Adductor canal block versus intra-articular steroid and lidocaine injection for knee osteoarthritis: a randomized controlled study

  • Background: This study aimed to assess the efficacy of the adductor canal block (ACB) in comparison to intra-articular steroid-lidocaine injection (IASLI) to control chronic knee osteoarthritis (KOA) pain. Methods: A randomized, single-blinded trial in an outpatient rehabilitation clinic recruiting chronic KOA with pain ≥ 6 months over one year. Following randomization, subjects received either a single ACB or IASLI under ultrasound guidance. Numerical rating scale (NRS) scores for pain, and Knee Injury and Osteoarthritis Outcome Scores (KOOS) were recorded at baseline, 1 hour, 1 month, and 3 months postinjection. Results: Sixty-six knees were recruited; 2 were lost to follow-up. Age was normally distributed (P = 0.463), with more female subjects in both arms (P = 0.564). NRS scores improved significantly for both arms at 1 hour, with better pain scores for the IASLI arm (P = 0.416) at 1st month and ACB arm at 3rd month (P = 0.077) with larger effect size (Cohen's d = 1.085). Lower limb function improved significantly in the IASLI arm at 1 month; the ACB subjects showed greater functional improvement at 3 months (Cohen's d = 0.3, P = 0.346). Quality of life (QoL) improvement mirrored the functional scores whereby the IASLI group fared better at the 1st month (P = 0.071) but at the 3rd month the ACB group scored better (Cohen's d = 0.08, P = 0.710). Conclusions: ACB provides longer lasting analgesia which improves function and QoL in chronic KOA patients up to 3 months without any significant side effects.

정신과 환자의 낮병원 직업재활 프로그램 참여 경험에 관한 질적 연구 (Qualitative Study on Vocational Rehabilitation Program Participation Experience of Mentally Ill Patients in Psychiatric Day Hospital Care)

  • 본 연구는 일반적 질적 연구와 집단 포토보이스(G-Photovoice) 방법을 활용하여 정신과 환자의 낮병원 직업재활 프로그램 참여 경험을 연구하였다. 연구자는 낮병원 직업재활 프로그램 참여 경험이 있는 정신과 환자 5명을 대상으로 2회 이상 면담을 실시하였으며 낮병원 이용과 외래 진료 이후 추가 면담을 실시하였다. 더불어 사진을 활용한 반 구조화된 집단 면담을 진행하였다. 연구결과 연구 참여자들의 낮병원 직업재활 프로그램 참여 경험은 '직업 활동의 어려움을 경험함', '함께 일하는 것에 대한 즐거움', '안정감을 얻음', '자신이 성장했음을 느낌'으로 나타났다. 이러한 연구결과에 기반하여 본 연구에서는 낮병원 직업재활을 활성화하고 정신과 환자의 성장을 촉진시킬 수 있는 함의를 논의하였다.

재가 뇌졸중 환자 가족의 주간재활간호 서비스 요구와 관련요인 (The Need for Rehabilitation Day Care Program Service of Stroke Survivor's Family)

  • This study was carried out to find out the basic data required to plan and develop Rehabilitation Day Care Program for the stroke survivor's family in Korea. The subjects comprised of 92 stroke survivor's family who discharged from 4 hospitals in Seoul during the past 2 years. The data were collected from August 3, 1998 to September 18, 1998, through interviews with questionnaires about general characteristics, activities of daily living, depression and service need of rehabilitation day care program at the outpatient clinics by trained nursing graduates. Data were analyzed with descriptive analysis, Pearson's correlation analysis, and Stepwise multiple linear regression analysis using SPSS/WIN 10.0 program. The results obtained are as follows; 1. The mean score of the general need of rehabilitation day care program of stroke survivor's family was 3.10(range 1-4). The highest need among the service categories of the rehabilitation day card program was self-care and restorative activities category(3.30), and health services referral category, recreation category, psychosocial activities category in order. The needs of each category are as follows. In the health services referral category, the need for dental examination and medical examination were highest, followed by the need for physical therapy and occupational therapy. In the psychosocial activities category, the need for family counselling was highest. In the self-care and restorative activities category, the need for ROM exercise training was highest, followed by bowel training, and ambulation training. 2. The need of family for rehabilitation day care program service displayed a correlation with the level of education, ADL, and the level of depression, and a reverse correlation with age, illness intrusiveness, depression, knowledge, subject and object burden and relationship with stroke survivors. 3. The stepwise multiple linear regression analysis revealed following results. For the need for rehabilitation day care program service, 22.6% of the variance was initially explained by level of family's knowledge about caring method for stroke survivors, 8.8% was the level of subjective burden and 5.4% was relationship with stroke survivors. In conclusion, above characteristics should be considered to develop stroke survivors' rehabilitation day care program.

관상동맥질환자를 위한 타이치 적용 심장재활 프로그램이 심혈관 위험요소, 심혈관질환 재발 위험도 및 건강 관련 삶의 질에 미치는 효과 (Effects of Tai Chi Exercises on Cardiovascular Risks, Recurrence Risk, and Quality of Life in Patients with Coronary Artery Disease)

  • Purpose: The study aims to evaluate the effects of Tai Chi applied cardiac rehabilitation program(TCCRP) on cardiovascular risks, recurrence risk in ten years, and cardiac specific quality of life in individuals with coronary artery disease. Methods: The sample was comprised of individuals diagnosed with coronary artery disease within six months of the study who were referred by their primary physicians to participate in the TCCRP. The design was a pretest/posttest with non-equivalent groups with 30 in TCCRP program and 33 wait-listed comparison group. Results: The average age of all participants was sixty seven years. At the completion of the TCCRP, the Tai Chi group showed significant reduction in their ten year recurrent risk for coronary artery disease measured by Framingham's algorithm. The quality of life for the experimental group was reported as significantly higher than the comparison group, especially the area of general symptom. Conclusion: The outpatient cardiac rehabilitation with Tai Chi was applied effectively and safely without any complication to individuals with coronary artery disease. Tai Chi can be useful as an alternative exercise for cardiac rehabilitation program which may provide more access to individuals for cardiovascular risk management in the community settings.

장애인의 의료기관 접근성 조사: 서울시 종로구 병의원을 대상으로 (Accessibility of the disabled to Health Care Institution : A Case Study of Chongno-Gu in Seoul)

  • The purpose of this study was to assess adequately equipped with convenience facilities for the disabled in 160 healthcare institutions in Jongno district, Seoul. Healthcare institutions were equipped an average of 3.7 facilities out of 10. General hospitals had an average of 5.0 facilities, which was higher than an average of 3.6 facilities for private clinics (p<0.05). Of 160 healthcare institutions, only 13 (8.1%) offered easy access to the outpatient setting from the entrance for wheelchair users, highlighting difficult wheelchair access within hospitals. To provide easy access to medical service for the disabled, more accessible designs need to be adopted as part of the effort to improve public facilities for the disabled. Also, universal designs could be applied for newly constructed roads, structures and transportation vehicles to maximize accessibility for the disabled. Increased accessibility for the disabled in the community will eventually increase the use of healthcare institutions.

Evaluation of a Smart After-Care Program for Patients with Lung Cancer: A Prospective, Single-Arm Pilot Study

  • Background: The efficacy of telemedicine among cancer survivors is uncertain. The Smart After-Care Program (SAP), which is an interactive, smartphone-based remote health monitoring system, was developed to help patients manage their health after leaving the hospital. This study was designed to evaluate the efficacy of our remote health care program for lung cancer patients. Methods: We enrolled 50 patients with lung cancer. Self-monitoring devices were supplied to all patients, who were instructed to enter their daily vital signs and subjective symptoms to the Smart After-Care app. The app also provided information about rehabilitation exercises and a healthy diet for lung cancer patients. All patients received health counseling via telephone once a week and visited an outpatient clinic during weeks 6 and 12 to assess satisfaction with the SAP and changes in quality of life and physical performance. Results: Overall satisfaction with the SAP was very high (very good, 61.9%; good, 26.2%). In the multivariate analysis to identify factors affecting satisfaction, the distance between the patient's residence and the hospital was the only significant independent factor (p=0.013). Quality of life improved along all functional scales (p<0.05). Muscle strength significantly improved in the lower limbs (p=0.012). Two-minute walk distance also significantly improved (p=0.028). Conclusion: This study demonstrated that the SAP was acceptable for and supportive of patients with reduced pulmonary function after lung cancer treatment. The SAP was found to be particularly useful for patients living far from the hospital.

  • Purpose : This study was conducted to investigate correlation between patient satisfaction and rehabilitation motivation in a physical therapy environment. Methods : This study conducted a survey on musculoskeletal and neurological patients receiving rehabilitation treatment at a hospital. The subjects of the study were patients who were currently receiving rehabilitation treatment, either hospitalized or outpatient. 234 people were collected. A questionnaire was consisted of a total of 55 questions, including 27 questions about motivation for rehabilitation, 14 questions about physical therapy service environment, and 14 questions about patient satisfaction and intention to revisit. The detailed items in rehabilitation motivation consisted of 8 questions about task-oriented motivation, 7 questions about change-oriented motivation, 4 questions about obligatory motivation, 4 questions about external motivation, and 4 questions about intrinsic motivation, and in the physical therapy service environment, 4 questions about facility service and therapist service. , 6 questions, 4 questions about services used, 3 questions about friendliness, 4 questions about professionalism, 3 questions about treatment satisfaction, and 2 questions each about repeat visit and recommendation. Results : Facility service (r=.21) was highly correlated for task-oriented motivation, therapist service (r=.22) for change-oriented motivation, therapist service (r=.31) for mandatory motivation, therapist service (r=.19) for external motivation, and facility service (r=.56) for internal motivation. Revisit for task-oriented motivation (r=.47) is kind to change-oriented motives (r=-.13) was highly correlated with kindness (r=.19) for mandatory motives, recommendation (r=.14) for external motives, and expertise (r=.52) for internal motives. There was a high correlation between professionalism (r=.61) for facility services and kindness (r=.53) for therapist services, and revisit (r=.40) for service use. Conclusion : According to the results of this study, it was found that there was a correlation between patient satisfaction and rehabilitation motivation in a physical therapy environment.

당뇨여성의 성기능 장애 조사연구 (A Study on the Sexual Dysfunction for Diabetic Women)

  • Purpose: The purpose of this study was to investigate the sexual dysfunction of diabetic women in order to provide basic data contributed in nursing intervention. Method: A convenience sample consisted of 67 subjects with diabetes who attended outpatient department of 2 university hospitals. After verbal consent was obtained, subjects were asked to complete a questionnaire including BISF-W developed by Taylor et al. Data were collected from April 27 to May 22, 2006, and were analyzed by descriptive statistics, t-test, oneway ANOVA using SPSS 12.0 program. Results: This study discovered that the degree of sexual dysfunction of women with diabetes was high, and in particular that most of the women with diabetes did not have satisfactory sex life because of lack of vaginal lubrication, lower sexual desire, lower orgasm, and sexual pain. The test of differences in sexual dysfunction according to demographic and disease-related characteristics revealed that significant differences existed only with regard to menopause and age. Conclusion: As the sexual function of women varies widely among individuals, we would like to emphasize the importance of sexual counseling and education programs to improve the quality of life of diabetic women to prevent or relieve their sexual dysfunction.

일 호스피스실 이용 환자와 가족의 상담내용 분석 (Content Analysis with Counseling Cancer Patients and Their Relatives in Outpatient Hospice Office)

  • Purpose: The purpose of this study was to increase our understanding of the terminal cancer patients and their families concerns. I analyzed the counseling contents of terminal cancer patients and their relatives who referred to hospice office. Method: Data was collected from January 2004 to November 2004. During the counseling, I took notes the key points and contents. 109 patients and/or their families's counseling records were analyzed with the descriptive statistics and content analysis. Result: 73.4% of patients knew their current terminally ill status. The mobility of 86.2% of the patients was worse than ECOG 3 level. Patients have uncontrolled pain(28.4%), emotional distress(55.0%), and physical distress(49.5%). Caregivers of the patients were spouse(46.8%), sons and daughters(24.8%). Family members had problems to tell the bad news to their family cancer patient. 95.4% of the patient and family members was informed about the hospice and palliative care services, and 35.8% of them was referred to the hospice and palliative organizations. Frequently asked questions were 'what is hospice?', 'how much is the cost of hospice services?' etc. Conclusion: Care planning for terminal cancer patients must include patient. Also, professionals should consider and offer accessible, effective and empathetic counseling services to patients and families.

근위축성 측삭경화증 환자의 삶의 질 (Quality of Life in Patients with Amyotrophic Lateral Sclerosis)

  • Purpose: This study measured the quality of life in patients with amyotrophic lateral sclerosis (ALS). Methods: The participants consisted of consecutive patients with ALS who visited the neurology outpatient department from January to July, 2008. To collect the data, face-to-face interviews were applied at S national university hospital in Seoul. Using multivariate analysis, factors to predict QOL in ALS patients including demographic factors, functional independence, depression, anxiety, social support and quality of life were analyzed. Results: The final analysis included 69 patients. The mean score for the physical health and mental health components was $34.4{\pm}21.3$ and $44.7{\pm}20.6$, respectively. The mean score for the ALS functional rating scale was $24.3{\pm}10.8$ out of 40. Anxious and depressed patients accounted for 44.9% and 71%, respectively. Quality of life in ALS patients was significantly affected by functional independence, depression and anxiety level. Conclusion: To improve the quality of life in ALS patients with declining functional independence, comprehensive interventions are necessary to manage depression and anxiety.
