  • The healthcare industry is expected to the range of services will be expanded and management will be done throughout the life cycle by combining Big Data, Information and Communications Technologies (ICT), and IT equipment. This paper describes a metacarpophalangeal joint flexion and interphalangeal joint flexion of the thumb as one area of healthcare. Metacarpophalangeal joint flexion refers to bending the thumb metacarpophalangeal joint, and interphalangeal joint flexion refers to bending the thumb interphalangeal joint. When bending the joints, if the angle is below a certain angle, or if you feel pain, you have an abnormality. In addition, prevention and therapy of the thumb joint were also suggested. Experiments were performed on metacarpophalangeal joint flexion and interphalangeal joint flexion in the thumb joint in the case of 50 people in their 50s. The system is limited to a portion of the body, but if it is partially expanded, it will be the basis for building a Big Data healthcare system.

  • Hyperhidrosis, one of the abnormalities in autonomic nervous system, has been treated with dermatologic principles or thoracic sympathectomy via conventional axillary thoracotomy or dorsal spinal approach. But these techniques were rather ineffective or invasive. Recently, VATS is widely applied in thoracic surgical area, and hyperhidrosis is not the exception of these cases. From May 1993 to August 1994, 30 patients with bilateral palmar hyperhidrosis underwent bilateral thoracic [T2, T3 sympathectomy with thoracoscopic surgery at Seoul National University Hospital. There were 20 men and 10 women and the mean age was 23.0 years.Mean operating time was 115 min and there was no thoracotomy conversion. Operative complications were anesthetic overdose in 1, Horner`s syndrome in 1, and small amount of residual pneumothorax in 6. Mean postoperative hospital stay was 2.3 days [range from 1 to 4 days and postoperative analgesics were required in 17 cases with a single dose.Sweating amount was measured in 12 patients, showing significantly decreased amount from 284.5 mg preoperatively to 18.9 mg postoperatively in 5 minutes [p=0.004 . There was no recurrence during mean 6 months follow up. Twenty two patients [73.3 % complained moderate compensatory hyperhidrosis on the trunk. In conclusion, all patients were greatly satisfied with those results including no more palmar sweating, less pain, better cosmetics, short hospital stay. In addition, recent use of sweating amount measurement and intraoperative temperature monitoring could make this technique more accurate, so we easily applied thoracoscopic sympathectomy with minimal risk.

  • Research involving discomfort or pain related to haptic vibratory stimulation the for prosthesis users of myoelectrical hand is very lacking. Our objective of this study was to evaluate the displeasure and sensitivity of areas in forearm using vibration stimulation system between upper limb amputees and non-amputees. Twenty transradial amputees and forty non-amputees (20 youth, 20 elderly) were involved. We set up custom-made vibration stimulation system including eight actuators (4 medial parts and 4 lateral parts) and GUI-based acquisition system, to investigate changes of residual somatosensory sensibility and displeasure at proximal 25% of forearm. Eight vibration actuators were attached to the circumference of proximal 25% point of forearm at regular intervals. Sensitivity tests were used to stimulate the 120Hz and discomfort experiment was used to 37 ~ 223Hz. The subjective responses were evaluated by 10 point scale. The results showed that both groups were similar in sensitive areas. Response at around of radius was most sensitive than other areas in all subjects. Elderly group do not appear discomfort of vibrotactile; however, youth group and amputee presented discomfort of vibrotactile. Prosthesis with a vibrotactile feedback system should be developed considering the sensitivity. Furthermore, Future studies should investigate the scope of application of that principle.

  • Purpose: This study was a prospective survey to evaluate the Korean Triage and Acuity Scale (KTAS) inter-rater reliability and to identify discordant areas in the KTAS classification between nurses and paramedics in emergency department (ED). Methods: Inter-rater reliability were evaluated using triage deta from January 11 to May 31, 2019. A convinience sample of a total of 800 patients who visited an emergnecy medical center in Incheon (400 adult and 400 pediatric patients in this study) were selected. The triages of this study a pair of one nurse and one paramedic performed triage at the same time, using the KTAS, Inter-rater reliability for the KTAS was evaluated with the weighted kappa. Results: The inter-rater reliability of the KTAS evaluated by weighted Kappa between nurse-paramedic KTAS score of adult patients was .71, and .66 for pediatric patients. Both were in the level considered as substantial. Among causes of triage discordance, the most frequently identified discordant item in triaging adults was the difference in the measurement of pain intensity (28.0%). For both adult and pediatric patients, multiple chief complaint was identified as the major discordant factor. In triaging pediatric patients the body temperature assessment by subjective judgments was the highest discordant item (50.7%). Conclusion: The study result suggested that the KTAS demonstrated a reasonable level of inter-rater reliability and functioned as a standardized triage tool for emergency medical services. In order to improve the inter-rater reliability and classification competence, it is necessary to revise the current all-integrated KTAS provider course to be differentiated for each job group.

  • Objectives: The purpose of this study is to find out the characteristics of dysmenorrhea patients with the Inbody test results by Sasang constitutions. Methods: The data from the 541 participants were collected using a structured measurement of menstrual pain. Based on the survey responses, we had 329 women with dysmenorrhea as the test group and 212 women without dysmenorrhea as the control group. The clinical trials subjects were asked to respond to another questionnaire for identifying their constitutional types and undergo Inbody test. Results: The result of a comparison of the test and control groups showed that the DSOM test showed that the scores of blood deficiency, qi stagnation, blood stasis and phlegm were significantly higher in dysmenorrhea. The result of the taeumin's test groups showed that the DSOM test showed that the scores of heat were significantly higher. For the Sasang constitution, there is a difference on the cause of the outbreak. Taeumin from blood deficiency, blood stasis, dampness, heart, kidney, phlegm and lung is associated with dysmenorrhea. Soeumin from blood deficiency, qi stagnation, blood stasis, liver, heart, spleen and phlegm is associated with dysmenorrhea. The ratio of overweight of taeumin was low in blood deficiency and yin deficiency. The ratio of lowweight of soeumin was high in heat. Conclusions: The DSOM test showed that the scores of blood deficiency, qi stagnation, blood stasis and phlegm were significantly higher in dysmenorrhea.

  • Postinfectious glomerulonephritis (PIGN) is most commonly caused by Streptococcus pyogenes in children, but PIGN associated with other pathogens has been described in the literature. A previously healthy 6-year-old boy was admitted with complaints of cough, fever, and right chest pain. The patient was diagnosed with pneumococcal bacteremia and influenza A virus infection and treated with antibiotics and antiviral agent. During hospitalization, generalized edema, hematuria, proteinuria, and increased blood pressure were observed; therefore, we started administering diuretics. The boy was discharged with gross hematuria, and even microscopic hematuria disappeared 14 weeks after discharge. We report a case of PIGN associated with bacteremic pneumococcal pneumonia and influenza A virus infection in children. A urine test and blood pressure measurement should be considered for the early detection of PIGN in children with pneumococcal or influenza A virus infection when they present with nephritic symptoms.

  • Purpose: The clinical implications of bowel wall thickening (BWT) on abdominal computed tomography (CT) among children are unknown. We aimed to suggest a new method for measuring BWT and determining its clinical significance in children. Methods: We retrospectively analyzed 423 patients with acute abdomen who underwent abdominal CT; 262 were classified into the BWT group. For this group, the pediatric radiologist described the maximal bowel wall thickness (MT), normal bowel wall thickness (mm) (NT), and their ratios for each segment of the bowel wall. Results: In the thickened bowel walls, the thickness differed significantly between the small bowel (6.83±2.14 mm; mean±standard deviation) and the colon (8.56±3.46 mm; p<0.001). The ratios of MT to NT in the small bowel (6.09±3.17) and the colon (7.58±3.70) were also significantly different (p<0.001). In the BWT group, 35 of 53 patients had positive fecal polymerase chain reaction results; 6 patients infected with viruses predominantly had BWT in the small intestine, while the terminal ileum and the colon were predominantly affected in 29 patients with bacterial infections. In the initially undiagnosed 158 patients with BWT, the symptoms improved spontaneously without progression to chronic gastrointestinal disease. Conclusion: This study provides a clinical reference value for BWT in the small intestine and colon using a new method in children. The BWT on abdominal CT in children might indicate nonspecific findings that can be observed and followed up without additional evaluation, unlike in adults.

  • Among non-invasive blood glucose detection technologies, the optical technique is a method that uses light reflection, absorption, and scattering characteristics when passing through a biological medium. It reduces pain or discomfort in measurement and has no risk of infection. So it is becoming a major flow of blood glucose detection research. Among them, near-infrared spectroscopy has a disadvantage in that the complexity increases when analyzing signals detected due to interferences between proteins and acids that share a similar absorption function with blood glucose molecules. In this study, a non-invasive sensor system with multiple near-infrared bands was designed and manufactured to alleviate the deterioration of blood glucose detection function that may occur due to skin absorption of near-infrared rays. A blood survey was conducted to verify the system, and the degree of blood glucose response in the blood was collected as spectral data, and the results of this study were quantitatively verified in terms of correlation between the data and blood glucose.
