The herpes zoster infecting ophthalmic branch of trigeminal Nerve that is similar to migraine at first stage symptom has been treated with oriental medication at Dept. of Internal Medicine, Semyung University Oriental Hospital. The fIrst symptom of roster is burning pain, tingling or extreme sensitivity in one area of the skin, usually limited to one side of the body. This may be present for one to three days before a red rash appears at that site. There may also be a fever or headache. The rash soon turns into groups of blisters. The blisters start out clear but then pus or dark blood collects in the blisters before they crust over (scab) and begin to disappear. The pain may last longer. In this case, the severe pain was present for five days, the blisters and scabsdisappeared entirely on the seventeenth day, but postherpetic neuralgia, the most common complication and is observed most frequently in the ophthalmic branch of trigeminal nerve, was not prevented entirely. We have observed this case and report to help treatment on this disease at oriental medicine clinic.

During the early stage of postherpetic neuralgia, an epidural block on the affected segment is helpful in controlling pain and preventing progression to a chronic state. The main neurologic complication following an epidural block is cord compression symptom due to an epidural hematoma. When neurologic complications arise from an epidural block for the treatment of postherpetic neuralgia, it is important to determine whether the complications are due to the procedure or due to the herpes zoster itself. We report a case of a patient who was diagnosed with herpes zoster myelitis during treatment for postherpetic neuralgia. The patient complained of motor weakness in the lower extremities after receiving a thoracic epidural block six times. Although initially, we believed that the complications were due to the epidural block, it was ultimately determined to be from the herpes zoster myelitis.

Acute viral meningitis and myositis are rare complications of varicella-zoster virus (VZV) reactivation. A 71-years-old immunocompetent man, who presented with lower back pain radiating to the left lower extremities, developed vesicles on the L5 dermatomal area. The next day, he had complained of aberrant vesicles on the trunk, face and scalp, with generalized myalgia, headache and dizziness. He was confirmed with VZV meningitis and myositis, as demonstrated by the presence of VZV DNA in the blood and cerebral spinal fluid using a polymerase chain reaction (PCR) amplification. PCR has been used in patients with a VZV infection associated neurological symptoms, and provides a useful tool for the early diagnosis of VZV-associated neurological disease. The patient was treated with bed rest, with intravenous acyclovir for the VZV infection, and intravenous Patient-controlled Analgesia for pain management and the prevention of postherpetic neuralgia. When he visited the outpatient department 3 months later, the skin lesion, leg pain, headache and myalgia had all improved, without sequelae. Here, this case is reported, with a discussion of the relevant literature on its diagnosis and management.

A 34 year old male patient visited to our neuro-pain clinic with symtoms of a left frontal headache, eyeball throbbing and occipital pain. Two days after the first visit to our clinic. pain was aggrevated and the skin eruption appeared on the left forehead. He was diagnosed as raving Herpes Zoster Opthalmicus(HZO). We performed stellate ganglion block(SGB), but pain did not subsid. So a continuous cervical epidural block was perfomed(CCEB) and it could relieve the pain promptly. In this case, VAS(visual analogue scale) was diminished from 10 to 3 and the skin eruption was healed 24 days after the treatment with CCEB and SGB. We experienced that CCEB is more effective rather than intermittent SGB in intractable HZO. CCEB should be considered to the treatment of choice in patients with HZO.

Purpose: Herpes zoster is a common dermatologic disease characterized by unilateral pain and vesicular lesions over the unilateral sensory dermatomes being caused by the reactivation of Varicella zoster virus and its incidence seems to be increasing recently. In case of involving the ganglion of the fifth cranial nerve(trigeminal nerve), it can descend down the affected nerve into skin, then producing an eruption in the dermatome. Among the patients, about 40 - 50% had associated conditions such as diabetes mellitus, hypertension, pulmonary tuberculosis, liver diseases, peptic ulcer, hypothyroidism, pharyngitis but rare facial trauma. Methods: Retrospective study was done for 3 cases of Herpes zoster from May 2000 to May 2007, which had been treated with acyclovir and steroid. Results: The clinical course was uneventful. Follow-up length was about 3 months. After treatment, the patients became stable and there was no complications. Conclusion: Herpes zoster was commonly associated with systemic disorders and the treatment duration was prolonged in associated diseases. But herpes zoster occurring specifically at the site of previously traumatized facial skin has not yet been reported. We experienced the treatment of herpes zoster developing within recent operative facial scar and three cases are presented with the review of literatures. Finally, facial trauma might be a risk factor for herpes zoster in traumatized patients.

Segmental zoster paresis is characterized by focal, asymmetric motor weakness in the myotome corresponding to the dermatome of the rash. A 73-year-old man, who presented with severe right shoulder pain and shoulder girdle muscle weakness, was diagnosed with segmental zoster paresis involvement of the C5⁣-C6 motor roots as a complication of herpes zoster. Girdle muscles (supraspinatus, deltoid and infraspinatus) atrophy had developed in his right shoulder. An MRI showed rotator cuff tearing in his right shoulder; therefore, an arthroscopic rotator cuff repair was performed. Herein, this case is presented to emphasize the importance of considering post-herpetic segmental motor paresis in the differential diagnosis of acute painful motor weakness of the upper extremities.

Objectives : The purpose of this study is to analyze the trend of Korean medical treatments on acute stage of herpes zoster. Methods : Domestic databases(OASIS, KTKP, RISS, NDSL, KISS) were used to search case study papers related to the acute herpes zoster. Total 33 studies were selected and analyzed. Results : According to clinical type, Ramsay Hunt syndrome was the most frequent type, followed by herpes zoster genralisatus, facial herpes zoster. Manual acupuncture was the most frequently used treatment modalities, followed by Korean herbal medicine. LI4(合谷), LR3(太衝), ST36(足三里), GB20(風池) were the major acupoints used for the treatment of acute herpes zoster. Yongdamsagan-tang(龍膽瀉肝湯) was the most frequently used prescription followed by Ligigeopoong-san(理氣祛風散). As principal herb, Gentianae Scabrae Radix et Rhizoma(龍膽) and Bupleuri Radix(柴胡) were the most frequently used herbs followed by Alismatis Rhizoma(澤瀉). Conclusions : Though several limits remains, this is the first study to analyze the trends of Korean medicine treatment for the acute stage of herpes zoster, it may helpful for the clinical practitioner.

Authors have experienced the treatment for the uppermost frequent cases, herpes zoster, frozen shoulder and low back pain among those who visited our pain clinic. 1. Herpes zoster The Patients who received the treatment within 4 weeks of the onset of the disease, not only healed herpes without leaving post herpetic neuralgia but also crust formation was fast. 2. Frozen shoulder Over 90% of the patients who received suprascapular nerve block accompanied with trigger points electric stimulation was very efficient procedure, futhermore, authors felt keenly the necessity each patients co-operation and individual endurance for this treatment. 3. Low back pain An epidural steroid administration to the patients who did not have any organic disturbances was effective treatment. If this non-effective, search other cause of the disease or operation was advisable one.

1. Objectives The purpose of these 2 case studies is to evaluate the effects of Gihwangbeakho-tang which is based on the Sasang Constitutional Medicine for the Herpes zoster. 2. Methods We used Visual analogue sign(VAS) and pictures of the affected part for the assessment. 3. Results After two patients were treated with Gihwangbeakho-tang, the vesicles were cleared and the pain was decreased. 4. Conclusions These 2 case studies show an efficient result of using Gihwangbeakho-tang in the treatment of Herpes zoster.
