Evaluation of Parameters of Gas Exchange During Partial Liquid Ventilation in Normal Rabbit Lung (토끼의 정상 폐 모델에서 부분액체환기 시 가스교환에 영향을 주는 인자들에 대한 연구)

  • Background: The opitmal ventilator setting during partial liquid ventilation(PLV) is controversial. This study investigated the effects of various gas exchange parameters during PLV in normal rabbit lungs in order to aid in the development of an optimal ventilator setting during PLV. Methods: Seven New-Zealand white rabbits were ventilated in pressure-controlled mode with the following settings; tidal volume($V_T$) 8 mL/kg, positive end-expiratory pressure(PEEP) 4 $cmH_2O$, inspiratory-to-expiratory ratio(I:E ratio) 1:2, fraction of inspired oxygen($F_TO_2$) 1.0. The respiration rate(RR) was adjusted to keep $PaCO_2$ between 35~45 mmHg. The ventilator settings were changed every 30 min in the following sequence : (1) Baseline, as the basal ventilator setting, (2) Inverse ratio, I:E ratio 2:1, (3) high PEEP, adjust PEEP to achieve the same mean inspiratory pressure (MIP) as in the inverse ratio, (4) High $V_T$, $V_T$ 15 mL/kg, (5) high RR, the same minute ventilation (MV) as in the High $V_T$. Subsequently, the same protocol was repeated after instilling 18 mL/kg of perfluorodecalin for PLV. The parameters of gas exchange, lung mechanics, and hemodynamics were examined. Results: (1) The gas ventilation(GV) group showed no significant changes in the $PaO_2$ at all phases. The $PaCO_2$ was lower and the pH was higher at the high $V_T$ and high RR phases(p<0.05). No significant changes in the lung mechanics and hemodynamics parameters were observed. (2) The baseline $PaO_2$ for the PLV was $312{\pm}$ mmHg. This was significantly lower when decreased compared to the baseline $PaO_2$ for GV which was $504{\pm}81$ mmHg(p=0.001). During PLV, the $PaO_2$, was significantly higher at the high PEEP($452{\pm}38$ mmHg) and high $V_T$ ($461{\pm}53$ mmHg) phases compared with the baseline phase. However, it did not change significantly during the inverse I:E ratio or the high RR phases. (3) The $PaCO_2$ was significantly lower at high $V_T$ and RR phases for both the GV and PLV. During the PLV, $PaCO_2$ were significantly higher compared to the GV (p<0.05). (4) There were no important or significant changes in of baseline and high RR phases lung mechanics and hemodynamics parameters during the PLV. Conclusion: During PLV in the normal lung, adequate $V_T$ and PEEP are important for optimal oxygenation.

Detection of the expression of a Bombyx mori Atypical Protein Kinase C in BmPLV-Infected Larval Midgut

  • Protein kinase C (PKC) is involved in many cellular signaling pathways, it participates in many physiological processes, such as cell cycle, growth, proliferation, differentiation and apoptosis. To investigate the effect of PKC on the silkworm midgut tissue infection of Bombyx mori parvo-like virus (BmPLV), a B. mori atypical protein kinase C (BmaPKC) gene was cloned from larval midgut tissue, expressed in E. coli and purified. Additionally, the BmPLV susceptible silkworm strain and resistant silkworm strain were used to test the effect of the B. mori infection on BmPLV. The result showed that BmaPKC encodes a predicted 586 amino acid protein, which contains a C-terminal kinase domain and an N-terminal regulatory domain. The maximum expression amount of the soluble (His)6-tagged fusion protein was detected after 0.8 mmol/L IPTG was added and cultured at $21^{\circ}C$. The (His) 6-tagged fusion protein revealed about 73 kDa molecular weight which confirmed by western blot and mass spectrography. Furthermore BmaPKC protein were detected at 0-72 h post-infection in BmPLVinfected larval midgut tissue, western blot showed that as time went on, the expression of BmaPKC increased gradually in susceptible strain, the expression quantity on 72 h is 5 times of 0 h. However, in resistant strain, the expression quantity is slightly lower than susceptible strain. But no significant change in resistant strain was observed as time went on. The available data suggest that BmaPKC may involve in the regulation of BmPLV proliferation.

First Report of Sweet potato latent virus and Sweet potato chlorotic stunt virus Isolated from Sweet Potato in Korea

  • Infected sweet potato (Ipomoea batatas) showing symptoms of sunken veins, stunting, mosaic, and mottling were collected from Gimje, Cochang, Iksan, and Haenam provinces in Korea. Electron microscopic (EM) observation of the infected tissue revealed rod and filamentous rod type virus particles of various lengths. Western blot analysis of the protein samples extracted from infected sweet potato and partially purified virus identified the isolates as Sweet potato feathery motile virus (SPFMV), Sweet potato latent virus (SwPLV), and Sweet potato chlorotic stint virus (SPCSV). Sweet potatoes were occasionally infected with more than one of these viruses. This is the first report of SwPLV and SPCSV in Korea.

  • In 2003, a survey of sweet potato virus disease was carried out in seed boxes as well as in various sweet potato fields. Virus infection rate was $5\sim100%$ and 100% at seed boxes and fields, respectively. No relationship of the disease incidence and severity was observed between sweet potato cultivating areas and cultivars. A total of 179 samples were collected and analyzed based on serological, electron microscopic and molecular properties. Field-grown sweet potatoes were examined to inspect 8 different viruses using NCM-ELISA, resulting that 30% of sweet potato was infected by one virus, whereas 70% was by more than 2 viruses. However, RT-PCR using primers selected for seven viruses, such as Sweet potato feathery mottle virus (SPFMV) revealed that of one-hundred seventy-nine tested; 71 of SPFMV, 29 of SPGV, 19 of SPFMV+SPGV, 1 of SPFMV+SwPLV, 1 of SPFMV+SPLCV, 2 of SPFMV+SPGV+SwPLV, 6 of SPFMV+SPGV+SPLCV, 2 of SPFMV+SPGV+SwPLV+SPLCV and 48 of unknown viruses were identified from the field samples. In root, viral diseases were severer in Yeoju than in Mokpo Experiment Station and infection rate was much different depending on sweet potato cultivars.

  • The patients' demand for treatment of unesthetic anterior teeth is steadily growing. Accordingly, several treatment options such as porcelain laminate veneers(PLV) have been proposed to restore the esthetic appearance of the dentition. Porcelain laminate veneers are considered minimally invasive, but they also require removal of sound enamel. One critical step in the porcelain laminate veneer technique is the achievement of sufficient ceramic thickness, and several different strategies for tooth preparation can be found in the literature. This clinical report describes a step-by-step protocols for preparation of these restorations used with the silicone index from diagnostic wax-up and the acrylic resin mock-up. Conservative use of porcelain laminate veneers provided satisfactory esthetic outcomes and preserved sound tooth structure.

Partial Left Ventriculectomy as a Bridge to Cardiac Transplantation in a Patient of End-Stage Heart Failure -Case Report- (말기 심부전 환자에서 심장이식의 교량으로서 좌심실 축소술 - 중례보고 -)

  • A 40-years-old male with dilated cardiomyopathy(DCMP) and end-stage heart failure had undergone partial left ventriculectomy(PLV) in July 1997 and then underwent cardiac transplantation in January 1999. Three months later he showed increased ejection fraction (EF) from 26% to 42.6%, decreased left ventricular end diastolic diameter(LVEDD) from 71mm to 45mm, cardiac output(CO) 3.95 L/min and cardiac index(CI) 2.28 L/min/m$^2$echocardiographically. Eight months later, left ventriclular end diastolic and systolic diameters increased to 56 and 51 mm respectively and EF decreased to 17% in echocardiographic follow-up. He had been on maximum medication until he underwent cardiac transplantation 18 months after the PLV. Consecutive myocardial biopsies (1, 3 and 6 month later) showed ISHLT (international society of heart and lung transplantation) class la and the treatment for rejection was not needed until now on. We report a partial left ventriculectomy as a successful bridge to cardiac transplantation in a patient with DCMP and end-stage heart failure.

Esthetic improvement in patient with deep bite using porcelain laminate veneers and a full veneer crown (과개교합 환자에서 도재 라미네이트 비니어와 전부도재관을 이용한 심미수복 증례)

  • This case is a porcelain laminate veneer (PLV) and all-ceramic crown treatment case on a female patient in her thirties who visited for esthetic improvement. The patient's chief complaint was protrusive and uneven anterior teeth, and she wanted minimum tooth reduction. The patient was in skeletal class II deep bite. Considering this, check bite method was used for mounting on articulator during diagnostic wax-up. Tooth reduction was done using mock-up base on diagnostic wax-up, and the amount of deletion was checked by index. For the accordance of shade, PLVs were restored in prior to all ceramic crown. After using all-ceramic crown, satisfactory esthetic improvement was obtained.

  • 본 연구는 만성 뇌졸중 환자 5 명을 대상으로 상지 운동(Affected hand의 주먹 쥐기/펴기운동)시 참가자의 운동의지와 운동 수행의 유무에 따라 차이가 있을 것을 가정하고, 운동 수행 및 운동의지가 존재하는 Active movement와 운동 수행을 하지만 운동의지가 없는 Passive movement, 운동 수행은 없지만 운동의지가 있는 Motor Imagery(MI)의 세가지 task에 따른 뇌파의 연결성을 비교하고자 한다. 이 때 EEG 영역 간의 연결성을 보기 위한 분석 방식 중 하나인 Phase locking value(PLV)를 통해 각 task 간의 차이를 비교 및 분석했다. 운동 수행은 동일하지만 운동의지 유무에 따른 차이는 Passive movement가 전반적으로 뇌 영역간 연결이 감소하고 Active movement가 motor task 시작 후 375ms를 기점으로 급격히 증가함을 보이는 데에서 발견할 수 있었으며, 운동 수행 유무에 따른 차이는 687.5ms 이후 Active movement에 비해 MI에서 뇌 영역 간 연결 수가 확연히 감소하는 데에서 큰 차이를 나타내었다. 이에 따라 본 연구에서는 만성 뇌졸중 환자의 상지운동 시의 motor task에 따른 EEG 영역간의 연결성을 토대로 운동의지 검출이 가능성이 있음을 밝혔다.

Comparison of the degree of conversion of light-cured resin cement in regard to porcelain laminate thickness, light source and curing time using FT-IR (도재 라미네이트 두께와 광원 및 광조사 시간에 따른 광중합형 레진 시멘트의 FT-IR을 이용한 중합도 비교)

  • Statement of problem: The degree of light attenuation at the time of cementation of the PLV restoration depends on characteristics such as thickness, opacity and shade of the restorations, which interfere with light transmittance and, as a result, may decrease the total energy reaching the luting cement. Purpose: The purpose of this study was to compare the degree of conversion of light-cured resin cements measuring by FT-IR in regard to different thickness, light devices and curing time. Material and methods: In the control group, a clear slide glass (1.0 mm) was positioned between the light cured resin cement and light source. The specimens of ceramics were made with IPS Empress Esthetic. The ceramics were fabricated with varying thicknesses-0.5, 1.0, 1.5 mm with shade ETC1. Rely $X^{TM}$ Veneer with shade A3, light-cured resin cement, was used. Light-activation was conducted through the ceramic using a quartz tungsten halogen curing unit, a light emitting diode curing unit and a plasma arc curing unit. The degree of conversion of the light-cured resin cement was evaluated using FT-IR and OMNIC. One-way ANOVA and Tukey HSD test were used for statistical analysis ($\alpha$< .05). Results: The degree of conversion (DC) of photopolymerization using QTH and LED was higher than results of using PAC in the control group. After polymerization using QTH and LED, the DC results from the different ceramic thickness- 0.5 mm, 1.0 mm, 1.5 mm- did not show a significant difference when compared with those of control group. However, the DC for polymerization using PAC in the 1.5mm ceramic group showed significantly lower DC than those of the control group and 0.5 mm ceramic group (P<.05). At 80s and 160s, the DC of light-cured resin cement beneath 1.0 mm ceramic using LED was significantly higher than at 20s (P<.05). Conclusion: Within the limitation of this study, when adhering PLV to porcelain with a thickness between 0.5-1.5 mm, the use of PAC curing units were not considered however, light cured resin cements were effective when cured for over 40 seconds with QTH or LED curing units. Also, when curing the light cured resin cements with LED, the degree of polymerization was not proportional with the curing time. Curing exceeding a certain curing time, did not significantly affect the degree of polymerization.

Esthetic improvements through systematic diagnosis and treatment procedures in patients with unesthetic maxillary anterior teeth proportion after orthodontic treatment: Case report (교정치료 후 비심미적인 상악 전치부 비율을 가진 환자에서 체계적인 진단 및 치료과정을 통해 심미성이 개선된 보철 수복 증례)

  • The treatment of areas demanding esthetic requirements, such as maxillary anterior teeth, should take into account the achievement of a healthy, harmonious to the surrounding tissue, and an attractive smile line. In this case report, smile line, soft tissue and hard tissue morphology, and the anatomy and proportion of the tooth, must be considered. In patients with unesthetic maxillary anterior ratio due to inadequate gingival contour and diastema caused by peg lateralis, it would be challenging to achieve an esthetic restoration by orthodontic treatment alone. In such case, after orthodontic treatment, dento-gingivo-facial compositional diagnosis and analysis, followed by gingivectomy and prosthodontic restoration is needed to improve the interdental mesial/distal, width/length ratio to achieve a satisfactory esthetic result. In addition, when improving the tooth proportion of peg lateralis by prosthodontic treatment, Porcelain laminate veneer (PLV), which results in less tooth structure loss, reproduction of similar shade to that of the proximal tooth and high transparency, is recommended. This case report demonstrates esthetic improvements by prosthodontic restoration through systematic diagnosis and treatment procedure in patients with unesthetic maxillary anterior proportion after orthodontic treatment due to peg lateralis by means of two female patients aged twenty years old.
