Purpose: This study was done to develop a prediction model for postpartum depression by verifying the mediation effect of antepartum depression. A hypothesized model was developed based on literature reviews and predictors of postpartum depression by Beck. Methods: Data were collected from 186 pregnant women who had a gestation period of more than 32 weeks and were patients at a maternity hospital, two obstetrics and gynecology specialized hospitals, or the outpatient clinic of K medical center. Data were analysed with descriptive statistics, correlation and exploratory factor analysis using the SPSS/WIN 18.0 and AMOS 18.0 programs. Results: The final modified model had good fit indices. Parenting stress, antepartum depression and postpartum family support had statistically significant effects on postpartum depression, and defined 74.7% of total explained variance of postpartum depression. Antepartum depression had significant mediation effects on postpartum depression from stress in pregnancy and self-esteem. Conclusion: The results of this study suggest that it is important to develop nursing interventions including strategies to reduce parenting stress and improve postpartum family support in order to prevent postpartum depression. Especially, it is necessary to detect and treat antepartum depression early to prevent postpartum depression as antepartum depression can affect postpartum depression by mediating antepartum factors.
Purpose: The purpose of this study was to investigate the influencing factors on Posttraumatic Growth(PTG) in patients with colorectal cancer. Methods: Eighty patients who were diagnosed with colorectal cancer at least twelve months ago were recruited from the oncology outpatient clinic of university hospital in Y city. Participants completed four survey questionnaires: Korean versions of PTG Index, Korean versions of Cancer Coping Questionnaire, Multidimensional Scale of Perceived Social Support, and a Resilience Scale. The data were analyzed by ANOVA, Pearson-correlation coefficient, and multiple regression analysis. Results: The mean score of PTG in these patients was 51.16 indicating relatively high growth. Posttraumatic Growth had significant correlations with coping, social support, and resiliency. In addition to the variables of age and education that are found to be associated with the PTG, the PTG was significantly influenced by resilience, coping, and importance of religion. Forty-nine percent of the variations in the PTG were explained by these three variables. Conclusion: The identified factors influencing colorectal cancer related to PTG could be considered in developing nursing interventions to promote positive psychological changes in response to adversity which colorectal cancer survivors might experience.
Purpose: The objective of this study was to identify factors related to self-care adherence in symptomatic patients with heart failure (HF). Methods: Using a cross-sectional design, a convenience sample 209 outpatient clinic patients were recruited at two medical centers. Between October 2011 and August 2012, data were collected using the structured questionnaire. Factors related to self-care adherence were examined using hierarchical multiple regression. Results: Mean age of participants was 67.71 years and a half of them (53.6%) were female. They showed relatively low self-care adherence with mean scores of $61.88{\pm}12.92$. Lower self-care adherence was reported in asking for low sodium items, weighing oneself, checking for ankle edema, and exercising for 30 minutes. The overall model significantly explained 23.9% of variance in self-care adherence. Among the predictors, education, New York Heart Association functional classification, and social support were statistically significant in influencing self-care adherence. The variable of negative emotional status such as anxiety and depression were not found to be significant. Conclusion: These findings demonstrate that social support could help self-care adherence among symptomatic patients with HF. Thus, programs targeting self-care adherence in this population should consider the strategies improving social support.
48세 여자 환자가 좌측 팔의 통증으로 1개월간 한의원에서 침술치료를 받던 중 좌측 경부의 부종과 통증으로 내원하였다. 컴퓨터 촬영상 좌측 내경정맥 혈전증이 발견되었다. 환자는 항생제와 함께 항응고 치료를 받고는 증세 호전이 되어 현재 통원 치료 중이다. 침술치료에 따른 내경정맥 혈전증은 아직 증례보고가 없는 바 이에 문헌고찰과 함께 보고하는 바이다.
Purpose: The purpose of this study was to identify the factors influencing symptom self management (SSM) in the patient with a chronic mental disorder. Method: Data was collected by questionnaires from 204 chronic mental disorders in an outpatient clinic in a General Hospital and Public Mental Health Centers in Seoul and Kyunggi Province. The data was analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression, Results: The score of SSM showed a significantly positive correlation with the score of the level of self efficacy (SE) (r=.33, p=.00), social support (SS) (r=.27, p=.00), self care agency (SCA) (r=.36, p=.00) and daily living ability (DLA) (r=.34, p=.00). The score of DLA showed a significantly positive correlation with the score of level of SE (r=.46, p=.00), SS (r=.51, p=.00), and SCA (r=.52, p=.00), The most powerful predictor of SSM was SCA ($14\%$). A combination of SCA, DLA, and SS account for $20 \%$ of the variance in SSM. Conclusion: This study suggests that SCA, DLA, and SS are significant influencing factors on SSM in patients with chronic mental disorders.
Background: We aimed to investigate the relationship between blood groups and pancreatic cancer in a Turkish population in Western Blacksea region. Methods: This is a retrospective study. Zonguldak Karaelmas University outpatient oncology clinic records were screened for the period between 2004 and 2011. Results: The median age of patients were 56 (${\pm}16$) and 132 of 633 study population had pancreatic cancer. Pancreatic cancer patients had significantly higher rates of blood group A compared to controls (OR 1.8, 95%CI, p 0.005). Rates of blood group AB was significantly lower than the control group (OR 0.37, 95% CI, p 0.04). The median survival (IR) time in subjects having the blood groups A, B, AB and O were 7.0 (1-28), 7.0 (2-38), 10 (2-36) and 9.0 (2-48) months respectively; the blood group 0 had significantly higher overall survival (OS) compared to the non-0 groups (p 0.04). Conclusions: Pancreatic cancer patients had more common blood group A in our population. Moreover, blood group AB appeared to be a protective factor against pancreatic cancer in our population. Blood group 0 had a significantly longer survival compared to non-0, regardless of prognostic factors.
Purpose: To examine the effects of a case management program on self-efficacy, depression and anxiety in pregnant women with gestational diabetes mellitus. Methods: Thirty-seven patients who enrolled in a diabetes outpatient clinic were randomly assigned to either an experimental group (n=19) or a control group (n=18). The experimental group received a 2-week intervention composed of a series of one face-to-face interactive interview and five telephone interviews based on the National standards for diabetes self-management education and Bandura's self-efficacy resources of performance accomplishment, vicarious experience, verbal persuasion, and emotional arousal. The effects of the intervention were assessed by completion of a 9-item self-efficacy questionnaire, a 20-item depression questionnaire and a 20-item anxiety questionnaire prior to, and after the intervention. The statistical significances were examined using t-test. Results: The change in self-efficacy was significantly greater in the experimental group than in the control group. Depression and anxiety changes in the experimental group were significantly greater compared to those in the control group. Conclusion: The results of study suggest that a case management program improves self-efficacy, and decreases depression and anxiety in pregnant women with gestational diabetes mellitus.
Purpose: To explore what particular types of information were important to patients diagnosed with cancer. Methods: Seventy three patients with cancer at an outpatient clinic and hospitalized patients in W Christian Hospital Korea, responded. The structured questionnaire developed by the investigator based on previous studies. Results: There was a significant negative relationship between age and the score of informational need (r=-.307, p<.05). Level of education, and level of monthly income were related to level of informational need. The top three informational priorities according to the time since diagnosis were 'Self care during treatment', 'Health food and diet', 'Likelihood of recurrence', 'Follow up care' and 'Side effects'. The top three informational priorities for patients with breast cancer were 'Likelihood of recurrence', 'Metastasis possibility', 'Treatment options', and 'Side effects. For patients with stomach cancer, they were 'Follow up care', 'Healthy food and diet', 'Likelihood of recurrence', and 'Metastasis possibility', and for patients with colon/rectal cancer, they were 'Side effects', 'Healthy food and diet', 'Likelihood of recurrence', and 'Self care during treatment'. Conclusion: The assessment of information needs based on demographic factors and disease-related factors is critical in helping patients with cancer to manage their illness.
Purpose: As a result of dramatic advances in the medical and surgical management of congenital heart disease (CHD), many babies born with cardiac anomalies today can expect to reach adulthood. The main purpose of this study was to examine the relationship between parenting stress and personal resources of mothers of children with CHD. Method: Fifty-one mothers of children with CHD were recruited at the pediatric cardiac outpatient clinic from July 14th to September 25th 2006. Abidin's parenting stress index/short form (PSI/SF) and Brandt and Weinert's personal resource questionnaire (PRQ) were used. PSI has 3 sub-concepts; parental role distress, dysfunctional parent-child interaction, and difficult child. PRQ has 4 sub-concepts; intimacy, social integration, worth, and assistance. Data were analyzed using SPSS 13.0 version. Results: Correlation analysis showed that parenting stress was significantly related to 'intimacy', 'social integration', and 'worth' of mothers. Multiple regression analysis showed that parenting stress was significantly related to personal resource of mother and information by internet. Conclusion: Mothers who felt they had supportive friends and family, high self esteem, and social integration reported lower parenting stress. Also, internet may be an effective method to provide information and share experience for mothers of children with CHD.
The present study aimed to investigate the possible association of Toxoplasma gondii and Toxocara spp. infections with cryptogenic epilepsy in children. The study was carried out between June 2014 and March 2015. Total 90 children (40 with cryptogenic epilepsy, 30 with non-cryptogenic epilepsy, and 20 healthy control children) were evaluated to determine the anti-Toxocara and anti-T. gondii IgG seropositivity using ELISA kits. Epileptic cases were selected from those attending the pediatrics outpatient clinic of Benha University Hospital, Pediatrics Neurology Unit, and from Benha Specialized Hospital of children. The results showed that the level of anti-T. gondii IgG seropositivity was significantly higher among children with cryptogenic epilepsy (20%) than among children with non-cryptogenic children (0%). In healthy controls (10%), there was no association between toxocariasis seropositivity and cryptogenic epilepsy (only 5.7%; 4 out of 70 cases) among cases and 10% (2 out of 20) among controls. Among toxocariasis IgG positive cases, 3 (7.5%) were cryptogenic, and only 1 (3.3%) was non-cryptogenic. These statistically significant results support the association between T. gondii infection and cryptogenic epilepsy while deny this association with toxocariasis.
