Purpose: This study was a prospective survey to evaluate the Korean Triage and Acuity Scale (KTAS) inter-rater reliability and to identify discordant areas in the KTAS classification between nurses and paramedics in emergency department (ED). Methods: Inter-rater reliability were evaluated using triage deta from January 11 to May 31, 2019. A convinience sample of a total of 800 patients who visited an emergnecy medical center in Incheon (400 adult and 400 pediatric patients in this study) were selected. The triages of this study a pair of one nurse and one paramedic performed triage at the same time, using the KTAS, Inter-rater reliability for the KTAS was evaluated with the weighted kappa. Results: The inter-rater reliability of the KTAS evaluated by weighted Kappa between nurse-paramedic KTAS score of adult patients was .71, and .66 for pediatric patients. Both were in the level considered as substantial. Among causes of triage discordance, the most frequently identified discordant item in triaging adults was the difference in the measurement of pain intensity (28.0%). For both adult and pediatric patients, multiple chief complaint was identified as the major discordant factor. In triaging pediatric patients the body temperature assessment by subjective judgments was the highest discordant item (50.7%). Conclusion: The study result suggested that the KTAS demonstrated a reasonable level of inter-rater reliability and functioned as a standardized triage tool for emergency medical services. In order to improve the inter-rater reliability and classification competence, it is necessary to revise the current all-integrated KTAS provider course to be differentiated for each job group.
Objectives: The purpose of this study is to find out the characteristics of dysmenorrhea patients with the Inbody test results by Sasang constitutions. Methods: The data from the 541 participants were collected using a structured measurement of menstrual pain. Based on the survey responses, we had 329 women with dysmenorrhea as the test group and 212 women without dysmenorrhea as the control group. The clinical trials subjects were asked to respond to another questionnaire for identifying their constitutional types and undergo Inbody test. Results: The result of a comparison of the test and control groups showed that the DSOM test showed that the scores of blood deficiency, qi stagnation, blood stasis and phlegm were significantly higher in dysmenorrhea. The result of the taeumin's test groups showed that the DSOM test showed that the scores of heat were significantly higher. For the Sasang constitution, there is a difference on the cause of the outbreak. Taeumin from blood deficiency, blood stasis, dampness, heart, kidney, phlegm and lung is associated with dysmenorrhea. Soeumin from blood deficiency, qi stagnation, blood stasis, liver, heart, spleen and phlegm is associated with dysmenorrhea. The ratio of overweight of taeumin was low in blood deficiency and yin deficiency. The ratio of lowweight of soeumin was high in heat. Conclusions: The DSOM test showed that the scores of blood deficiency, qi stagnation, blood stasis and phlegm were significantly higher in dysmenorrhea.

Purpose : The study investigated the effects of Pilates exercise on strengthening trunk muscles of females who perform such exercise for the purpose of comparing activation of trunk muscles that contact while performing Pilates motions between females with or without at least 8 weeks of Pilates experience. Methods : The study investigated 10 females with at least 8 weeks of Pilates experience in the past 6 months (experienced group) and 10 healthy females without Pilates experience (non-experienced group). The study used basic Pilates postures involving hip abduction, lift, and leg swing motions as the measurement postures in comparing the activation of muscles used for stabilization, such as the rectus abdominis, external oblique abdominal, and transverse abdominis & internal oblique abdominal muscles. Surface electromyography was used for measuring muscle activation, and the measurements targeted activation of the rectus abdominis, external oblique abdominal, transversus abdominis, and internal oblique abdominal muscles. Results : The study results showed that, as compared to the non-experienced group, the experienced group had significantly higher muscle activation in the transverse abdominis and internal oblique abdominal muscles during hip abduction (p<.05) and significantly higher muscle activation in the rectus abdominis, external oblique abdominal, and transverse abdominis & internal oblique abdominal muscles during lift and leg swing motions (p<.05). Conclusion : Pilates exercise performed over a long period can be recommended as an effective exercise method that can increase the activation of trunk muscle, and especially, repeated performance of highly difficult motions can increase muscle activation even more, which can help promote spinal stabilization, prevent pain, and improve performance of activities of daily living.

Postinfectious glomerulonephritis (PIGN) is most commonly caused by Streptococcus pyogenes in children, but PIGN associated with other pathogens has been described in the literature. A previously healthy 6-year-old boy was admitted with complaints of cough, fever, and right chest pain. The patient was diagnosed with pneumococcal bacteremia and influenza A virus infection and treated with antibiotics and antiviral agent. During hospitalization, generalized edema, hematuria, proteinuria, and increased blood pressure were observed; therefore, we started administering diuretics. The boy was discharged with gross hematuria, and even microscopic hematuria disappeared 14 weeks after discharge. We report a case of PIGN associated with bacteremic pneumococcal pneumonia and influenza A virus infection in children. A urine test and blood pressure measurement should be considered for the early detection of PIGN in children with pneumococcal or influenza A virus infection when they present with nephritic symptoms.
Purpose: The clinical implications of bowel wall thickening (BWT) on abdominal computed tomography (CT) among children are unknown. We aimed to suggest a new method for measuring BWT and determining its clinical significance in children. Methods: We retrospectively analyzed 423 patients with acute abdomen who underwent abdominal CT; 262 were classified into the BWT group. For this group, the pediatric radiologist described the maximal bowel wall thickness (MT), normal bowel wall thickness (mm) (NT), and their ratios for each segment of the bowel wall. Results: In the thickened bowel walls, the thickness differed significantly between the small bowel (6.83±2.14 mm; mean±standard deviation) and the colon (8.56±3.46 mm; p<0.001). The ratios of MT to NT in the small bowel (6.09±3.17) and the colon (7.58±3.70) were also significantly different (p<0.001). In the BWT group, 35 of 53 patients had positive fecal polymerase chain reaction results; 6 patients infected with viruses predominantly had BWT in the small intestine, while the terminal ileum and the colon were predominantly affected in 29 patients with bacterial infections. In the initially undiagnosed 158 patients with BWT, the symptoms improved spontaneously without progression to chronic gastrointestinal disease. Conclusion: This study provides a clinical reference value for BWT in the small intestine and colon using a new method in children. The BWT on abdominal CT in children might indicate nonspecific findings that can be observed and followed up without additional evaluation, unlike in adults.

Among non-invasive blood glucose detection technologies, the optical technique is a method that uses light reflection, absorption, and scattering characteristics when passing through a biological medium. It reduces pain or discomfort in measurement and has no risk of infection. So it is becoming a major flow of blood glucose detection research. Among them, near-infrared spectroscopy has a disadvantage in that the complexity increases when analyzing signals detected due to interferences between proteins and acids that share a similar absorption function with blood glucose molecules. In this study, a non-invasive sensor system with multiple near-infrared bands was designed and manufactured to alleviate the deterioration of blood glucose detection function that may occur due to skin absorption of near-infrared rays. A blood survey was conducted to verify the system, and the degree of blood glucose response in the blood was collected as spectral data, and the results of this study were quantitatively verified in terms of correlation between the data and blood glucose.

Objectives This study is aimed to evaluate cost-effectiveness of treatment of Korean Medicine for whiplash associated disorders (WAD) and to suggest the direction of future research. Methods We searched papers in Pubmed database to use some keywords indicating whiplash injury syndrome, treatment of Korean Medicine and cost-effectiveness. After searching, appropriate papers were selected depending on the exclusion criteria. The selected papers were analyzed in the sections of author, publication year, intervention and control groups, outcome measurement, the list including in the cost, cost-effectiveness, cost-utility and study design. Results Four studies about effect of Korean medicine and 3 studies about economic evaluation were finally included. Acupuncture was effective for balance disorder and neck pain. Economic evaluation studies used analytical decision model or cost-consequence analysis. Cost-effectiveness analysis using visual analog scale and cost-utility analysis using quality adjusted life years were performed. Initiating timing of proper management affected the consequence of treatments. Direct and indirect medical costs including supportive devices, and non-medical costs such as litigation were considered. Conclusions We conclude that studies haven't been conducted so far to evaluate the cost-effectiveness of Korean Medicine in whiplash injury syndrome. Thus, future studies are needed in this section.

The purpose of this study is to observe the effectiveness of the Korean medical complex treatments for functional dyspepsia (FD). And we observed NDI-K (Nepean dyspepsia index-Korean version) and algometer on acupoints to confirm the treatment effectiveness. In this retrospective study, we investigated 77 patients with FD symptoms who admitted to 00 Korean Hospital for 2 weeks from April 14, 2020 to March 31, 2021. All patietns were treated with herbal medicines, acupuncture, pharmacopuncture, moxa treatment, aroma therapy, So-jeok and So-jeok II therapy. On the day of admission and discharge, NDI-K, algometer on acupoints (CV12, CV10, ST25, ST27, SP14) were examined twice. Among the NDI-K values, all 14 items except for poor breathing were decreased statistically significantly. The algometer values were increased statistically significantly in all acupoints (CV12, CV10, ST25, ST27, SP14). These results suggest that Korean medical treatment can be useful in the treatment of FD.

Background: The purpose of this study is to analyze the effect of shoulder and neck stabilization exercise using a gym ball for healthy women in their 20s with round shoulder and forward head posture, it is intended to try and find improvement for posture. Methods: The subjects of the study were 25 women who understood the purpose of the research and voluntarily agreed to participate in the experiment. They carried out the shoulder and neck stabilization exercise program using a gym ball, which was conducted for 35 minutes, 3 times a week for 4 weeks. The position of the round shoulder and shoulder bones was measured using an tape measurement and were recorded in both at pre, 2 weeks, and post intervention. The craniovertebral angle (CVA) and craniorotational angle (CRA) was measured using an image J. Results: The round shoulder posture left was statistically significantly different within group (p<.05); however, the round shoulder posture right wasn't statistically significantly different (p>.05). As a result of this contrast, test were significant different in both at pre, 2 weeks and post. The scapular position on the left and right were statistically significantly different within the group (p<.05). As a result of the contrast, test were significant different in both at pre, 2 weeks and post (p<.05). The CVA and CRA on the left and right were statistically significantly different within the group (p<.05). As a result of the contrast, test were significant different in both at pre, 2 weeks and post (p<.05). Conclusion: The gym ball exercises improved the posture of women in their 20s. Therefore, gym ball exercises can help improve the quality of life of those with shoulder and neck pain.
Background: In the present study, the age- and sex-adjusted Constant score (CS) in a normal Indian population was calculated and any differences with other population cohorts assessed. Methods: The study participants were patients who visited the outpatient department for problems other than shoulder and healthy volunteers from the local population. Patients without shoulder pain/discomfort during activity were included in the study. Subjects with any problem that might affect shoulder function (e.g., cervical, thoracic spine, rib cage deformity, inflammatory arthritis) were excluded. Constant scoring of all participants was performed by trained senior residents under the supervision of the senior faculty. Shoulder range of movement and strength were measured following recommendations given by the research and Development Committee of the European Society for Shoulder and Elbow Surgery (2008). A fixed spring balance was used for strength measurement; one end was fixed on the floor and the other end tied with a strap to the wrist of the participant, arm in 90° abduction in scapular plane with palm facing down. Results: Among the 248 subjects (496 shoulders), the average age was 37 years (range, 18-78 years), 65.7% were males (326 shoulders) and 34.3% females (170 shoulders). The mean CS was 84.6±2.9 (males, 86.1±3.0; females, 81.8±2.9). CS decreased significantly after 50 years of age in males and 40 years of age in females (p<0.05). The mean CS was lower than in previous studies for both males and females. Heavy occupation workers had higher mean CS (p<0.05). A linear standardized equation was estimated for calculating the adjusted CS for any age. Conclusions: Mean CS and its change with age differed from previous studies among various population cohorts.
