The Effects of Tidal Volume on Minimal Occlusion Pressure of Endotracheal Tube Cuff in Patients with Same Peak Inspiratory pressure (동일한 최고 흡기압(Peak inspiratory pressure)에서 기관 내관 풍선(Endotracheal tube cuff)의 최소 밀폐압(Minimal occlusion pressure)에 대한 상시량의 영향)

  • Background : An excessive endotracheal cuff pressure can cause tracheal injury, and insufficient cuff pressure may not generate an effective cuff seal. The peak inspiratory pressure influences the minimal occlusion pressure of the endotracheal tube cuff. However, the relationship between the minimal occlusion pressure and the tidal volume has not been investigated. This study was conducted to estimate the relationship between the tidal volume and the minimal occlusion pressure of the cuff. Methods : Ten mechanically ventilated patients were included. The minimal occlusion pressure of the cuff was measured using a pressure gauge. The basal tidal volume was increased and decreased as much as 10% whilst maintaining the same peak inspiratory pressure. The, minimal occlusion pressures were then measured in the high and low tidal volume state, respectively. Results : The peak inspiratory pressure was $32.6{\pm}4.72cmH_2O$ and the minimal occlusion pressure was $19.0{\pm}2.26$ mmHg in the basal ventilator setting. There was a significant relationship between the peak inspiratory pressure and the minimal occlusion pressure(r=0.77, p<0.01). The minimal occlusion pressure of the cuff was increased to $20.3{\pm}2.4$ mmHg in the high tidal volume state(p<0.05), and decreased to $16.8{\pm}3.01$ mmHg in the low tidal volume state (p<0.001). Conclusion : The minimal occlusion pressure of the cuff can be influenced by changes in the tidal volume as well as by the peak inspiratory pressure.

  • Sloshing of the fluid having a free surface produces an impact force on a tank wall subjected to external excitation. This paper investigates the effect of cylindrical structures in a rectangular sloshing tank under translational harmonic excitations. By varying the number of installed cylinders in the tank, the characteristics of the free-surface deformation is experimentally observed, and the peak pressure on the tank wall is extracted by threshold values. To predict the peak pressure, the numerical simulation is also conducted using smoothed particle hydrodynamics (SPH), and the peak values are compared with the experimental results. Furthermore, pressure and velocity fields in the tank and free-surface shape are analyzed at the moment of impact.

  • The torque is an important measure that represents the performance of a particular engine. Furthermore the information of engine torque can be used as a primary feedback parameter in modem engine management system. In this paper, a methodology is proposed for torque estimation of SI-engine. Since the proposed method uses cylinder pressure sensor, the torque can be estimated in a simple manner. The indicated torque is estimated from the peak pressure and its location, and the load torque is observed by the state observer based on the estimated indicated torque. The proposed method is accurate and robust against the variations that affect the torque production such as spark timing, mass air flow and others. This torque estimation method may be an alternative solution to the use of engine torque maps in a modem torque-based engine management system.

Field measurements of wind pressure on an open roof during Typhoons HaiKui and SuLi

  • Full-scale measurements of wind action on the open roof structure of the WuXi grand theater, which is composed of eight large-span free-form leaf-shaped space trusses with the largest span of 76.79 m, were conducted during the passage of Typhoons HaiKui and SuLi. The wind pressure field data were continuously and simultaneously monitored using a wind pressure monitoring system installed on the roof structure during the typhoons. A detailed analysis of the field data was performed to investigate the characteristics of the fluctuating wind pressure on the open roof, such as the wind pressure spectrum, spatial correlation coefficients, peak wind pressures and non-Gaussian wind pressure characteristics, under typhoon conditions. Three classical methods were used to calculate the peak factors of the wind pressure on the open roof, and the suggested design method and peak factors were given. The non-Gaussianity of the wind pressure was discussed in terms of the third and fourth statistical moments of the measured wind pressure, and the corresponding indication of the non-Gaussianity on the open roof was proposed. The result shows that there were large pulses in the time-histories of the measured wind pressure on Roof A2 in the field. The spatial correlation of the wind pressures on roof A2 between the upper surface and lower surface is very weak. When the skewness is larger than 0.3 and the kurtosis is larger than 3.7, the wind pressure time series on roof A2 can be taken as a non-Gaussian distribution, and the other series can be taken as a Gaussian distribution.

The Change of Plantar Pressure According to the Height of Heel Lifts in Obese and Non-Obese and Non-Obese Adults

  • The purpose of this study was to assess the peak plantar pressure distribution under foot areas according to the height of heel lifts in obese adults and non-obese adults during walking. Thirty-one participants volunteered for this experiment. The average body mass index (BMI) value of the fourteen subjects in the obese group was $26.5{\pm}1.4kg/m^2$ (from 25.1 to 29.3 $kg/m^2$), and of seventeen subjects in the non-obese group was $20.0{\pm}1.1kg/m^2$ (from 18.7 to 22.7 $kg/m^2$). The subject ambulated while walking in the sneakers, walking with 2 cm heel lifts, and walking with 4 cm heel lifts in the shoes. We measured the peak plantar pressure under the hallux, 1st, 2nd, 3~4th, and 5th metatarsal head (MTH), mi foot, and heel using F-scan system. The obese group had significantly higher peak plantar pressure under all foot areas than the non-obese group regardless of the height of heel lifts (p<.05). The peak plantar pressure under the 5th MTH and heel was significantly decreased, also the peak plantar pressure under hallux, 1st, and 2nd MTH was significantly increased according to the height of heel lifts in the obese group and non-obese group (p<.05), We proposed that individuals with heel lifts in shoes should be careful, as there is high plantar pressure under the forefoot.

  • Purpose : The purpose of this study was to investigate the change of the peak plantar pressure distribution under the foot areas and the range of motion (ROM) of ankle joint according to gradients in treadmill gait. Method : Thirty normal subjects (15 male and 15 female) walked on treadmill at three gradient conditions ($0^{\circ}$, $10^{\circ}$, and $15^{\circ}$) in normal speed. The ankle ROM was measured using the CMS70P that is three dimensional analyzer for excursion of ankle ROM, plantar flexion, and dorsi flexion. The peak plantar pressure distribution under the hallux, 1st metatarsal head (MTH) and heel was measured using the F -Scan system with an in-shoe sensor. Data was collected from 9 steps of left sife foot in at each gradient condition while all subjects walked. Result : As the treadmill gradient increased, the excursion of ankle joint was significantly increased (p<.05). Also, plantar flexion and dorsi flexion was significantly increased according to treadmill gradients (p<.05). The peak plantar pressure under the 1st MTH was significantly increased (p<.05) and the peak plantar pressure under the heel was significantly decreased (p<.05) as the treadmill gradient increased. No significant different in the peak plantar pressure under the hallux was observed. Conclusion : This study suggests that physical therapy for patients who have limited ankle ROM should be considered sufficient range of motion for functional ambulation. And individuals that have painful forefoot syndromes, including metatarsalgia, hallux valgus, and plantar ulceration should be careful in walking to uphill, as there is high plantar pressure under the forefoot.

  • This study is to develop automatic extraction system of continuous blood pressure using ECG (Electrocardiogram) and PPG(Photoplethysmography) for u-health care technology. PTT (Pulse Transit Time) was determined from peak difference between ECG and PPG and its inverse made to get blood pressure. Since the peaks were vulnerable to be contaminated from noise and variation of amplitude, this study developed the adaptive algorithm for peak calculation in any noise condition. The developed method of the adaptive peak calculation was proven to make the standard deviations of PPT decrease to 28% and the detection of noise increase to 18%. Also, the correlation model such as blood pressure = -0.044 $\cdot$ PTT + 133.592 has successfully been determined for predicting the continuous pressure measured without using cuff but with using PPG and ECG, only.

  • Measurements of plantar pressure provide an indication of foot and ankle function during gait and other functional activities because the foot and ankle provide necessary support and flexibility for weight bearing and weight shifting while people are performing these activities. Plantar pressure is being increasingly used in both research and clinical practice to measure the effects of various footwear and physical therapy intervention. The influence of walking speed and ground inclination on plantar pressure parameters However has not been evaluated in detail. So, in this study to determine the effect of changes in walking speed and ground inclination on plantar pressure treadmills with different walking speeds and inclination were used. Plantar pressure parameters were measured with the Parotec system using the walking and running in 20 healthy participants(10 male, 10 female) aged $20{\sim}28$(mean 22.22, S.D.2.26 years) when slow walking and running. The result of this study with increased die walking speed, the peak pressure of 1st, 5th metatarsal head and total contact time and impulse total at the forefoot was affected by walking speed; however, die peak pressure, contact time and impulse total at the forefoot was not affected by ground inclination.

Peak Net Pressure Coefficients for Cladding Design of Retractable Dome Roofs according to Rise-Span Ratio (라이즈-스팬 비에 따른 개폐식 돔 지붕의 외장재 설계용 피크순압력계수)

  • In this study, the characteristics of wind pressure distribution on circular retractable dome roofs with a low rise-to-span ratio were analyzed under various approaching flow conditions by obtaining and analyzing wind pressures under three different turbulent boundary layers. Compared to the results of previous studies with a rise-to-span ratio of 0.1, it was confirmed that a lower rise-to-span ratio increases the reattachment length of the separated approaching flow, thereby increasing the influence of negative pressure. Additionally, it was found that wind pressures varied significantly according to the characteristics of the turbulence intensity. Based on these experimental results, a model for peak net pressure coefficients for cladding design was proposed, considering variations in turbulence intensity and height.

  • Accurate description of starting transient history allows and justifies the use of small margin of safety for the engine parts, resulting in high motor mass ratio in addition to satisfying the control and guidance requirements of the vehicle. Studies have been carried out for the prediction and reduction of ignition peak and pressure-rise rate during the starting transient of solid rocket motors. Numerical studies have been carried out using a two dimensional Navier-Stokes solver. It has been inferred through the parametric studies that, in the case of solid rocket motors with uniform port, high ignition peak is observed at high spread rate and low pressure-rise rate. In the case of the port with sudden expansion configuration, high ignition peak is observed at relatively high average spread rate and high-pressure rise rate. These studies are expected to aid the designer in reducing the ignition peak by altering the propellant properties or igniter characteristics without sacrificing the motor performance.
