Increased procalcitonin level is a risk factor for prolonged fever in children with Mycoplasma pneumonia

  • Purpose: Macrolide-resistant Mycoplasma pneumoniae pneumonia (MPP) is characterized by prolonged fever and radiological progression despite macrolide treatment. Few studies have examined serum procalcitonin (PCT) level in children with MPP. We aimed to investigate the association of acute inflammation markers including PCT with clinical parameters in children with MPP. Methods: A total of 147 children were recruited. The diagnosis of MPP relied on serial measurement of IgM antibody against mycoplasma and/or polymerase chain reaction. We evaluated the relationships between C-reactive protein (CRP), PCT, and lactate dehydrogenase (LDH) levels and white blood cell (WBC) counts, and clinical severity of the disease. We used multivariate logistic regression analysis to estimate the odds ratio for prolonged fever (>3 days after admission) and hospital stay (> 6 days), comparing quintiles 2-5 of the PCT levels with the lowest quintile. Results: The serum PCT and CRP levels were higher in children with fever and hospital stay than in those with fever lasting ${\leq}3days$ after admission and hospital stay ${\leq}6days$. CRP level was higher in segmental/lobar pneumonia than in bronchopneumonia. The LDH level and WBC counts were higher in children with fever lasting for >3 days before compared to those with fever lasting for ${\leq}3days$. The highest quintile of PCT levels was associated with a significantly higher risk of prolonged fever and/or hospital stay than the lowest quintile. Conclusion: Serum PCT and CRP levels on admission day were associated with persistent fever and longer hospitalization in children with MPP.

A Study on Application of GPS for Deflection Management of Curved PCT Girder Bridge under Construction (시공 중 곡선형 PCT 거더교의 처짐 관리를 위한 GPS 적용 연구)

  • In order to manage the deflection of a curved PCT girder bridge during construction, a GPS receiver was installed at the spot predicted to be the weak point during the incremental launching so as to measure the deflection at each construction stage. The deflections obtained in the experiment were compared with those derived from the monitoring of stress, temperature and inclination. The comparative analysis of the GPS measurement and analytical values obtained from finite element modeling with respect to the launching distance showed that the measured values differ by 0.6 to 1.6 times to the analytical results. This difference could be significantly reduced by thermal calibration. From the analysis of the behavioral pattern of the bridge, deflection occurred during construction in the concrete tip due to the deflection at the head of the nose at the 95m and 75m-spots, and compression and tension developed respectively at the compression weak zone and tension weak zone. The application of GPS appeared to enable more efficient management of the deflection during the erection of the curved PCT girder bridge and is expected to be helpful for the prediction and management of the behavior in future ILM construction sites.

  • Two candidate glasses, AG8W1 and DG-2, have been developed for vitrifying the mixture of low activity resin, zeolite and Dry Active Waste(DAW), and DAW solely, respectively. In order to evaluate the chemical durability of the glasses, two different leaching tests, Product Consistency Test(PCT) and Vapor Hydration Test(VHT), have been performed. As the results of PCT performed from 7 to 120 days, the leach rates of B, Na, Si and Li in the glasses were much lower than those of the benchmark glass(SRL-EA). As the result of VHT peformed for 7 days, the leach rates were 2 and $10g/m^2/day$ for AG8W1 and DG-2, respectively, The results of VHT met the regulatory guideline( $<50g/m^2/day$) for the low activity glasses of Hanford in the USA. Consequently, two candidate glasses to be used at a commercial operation in the future showed that their chemical durability is satisfactory according to the results of two leaching tests.

  • The $(Pb_{0.9}Ca_{0.1})TiO_3$[PCT] thin films have been deposited by sol-gel processing on Si-wafer and ITO glass substrates. The creak-free films have been obtained by rapid thermal annealing at $700^{\circ}C$ for 10 minute and characterized by XRD, SEM and electrical measurements. Their tetragonality c/a was 1.041 and grain size was $0.15{\sim}0.2{\mu}m$. When the electrode system of sample was Au/PCT/ITO(MFM) and film thickness was $0.8{\mu}m$, dielectric constant, dielectric loss and Curie temperature were about 149, 0.085 and $449^{\circ}C$ at 10kHz, respectively. Spontaneous polarization $P_s$, remnant polarization $P_r$ and coercive field $E_c$ were about $5.29{\mu}C/cm^2$, $4.15{\mu}C/cm^2$ and 82kV/cm calculated by hysteresis loop.

Some Phonetic Characteristics of Mid-vocalic Lax Stops and Pre/Post-stop Vowels in Korean

  • It has been claimed that Korean mid-vocalic voiceless unaspirated lax stops are phonetically realized with voicing throughout the oral closure phase. Acoustic measurements were undertaken to examine the claim with four Korean native speakers using /$V_1CV_2$/ words where the vowel ($V_1\;=\;V_2$) was /i, a, u/ and the C was voiceless unaspirated lax stops /p, t, k/. Findings: (1) During mid-vocalic stops /k/ and /p/ the vowel /u/ was accompanied generally by a significant increase in voice cessation time as percentage of the oral closure interval (PCT) than the vowel /a/, regardless of subjects, whereas in mid-vocalic alveolar stop /t/ the effects of vowels on PCT were subject-dependent, (2) The effects of vowels on PCT were significantly greater in mid-vocalic /k/ than /p/, regardless of subjects, (3) The mean PCT, averaged across six tokens, ranged from 17% to100%, giving overall mean 61% in which the standard deviation was ${\pm}30$, and (4) Overall % of the total of mid-vocalic unaspirated lax stops were produced with a substantial period of devocing and voicing lag. Considering these results, it is difficult to agree with the existing claims that Korean voiceless unaspirated lax stops are phonetically realized with voicing throughout the oral closure phase. Other phonetic variables, including the durations of pre/post-stop vowels, voice onset time, voice cessation time, and the duration of oral closure, were measured.

Comparison of Serum CRP and Procalcitonin in Patients after Spine Surgery

  • Objective: Classical markers of infection cannot differentiate reliably between inflammation and infection after neurosurgery. This study investigated the dynamics of serum procalcitonin (PCT) in patients who had elective spine surgeries without complications. Methods: Participants were 103 patients (47 women, 56 men) who underwent elective spinal surgery. Clinical variables relevant to the study included age, sex, medical history, body mass index (BMI), site and type of surgery, and surgery duration. Clinical and laboratory data were body temperature, white blood cell count (WBC), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) and PCT, all measured preoperatively and postoperatively on days 1, 3, and 5. Results: PCT concentrations remained at <0.25 ng/mL during the postoperative course except in 2 patients. PCT concentrations did not correlate with age, sex, DM, hypertension, BMI, operation time, operation site, or use of instrumentation. In contrast, CRP concentrations were significantly higher with older age, male, DM, hypertension, longer operation time, cervical operation, and use of instrumentation. Conclusion: PCT may be useful in the diagnosing neurosurgical patients with postoperative fever of unknown origin.

Relations of Platelet Indices with Endometrial Hyperplasia and Endometrial Cancer

  • Background: Platelets are blood elements thought to play a role in the immune system and therefore tumor development and metastasis. Platelet activation parameters such as mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit (PCT) can be easily evaluated with the whole blood count and have been studied as markers of systemic inflammatory responses in various cancer types. Our aim in this study was to evaluate the correlation between endometrial pathologies and MPV, PDW and PCT. Materials and Methods: A total of 194 patients who presented to our clinic with abnormal vaginal bleeding were included in our study. The patients were divided into 3 groups (endometrial hyperplasia, endometrial cancer, control) according to their pathology results. The groups were compared for MPV, PDW, and PCT values obtained from the blood samples taken on endometrial biopsy day. Results: The endometrial cancer patients were the oldest group (p=0.04). There was no significant difference between the three groups in terms of white blood cell count (WBC), platelet count (PC), and hemoglobin (Hb) level. The highest MPV (p<0.001), PDW (p=0.002), and PCT (p<0.001) levels were in the endometrial cancer group, and the lowest levels were in the control group. Conclusions: The easy evaluation of platelet parameters in patients who are suspected of having endometrial pathology is a significant advantage. We found MPV, PDW, and PCT to be correlated with the severity of endometrial pathology with the highest values in endometrial cancer. Studies to be conducted together with different laboratory parameters will further help evaluate the diagnosis and severity of endometrial cancer and precursor lesions.

  • WIPI is developed by KWISF(Korea Wireless Internet Standardization Forum) and a wireless internet standard platform adopted by TTA. It needs the certification Process for standard specification in order to confirm interoperability. The WIPI is composed of the HAL, the Runtime Engine, and APIs(WIPI-C, WIPI-Java). nl applications are implemented through WIPI APIs that can be finished by themselves or provided essential functions from runtime engine or HAL. Therefore it needs to certify where the problems occur when errors occurred in a application. In this paper we propose the PCT that certifies a WIPI platform's functionality and APIs and the HCT that certifies HAL APIs. Because the PCT reports the final certification results for the platform it is impossible to know where the problems occur when it fails to certify platform. So, it needs to certify the HAL regardless of platform certification.

Significance of Biomarkers as a Predictive Factor for Post-Traumatic Sepsis

  • Purpose: Many traumatic patients die from sepsis and multiple organ failure. Early recognition of post-traumatic sepsis in traumatic patients will help improve the prognosis. Recently, procalcitonin (PCT), macrophage migration inhibitory factor (MIF), and lactic acid have emerged as predictive factors. Our study aims to explore the significance of PCT, MIF and lactic acid as a predictor of posttraumatic-sepsis in trauma patients. Methods: This study was conducted on prospective observational study patients who visited an emergency medical center in a university hospital from March 2014 to February 2016. We measured the white blood cells, c-reactive protein (CRP), lactic acid, PCT, and MIF with serum taken from the patient's blood within 1 hour of the occurrence of the trauma. The definition of post-traumatic sepsis was defined as being part of systemic inflammation response syndrome criteria with infections within a week. Results: A total of 132 patients were analyzed, wherein 74 patients were included in the low injury severity score (ISS) group (ISS <15) and 58 patients were included in the high ISS group (ISS ${\geq}15$). The mean PCT, MIF, and lactic acid levels were higher in the high ISS group (p<0.05). Meanwhile, 38 patients were included in the early sepsis group and 94 patients were included in the non-sepsis group. The mean MIF levels were higher in the sepsis group than the non-sepsis group (p<0.05) and there were no significant differences in the initial CRP, lactic acid, and PCT levels in these two groups. Conclusions: MIF may be considered as a predictive factor for sepsis in trauma patients.
