Comparison of immediate complete denture, tooth and implant-supported overdenture on vertical dimension and muscle activity

  • PURPOSE. To compare the changes in the occlusal vertical dimension, activity of masseter muscles and biting force after insertion of immediate denture constructed with conventional, tooth-supported and Implant-supported immediate mandibular complete denture. MATERIALS AND METHODS. Patients were selected and treatment was carried out with all the three different concepts i.e, immediate denture constructed with conventional (Group A), tooth-supported (Group B) and Implant-supported (Group C) immediate mandibular complete dentures. Parameters of evaluation and comparison were occlusal vertical dimension measured by radiograph (at three different time intervals), Masseter muscle electromyographic (EMG) measurement by EMG analysis (at three different positions of jaws) and bite force measured by force transducer (at two different time intervals). The obtained data were statistically analyzed by using ANOVA-F test at 5% level of significance. If the F test was significant, Least Significant Difference test was performed to test further significant differences between variables. RESULTS. Comparison between mean differences in occlusal vertical dimension for tested groups showed that it was only statistically significant at 1 year after immediate dentures insertion. Comparison between mean differences in wavelet packet coefficients of the electromyographic signals of masseter muscles for tested groups was not significant at rest position, but significant at initial contact position and maximum voluntary clench position. Comparison between mean differences in maximum biting force for tested groups was not statistically significant at 5% level of significance. CONCLUSION. Immediate complete overdentures whether tooth or implant supported prosthesis is recommended than totally mucosal supported prosthesis.

한 환자에서 실패한 임플란트 부착장치들의 원인과 이중관 임플란트 피개 의치의 적용 (Causes of failed implant attachments and application of double crown implant overdenture in one patient)

  • 본 증례의 환자는 약 10여년 전 2개의 임플란트를 이용하여 Gold Hader bar 하악 임플란트 피개 의치로 수복하였고, 3년 6개월의 사용으로 인한 bar의 심한 마모로 유지력을 상실하였다. 이를 Locator 부착장치를 이용하여 하악 피개 의치를 수리하였으나, Locator 사용 6년 6개월 후 male 부위가 완전히 닳은 상태로 내원하게 되었다. 다음으로는 하이브리드 이중관(Hybrid telescopic double crown with friction pin)을 사용하여 임플란트 피개 의치를 새로 제작하였고, 이를 2년동안 관찰한 결과 우수한 예후를 보여주고 있다. 여기에서 한 환자에서 2가지(Gold Hader bar, Locator)방법으로 수복한 임플란트 피개 의치에서 각 부착장치의 실패원인에 대해서 알아보고, 이중관 피개 의치의 장단점과 성공을 위한 조건에 대해서 보고하고자 한다.

  • 임플란트 피개의치는 전통적인 총의치에 비해 높은 유지력 및 저작능력, 환자의 만족도를 보인다. 특히 골흡수가 심한 하악 무치악 환자의 치료에서는 우선되는 치료방법으로 고려된다. 임플란트 피개의치를 위한 attachment에는 임플란트간 연결된 bar type과 연결되어있지 않은 solitary type으로 구분할 수 있다. 최근 임상에서 많이 사용하는 solitary type의 경우 임플란트간의 평행관계가 중요하며, 임플란트간의 평행관계가 불량한 경우 attachment의 마모 및 유지력 감소로 유지관리를 내원횟수가 증가할 수 있다. 본 증례는 평행하지 못한 두 개의 하악 임플란트 식립 환자에서 milled bar를 이용하여 제작한 임플란트 피개의치로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

  • The term, 'crossed occlusion' implies clinical situation in which the residual teeth in one arch have no contact with those in the antagonistic arch, resulting in the collapse of occlusal vertical dimension. The treatment goal of this pathologic condition is restoration of the collapsed vertical dimension and stabilization of abnormal mandibular position. Previously, konus removable prostheses or tooth supported overdentures were suggested to solve crossed occlusion. Nowadays, dental implants have been used for definitive support to solve this problem. In this case report, a 65 years old female patient had a crossed occlusion, in which the maxillary posterior residual teeth and mandibular anterior residual teeth cross. Interim removable and fixed dental prostheses were used to confirm the proper vertical and horizontal jaw relation. After that, the mandibular posterior edentulous region was restored with implant-supported fixed dental prostheses. Computer tomography guided implant surgery was performed according to the concept of the restoration-driven implant placement. The maxillary anterior edentulous region was restored with Kennedy class IV removable prosthesis, considering the patient's economic status. The patient's jaw position and prostheses have been well maintained at the follow-up after 6 months of definitive restoration. The antero-posterior crossed occlusion problems appeared to be effectively solved with the combination of removable in one arch and implant-supported fixed prostheses in the other.

CAD/CAM 맞춤형 지대주를 이용한 milled-bar 피개의치 증례 (Rehabilitation of edentulous maxilla with implant-supported milled bar overdenture using CAD/CAM customized abutment: A case report)

  • 상악 완전 무치악에 6개 이상의 임플란트가 식립된 경우 임플란트-지지형 피개의치 제작이 가능하다. 임플란트-지지 피개의치에 사용되는 다양한 부착장치 중에 milled-bar의 경우 의치의 회전을 허용하지 않고, 안정성이 뛰어나며, 여러 개의 임플란트를 연결 고정하는 효과를 가진다는 장점이 있다. 본 증례에서는 전치부에 심한 골소실을 동반한 상악 완전 무치악 환자에서 세 가지 다른 시스템으로 구성된 7개의 임플란트 상부에 CAD/CAM 맞춤형 지대주와 시멘트 접착형의 milled bar를 이용한 피개의치를 제작하여 상실된 심미와 기능을 효과적으로 회복하였다.

Clinical evaluation of mandibular implant overdentures via Locator implant attachment and Locator bar attachment

  • PURPOSE. The aim of this study was to evaluate the clinical findings and patient satisfaction on implant overdenture designed with Locator implant attachment or Locator bar attachment in mandibular edentulous patients. MATERIALS AND METHODS. Implant survival rate, marginal bone loss, probing depth, peri-implant inflammation, bleeding, plaque, calculus, complications, and satisfaction were evaluated on sixteen patients who were treated with mandibular overdenture and have used it for at least 1 year (Locator implant attachment: n=8, Locator bar attachment: n=8). RESULTS. Marginal bone loss, probing depth, plaque index of the Locator bar attachment group were significantly lower than the Locator implant attachment group (P<.05). There was no significant difference on bleeding, peri-implant inflammation, and patient satisfaction between the two denture types (P>.05). The replacement of the attachment components was the most common complication in both groups. Although there was no correlation between marginal bone loss and plaque index, a significant correlation was found between marginal bone loss and probing depth. CONCLUSION. The Locator bar attachment group indicates lesser marginal bone loss and need for maintenance, as compared with the Locator implant attachment group. This may be due to the splinting effect among implants rather than the types of Locator attachment.

파킨슨 병을 가진 환자의 상악 마그네틱 피개의치 및 하악 임플란트 피개의치 수복 증례 (Overdentures of the patient with Parkinson's disease: A case report)

  • 파킨슨 병을 가지고 있는 환자의 총의치는 혀와 입술의 불수의적 운동으로 인해 유지와 안정이 좋지 않은 경우가 많다. 또한 제한된 수의 운동으로 인해 피개의치의 장착에 어려움이 어려울 수 있다. 본 증례는 파킨슨병을 가진 상, 하악 부분 무치악 환자에서 상악에서는 잔존치근을 이용한 magnetic attachment와 하악에서는 2개의 임플란트를 이용한 $Locator^{(R)}$ attachment를 통해 유지와 안정을 증진시킨 피개의치 수복증례로서, 기능적으로 개선된 보철물을 제작했기에 보고하는 바이다.

Effect of abutment angulation in the retention and durability of three overdenture attachment systems: An in vitro study

  • This in vitro study investigated and compared the durability and retention of three types of attachments. MATERIALS AND METHODS. Three commercially available attachments were investigated: $Clix^{(R)}$, Dalbo-$Plus^{(R)}$ and $Locator^{(R)}$. In total, 72 samples of these attachments were placed in the acrylic resin forms and subjected to mechanical testing (5400 cycles of insertion and removal) over the respective ball or Locator abutments immersed in artificial saliva at pH 7 and $37^{\circ}C$. The abutments were placed at angulations of $0^{\circ}$, $10^{\circ}$ and $20^{\circ}$. The retention force was recorded at the beginning and after 540, 1080, 2160, 3240, 4320 and 5400 insertion-removal cycles. RESULTS. The results revealed that there were significant differences in the average values of the insertion/removal force due to angulation ($F_{(2.48)}=343619$, P<.05) and the type of attachment ($F_{(7.48)}=23.220$, P<.05). CONCLUSION. Greater angulation of the abutments was found to influence the retention capacity of the attachments, and the fatigue test simulating 5 years of denture insertion and removal did not produce wear in the metal abutments.

Recruitment and enrollment in a randomized clinical trial of mandibular two-implant overdenture

  • PURPOSE. The purpose of this study was to evaluate the effectiveness of a promotion campaign for subject recruitment and selection, and reasons of withdrawal from a prospective clinical trial of mandibular two-implant supported overdenture. MATERIALS AND METHODS. The subjects of this study were participants in a randomized controlled clinical trial for investigating prognosis of implants and overdentures with attachments. Recruited subjects were classified by gender, age, and participation motives. Withdrawal rate of the participants before and after enrollment were evaluated. RESULTS. 177 patients were recruited and 51 patients were enrolled for the trial. Among them, 40 participants eventually took part in the trial. 116 subjects (65.5%) were recruited by advertisement and 61 (34.5%) were referred by patients of the hospital or local clinics. Regarding recruitment effectiveness, newspaper recruited the largest number of participants. With respect to referral patients, the proportion of our hospital patients was higher (37/61). Subjects in their 70s comprised the largest proportion (22/51). The male to female ratio was similar (25:26). Final withdrawal rate of all subjects were 74.0%. Among the reasons for withdrawal from enrollment (n=126) presence of remaining teeth and lack of motivation were the most common reasons. CONCLUSION. To facilitate recruitment of clinical trial subjects and improve enrollment rate, it is important to obtain a sufficient number of researchers, perform promotion activity with diverse strategies, cooperate with local dentists, increase the research funding, and alleviate subjects' fear against clinical trials by thorough consultation.

Evaluation of two interforaminal implants and implant-assisted removable dentures on stress distribution: an in vitro study

  • Objectives: In mandibular edentulism, the treatment option with a two-interforaminal implant-retained bridge and a removable partial denture is rarely evaluated in literature. The aim of this in vitro study was to evaluate the stress distribution of this treatment option by comparing it with traditional treatment options with interforaminal implants in the edentulous mandible. Materials and Methods: Two interforaminal implants were placed in a formalin-fixed cadaver mandible, and overdentures with three different types of attachments were fabricated: (1) two ball attachments and an overdenture, (2) a Dolder bar and an overdenture, and (3) screw-retained two-implant inter-canine porcelain fused to a metal bridge and an implant-assisted removable denture (IARD) with precision attachments. Three biting conditions were generated for each denture type, and the strains were documented under vertical loading of 100 N. Results: The calculated strain values from measured strains in all measurement sites and loading conditions for the screw-retained two-implant intercanine porcelain fused to a metal bridge and a cast framework partial denture with precision attachments situation were lower than in the other scenarios (P<0.05). Conclusion: Within the limitations of the present study, it can be concluded that an IARD may be a reasonable and valuable alternative to ball attachments or a bar in two interforaminal implants, especially when the patients prefer to be able to show their teeth even when they take out their removable dentures.
