Development of Safety Climate Measuring Software Tool (안전문화측정 전산화 프로그램 개발)

  • The safety culture of an organization is very complex and hard to study, but it is possible to examine norms that make up the culture. This paper describes of a Process Safety Management (PSM) climate assessment tool developed in high risk industry such as chemical industries. The main purpose is to develop measuring software tool behaviors by examining their nature and strength and by analysing underlying factors that offer explanations for attitudinal differences. We reviewed the current techniques and literatures available to assess safety climate and culture, conducted focus group interviews and discussions. Based on the reviews and focus group, PSM climate assessment questionnaires and an online application program were developed. A pilot assessment was done at a chemical plant in Korea and the safety climate profile was completed. It was suggested to have a constant feedback from different sectors of industry to improve the application.

  • Even if several chemical accident prevention systems such as PSM(Process Safety Management), RMP(Risk Management Plan), etc. have been carried out, many chemical accidents have still occurred at industrial plants in Korea. We describe the status of chemical industry and the trend of occurrence of chemical accidents in Korea. And this paper analyzes the recent chemical accidents in eight ways. These ways include chemical accident forms, ignition sources, sources of chemical equipment, human vs equipment/material causes, worker's working situation, employee scale, hazardous substances, week & time, fatalities of manufacture & contractor's workers. Finally we proposes the four representative prevention measures brought to result of cause analysis by accident statistics.

A Study on Improvement of Safety Management in Subcontractor through Process Safety Management (공정안전관리를 통한 도급업체 안전관리 강화에 관한 연구)

  • Looking at the large and small fire, explosion, or release accidents that occurred recently, we can see that the accident of subcontractor workers has been increased in the contractor workplace while a harmful or dangerous maintenance works. In this study, the actual status of subcontractor's safety management was examined by using the questionnaire to the contractor who submitted the process safety report. In order to improve the safety management of subcontractors, the responsibility and rational role-sharing between the contractors and subcontractors were reviewed. Also, Providing safety and health information and worker protection measures during maintenance were investigated. The results of this study can be used to help strengthen and improvement safety management of the subcontractor at the time of a hazardous or dangerous works. In addition, it will be utilized to reduce industrial accidents and to build mutual cooperation relationship between contractor and subcontractor.

  • Establish standards and review procedures for on-site implementation of the emergency response plan required to establish by substances requiring preparation for accidents. Investigated for the examination content and standards for Process Safety Management (PSM) and Safety Management System (SMS). Safety Management System operator's comments and problems to be expected in the review process. Standards for on-site implementation and review of a person's ability to do the job does not require a high level of expertise. However, it has been created mainly to determine whether appropriate items to be included in the report and the contents presented.

  • The consequence analysis (CA) is widely using in the petrochemical plant through adoption of the process safety management (PSM) system, but it has not practical problem that the CA was not reflected effects for employee count, business interruption loss, utility usage, and etc.. In this study, to establish the practical emergency response plan and to achieve risk based management, the consequence based on the damage area and the financial loss were estimated and compared through application on facilities in the petrochemical plant. If the damage area is used, the consequence category must be determined by safety area considering simultaneously damage area, fatality area and toxic area. Also, the consequences based on the financial loss is more practical method for the case of considering process properties and circumstances.

  • A lot of hazardous materials have been used for product processing and utility plant. Many accidents including toxic release, fire and explosions occur in the ammonia related facility and plant. Various safety and environment management program including PSM, SMS, ORA etc. are being implemented for risk management and accident prevention in the production industry. Also much study and research have been carried about risk assessment of accident scenario in the academic and research area. In this paper, firstly risk level was assessed by using a typically used KORA program and LOPA PFD method for the selected ammonia unloading and storage facility. And then risk reduction measures for the risk assessed facility were studied in 3 aspects and some measures were proposed. Those Risk Reduction measures are including a leak detection and emergency isolation, water spray, dilution tank, dike and trench, scattering protection in hardware impovement aspect, and a applicable risk criteria, conditional modifier for existing LOPA PFD, alternative supporting modeling program in risk estimation methodology aspect, and last RBPS(Risk Based Process Safety) program, re-doing of process hazard analysis, management system compliance audit in managerial activity aspect.

  • 화학플랜트는 폭발성, 유독성 물질의 제조와 취급하므로 사고 발생에 의한 영향은 대단히 크고, 또한 사고 후 몇 년에 걸쳐 그 주변사회에 영향을 미친다. 따라서 화학공장의 화재, 폭발 및 독극물 누출에 의한 사고 등과 같은 가능한 모든 유형의 위험을 예측하고 예방하기 위해서 위험성 평가를 수행해야 한다. 이에 공정안전관리(Process Safety Management, PSM)에 대한 사회적인 관심이 증가하면서 많은 연구자들이 위험성 평가방법을 개발하여 잠재적 위험을 발견하고 안전대책(safety guard)을 제시하여 공정의 안전성을 확보하고자 노력해 왔다. 그러나 개발된 위험성 평가기법은 대부분 전문가들의 수작업을 통해서 이루어짐으로 신뢰성과 안전성을 항상 보장해 주지 못한다. 따라서 근래에 위험성 평가과정을 자동화하고자 하는 연구가 활발히 진행되고 있다.   • This study aims to suggest a measure to reduce industrial accident and secure adaptation to the industrial safety regulation policies by empirically establishing the effects of stress from adaptation to the industrial safety regulation on the human error, one of direct causes of industrial accident to the target groups of the industrial safety regulation policies. This study as for content range examines the stress from adaptation to the industrial safety regulation, job satisfaction, human error, and as for spatial range, this study performed an empirical research on the workers of 24 companies located within Chungcheong region centers district among 153 companies nationwide that uses 7 chemicals by introducing PSM (process safety management) system of the Ministry of Employment & Labor as of January 2012. Based on these results and suggestions there are five(5) necessities to reduce industrial safety regulations adaptation stress and human error generation, first(1st) is the necessity of understanding influential factors to stress, second(2nd) is the necessity to customized vocational training, third(3rd) is improving enhance system of job satisfaction, forth(4th) is preparing stress-reduction program, and fifth(5th) is introducing on-site restrictive action of advanced country as politics items.

Process Risk Assessment for a Batch Condensation Reaction of Polyester Resin using K-PSR Technique (K-PSR 기법을 활용한 회분식 폴리에스터 축합반응에서의 공정 위험성 평가 연구)

  • Risk assessment and analysis for a medium-to-small sized chemical plant that manufactures a polyester resin by the process of batch-type condensation reaction was conducted using K-PSR technique which is one of the risk assessment methods used to implement the Process Safety Management System (PSM). K-PSR is a risk assessment technique developed by KOSHA to compensate for difficulties caused by the lack of infrastructure of medium-to-small sized chemical plants in the re-evaluation. To apply the K-PSR technique, the entire process of a selected chemical palnt was classified in two review sections, i.e., the condensation reaction process and the dilution/filtration process, and the potential risks of the process about these review sections were identified and classified based on the four guide-words (release, fire.explosion, process trouble, and injury). As the results of the research, refer to recommend of risk rating has been confirmed that non-destructive testing of old facilities and the preparation of LOTO procedures for the electrical equipments are necessary as specific measures to prevent the risk of release and fire.explosion. It was also shown that pressure gauges and thermometers should be installed on the hot-oil supply piping to minimize the process trouble, and exhausting hood should be installed to prevent potential injury.

Review half configuration to enhance the professionalism and efficiency of the work of this research was performed.
