Management of Acute Gastroenteritis in Children: A Survey among Members of the Korean Society of Pediatric Gastroenterology, Hepatology, and Nutrition

  • Purpose: No national survey has yet described the guidelines followed by Korean pediatricians to treat acute gastroenteritis (AGE). An online survey was performed to investigate the management of AGE followed by members of The Korean Society of Pediatric Gastroenterology, Hepatology, and Nutrition, and the results were compared between pediatric gastroenterologists (PG) and general pediatricians (GP). Methods: Questionnaires were sent to pediatricians between June 2 and 4, 2018 regarding the type of hospital, indications for admission, antiemetic and antidiarrheal drugs and antibiotics prescribed, and dietary changes advised. Results: Among the 400 pediatricians approached, 141 pediatricians (35.3%) responded to the survey. PG comprised 39% of the respondents and 72.7% worked at a tertiary hospital. Both PG and GP considered diarrhea or vomiting to be the primary symptom. The most common indication for hospitalization was severe dehydration (98.8%). Most pediatricians managed dehydration with intravenous fluid infusions (PG 98.2%, GP 92.9%). Antiemetics were prescribed by 87.3% of PG and 96.6% of GP. Probiotics to manage diarrhea were prescribed by 89.1% of PG and 100.0% of GP. Antibiotics were used in children with blood in diarrheal stool or high fever. Dietary changes were more commonly recommended by GP (59.3%) than by PG (27.3%) (p<0.05). Tests to identify etiological agents were performed primarily in hospitalized children. Conclusion: This survey assessing the management of pediatric AGE showed that the indications for admission and rehydration were similar between GP and PG. Drug prescriptions for diarrhea and dietary changes were slightly commonly recommended by GP than by PG.

필라테스 운동이 여중생의 요통과 체간 등속성 근력 및 평형성에 미치는 영향 (Effects of Pilates Exercises on Isokinetic Trunk Strength and Balance in Female Middle School Students with Lumbago)

  • Purpose: The purpose of this research was to investigate the effects of performing Pilates exercises for eight weeks on the isokinetic trunk strength and balance of female middle school students with lumbago. Methods: Twenty-four female students who met the test requirements were recruited and randomly divided into a control group (CG, n = 16) and a Pilates exercise group (Pilates group; PG, n = 8). The PG performed a Pilates program, which consisted of lumbar muscle strengthening exercises, for 60 minutes three times a week for eight weeks. Data analysis was conducted by two-way repeated ANOVA, and a Bonferroni test was carried out when significant differences appeared. The alpha level was set at 0.05. Results: Following the experimental treatment, the PG showed an increased trunk flexion peak torque of 17% and 13% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased about 31% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Following the experimental treatment, the PG showed an increased trunk extensor peak torque of approximately 29% and 21% at angular velocities of 180°/sec and 300°/sec, respectively. The intergroup comparison showed no significant difference at pre-measurement, while the PG increased approximately 27% (180°/sec) and 15% (300°/sec) higher than the control group at post-measurement. Both the CG and the PG showed a 20% and 50% decrease in pain index after the experimental treatment, respectively. After the experimental treatment, the PG had about 50% lower pain than the CG. The average error rate of static balance in the PG reduced by 19% from 3.28±0.45 to 2.65±0.36, confirming a significant improvement. Conclusion: Regular Pilates exercise for eight weeks improved the isokinetic trunk muscle strength of female middle school students with lumbago and relieved their pain. Pilates was also shown to be effective in improving balance.

A Combination of PG490 and Lipopolysaccharide Induce Apoptosis through Activation of Casapase-3 and Downregulation of cIAP1 and XIAP in Human Astroglioma Cell

  • Background: Malignant gliomas are the most common primary tumors in the central nervous system. Methods: We investigated the combined effect of PG490 and LPS on the induction of the apoptotic pathway in human astroglioma cells. Results: Treatment of U87 cells with combination of 50nM of PG490 and $50{\mu}g/ml$ of LPS resulted in increased internucleosomal DNA fragmentation, cleavage of PLC-${\gamma}1$, and downregulation of cIAP1 and XIAP. The combination of LPS and PG490 treatment-induced apoptosis is mediated through the activation of caspase, which is inhibited by the caspase inhibitor, z-VAD-fmk. Also, release of cytochrome c was found in PG490 and LPS-cotreated U87 cell. Conclusion: Taken together, combination of PG490 and LPS appears to be a potent inducer of apoptosis in astrogliaoma cells, and might have some benefit in the treatment of glioma patients.

  • In 1990, Tolerable Daily Intake (TDI) of 10 pg TCDD/kg/day for dioxins based on carcinogenicity and reproductive toxicity was determined by WHO/EURO, that resulted in the establishment of TDIs in other countries. In Japan, Ministry of Health and Welfare and Environment Agency, respectively established the TDI of 10 pg TCDD/kg/day and Health Risk Assessment Index of 5 pg TCDD/kg/day in 1996. Accumulation of new scientific data, especially by molecular toxicology since 1990, resulted in the reevaluation of TDI by WHO-ECEH and IPCS in May, 1998. At this meeting, it was stressed that \circled1 toxic effects of dioxin is mediated through Ah-receptor in both animals and humans, \circled2 use of ebody burdeni concept is better than the use of traditional NOAEL/UF approach, \circled3 inclusion of coplanar PCBs in the TDI by the use of new WHO-TEF. LOAELs (0.16~200 ng TCDD/kg/day) obtained from reproductive toxicity and immunotoxicity in rats, and neurobehavioral toxicity and induction of endometriosis in rhesus monkeys are calculated to be the body burden of 10~50 ng TCDD/kg that is 14~37 pg TEQ/kg/day as human daily intake. Finally TDI of 1~4 pg TEQ/kg/day was established by applying the UF of 10. In Japan, reproductive toxicity and immunotoxicity in rats were used to obtain LOAELs (100~200 ng TCDD/kg/day). Finally TDI of 4 pg TEQ/kg/day was established in June 1999 by applying the UF of 10 to human daily intake of 43.6 pg TEQ/kg/day which corresponds to the body burden of 86 ng TCDD/kg.

동결정액과 PG600의 이용이 이유종빈돈의 번식능력에 미치는 영향 (Reproductive Performance in Post-weaning Sows Using PG600 and Inseminated with Frozen Semen)

  • This study was carried out to investigate the fertility and farrowing date in post-weaning sows using PG600 and inseminated with frozen semen. A total of 48 sows of Landrace, Large White and Duroc after 7-week lactation were used at the Chungnam Provincial Animal Breeding Station. The results obtained were summarized as follows: 1. Motility had no significant differences between the breeds, but NAR acrosome was highest in Landrace, followed by Duroc and Large White(p<.01). 2. Interval from weaning to estrus and length of estrus were, respectively, 3.7 days and 52.6hours of sows treated with PG600, and 6.5 days and 53.8 hours for control sows. The average interval from weaning to onset of estrus was significantly(p<.01) shorter by 2.8 days in PG600 treated sows compared to control sows. 3. In Landrace, Duroc and Large White, farrowing rate and number of pigs born alive per litter were 55.0%, 10.0; 43.8%, 8.1; and 16.7%, 3.5, respectively. Average pig weight at birth and survival rate at 56 days had no significant differences between the breeds. 4. Farrowing rate, number of pigs born alive per litter, average pig weight at birth and survival rate at 56 days were, respectivey, 45.8%, 101, 1.56kg and 94.5% for sows treated with PG600, and 37.5%, 7.0, 1.66kg and 93.8% for control sows. Sows treated with PG 600 had an average of 3.1 more pigs at farrowing compared to control sows.

The Therapeutic Effect of Tissue Cultured Root of Wild Panax ginseng C.A. Mayer on Spermatogenetic Disorder

  • This study examined the possibility of using a tissue cultured root of wild Panax ginseng (tcwPG) as a fertility agent. The effect of tcwPG on spermatogenesis was studied using male rats. The tcwPG crude powder was administered orally to 7-week-old rats over a 6-week period. The number of sperm in the testes and epididymides was significantly higher than the control. A histological examination did not reveal any morphological changes in the testes from the tcwPG powder treated rats. Moreover, there were no significant differences in the weights of the heart, spleen, liver, kidney, brain, testes and epididymides. Oligospermia was also induced by administering 2,3,7,8-tetrachlorodaibenzo-p-dioxin (TCDD) to the rats in order to estimate the feasibility of using tcwPG as treatment for infertility caused by spermatogenic disorders. After exposing the rats to TCDD, the tcwPG saponin fraction treated rats showed some improvement in the body weight, sperm number and testis morphology. It was estimated that tcwPG had feasibility as a therapeutic agent on spermatogenic disorder.

Utilization of a Scored Patient-Generated Subjective Global Assessment in Detecting a Malnourished Status in Gynecologic Cancer Patients

  • Purpose: To assess the prevalence of malnutrition in gynecologic cancer patients using the Scored Patient-Generated Subjective Global Assessment (PG-SGA) questionnaire. Materials and Methods: A total of 97 gynecologic cancer patients who never had any treatment but were planned for surgery were enrolled. The patients were asked to complete the scored PG-SGA form before the treatment was started. Attending physicians were also asked to complete other information in the PG-SGA form. Total scores were calculated and the patients were classified into 3 nutritional status levels. Results: Mean age was 54 years. Postoperative diagnoses were endometrial cancer in 42 cases (43.2%), ovarian cancer in 29 cases (29.9%), and cervical cancer in 26 cases (26.8%). Mean PG-SGA score was 5.2+4.7. Malnutrition (PG-SGA B and C) was found in 52 patients (53.6%, 95% CI 43.7% - 63.2%). Preoperative BMI, hemoglobin, serum albumin, and cancer stage were not significantly associated with nutritional status. Malnutrition was significantly more common among patients diagnosed with ovarian cancer, compared to other types of cancer (79.3% vs. 42.6%, p 0.004). Conclusions: Prevalence of malnutrition among gynecologic cancer patients was 53.5%, according to the scored PG-SGA. Malnutrition was significantly more common among patients with ovarian cancer.

인삼 및 틀의 초 saponin분획의 항당뇨 및 항고지혈 효과 (Hypoglycemic and Hypolipidemic Effects of Crude Saponin Fractions from Panax ginseng and Gynostemma pentaphyllum)

  • Crude saponin fractions were isolated using non-ionic resin chromatography from Ginseng Radix Alba (PG) and Gynostemmae Herba+ (GP). These saponin fractions were orally administered to strep- tozotocin (STZ)-induced diabetic rats for 2 weeks and to high-fat diet-induced obese rats for 4 weeks. Treatment with either PG saponin or GP saponin at a dose of 1 mg/kg significantly decreased the plasma glucose level to that of glibenclamide treated or normal groups. The increased plasma triglyceride (TG) level in diabetic rats was decreased by 50% with PG or GP saponin treatment. Combined administration of PG and GP saponins with different ratios (total dose of 1 mg/kg) also had the similar effects on the blood glucose and TG levels with that of PG or GP alone. Treatments with GP (1 mg/kg) or GP (0.5 mg/kg) and PG (0.5 mg/kg) together significantly suppressed the rise in TG levels induced by high-fat diet whereas slightly suppressed the rise in the total cholesterol level. The body weight gain was also decreased both in the two saponin treated groups. These results demonstrate that either alone or mixture of PG and GP have similar degree of effects on hyperglycemia and hyperlipidemia.

경기육괴 중부에 분포하는 대보 흑운모화강암류의 응용암석학적 연구 (Applied Petrologic Study of the Daebo Biotite Granites in the mid Gyeonggi Massif)

  • 경기육괴 중부인 포천지역과 양주지역에는 국내 주요 암석자원의 하나인 대보 흑운모화강암이 각각 분포한다. 이 연구의 주 목적은 상기한 두 지역의 흑운모화강암류의 입도, 암색, 광물조성, 물성 및 열극체계 등의 고유한 특성을 밝히는 데 있다. 포천지역의 흑운모화강암(PG)는 조립질과 담회색, 양주지역의 흑운모화강암(YG)은 중립질-조립질과 회색-담회색으로 대표된다. 구성광물의 모우드에서 주성분인 석영+알칼리장석+사장석은 PG에서, 부성분인 흑운모는 YG에서 증가하며, 이러한 광물함유량 차이로 인하여 PG가 YG보다 밝은 회백색을 띠게 된다. 물성 변화 및 특성에서, 비중은 PG보다 YG에서 약간 증가하며 이러한 차이는 전자보다 후자에서 주성분 광물에 비해 높은 비중을 갖는 흑운모의 함량 증가로 볼 수 있다. PG와 YG의 흡수율과 공극률은 0.33%와 0.86%를 가져 동일한 값을 각각 보여준다. 물성간의 상관도에서, 공극률 대 비중은 완만한 부의 경향, 공극률 대 흡수율은 뚜렷한 정의 경향을 보여준다. 압축 및 인장강도는 PG가 1,775 $kg/cm^2$ 와 87 $kg/cm^2$ 그리고 YG가 1,647 $kg/cm^2$와 79 $kg/cm^2$를 각각 가져 모두 PG에서 증가한다. 이러한 강도특성은 내재하는 암석조직에 기인될 수 있다. 마모경도는 PG에서 약간 증가하며, 이러한 마모특성은 다른 주성분 광물에 비하여 보다 높은 경도를 갖는 석영의 함량증가에 기인될 수 있다. 한편 포천 및 양주지역에서 발달하는 단열 조의 방향성에 대하여 국내 중생대 화강암류에서 발달하는 수직 결의 방향성과 비교하였다. 중첩도를 통하여, 단열 조와 상기한 수직 결 사이의 분포형태가 부합된다는 사실은 국내의 중생대 화강암류에 내재하는 미세균열 계는 경기육괴 중부의 대보 흑운모화강암류에서도 발달함을 시사한다. 상기한 두 지역간의 단열형태 특성을 보여주는 도면에서, PG에서는 규격석 그리고 YG에서는 비규격석이 보다 많이 산출될 잠재성이 있다.

급성호흡곤란의 감별진단에서 혈장 B-type Natriuretic Peptide의 역할 (The Role of Plasma B-type Natriuretic Peptide Measurements in the Differential Diagnosis of Acute Dyspnea)

  • 배 경 : 급성호흡곤란으로 응급실에 내원한 환자에서 그 원인질환을 감별하는 것은 효율적인 치료에 매우 중요하나 임상양상만으로 감별진단하기는 어려운 경우가 많다. 최근에 쉽게 측정하고 확인이 가능한 생화학적 표지자인 B-type natriuretic peptide에 대해 연구가 이루어지고 있다. 방 법 : 2004년 3월부터 7월까지 한양대학교병원 응급실로 급성호흡곤란을 주소로 내원한 261명의 환자를 대상으로 혈청 BNP를 측정하였고 임상적 소견을 후향적으로 분석하였다. 결 과 : 대상환자를 증상의 원인에 따라 크게 심부전군(n=119, 382, 113-1230 pg/ml)과 비심부전군(n=142, 29, 7-81 pg/ml)으로 나누었고(사람수, 중위수, 백분위수 25-백분위수 75), 하위집단으로 심부전군은 COPD와 좌심부전이 동반된 군(n=5, 820, 354-1620 pg/ml), 폐성심군(n=3, 1650, 239-1990 pg/ml), 좌심부전군(n=111, 378, 106-1120 pg/ml)으로, 비심부전군은 COPD군(n=20, 39, 21-101 pg/ml), 기타 호흡기질환군(n=56, 59, 10-129 pg/ml), 기타 다른 원인군(n=66, 15, 6-47 pg/ml)으로 분류하였다. 혈장 BNP는 원인 질환에 따라 유의한 차이를 보였으며(p<0.001), COPD군과 좌심부전이 동반된 COPD군을 비교했을 때 심부전이 동반될 때 혈장 BNP가 의미있게 높았다(p=0.002). 당뇨병, 고혈압, 흡연 및 신부전 등 심질환의 위험인자가 없는 환자들에서 심부전의 동반 유무와 BNP값을 비교해 보았을 때, 심부전이 동반된 경우 의미있게 BNP값이 증가되어 있었다(p<0.001). 또한, BNP가 133 pg/ml이상일 경우 예민도 0.73, 특이도 0.87로 심부전에 대한 치료에 반응할 수 있는 환자군을 감별할 수 있었다. 결 론 : 혈장 BNP는 급성호흡곤란의 감별진단에서 심부전의 유무를 알아내고 효율적인 치료를 앞당기는데 유용한 검사이다. 특히, 기존의 호흡기 질환을 가지고 있거나 심장질환의 위험요인이 없더라도 BNP가 높은 수치를 보일 경우에는 심부전의 가능성을 고려해서 환자에게 접근해야 한다.
