Vitamin B12 Deficiency after a Total Gastrectomy in Patients with Gastric Cancer (위암으로 위 전절제술 후 비타민 Bl2 결핍과 치료)

  • Purpose: The most common metabolic defect appearing following a gastrectomy is anemia. Two types have been identified: One is related to a deficiency in iron and the other is related to an impairment in vitamin B12 metabolism. The purpose of this study is to evaluate the incidence and treatment of vitamin B12 deficiency after a total gastrectomy. Materials and Methods: Serum vitamin B12 concentrations were measured in 128 patients who had undergone a total gastrectomy. The group with a serum concentration under 200 pg/ml was supplemented at one-month intervals with Actinamide and five-six months later, serum concentrations of the vitamin B12 were rechecked. Results: The group with vitamin B12 under 200 pg/ml was 61 (47.6%) of the 128 patients who had undergone a total gastrectomy. In this group, the cumulative incidences of vitamin B12 deficiency were 7.0, 23.4, 33.6, 39.1, 41.4, and 47.7% at 6 months, 1, 2, 3, 4, and 5 or more years, respectively. The treated group with vitamin B12 under 200 pg/ml had 36 (28.17%) of the 128 patients. The 16 of those cases with vitamin B12 levels of $100{\sim}150\;pg/ml$ were supplemented 5.21 times and the vitamine B12 was elevated to above 650 pg/ml. The other 20 cases with an average of vitamin B12 levels of $150{\sim}200pg/ml$ were supplemented an average of 4.75 times, and the vitamin B12 was elevated to above 780 pg/ml. Conclusion: It is necessary to supplement vitamin B12 even 1 year later after a total gastrectomy. The group with vitamin B12 under 200 pg/ml was supplemented $5{\sim}6$ times at one-month intervals with Actinamide $1,000\;{\mu}g$ IM injections and reached normal levels. Effects of Vehicles and Enhancers on the Permeation Properties of Tizanidine Hydrochloride through Strat-MMTM Artificial Membrane and Hairless Mouse Skin (용제와 투과촉진제가 Strat-MTM 인공막 및 무모마우스 피부를 통한 티자니딘염산염의 투과 특성에 미치는 영향)

  • This study was aimed to enhance the percutaneous absorption of tizanidine hydrochloride (TZ) across Strat-M$^{TM}$ artificial membrane and excised hairless mouse skin using various vehicles and chemical permeation enhancers. Solubility studies were performed using hydrophilic and lipophilic vehicles. To initially evaluate vehicle effects on skin permeation, Strat-M$^{TM}$ membrane was adopted using Franz-type diffusion cells loaded with 0.4 mg donor dose. Effects of fatty acids on the permeation of TZ from PG and PGMC were compared, and the effects of various hydrophilic vehicles in the presence of linoleic acid were studied using excised hairless mouse skin specimens. The mean solubility (mg/ml) of TZ in hydrophilic vehicles was higher: water > PG > DMSO > ethanol > PEG 200 > NMP > PEG 300 > PEG 400 > DGME, and solubilities in lipophilic vehicles such as PGMC, PGMC, IPM, Captex 200 and Captex 300 were much less than 1.0 mg/ml. Permeation rates through StratTM membrane from pure vehicles were in the rank order: PGMC ${\geq}$ LBF > DMSO ${\geq}$ NMP ${\geq}$ PGML ${\geq}$ PG ${\geq}$ PEG 200 ${\geq}$ DGME ${\geq}$ EtOH. However, permeation rates of TZ through hairless mouse skin from pure vehicles were very low, although PG showed the highest flux ($1.66{\pm}0.28{\mu}g/cm^2{\cdot}hr$). Therefore, PG was selected in further studies. Addition of enhancers (3 v/v%) into PG markedly increased the flux (${\mu}g/cm^2{\cdot}hr$): oleyl alcohol ($14.9{\pm}3.1$) ${\geq}$ oleic acid ($14.5{\pm}1.6$) ${\geq}$ linoleic acid ($13.7{\pm}1.3$) > capric acid ($4.4{\pm}0.6$) > caprylic acid ($2.1{\pm}0.4$). Among hydrophilic vehicles with linoleic acid, PG and DMSO revealed relatively higher permeation for TZ. Increase of donor dose in PG resulted in dose-dependent permeation fluxes. These results suggest that permeation properties of TZ from nonaqueous solutions are markedly different between Strat-$M^{TM}$ membrane and excised hairless mouse skin, and transdermal delivery of TZ would be feasible with a combination of PG and enhancers.

주암호의 계절별 용존가스상 수은의 변화 및 특성

  • DGM은 상온에서 휘발성을 가지는 수은의 독특한 화학적 성질 때문에 쉽게 대기로 방출될 수 있고, 대기 중으로의 DGM 방출은 수체 내 수은의 유일한 제거 기작일 뿐만 아니라, 고정상으로 존재하던 수은이 다시 이동상의 수은으로 변화하는 중요한 과정이다. 본 연구는 2005년 8월과 10월에 주암호를 대상으로 실시간 자동분석 장치(Tekran 2537A)를 이용하여 DGM 농도변화를 관찰하였다. 전반적인 주암호(n=23)의 여름철 평균 BGM 농도는 110 ${\pm}$ 21 pg $L^{-1}$으로 기존에 발표된 국외 호수에서의 DGM 농도보다 약 3배정도 높았다. 강우에 따른(강우량 : 0.4 mm) DGM 농도 변화는 비가오기 전 89 pg $L^{-1}$ 에서 비온 뒤 89 pg $L^{-1}$ 으로 약 9%가 증가했다. 그러나 가을철 평균 DGM 농도(n=21)는 20 ${\pm}$ 0.4pg $L^{-1}$ 로 여름철보다 약 5.5배 감소했고, 비가 내리는 날씨에서의 DGM 농도는 13.7 pg $L^{-1}$ 로 맑은 날 같은 시간에 비해(32 pg $L^{-1}$) 약 58% 감소했다.

Isolation of Intestinal Glucose Uptake Inhibitor from Punica granatum L.

  • Inhibition of intestinal glucose uptake is beneficial in reducing the blood glucose level for diabetes. To search for an effective intestinal glucose uptake inhibitor from natural sources, 70 native edible plants, fruits and vegetables were screened using Caco-2 cells and fluorescent D-glucose analog 2-[N-(7-nitrobenz-2-oxa-1,3-diazol-4-yl) amino]-2-deoxy-D-glucose (2-NBDG). A compound that was able to inhibit glucose uptake was isolated from methanol extract of Punica granatum L. and called PG-1a. PG-1a appears to be a phthalic acid-diisononyl ester- like compound (PDE) with molecular weight of 418. The inhibitory effect of PG-1a on intestinal glucose uptake was dose-dependent with 89% inhibition at $100\;{\mu}g$/mL. Furthermore, the intestinal glucose uptake inhibitory effect of PG-1a was 1.2-fold higher than phlorizin, a well known glucose uptake inhibitor. This study suggests that PG-1a could play a role in controlling the dietary glucose absorption, and that PG-1a can effectively improve the diabetic condition, and may be used as an optional therapeutic and preventive agent.

Comparison of Regional Differences of PCBs Concentration Using Pine Needles and Soil (지역별 소나무잎과 토양에 침착된 PCBs 농도 비교)

  • This study was conducted to measure the concentration of PCBs in pine needles and soil in urban (Seoul, many artificial sources of PCBs), semi-rural (Anseong, small town located below Seoul in wind direction) and rural areas (Jincheon, rarely artificial sources of PCBs) in which the artificial production amount of PCBs are different. The total PCBs concentrations in pine needles, which did not show big difference in three sampling sites, were 107.5 pg/g (urban), 94.8 pg/g (semi-rural) and 78.8 pg/g (rural) respectively. The low chlorinated PCBs were major component in pine needles and the PCBs congener concentration profile of each sampling area were similar each other, and the octanol-air partitioning coefficient, Koa, highly correlated with the PCBs concentrations in pine needles. The total PCBs concentrations in soil which did show big difference in three sampling sites, were 830.0 pg/g (urban), 314.1 pg/g (semi-rural) and 136.5 pg/g (rural) respectively. The high chlorinated PCBs were major component in soil and the PCBs congener concentration profile of each sampling area were similar each other. There was no similarity between the PCBs concentration of pine needles and those of soil at each site, because of the different mechanism of deposition and volatilization processes of PCBs. The total PCBs concentrations of 2009 became 12.9 times lower than those of 2001. The reduce rate of PCB 28 was the greatest.

Molecular characterization and biological changes caused by Agrobacterium-mediated infiltration of PgTRX1

  • In order to test the functionality of Panax ginseng thioredoxin 1 (PgTRX1) isolated from fermented wild ginseng roots, a transient effect on physiological activity were performed over a short time frame using the Agrobacterium infiltration technique. The PgTRX1 gene isolated from fermented wild ginseng was confirmed to have a size of 579 bp, and the expression of PgTRX1 was the highest in the sample after 6 h of fermentation. As a result of constructing this gene and confirming the infiltration reaction mediated by Agrobacterium in tobacco leaves, it was found that the expression of the NbHSR203j gene was also induced as PgTRX1 expression increased. As a result of measuring the biological activity of the infiltration samples, the total phenol content increased by 35.45±1.84 to 49.01±1.84 ㎍ GAE/mL compared to the control, and the total flavonoid amount of 9.52±0.41 to 9.82±0.25 ㎍ QE/mL was slightly high. From these results, Agrobacterium-mediated PgTRX1 appears to be related to the hypersensitive response induction mechanism of plants and the production of secondary metabolites such as phenolic substances.

Sex Morph, Fruiting Habit, and Seed Viability of Acer palmatum (단풍나무의 성 형태, 결실특성 및 종자활력)

  • To examine the sex morph, heterodichogamy, fruiting characteristics and seed viabilities of Acer palmatum, sixty three planted trees were monitered in Wonju-si for two years in terms of their sex morph, flowering and fruiting habit. Most flowers of A. palmatum are pollinated by Apis mellifera, Helophilus virgatus, a few (3.57%) are pollinated by wind. Twenty four trees (42.9%) in 2010, and twenty trees (31.7%) in 2011 were proved protandrous (PA: pollen is shed before stigmas are receptive), thirty two (50.8%) in 2010, thirty trees (47.6%) in 2011 were proved protogynous (PG: stigmas are receptive before pollen is shed). This result means that Acer palmatum has heterodichogamous sexual system. Five trees (7.9%) have changed their sex morphs; 4 trees change the sex morp PA to PG, and one does the sex morph PG to PA. Rates of sound seeds and decayed seeds were significantly different between sex morphs. Rates of sound seeds were 51.6% (2010) and 64.0% (2011) in PA, but 30.4% (2010) and 40.4% (2011) in PG. and rates of decayed seeds did 40.0% (2010) and 31.2% (2011) in PA, but 65.6% (2010) and 56.8% (2011) in PG. The differences of decayed seeds' rates between sex morphs might be related with the timing of insect pests' outbreak.

  • Lead has been determined by Resonance Ionization Mass Spectrometry (RIMS) through one-color-two-photon ionization, two-color-two-photon ionization and three-color-three-photon ionization in a vacuum chamber equipped with Time-of-Flight(TOF) mass spectrometer. In all cases, the first excited state chosen was 6p7s($^3P_1$) state and the transition was at 283.3 nm in wavelength from the ground state. By using various concentrations of lead standard solutions, the calibration curve is obtained in the range of 0.1 ${\mu}g$ to 1.0 pg in both ionization schemes. The detection limit was estimated as 20 pg for the two-color ionization, while 10 pg for the three-color ionization experiment.

Screening for the 3' UTR Polymorphism of the PXR Gene in South Indian Breast Cancer Patients and its Potential role in Pharmacogenomics

  • Background: Breast cancer, the commonest cancer among women in the world, ranks top in India with an incidence rate of 1,45,000 new cases and mortality rate of 70,000 women every year. Chemotherapy outcome for breast cancer is hampered due to poor response and irreversible dose-dependent cardiotoxicity which is determined by genetic variations in drug metabolizing enzymes and transporters. Pregnane X receptor (PXR), a member of the nuclear receptor superfamily, induces expression of drug metabolizing enzymes (DMEs) and transporters leading to regulation of xenobiotic metabolism. Materials and Methods: A genomic region spanning PXR 3' UTR was amplified and sequenced using genomic DNA isolated from 96 South Indian breast cancer patients. Genetic variants observed in our study subjects were queried in miRSNP to establish SNPs that alter miRNA binding sites in PXR 3' UTR. In addition, enrichment analysis was carried out to understand the network of miRNAs and PXR in drug metabolism using DIANA miRpath and miRwalk pathway prediction tools. Results: In this study, we identified SNPs rs3732359, rs3732360, rs1054190, rs1054191 and rs6438550 in the PXR 3; UTR region. The SNPs rs3732360, rs1054190 and rs1054191 were located in the binding site of miR-500a-3p, miR-532-3p and miR-374a-3p resulting in the altered PXR level due to the deregulation of post-transcriptional control and this leads to poor treatment response and toxicity. Conclusions: Genetic variants identified in PXR 3' UTR and their effects on PXR levels through post-transcriptional regulation provide a genetic basis for interindividual variability in treatment response and toxicity associated with chemotherapy.

Change in Plasma Vascular Endothelial Growth Factor after Gamma Knife Radiosurgery for Meningioma : A Preliminary Study

  • Objective : The purpose of this study was to investigate changes in the plasma level of vascular endothelial growth factor (VEGF) after Gamma Knife radiosurgery (GKRS) for the treatment of meningioma. Methods : Fourteen patients with meningiomas had peripheral venous blood collected at the time of GKRS and at 1 week, 1 month, 3 month and 6 month visits. Plasma VEGF levels were measured using commercially available enzyme-linked immunosorbent assay. For controls, peripheral blood samples were obtained from 20 healthy volunteers. Results : The mean plasma VEGF level (29.6 pg/mL) in patients with meningiomas before GKRS was significantly lower than that of the control group (62.4 pg/mL, p=0.019). At 1 week after GKRS, the mean plasma VEGF levels decreased to 23.4 pg/mL, and dropped to 13.9 pg/mL at 1 month, 14.8 pg/mL at 3 months, then increased to 27.7 pg/mL at 6 months. Two patients (14.3%) with peritumoral edema (PTE) showed a level of VEGF 6 months after GKRS higher than their preradiosurgical level. There was no significant association found in an analysis of correlation between PTE and tumor size, marginal dose, age, and sex. Conclusion : Our study is first in demonstrating changes of plasma VEGF after stereotactic radiosurgery (SRS) for meningioma. This study may provide a stimulus for more work related to whether measurement of plasma level has a correlation with tumor response after SRS for meningioma.
