Background: The management of contralateral bullae incidentally found in radiological studies is controversial, largely due to the unpredictability of the natural course of incidentally found contralateral bullae. This study aimed to identify the factors associated with the contralateral occurrence of primary spontaneous pneumothorax (PSP), and to characterize the outcomes of contralateral bullae incidentally found in radiological studies. Methods: From January 2005 to December 2008, 285 patients were admitted to our institution for PSP, and the patients underwent follow-up until August 2012. The relationships between the following variables and contralateral pneumothorax occurrence were evaluated: age, sex, smoking history, body mass index, ipsilateral recurrence, ipsilateral bullae size, the number of ipsilateral bullae, contralateral bullae size, and the number of contralateral bullae. Results: The study group consisted of 233 males and 29 females. The mean age and mean body index of the patients were $23.85{\pm}9.50years$ and $19.63{\pm}2.50kg/m^2$. Contralateral PSP occurred in 26 patients. The five-year contralateral PSP occurrence-free survival rate was 64.3% in patients in whom contralateral bullae were found. Conclusion: The occurrence of contralateral PSP was associated with younger age, ipsilateral recurrence, and the presence of contralateral bullae. Contralateral PSP occurrence was more common in young patients and patients with recurrent PSP. Single-stage bilateral surgery should be considered if an operation is needed in young patients, patients with recurrent pneumothorax, and patients with contralateral bullae.

The Personal Software Process(PSP) is a rigorous, measurements based process designed to help software developers produce high-quality codes. Learning PSP requires substantial and tedious record keeping including clerical computations with plenty of data occurred at every project. The required task reduces the educational effect and motive by time consuming characteristic and easy occurrence of errors. To alleviate these problems, a web-based PSP log system having streamlined mechanism of data manipulation is designed and implemented at this study. Three years of experiment shows that the system not only improves productivity and accuracy of the process but also allows students and instructors to focus on learning and teaching PSP, rather than PSP data collection.

We analyzed the paralytic shellfish poisoning (PSP) toxins of the toxic marine dinoflagellate Alexandrium tamarense collected from Dadaepo and Gaduck-do in Busan and from Sujeong-ri in Jinhae Bay, Korea, in April 2003. We also analyzed the PSP toxin of mussels (Mytilus galloprovincialis) and oysters (Crassostrea gigas) collected around Busan and Jinhae Bay. PSP toxin analyses were conducted by high performance liquid chromatography (HPLC). Fifteen cultured A. tamarense isolates contained 2.78 to 57.47 fmol/cell, with nearly identical toxin profiles: major components C2, GTX4; minor components C1, GTX1, NEO; and trace components GTX2, GTX3, STX. PSP toxin contents were 0 to $492\;\mu{g}$ STXeq/100 g in mussels and 0 to $48\;\mu{g}$ STXeq/100 g in oysters. Mussels at Gijang and Sujeong-ri contained the most PSP toxin contents ($492\;\mu{g}\;STXeq/100\;g\;and\;252\;\mu{g}\;STXeq/100\;g,\;respectively$), exceeding the quarantine level ($80\;\mu{g}$ STXeq/100 g). Their dominant toxin components were C2, C1, GTX2, and GTX3; the minor components GTX1, GTX4, GTX5, and NEO were sporadically detected. Phytoplankton contained 0.774 fmol/L seawater and 1.228 fmol/L seawater at Gijang and Sujeong-ri in April. At that time, Alexandrium cells were present in the water column at Gijang at 2,577 cells/mL and at Sujeong-ri at 6,750 cells/mL. Overall, we found the high and similar PSP toxin contents in AZexandrium isolates and mussels, and a correlation between occurrence of toxic Alexandrium cells in the water column and mussel intoxification. High densities of toxic Alexandrium cells in the water column immediately preceded shellfish intoxification at Gijang and Sujeong-ri in April.

Background: We aimed to investigate the characteristics of primary spontaneous pneumothorax (PSP) in adolescents and to analyze the relationship between the occurrence of PSP and air pollutants. Methods: Data pertaining to age, sex, body mass index, smoking status, initial pneumothorax volume, presence of bullae, treatment methods, and city of residence were retrospectively obtained from January 2010 to December 2014. We investigated the association between short-term exposure to air pollutants (SO2, NO2, O3, CO, and PM10) and the occurrence of PSP using a case-crossover design with conditional logistic regression. Results: We collected information from 598 patients who were admitted for PSP, with a mean follow-up duration of 62.9 months. The majority (91.1%) of the patients were male. In the case-crossover design, conditional logistic regression showed that no air pollutant was associated with the occurrence of pneumothorax. The results were consistent across all city subgroups (Anyang, Gunpo, Uiwang, and Gwacheon). Conclusion: In our study, the incidence rate of pneumothorax was 153.8 per 100,000 person-years in male adolescents and 16.7 per 100,000 person-years in female adolescents. The case-crossover design showed that PSP in adolescents is unlikely to be related to air pollution.
To prevent paralytic shellfish poisoning (PSP) due to the consumption of shellfish contaminated with PSP toxins, the quantitative analysis of these toxins is very crucial. The AOAC International mouse bioassay (MBA) has been used widely for the routine monitoring of PSP toxins for more than 50 years. However, this method has low sensitivity and high limit of quantification (LOQ) and interferences from other components in the extract, and it cannot determine toxic profiles. Ethical problems also exist with the continued use of this live mouse assay. To establish an alternative method to the MBA used for PSP toxins analysis, we attempted to optimize the analysis conditions of a precolumn high-performance liquid chromatography (HPLC) oxidation method and succeeded in validating its accuracy and precision in quantifying PSP toxins. A clear peak and the isolation of PSP toxins were obtained by injecting the working standards of Certified Reference Materials using HPLC. The LOQ of the precolumn HPLC oxidation method for PSP toxins was about $0.1002{\mu}g/g$, which represented an approximately fourfold improvement in detection capability versus the AOAC MBA. The intra-accuracy and precision for PSP toxins in oysters were 77.0-103.3% and 2.0-5.7%, respectively, while the respective inter-accuracy and precision were 77.3-100.7% and 2.4-6.0%. The mean recoveries of PSP toxins from oysters were 75.2-112.1%. The results of a comparison study showed good correlation between the results of the precolumn HPLC oxidation method and those of MBA, with a correlation factor of 0.9291 for mussels. The precolumn HPLC oxidation method may be used as an alternative to, or supplementary method with, MBA to monitor the occurrence of PSP toxins and to analyze the profiles of these toxins in shellfish.
To compare the accumulation of paralytic shellfish poison (PSP) in different marine organisms, the occurrence and variation of PSP were surveyed in blue mussel Mytilus edulis, oyster Crassostrea gigas, short neck clam Ruditapes philippinarum, bay scallop Argopecten irradians, and warty sea squirt Styela clava collected from Jinhae Bay, Korea, in 2005 and 2006 year. We also investigated the ability of the blue mussel to detoxify PSP by relaying and depuration (via the water flow or water circulation system). In the marine organisms examined, PSP levels were the highest in blue mussel, followed in order by bay scallop, oyster, short neck clam, and warty sea squirt. Comparing the maximum PSP levels in the bivalve species examined in 2005 and 2006, PSP in blue mussel was 1.6-2.0, 4.0-5.9, and 5.1-6.0 times higher than in bay scallop, oyster, and short neck clam, respectively. Therefore, blue mussel could be useful as a bioindicator for PSP monitoring. With the increasing PSP levels in blue mussel in 2006, the proportion of PSP in its digestive gland increased to 95.1% when the maximum level was detected from the whole tissues of blue mussel on May 29. Subsequently, the PSP proportion in the digestive gland decreased as the PSP level in whole tissue decreased. The detoxification of PSP in blue mussel was greatest with relaying, followed by the water flow, and water circulation systems. Relaying decreased the PSP level below the regulatory limit of $80{\mu}g$/100 g after 2 days in low toxic sample with $124{\mu}g$/100 g, and after 7 days in high toxic sample with $401{\mu}g$/100 g. During depuration in the blue mussel with $401{\mu}g$/100 g via the water flow system, the PSP amounts in the digestive gland decreased by about 50% after 1 day, and about 77% after 7 days. In contrast, the PSP amounts in the soft body, gill, and mantle did not change significantly with depuration.
Paralytic shellfish poison (PSP) occurrence and variation in th\ulcorner different bivalve species including oyster,Crassostrea gigas and mussel, Mytilus edulis Jinhae bay Korea from January to December in 1997 were surveyed. And also compositional characteristics of PSP ingredients in the different bivalve species were investigated. PSP in shellfish was detected from late February and increased continuously until the middle of April in Jinhae bay. And after April PSP level had been decreased gradually and the toxicity was not detected by mouse bioassay in the early of June. Of the examined bivalve species, PSP content in the mussel exhibited the highest value and the PSP content in the mussel in the middle of April, PSP high season in Jinhae bay, was 6 times higher than that in the oyster. Gonyautokin (GTX) 1~4 group occupied 59.0~78.8% of whole PSP contents and identified as dominant ingredient in the examined bivalves except oyster. And it is also identified that the PSP toxicity in the tested species were derived from the GTX group. And the dominant ingredient of PSP in the oyster was carbamoyl-N-sulfo-11$\alpha$-hydroxysaxitoxin sulfate(Cl)(37.9%) and neosaxitoxin(neoSTX)(26.2%). But the toxicity of Cl in the tested oyster could be ignorable and most toxicity (80.0%) was derived from saxitoxin (STX) group.

Paralytic shellfish poisoning (PSP) occurs when saxitoxin (STX), which is produced by harmful algae (dinoflagellates) and then accumulated in bivalve shellfish by filter-feeding, is consumed by humans. With recent advances in analysis technology, it has been reported that dinoflagellates also produce a variety of analogues such as the gonyautoxin (GTX) group and the N-sulfo-carbamoyl toxin (C toxin) group, in addition to STX. Accordingly, CODEX and the EFSA are stepping forward to manage STX and analogues as STX groups requiring safety management. In Korea, the occurrence of dinoflagellates producing STX analogues has already been reported, and contamination of analogues (GTX group, C toxin group) in live mussels has also been reported. In this study, in order to provide the basis for systematic monitoring and safety management of STX and analogues, their physicochemical characteristics, occurrence of dinoflagellates, toxicity and toxic equivalency factor, analytical method and occurrence were widely reviewed. This review is expected to contribute to strengthening the safety management of STX and its analogues.

AOAC Mouse Bioassay Analysis (MBA) has been the gold standard for the analysis of paralytic shellfish poisoning toxin (PSP toxin) for more than 50 years. However, this method has inaccurate limit of quantification and cannot be used to determine toxic profiles. An HPLC method (PCOX) was optimized for Korean shellfish to establish an alternative or supplementary method for PSP analysis and was intended to be used for the official monitoring and regulation of food. The recovery rate of the PCOX method was 83.5-112.1% and the limit of quantification for total toxin was about $8.6{\mu}g$/100 g. A long-term comparison study showed a good correlation of the PCOX results with the AOAC MBA results: the correlation factors were 0.9534 and 0.9109 for oyster and mussel matrices, respectively. The PCOX method may be used as an alternative or supplementary method for AOAC MBA to monitor the occurrence of PSP and to analyze PSP toxin profile in oysters and mussels.
