A Study for Improvement of Nursing Service Administration (병원 간호행정 개선을 위한 연구)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.

A Study on the Water Systems in the Rural Districts of Korea (농촌 생활용수에 관한 조사 연구)

  • This study was performed to estimate the daily water consumption for watering livestock and home use, and to organize the water systems in the rural districts of Korea. For these purposes, 560 farms and other 40 urban families were randomly chosen and investigated about their water systems and daily water consumption seasonally from July 20 to August 31, 1973, and other 82 livestock raisers' useful data for watering livestock helped the study to assess the consumption of livestock water and compare it to that of general farms which bred some domestic animals as their sidelines. The results of data showed that the daily consumption of water was varied with the difference of districts and seasons in which the investigations were conducted. The results were summarized as follows. 1. The mean consumption of water for home use was varied with the range of 30.2-48.7 lit./day in summer and 22.4-45.2 lit./day per a person, respectively, in the rural districts, which showed that the water consumption in summer was more than in autumn. 2. The mean consumption of livestock water in the general farms approached to about 15.8-37.1 lit./day per cattle, 3.87-16.45 lit./day per a pig, and 0.19-1.03 lit./day per chicken, but it was slightly reduced in autumn; 10.89-37.09 lit./day per cattle, 3.87-12.63 lit./day per a pig, and 0.06-0.94 lit./day per chicken. The ratio of livestock water consumption to home use amounted to the range of 10-25%. 3. The mean consumption of livestock water used at the livestock raisers amounted to 134 lit./day per dairy cow, 67.4 lit./day per cattle, 43.29 lit./day per a horse, 13.24 lit./day per a pig, 0.438 lit./day per a layer, and 0.177 lit./day per a broiler, which showed that the daily water consumption for larger livestock was about two times as much as the farms, but it was approximately of the same for small ones. 4. The factors affecting the amount of water consumption for home use were statistically analyzed to find the kind of water sources and the class of water systems by the factorial arrangement method to the fundamental experimental unit, Suwon area of 100 of families, in which the former was very significant while the latter was significant. However, the standard of living and their interactions were not significant. 5. Almost 83.24% of the sampled farms used some kinds of wells as their water sources, of which 32.06% was of hard water, and of which 11.l% or 2.6% contained much of iron or organice materials, respectively, examining them by a simple technique of sensibility, 6. The resulting chart of the farm water systems was summarized as Fig. IV-6. Observating the safe distance from contamination sources, the ratio of 42.4% of sampled farms whose water sources were wells, confronted highly to the danger of contamination to their water sources. And other dangerous sources of contamination such as toilets, stalls, and compost heaps were situated nearly to the water sources, and most of them were able to make their spoiled water infiltrated into the ground. Thereafter, it was recommended to develop some protecting methods and to install some suitable faculties to clean water.

An Analysis of Small Area Variations of Hospital Services Utilization in Korea (지역간 입원 이용 변이에 관한 연구)

  • This study was conducted to investigate whether variations in hospital services utilization across small geographic areas in Korea existed, and if so, what factors are responsible for the variation. The claims data of the fiscal year 1992 obtained from the regional health insurance societies were used for the study. Main findings of the research can be summarized as follows: 1 Extremal Quotients (EQ) of hospital expenditure per capita and hospital days per capita were 2.69 and 2.73, and Coefficient of Variation (CV) were 0.14, both, respectively. The EQ and the CV of admission rate were also 2.71, 0.15. The EQ and the CV of expenditure per admission were 1.73, 0.10 and those of hospital days per admission were 1.29, 0.06. All these statistics were statistically significant and this result provides strong evidence for the existence of small area variations. 2. Comparing patterns of variation among areas, the area which showed higher utilization amounts is Chansungp'o. Koje area, whereas the areas which showed lower utilization amounts are Yongju, Changhung, Miryang, Mokp'o, Koch'ang area. 3. Multivariate analytic methods were used to examine factors related to the variation across areas. In terms of the health resource availability variables, beds per capita or physicians per capita were positively associated with all utilization indices. As for the health service market structure variables, the proportion of health care institutions operating for less than f years was positively related to the expenditure per capita, hospital days per capita and expenditure per admission. In addition the proportion of the private health care institutions also had a negative relationship with total utilization amount and admission rate and the proportion of physicians under age 40 was negatively associated with expenditure per capita and expenditure per admission. With regard to the socio-demographic characteristics, proportion of medicaid population was positively related to hospital days per capita, and percentage of paved road was positively related to hospital days per admission. As a conclusion, wide variations existed across small areas in Korea and supply factors were found to be important in explaining the variation.

Statistical Analysis of Stillbirths in Different Genotypes of Sows

  • Statistical analysis was conducted on sow stillbirth traits of three genotypes with 2,400 litters including the Erhualian, Large White and the $F_1$ cross of these two breeds. Number of stillborn piglets per litter in the Erhualian, Large White and the $F_1$ averaged 0.85, 0.31 and 0.70, and percentage born alive averaged 95.0%, 97.0% and 95.5%, respectively. Erhualian sows with a greater litter size also had a higher stillbirth rate. Results of analysis of variance indicated that genotype, parity, farrowing year${\times}$farrowing season interaction and total number born had highly significant effects on both number of stillborn piglets per litter and percentage born alive in sows (p<0.0001). Farrowing year had no significant effect on number of stillborn piglets per litter (p>0.05), and highly significant effect on percentage born alive (p<0.01). Farrowing season had highly significant effects on both number of stillborn piglets per litter and percentage born alive (p<0.01). From parity one to parity ten, least squares means for number of stillborn piglets per litter progressively increased with increasing parity and least squares means for percentage born alive progressively decreased with increasing parity. Sows that farrowed in winter had the highest number of stillborn piglets per litter and the lowest percentage born alive, sows that farrowed in autumn had the lowest number of stillborn piglets per litter and the highest percentage born alive. With increasing total number born, least squares means for number of stillborn piglets per litter markedly increased and least squares means for percentage born alive markedly decreased. Results from analysis of paternal half sibs indicated that the heritabilities for number of stillborn piglets per litter and percentage born alive were 0.110 and 0.124, and the genetic, phenotypic and environmental correlations between them were -0.989, -0.951 and -0.948, respectively. These results indicated that number of stillborn piglets per litter and percentage born alive were traits with the similar genetic background.

CLINICO-RADIOLOGICAL STUDY OF RADICULAR CYST OF THE JAW (악골 치근단 낭종의 임상 방사선학적 연구)

  • The author analyzed clinically and radiologically 319 radicular cysts which were proven histologically in Infirmary College of Dentistry Yonsei University and Seoul National University Hospital from 1965 to 1983, and which occurred in 167 males and 139 females between the ages of 6 and 70. This analysis was undertaken to discover the frequency and distribution of the radicular cysts with regard to the age and sex, the chief complaints of the patients, the regions, the radiographical appearances, the tooth changes, and the anatomical structures involved by the lesions. The obtained results were as follows: 1. The most frequently involved age group for the radicular cysts was the third decade of the patients, and the greatest number 69.9 per cent occurred between the ages of 11 and 40 years. The statistical analysis did not show any significant difference between male and female (P>0.05) 2. The frequently involved region for the radicular cysts was orderly the maxillary anterior region (44.9 per cent), the mandibular anterior region (15.9 per cent), the maxillary premolar region (14.8 per cent), and the mandibular premolar region (11.0 per cent). The maxillo-mandibular ratio of the radicular cysts was about 7:3. The statistical analysis did not show any significant difference between right and left sides (P>0.05). 3. The greatest number was 92.2 per cent, which were involved in apical portions of the radicular cysts, 88.7 per cent of the radicular cysts were found in the round shapes which were oval and elliptical, and 66.8 per cent of the radicular cysts were observed with the completely hyperostotic borders. 4. The radicular cysts caused the root resorption in 55.2 per cent and the migration of the involved tooth or teeth in 23.5 per cent respectively.

Theoretical Design for the Production of Quinoa (Chenopodium quinoa Willd.) in a Closed Plant Factory

  • Quinoa (Chenopodium quinoa Willd.) is a grain crop with high nutritional value. The leaves and sprouts of quinoa can also be consumed either raw or cooked, providing considerably nutritional value as well as high antioxidant and anticancer activities. This study was carried out to obtain basic data to assist in the practical design of a plant factory with artificial lighting for the cultivation of quinoa as a leafy vegetable. We estimated the energy content of the quinoa and the electrical energy required to produce this crop. The yield was 1,000 plants per day, with a planting density and light intensity of $0.015m^2$ ($15{\times}10cm$) and $200{\mu}mol{\cdot}m^{-2}{\cdot}s^{-1}$, respectively. The total number of plants, cultivation area, and electricity consumption were estimated to be 25,000, $375m^2$, and $93,750{\mu}mol{\cdot}s^{-1}$, respectively. White fluorescent lamps were used at a power of 20.4 kW from 1,857 fluorescent lamps (FL, 55 W), and the cost for electricity was approximately 1,820 dollars (exchange rate of $1 = 1,200 won) per month. For a daily harvest of 1,000 plants per day in a closed plant factory, the estimated light installation cost, total installation cost, and total production cost would be 15,473, 46,421, and 55,704 dollars, respectively. The calculated production cost per plant, including labor costs, would be 27 cents for the 25-day cultivation period, with a marketable ratio of 80%. Considering the annual total expenses, income, and depreciation costs, the selling price per plant was estimated to be approximately 56 cents.

Relationship between Nutrient Intake Ratio and Sedentary Time of Female Adults by Dehydration Estimated with Blood Urea Nitrogen to Creatinine Ratio: Based on the 2016~2018 Korea National Health and Nutrition Examination Survey (혈중 요소질소와 크레아티닌 비율에 근거한 성인 여자의 탈수 여부에 따른 영양소 섭취 정도와 앉아서 보내는 시간과의 관련성: 2016~2018년 국민건강영양조사를 이용하여)

  • There is a dearth of Korean studies on dehydration, one of the health risks for adult women. This study analyzed the subjects of the 2016~2018 National Health and Nutrition Examination Survey (KNHANES) to investigate the relationship between nutrient intake ratios and sedentary time in dehydrated women. Body mass index and waist circumference in the dehydrated group (DG) were significantly higher than the normal group (NG). Intake of other liquids in the DG was significantly higher than in the NG, but total water intake in the DG was significantly lower than in the NG. Compared to the 600 sedentary minutes or more per day group, the odds ratio of dehydration was significantly higher in the less than 300 sedentary minutes per day group (1.871 [95% CI: 1.579~2.215], P<0.001). Compared to the drinking over 6.0 cups of water per day group, the odds ratio of dehydration was significantly higher in the 3 cups or fewer per day (OR [95% CI] is 1.198 times [1.019 to 1.408], P<0.05), and 6.0 cups or fewer group (OR [95% CI] is 1.162 times [1.018 to 1.326], P<0.05). The results of this study showed that the total water intake was relatively lower in the ≥600 sedentary minutes per day group compared to the 300 sedentary minutes per day group, making them more vulnerable to dehydration. The nutrient intake ratios were higher in the dehydration group than in the normal group. In conclusion, emphasis should be placed on the importance of hydration and nutrition education for adult women in the workplace.

  • 전통적인 방법에 의한 PER는 단순히 그 지표의 일반적인 높고 낮음에 따라 소위 '저평가 종목'이라는 이름으로 투자자들에게 추천되고 있다. 그러나 이런 방법은 개별기업의 구체적 내용을 정확하고 종합적으로 고려하지 못하고 있다. 본 연구는 전통적인 방법으로 사용되는 PER지표의 문제점을 개선코자 배당평가 모형으로부터 도출한 PER지표의 구성요소들을 독립변수로 활용 회귀분석을 했다. 그리고 이를 근거로 이론 지표를 만든 후, 그 이론 지표를 투자의사결정에 적용하였을 때의 유효성을 검증했다. PER지표를 구성하는 독립변수는 Kisor & Whitbeck(1963), Malkiel & Cragg(1970), A. Damodaran(1996)에 의해 연구된 것을 원용, PER지표의 구성요소들로 기업의 배당성향, 이익 성장을, 그리고 위험변수로서의 베타계수를 선정했다. 투자성과는 포트폴리오 투자가 일반적인 현실을 감안해 가치가중수익률을 사용한 포트폴리오의 투자성과를 측정했고, 표본은 국내 거래소 시장에 1991년부터 2001년까지 계속 상장된 금융업종을 제외한 전종목을 대상으로 했다. 실증분석에 사용된 기간은 1997년부터 2001년까지 5년 동안의 자료이며, 투자성과를 검증하기 위한 검증모형으로 위험 프리미엄 모형을 사용했다. 먼저 동 분석기간 중 전통적인 방법에 의한 PER효과는 나타나지 않았고, 아울러 기업규모 효과도 찾을 수 없었다. 그러나 회귀분석을 통해 구해진 이론 지표를 활용할 경우, 이론 지표에 비해 시장 지표가 과소 평가된 그룹이 과대 평가된 그룹과 비교할 때 투자성과가 더 우수한 것으로 나타났다. 또한 이론 지표를 통해 PER수준이 낮아짐에 따라 투자성과가 더 높아지는 PER효과도 발견됐다. 이와 같이 이론 지표에 의해 나타나는 PER효과는 기업규모 효과와는 독립적인 것으로 보인다. 외환위기 이후 우리시장에 나타난 차별화 장세 속에 아직도 PER효과나 기업규모 효과와 같은 시장이례 현상이 존재하는지는 관심의 대상이 됐다고 본다. 본 연구에 의하면 기업규모 효과와는 별개의 PER효과가 여전히 존재하며, 다만 이 PER 효과는 전통적 의미의 일반적으로 낮은 PER종목이 초과수익률을 내는 것이 아니라, 기업규모가 크더라도 그 기업의 개별특성을 고려했을 때 이와 비교해 상대적으로 PER가 낮은 종목에 투자하면 초과수익을 낼 수 있음을 의미한다.

Determination of Thiamine in Urine and Feces of the Rats by the Fluorometric Method (형광법(螢光法)에 의(依)한 Rat 분뇨중(糞尿中)의 Vitamin $B_1$의 정량(定量))

  • Two 3-month old Wistar Strain male rats (weights was 270g and 340g respectively) were used as samples in order to determine the daily excretion of thiamine contained in urine and feces by the thiochrome reaction. The results were as follows. 1. Daily excretion of urine was 5,2 cc per 100 grams of body weight and that of thiamine was $18.65\;{\gamma}$ per 100 grams of plasma. 2. Daily excretion of feces was 2.4 grams per 100 grams of body weight and that of thiamine was $7.24\;{\gamma}$ per 100 grams of plasma. 3. Daily excretion of thiamine contained in urine is about twice the amount of thiamine excretion contained in feces. Thus, it can be concluded that more of the excretion of thiamine was mainly excreted through urine.

Bioconversion of ethanol from various sugars and cellulosic materials by brown rot fungus Phaeolus schweinitzii

  • A novel brown rot fungus Phaeolus schweinitzii IUM 5048 was firstly used for ethanol production. It was found that this fungus produced ethanol with various sugars, such as glucose, mannose, galactose and cellobiose at 0.28, 0.22, 0.06, and 0.22 g of ethanol per g of sugar consumed, respectively. This fungus showed relatively good ethanol production from xylose at 0.23 g of ethanol per g of sugar consumed. However, the ethanol conversion rate of arabinose was relatively low (at 0.08 g of ethanol per g sugar). P. schweinitzii was capable of producing ethanol directly from rice straw and corn stalks at 0.11 g and 0.13 g of ethanol per g of substrates, respectively, when the fungus was cultured in a basal medium supplemented with 20 g/L rice straw or corn stalks. These results suggest that P. schweinitzii can hydrolyze cellulose or hemicellulose to fermentable sugars and convert them to ethanol simultaneously under oxygen limited condition.
