기혼여성의 Pap 도말검사 경험관련 정서와 검진 참여정도와의 관계연구 (A Study on the Relationship between Emotion Related to Pap Smear and Continuous Participation in Pap Smear Screening in Married Korean Women)

  • The purpose of this study was to determine the relationship between emotion related to Pap smear and continuous participation in Pap smear screening by married women. The subjects for this study were 209 married women who had at least one previous Pap smear. The data were collected using a self reported survey questionnaire which was developed by the researchers. It's Cronbach's alpha score ranged from .8799 to .9118. Data analysis was done by descriptive statistics. ANOVA, t-test and Pearson correlation using the SPSS Win program. The results are as follows: 1. The mean for continuous participation in Pap smear screening over the past three years was 1.95. 2. The mean score for negative emotions related to Pap smear screening was 3.2. 3. The mean score for positive emotions related to Pap smear screening was 3.9 4. Positive correlations were found between positive emotion and continuous participation in Pap smear screening (r=.417, p<.05). A weak negative correlation was found between negative emotion and continuous participation in Pap smear screening (r=-.213, p<.05). In conclusion, negative and positive emotions had a significant effect on continuous participation in the Pap smear screening among married women. This suggests that reducing negative emotions and activating messages which relate to positive emotions may be an effective strategy for increasing continuous participation in Pap smear screening.

Management of Precancerous Cervical Lesions in Iran: A Cost Minimizing Study

  • Background: Cervical cancer is a common, preventable and manageable disease in women worldwide. Objectives: This study was conducted to determine the cost of follow-up for suspicious precancerous cervical lesions within a screening program using Pap smear or HPV DNA test through the decision tree. Materials and Methods: Patient follow-up processes were determined using standard guidelines and consultation with specialists to design a decision tree model. Costs of treatment in both public and private sectors were identified according to the national tariffs in 2010 and determined based on decision tree and provided services (visits to specialists, colposcopy, and conization) with two modalities: Pap smear and HPV DNA test. The number of patients and the mean cost of treatment in each sector were calculated. The prevalence of lesions and HPV were obtained from literature to estimate the cost of treatment for each woman in the population. Results: Follow-up costs were determined using seven processes for Pap smear and 11 processes for HPV DNA test. The total cost of using Pap smear and HPV DNA process for each woman in the population was 36.1$ and 174$ respectively. Conclusions: The follow-up process for patients with suspicious cervical lesions needs to be included in the existing screening program. HPV DNA test is currently more expensive than Pap smear, it is suggested that we manage precancerous cervical lesions with this latter test.

Knowledge, Attitudes and Practices of Sudanese Women Regarding the Pap Smear Test and Cervical Cancer

  • Background: Despite the established role of the Pap smear test (PST) in prevention and early detection of cervical cancer, it is still rarely practiced in Sudan. Many challenges hinder the establishment of an effective cervical cancer screening program, including socio-cultural factors. Therefore, this study aimed to investigate the knowledge, attitudes and practices (KAP) of Sudanese women with regard to the Pap smear test and cervical cancer. Materials and Methods: A total of 500 married women aged 14 to 58 years were recruited from obstetric clinics, hospitals and universities in Khartoum in 2014. Data were collected using a standardized, pretested questionnaire that inquired socio-demographic characteristics and their KAP about cervical cancer and the PST. Results: More than 52% of participating women were above 30 years of age, and the majority (78.8%) were university degree holders. A total of 486 (97.2 %) of participants were resident in urban areas of Khartoum State. However about 48% of the respondents had never heard about PST, and only 15.8% of the participants had undergone a Pap smear test previously; 46.6% (233/500) knew that the human papilloma virus (HPV) was the causative agent, but only 39.2% (196/500) had heard about HPV vaccination, and only 11.4% (57/500) had received the vaccine. However 68% of the respondents agreed to do Pap smear if properly informed about the test and 75.4% of the respondents agreed to participate in a cervical cancer screening program. Conclusions: Despite a high educational level, less than half of our participants had accurate knowledge about cervical cancer, HPV, and cervical cancer screening. Health education about cervical cancer, HPV and sexually transmitted infections and the role of PST in cervical cancer prevention are crucial when designing interventions aimed at improving cervical cancer screening for Sudanese women.

The Relationship between Papanicolaou Smear Test and Human Papilloma Virus DNA Chip Test in the Uterine Cervix

  • The genotypes of Human Papilloma Virus (HPV) are important in the carcinogenesis of uterine cervical cancer. Diagnosis of uterine cervical cancer screening has been executed using Papanicolau method (Pap) and HPV DNA Chip method. We researched the interrelation of HPV DNA genotypes in single and multiple infections and analyzed the results of Pap and HPV DNA Chip tests at Gunsan Medical Center (GMC). The correlation analysis was surveyed on collected results from 599 patients who have been tested with both Pap and HPV DNA chip tests from November 2004 to May 2010 at GMC. The inconsistency between Pap and HPV DNA Chip tests was 41.1%. The HPV DNA Chip genotype related with high risk cases were type 16 (13.5%), type 52 (10.5%), type 58 (10.1%), and type 18 (3.4%). Those related with low risk cases were type 70 (8.9%), type 6 (1.7%), type 40 (1.2%), type 11 (1.3%), and other types (14.3%). Among the 195 cases of HPV positive status, 161 cases were associated with single infection; 108 (67.1%) cases were related with high risk genotype; 19 (11.8%) cases were low risk genotype; 31 (21.1%) cases were related with other types. 29 cases were associated with double infections; 23 (79.3%) cases were high risks; 5 (17.2%) cases were mixed high and low risks; 1 (3.5%) case was low risk.

Investigation of the Role of Training Health Volunteers in Promoting Pap Smear Test Use among Iranian Women Based on the Protection Motivation Theory

  • Background: Cervical cancer is known as one of the most prevalent types of cancers and a major public health problem in developing countries which can be detected by Pap test, prevented, and treated. Despite the effective role of Pap test in decreasing the incidence and mortality due to cervical cancer, it is still one the most common causes of cancer-related deaths among women, especially in developing countries. Thus, this study aimed to examine the effect of educational interventions implemented by health volunteers based on protection motivation theory (PMT) on promoting Pap test use among women. Materials and Methods: This quasi-experimental study was conducted on 60 health volunteers and 420 women. The study participants were divided into an intervention and a control group. Data were collected using a valid self-reported questionnaire including demographic variables and PMT constructs which was completed by both groups before and 2 months after the intervention. Then, the data were entered into the SPSS statistical software, version 19 and were analyzed using Chi-square test, independent T-test, and descriptive statistical methods. P<0.05 was considered as statistically significant. Results: The findings of this study showed that the mean scores of PMT constructs (i.e. perceived vulnerability, perceived severity, fear, response-costs, self-efficacy, and intention) increased in the intervention group after the intervention (P<0.001). However, no significant difference was found between the two groups regarding response efficacy after the intervention (P=0.06). The rate of Pap test use also increased by about 62.9% among the study women. Conclusions: This study showed a significant positive relationship between PMT-based training and Pap test use. The results also revealed the successful contribution of health volunteers to training cervical cancer screening. Thus, training interventions based on PMT are suggested to be designed and implemented and health volunteers are recommended to be employed for educational purposes and promoting the community's, especially women's, health.

미국 캘리포니아주에 거주하는 한인여성들의 자궁경부암 수검권고에 영향을 미치는 보건의료 접근성 및 보건의료인의 특성 분석 (Effect of healthcare access and healthcare provider status on recommendation for Pap test among Korean American women in Alameda and Santa Clam Counties, California)

  • 연구목적: 3년마다 정기적인 자궁경부암 조기검진을 받도록 권고하고 있는 수검권고안은 검진행동 유발요인으로써 자궁경부암 검진참여의 가능성을 증진시키는 주요 영향요인으로 작용하고 있다. 본 연구는 보건의료 접근성과 보건의료인의 특성이 자궁경부암 조기검진 수검권고에 미치는 영향을 살펴보고자 시도되었다. 연구방법: 본 연구에서 재분석된 한인여성 건강조사자료는 미국 캘리포니아주 알라메다와 산타클라라 카운티에 거주하는 한인여성 865명을 대상으로 2002년에 전화설문조사로 수행되었다. 수검권고안에 영향을 미치는 주요 예측인자를 살펴보기 위해 로지스틱 회귀분석을 수행하였다. 연구결과: 조사대상자 중 37.9%만이 지난 3년간 자궁경부암 조기검진 참여에 관한 수검권고를 받은 것으로 나타났다. 수검권고의 주요 예측인자는 건강보험 가입여부, 지난 1년간의 의사방문횟수, 정기적으로 방문하는 보건의료인의 특성인 것으로 나타났다. 또한 보건의료 접근성에서는 공공의료보험 또는 사보험 여부와 관계없이 건강보험에 가입한 여성들이 수검권고를 더 많이 받은 것으로 나타났다. 특히 보건의료인의 특성 중 인종과의 관련성을 살펴보면, 한인이 아닌 타 인종의 여성 의사로부터 가장 많이 검진참여에 관한 수검권고를 받았고(OR=6.21, 95% CI=2.63, 14.66), 다음으로 한인 남성 의사(OR=2.19, 95% CI=1.30, 3.68), 비한인 남성 의사로부터 수검권고를많이 받았다(OR=2.07, 95% CI=1.15, 3.71). 결론: 캘리포니아주의 2개 카운티에서 거주하는 한인여성을 대상으로 보건의료 접근성과 보건의료인의 특성이 자궁경부암 조기검진 수검권고에 미치는 영향을 분석한 결과, 한인여성들의 건강보험 가입여부 및 의사방문 수준, 보건의료인의 인종이 수검권고와 관련이 있는 것으로 나타났다. 이는 한인여성들의 자궁경부암 조기검진 수검률 향상 및 자궁경부암 이환률과 사망률을 감소시키기 위한 수행전략 개발에 중요한 기초자료로 활용될 수 있다.

Effects of Group Training Based on the Health Belief Model on Knowledge and Behavior Regarding the Pap Smear Test in Iranian Women: a Quasi-Experimental Study

  • The Pap smear test is recommended for early diagnosis of cervical cancer. The aim of this study was to assess knowledge and behavior regarding the Pap smear test based on the Health Belief Model (HBM) in women referred to premarital counseling classes, Hamadan, Iran. This quasi-experimental study was conducted on 330 women, who were allocated randomly to two case and control groups (n=165). Two educational session classes were performed in the case group. Two stages in before and after intervention groups were evaluated. Analysis of data was performed by SPSS/16.0, using t-test, $x^2$, and McNemar's test. P-values <0.05 were regarded as significant. There was no significant difference between the mean scores of the various structures of this model in two groups before the intervention. However, after the intervention there were significant increase in mean score of knowledge and all variables of HBM in the intervention group(P<0.001). The findings of this study highlight the important role of education about cervical cancer on changing women's beliefs about cervical screening.

  • PAP-I cDNA was synthesized from total RNA of Phytolacca americana leaves by RT-PCR, and then subcloned to recombinant vector pBluescript II SK-. Using PCR with primers designed in our laboratory, we could get the 9 deletion mutant PAP-I cDNA fragments. The first of the fragments was deleted by 66bp from immature N-terminal and then the rest were deleted by 90bp sequentially. Sequentially deletion mutant PAP-I cDNAs were inserted to pAc55M, on down-stream of gall promoter. Recombinant pAc55M was transformed to yeast cells, psy1 and the cells were spreaded on SC_urn-/glucose plate media. Colonies on SC_ura-/glucose plate were streaked on the same position of SC_ura-/glucose and SC_ura-/galactose plate, and we selected colonies growing on both plates, which carry non-cytotoxic deleted mutant PAP-I cDNA. We selected 4 deletion mutant PAP-I cDNAs which have not cytotoxicity.

Pap Smear Screening Results for Turkish Pregnant Women

  • Objective: Cervix cancer is one of the most common gynecological cancer types that cause cancer deaths among women. This study was planned based on a descriptive method in order to evaluate the results of PAP smear screening during pregnancy for prevention of cancer. Materials and Methods: The research involved 110 pregnant women registered at the Obstetrics and Gynecology Polyclinic of Bagcilar Training and Research Hospital and 86 non-pregnant women of the same ages as a control group. As criteria for acceptance were conditions such as not being in coitus within the last 48 hours, not using vaginal ovule, and not performing vaginal lavage. A survey consisting of 33 questions was conducted and the results were processed using Bethesda. Results: The average ages were $27.1{\pm}4.70$ for the pregnant women and $28.8{\pm}4.24$ for the control group. 60.7% of cases had previously heard of a PAP smear test, 49% were aware of why PAP smear tests were conducted, 26.4% of pregnant participants and 27.3% of non-pregnant participants had previously undergone a smear test. In this study, smear results of all cases were 95.4% sufficient. 18.2% of pregnant cases had an infection, 54.5% had reactive cellular change, and 0.9% had atypical squamous cells of undetermined significance (ASC-US). 16.3% of non-pregnant cases had an infection, 58.1% had reactive cellular change, 3.5% had atypical squamous cells of undetermined significance (ASC-US), and 1.2% had low-grade squamous intraepithelial lesions (LGSIL). Conclusion: PAP smear test is a good opportunity to identify pre-invasive lesions in early phases of pregnancy.

Development of an Instrument based on the Protection Motivation Theory to Measure Factors Influencing Women's Intention to First Pap Test Practice

  • Background: Given that there are many Iranian women who have never had a Pap smear, this study was designed to develop and validate a measurement tool based on the Protection Motivation Theory to assess factors influencing the Iranian women's intention to perform first Pap testing. Materials and Methods: In this psychometric research, to determine the Content Validity Index (CVI) and the Content Validity Ratio (CVR), a panel of experts (n=10) reviewed scale items. Reliability was estimated through the Intraclass Correlation Coefficient (n=30) and internal consistency (n=240). Also, factor analysis (exploratory and conformity) was performed on the data of the sample women who had never had a Pap smear test (n=240). Results: A 26-item questionnaire was developed. The CVI and CVR scores of the scale were 0.89 and 0.90, respectively. Exploratory factor analysis loaded a 26-item with seven factors questionnaire (perceived vulnerability and severity, fear, response costs, response efficacy, self-efficacy, and protection motivation (or intention)) that jointly accounted for 72.76% of the observed variance. Confirmatory factor analysis indicated a good fit for the data. Internal consistency (range 0.70-0.93) and test-retest reliability (range 0.72-0.96) of sub-scales were acceptable. Conclusions: This study showed that the designed instrument was a valid and reliable tool for measuring the factors influencing the women's intention to perform their first Pap testing.
