Guillain-Barré Syndrome after Lung Transplantation in the Immediate Postoperative Period: Case Report

  • A 58-year-old man, incapable of maintaining oxygen saturation with mechanical ventilation, was admitted to our hospital for veno-venous extracorporeal membrane oxygenation (ECMO) treatment. He was diagnosed with acute respiratory distress syndrome (ARDS) due to influenza A pneumonia. His condition stabilized with antibiotics and steroid administration, but weaning from ECMO failed due to post-infectious pulmonary sequelae. On day 84 after admission, he underwent bilateral lung transplantation. In the postoperative phase, he did not regain consciousness even after discontinuation of sedatives for 3 days. However, spontaneous pupillary reflex and eye movements were preserved, while communication and upper and lower limb movements were affected. The nerve conduction study was diagnostic of Guillain-Barré syndrome. He was managed with intravenous immunoglobulins and plasmapheresis. Mild recovery of the facial muscles was seen, but he died 24 days post-surgery due to progressive ARDS and sepsis.

Anterior Cingulate Cortex and Amygdala Dysfunction Among Patients with Alcohol Dependency During Exposure to Negative Emotional Stimuli

  • This study aimed to identify specific psychological and brain activation responses relating to the processing of negative emotions in patients with alcohol dependency. The authors hypothesized that patients with alcohol dependency would demonstrate the abnormal functioning of brain regions involved in negative emotions. Eleven male patients diagnosed with alcohol dependence in an inpatient alcohol treatment facility and 13 social drinkers with similar demographics were scanned using functional magnetic resonance imaging (fMRI) as they viewed film clips that evoked negative emotions. During exposure to negative emotional stimuli, the control group evinced significantly greater activity in the right anterior cingulate cortex (ACC) in comparison to patients with alcohol dependency. Correlation analyses demonstrated a negative association in the relationship between beta values from the right ACC and amygdala in participants classified in the control group. No statistically significant relationship was observed for blood oxygenation level-dependent (BOLD) changes between the two regions in the patient group during the elicitation of negative emotions. On the other hand, patients exhibited a greater activation of the amygdala as negative emotions were induced. These results suggest that alcoholism presents pathophysiology of brain activation that is distinct from the responses of healthy individuals functioning as controls.

  • Nutritional assessment and support are often overlooked in the critically ill due to other urgent priorities. Unlike oxygenation, organ dysfunction, infection, or consciousness, there is no consensus of indicators. Making it difficult to evaluate the effectiveness of an intervention. Nevertheless, appropriate nutritional support in the critically ill has been associated with less morbidity and lower mortality. But, nutritional support has been considered an adjunct, for body weight maintenance and to help patients during the inflammatory phase of illness. Thus, it has been assigned a lower priority, compared to mechanical ventilation or hemodynamic stability. Recent findings have shown that nutritional support may prevent cellular injury due to oxidative stress and help strengthen the immune response. Large-scale randomized trials and clinical guidelines have shown a shift from nutritional support to nutritional therapy, with an emphasis on the importance of protein, minerals, vitamins, and trace elements. Nutrition is also important in neurocritically ill patients. Since there are few studies or recommendations with regard to the neurocritical population, the general recommendations for nutritional support should be applied.

Successful treatment of central retinal artery occlusion using hyperbaric oxygen therapy

  • Central retinal artery occlusion (CRAO) is considered an ophthalmologic emergency. The prognosis of this disease is very poor. Currently, there is no generally effective therapy available to treat CRAO. Hyperbaric oxygen therapy (HBOT) can increase the volume of oxygen delivered to the ischemic retinal tissue until spontaneous or assisted reperfusion occurs. We report the case of a patient who experienced sudden visual loss due to CRAO that was treated with HBOT. The patient was an 81-year-old woman who presented with CRAO in her right eye (OD). She exhibited "hand motion" visual acuity before treatment. She underwent three sessions of HBOT at a pressure of 2.8 atmospheres absolute, performed over 3 days. After 4 days in hospital, her visual acuity improved to 0.4 (OD) for far vision and 0.5 (OD) for near vision. Her vision was stable without the supply of oxygen; therefore, she was discharged.

fNIRS 기반 실시간 집중력 모니터링 모바일 애플리케이션 (Mobile Application for Real-Time Monitoring of Concentration Based on fNIRS)

  • Learning assistance system that continuously measures user's concentration will be helpful to grasp the concentration pattern and adjust the learning method accordingly to improve the learning efficiency. Although a lot of various learning aids have been proposed, there have been few studies on the concentration monitoring system in real time. Therefore, in this study, we developed an Android-based mobile application that can measure concentration during study by using functional near-infrared spectroscopy, which is used to measure brain activity. First, the task accuracy was predicted at a maximum level of 93.75% from the prefrontal oxygenation characteristics measured while performing the visual Q&A task on 11 college students, and a concentration calculation formula based on a linear regression model was derived. Then, a survey on the usability of the mobile application was conducted, overall high satisfaction and positive opinions were obtained. From these findings, this application can be used as a customized learning aid application for users, and further, it can help educators improve the quality of classes based on the level of concentration of learners.

Non-Surgical Resolution of Inflow Cannula Obstruction of a Left Ventricular Assist Device: A Case Report

  • A 55-year-old woman who had received an implantable left ventricular assist device 3 months earlier presented with dyspnea and a low-flow alarm of the device. Computed tomography and log-file analysis of the device system suggested inflow cannula obstruction. Since the patient had cardiogenic shock due to pump failure, venoarterial extracorporeal membrane oxygenation (ECMO) was initiated. With ECMO, surgical exchange of the pump was considered. However, the obstruction spontaneously resolved without surgical intervention. It turned out that an obstructive thrombus was washed out by rebooting the pump. Moreover, the thrombus was embolized in the patient's left subclavian artery. The patient underwent heart transplantation 4 months after the pump obstruction accident and continued to do well.

  • Lung transplantation is considered a viable treatment option for patients with end-stage lung disease. Recent decades have seen a gradual increase in the number of lung transplantation patients worldwide, and in South Korea, the case number has increased at least 3-fold during the last decade. Furthermore, the waiting list time is becoming longer, and more elderly patients (>65 years) are undergoing lung transplantation; that is, the patients placed on the waiting list are older and sicker than in the past. Hence, proper management during the pre-transplantation period, as well as careful selection of candidates, is a key factor for transplant success and patient survival. Although referring and transplant centers should address many issues, the main areas of focus should be the timing of referral, nutrition, pulmonary rehabilitation, critical care (including mechanical ventilation and extracorporeal membrane oxygenation), psychological support, and the management of preexisting comorbid conditions (coronary artery disease, diabetes mellitus, gastroesophageal reflux disease, osteoporosis, malignancy, viral infections, and chronic infections). In this context, the present article reviews and summarizes the pre-transplantation management strategies for adult patients listed for lung transplantation.

HeartMate 3 Implantation via Only Left Thoracotomy: A Case Report

  • Median sternotomy is a standard surgical technique used for left ventricular assist device (LVAD) implantation. However, if sternotomy has a prohibitive surgical risk, LVAD implantation can be performed through only left thoracotomy. We managed a patient with end-stage heart failure who had recently undergone coronary artery bypass grafting (CABG) elsewhere. The patient also had a deep sternal wound infection and bacteremia. Because of refractory cardiogenic shock, we performed extracorporeal membrane oxygenation (ECMO). After multiple mediastinal washouts and omental flap placement, ECMO was converted to extracorporeal LVAD (from the left ventricular apex to the descending aorta) through a left thoracotomy. The extracorporeal LVAD was maintained for 18 days and replaced by the HeartMate 3 LVAD. The patient was discharged in good condition 115 days after CABG.

간호대학생의 디지털 리터러시, 가상현실에 대한 인식 및 가상현실을 적용한 간호 교육 요구도 (The digital literacy, awareness, and educational needs of virtual reality among nursing students)

  • Purpose: This study's aim was to investigate digital literacy, awareness of virtual reality, and educational needs of virtual reality-based nursing education among nursing students. Methods: Structured self-reported questionnaires were used to measure the nursing students' digital literacy, awareness, and educational needs of virtual reality. This study was conducted on 162 nursing students in Seoul between December 13 and December 24, 2021. The data were analyzed using descriptive statistics, t-test, and one-way ANOVA with SPSS/WIN 24.0. Results: The mean scores on digital literacy, awareness, and educational needs of virtual reality were 4.38±0.42, 4.71±0.64, and 4.08±0.61, respectively. Concerning the educational needs of the subjects, the need for safety education was the highest, followed by the need for oxygenation education, elimination education, and activity and exercise education. Significant relationships were found between digital literacy and the awareness of virtual reality (r=.34, p<.001), between digital literacy and the educational needs of virtual reality (r=.17, p=.028), and between awareness and the educational needs of virtual reality (r=.51, p<.001). Conclusion: This study suggests that it is necessary to develop a virtual reality-based nursing educational program that reflects the educational needs of nursing students.

단일 박동형 생명구조장치의 인공폐 적용 -실험연구- (Application of a Single-pulsatile Extracorporeal Life Support System for Extracorporeal Membrane Oxygenation -An experimental study -)

  • 생명구조장치(ECLS)는 호흡부전 및 심부전에 적용하는 장치로 세계적으로 많은 연구개발과 임상시도가 계속되고 있다. 현재까지 생명구조장치의 혈류펌프로는 비박동형 구동장치가 표준으로 사용되고 있으며, 박동형 구동장치는 생리적인 측면에서 유리하지만 급격한 회로압 상승과 그에 따른 혈구손상에 대한 우려가 있고 특히 막형산화기를 사용하는 인공폐 회로에서 기피되어 왔다. 본 연구는 인공폐 실험모델에서 단일 박동형 구동펌프와 막형 산화기 사이에 압력완충장치를 설치함으로써 언급한 부작용을 최소화시키면서 생리적인 박동혈류를 유도할 수 있을 것이라는 가능성을 검증하였다. 실험대상은 올레익산으로 급성 호흡부전증이 유발된 잡견(N=16)을 사용하였다. 실험군은 사용된 구동펌프의 종류와 압력완충장치의 유무에 따라 3개 군으로 분류하였다. 제1군(n=6)은 대조군으로 원심펌프를 이용하였고, 제2군(n=4)은 단일 박동형 구동펌프를 사용하였으며, 제3군(n=6)은 단일 박동형 구동펌프에 압력완충장치를 설치하였다. 실험모델은 흉강 절개 후 우심방-대동맥을 우회시키는 부분 체외순환 형태로, 1.8~2 L/min의 펌프박출량에서 2시간을 구동하였다. 관찰지표는 주로 혈역학 변화, 회로압, 혈액검사 및 혈구세포에 미치는 영향 등에 한정하였다. 맥동압(pulse pressure)은 박동형 구동펌프를 이용한 2군(47$\pm$10 mmHg)과 3군(41$\pm$9 mmHg)에서 비박동형 원심펌프를 이용한 1군(17$\pm$7 mmHg)에 비해 유의하게 높았다(p<0.001). 막형산화기에 걸리는 회로압은 1 군의 경우 222$\pm$8mmHg, 2군의 경우 739 $\pm$ 35 mmHg, 3군의 경우 470$\pm$ 17 mmHg였다(p<0.001). 순환 2시간째의 동맥혈 산소분압은 1군에 비해 박동혈류를 사용하는 2군과 3군에서 현저하게 높았다(77$\pm$41, 96$\pm$48, 97$\pm$25 mmHg; p<0.05). 혈구손상지표인 혈장 유리 헤모글로빈치는 1군에서 가장 낮았고, 2군에서 가장 높았으며, 3군에서 유의하게 감소하였다(55.7$\pm$43.3, 162.8$\pm$113.6, 82.5$\pm$25.1 mg%; p<0.05). 기타 혈액검사치는 특별한 차이가 없었다. 이상의 결과에서 단일 박동형 구동펌프는 비박동형 원심펌프에 비해 막형산화기의 산소교환에 유리한 반면, 회로압과 혈구세포 손상 측면에서 불리하다는 것을 알 수 있었다. 그러나 단일 박동형 구동펌프와 막형 산화기 사이에 압력완충장치를 설치하는 경우, 회로압 상승과 혈구세포손상을 유의하게 감소시킬 수 있었다.
