의료이용 전.후 기대와 만족수준 비교 (Consumer expectation and consumer satisfaction before and after health care service)

  • The purpose of this study is to analyze the consumer's expectation before the health care service and the consumer's satisfaction after it. The participants of the study are inpatients in a general hospital located in Seoul. The resources were collected from the self-administration questionnaire survey run parallel with face to face interview. In order to measure the degree of the consumer's expectation, 349 samples were collected from the first questionnaire survey on the date of admission to the hospital. The second questionnaire survey was carried out on the date of discharge to the hospital with the participants responding to the first questionnaire survey. There are 154 samples collected from this survey. The results from the analysis of these resources are as follow. First, the survey shows that one of the highest consumers' expectations was about the generosity, kindliness and sincerity from the staff at the hospital, specially from doctors. Second, according to the analysis of the factors affecting the expectations of the consumers, with regard to path of admission to a hospital relating to patient's features, outpatient who gets into a hospital expected good medical care much more than the other patients. In regard of doctor's features, patients usually and highly expect good medical care from doctors who have good carrier and much experience. Third, according to the second questionnaire survey, what patients are satisfied most with is about the generosity and sincerity from staff at a hospital, especially from doctors and their gem attitudes. The results from survey show that the differences among the degree of consumers' satisfaction are very variable, depending on surrounding environments and facilities. The only fact that expectation didn't meet with satisfaction appeared to the case about technology and skill of medical care and the case about updated medical skills and equipments. Fourth, comparing the degree of expectation with the degree of satisfaction of consumers, correlative analysis was concerned significantly and specifically about the part of overall cleanliness relating to facilities and surrounding environments, the items about medical examination and test plan procedure relating to skill of medical care, professional specialties and convenience for procedure, and the items about satisfying explanations and concern about patients from doctors relating to staff's generosity and sincerity. Fifth, the analysis of the factors affecting the degree of how much patients are satisfied with shows that relating to sociodemographical features, patients are not satisfied with the case when the time and process of medical treatment are getting longer. It is surveyed that consumer were satisfied with the motivation to visit a hospital and the insurance type in patient's feature and so were the medical department and the factor of the degree of the expectation in disease's feature. Sixth, according to analysis based on the survey, patients would join again a hospital when they get satisfaction from the medical care and also they want to come again regarding to doctor's capability. For example, when doctors are old, have a good carrier and much experience, patients would come again. As seen from the above, consumers are usually satisfied with the medical treatment more than that they expected before. They would intend to use again when they get satisfaction from the medical care provided at a hospital. Patients and consumers highly expect good attitude as well as capacity from medical doctors and they are also generally satisfied with those things. Therefore, in order to increase the degree of consumer's satisfaction and their intention to come again, the hospital staff would have to commit themselves to achieve high quality service continuously and would have to make an effort to offer the finest quality service.

여성 골관절염 환자의 일상생활 수행능력 (A Study on Degree of Daily Living Activities among Women with Osteoarthritis)

  • This study purports to examine sociodemographic characteristics and those factors associated with osteoarthritis in women experiencing the disease in order to make suggestions for the prevention and management of osteoarthritis in women. The subjects were 143 women who were diagnosed to have osteoarthritis and under follow-up care at an outpatient department of a university-affiliated hospital. The data were collected through personal interviews by using the structured questionnaire between October 6 and October 24, 1997. The degree of daily living activities was measured with 24 items, which were made based upon Katz Index, Barthel Index and Functional Status Index. Each item has 4 response categories : (1) being able to do alone with any difficulty, (2) being able to do alone with some difficulty, (3) being able to do alone with the help of a person or an instrument, (4) not being able to do at all. The data was analyzed with correlation analysis, t-test, ANOVA, and regression analysis. The results are summarized as follows : 1. As for the age of the subjects, 59.4% were between 45 and 64 years, while 28% were 65 years and above. The subjects having religion were 71.7% and 44.1% of the subjects were protestants. The educational level was low : 31.5% graduated from elementary school, while 30.1% had no formal education. Those married subjects were 72%. Fifty nine percent of the subjects evaluated their economic status as middle class. 2. Those people from whom the subjects currently receive some help were spouses in 35.8% of the subjects. Likewise, 50% of the subjects indicated spouse as the people from whom they want to receive help. 3. The score of the ability of daily living activities ranged between 48 and 96, with the mean of 78.94. Those means of physical activity, ADL(activities of daily living) and IADL(instrumental activities of daily living) were 14.89, 13.97 and 50.09, respectively. Except for the items of carrying heavy stuff and of washing clothes in the sitting position the subjects showed independence in more than 90% of the items, where Independence is defined as being able to do alone or with the help of a person or an instrument. 4. The increase in age was a significant predictor of the decrease in the ability of daily living activities. Those graduated from middle school and above showed a significantly higher degree in the ability of daily living activities than those with less educational level. The married women revealed significantly higher scores in physical activity, ADL, and IADL than the unmarried ones. The ability of daily living activities was not significantly related to having religion, economic status and living with family. 5. The average duration of experiencing osteoarthritis was 4 years and 7 months. Regarding the site of the onset, 65% of the subjects indicated knees. The women with osteoarthritis for less than 3 years were 65%. The ability of daily living activities was significantly different by the duration of illness : the longer the duration of illness, the less the ability of activities. The above findings suggest the need for developing those programs for prevention disabilities in performing daily living activities and for managing diseases targeting the women of middle and old age, with low educational level, and with unmarried status. In addition, there should be developed an instrument for measuring the ability of daily living activities which reflects daily lives of Korean women with osteoarthritis in order to examine comprehensive effects of osteoarthritis on women's daily lives in this country.

항 백혈병작용에 관련된 천연물의 자료조사 (Review of Anti-Leukemia Effects from Medicinal Plants)

  • 인삼, 호장근, 상산 등에서 분리한 성분들이 HL60, HL-60, Jurkat, Molt-4에 대한 억제작용이 있는 것으로 조사되었고, 익모초의 Leonunrine, 대청엽의 Indirubin, 천문동의 Aspargus polysaccharideA.B.C.D, 백합의 Colchicnamile, 익모초의 Lenunrine, 산두근, 자초근 추출물이 여러유형의 백혈병 환자에 대한 백혈병 억제효과가 있는 것으로 조사되었으며, mouse의 P388, L1210, L615, L120, S-180 등에 항 백혈병 효과가 있는 것으로는 완화, 로회, 원지, 오수유, 파두, 뇌공등, 석산, 백출, 단삼, 산약, 목단피, 청대, 감초, 당귀에서 분리한 성분들이 있으며 백굴채, 마전자, 가시오가피, 천초 추출물들이 동물실험에서 항암작용이 있는 것으로 조사되었다. 또 천연물에서 분리한 성분이 항백혈병 작용이 있는것으로는 ginsenoside Ro, ginseonoside Rh2, Emodin, Yuanhuacine, Aleemodin, phorbocdiester, Triptolide, Homolycorine, Atractylol, Colchicnamile, Paeonol, 당귀다당체, Aspargus polysaccharideABCD, Indirubin, Leonunrine, Acinosohic acid, Trichosanthin, G2 132, Schizandrin, allicin, Indirubin, cmdiumlactone chuanxiongol, 18A glycyrrhetic acid, Kansuiphorin A, 13 oxyingenol Kansuiphorin B 등이 조사되었고, 추출물이 항 백혈병 작용이 있는 것으로는 원지, 오수유, 백굴채, 대황, 산두근, 마전자, 가시오가피, 천초 등이 조사되었다.

당뇨병 환자에서 우울증상 및 불안증상과 혈당조절과의 연관성 (The Relationship of Anxiety Symptoms and Depressive Symptoms to Glycemic Control in Diabetic Patients)

  • 연구목적 : 당뇨병 환자들은 질환 자체에 대한 부담감과 혈당 관리를 위한 약물 및 식이요법과 운동 요법 등의 유지에 대한 부담감이 스트레스로 작용하고 있으며, 이러한 심리적 스트레스가 혈당조절에 악영향을 줄 수 있다. 당뇨병 환자들은 일반 인구에 비해 우울증 및 불안장애의 유병율이 높으며, 이러한 우울증과 불안장애의 증상은 또 다시 혈당조절에 부정적인 영향을 주게 된다. 이에 본 연구는 우울과 불안증상이 혈당조절에 어떤 영향을 미치는가를 알아보고자 하였다. 방법 : 단국대학교병원 내분비내과에서 외래통원치료 중인 30세 이상의 당뇨병 환자 65명을 대상으로 하여, 정맥 혈액채취로 당화혈색소(Hemoglobin A1c;이하 HbA1c) 수치를 얻었으며, 자가보고식 설문지인 Beck의 우울척도(Beck's Depression Inventory;이하 BDI) 및 Beck의 불안척도(Beck's Anxiety Inventory;이하 BAI)를 시행하여 통계분석을 하였다. 결과 : 65명의 당뇨병 환자 중 경한 우울증상을 보인 환자는 27명(41.5%), 중등도 이상의 우울증상을 보인 환자는 6명으로(9.23%) 일반 인구에 비해 높은 빈도를 보였다. HbA1c와 BDI간에는 유의한 상관관계를 보이지 않았으나 HbA1c와 BAI간에 유의한 상관관계를 보였으며(R=0.567, P<0.001), 다중회귀분석에서 BAI는 혈당조절에 영향을 미치고 있었으나($\beta=0.551$, T=4.912, P<0.001) BDI와 체질량지수, 당뇨병 합병증의 수 및 유병기간은 HbA1c에 유의한 영향을 미치지 않았다. 결론 : 본 연구에서는 일반 인구에 비해 우울증상을 보이는 당뇨병 환자의 빈도가 높았으며, 불안증상을 보이는 당뇨병 환자의 빈도는 높지 않았다. 당뇨병 환자에서 우울증상과 혈당조절간의 유의한 연관성을 보이지 않았으나 불안증상과 혈당조절 간에는 유의한 연관성을 보였다. 결론적으로 우울증상은 혈당조절에 별 영향을 미치지 않으나 불안증상은 혈당조절에 영향을 미치는 것으로 나타났다.

일개 도시·농촌 통합지역 주민의 보건기관 이용경험과 보건서비스 요구도 조사 (A Survey on Utilization of Health Center and Health Service Demand of Residents in a Urban and Rural Unified Community)

  • 본 조사는 행정구역 통합과 보건기관 시설 및 장비 개선이라는 변화를 겪고 있는 일개 통합 시 지역주민의 보건기관 이용경험과 보건서비스 요구도를 파악하여 지역사회에 적합한 보건사업계획에 필요한 자료를 얻고자 실시하였다. 조사 대상은 1995년 9월 현재 통합 구미시에 거주하는 86,305가구(284,775명) 중 읍 면 동 직원이 가정 방문하여 설문지로 방문 시 면접 가능했던 가구주 혹은 주부와 면담 조사했다. 분석 대상은 결측치가 있는 설문을 제외한 통합 전 도시 지역 67,607가구(220,187명) 중 3,337가구(4.9%)와 통합 전 농촌지역 18,698가구(64,588명) 중 690가구(3.7%)로 총 4,027가구(전체 가구의 4,6%)였다. 조사내용은 응답자의 건강관련 인구학적 특성과 응답자의 보건소 이용경험, 보건서비스에 대한 요구도, 보건지소 및 보건진료소의 필요성 등이다. 응답자의 성 연령 구성, 의료보장, 현주소지에 거주기간, 교육수준 등 건강관련 인구학적 특성에 대해 통합 전 도시와 농촌 지역간에 유의한 차이가 있었다. 급성질환 및 만성질환 유부 빈도는 농촌에서 모두 높았으며 급 만성질환 시 보건기관 이용률도 농촌지역에서 높았다. 과거 한번 이상 보건소를 이용한 경험률은 도시지역주민 64.8%, 농촌지역주민 55.6%이고, 이용한 보건의료서비스는 도시지역은 예방접종, 농촌지역은 일반진료가 가장 많고, 보건소 사업 중 가장 강화되기 바라는 사업은 양쪽 지역 모두 전염병 예방사업이었다. 순회진료 및 가정간호서비스에 대한 찬성률은 90%이상이고 보건지소와 보건진료소에 기대하는 사업은 노인을 중심으로 건강상담이 가장 많았다. 농촌지역에서는 보건지소에 대하여 90.3%가 필요한 것으로, 보건진료소에 대하여 88.3%가 필요하다고 답하였고, 도시지역에서는 주민의 86.0%가 도시 보건지소 설치가 필요하다고 답하였다. 위의 결과에 따르면 지역주민들은 보건기관이 지속적으로 필요하다고 생각하나 그 주 기능은 기존의 전염병관리사업으로 인식하고 있다. 도시지역은 예방접종을, 농촌지역은 일반진료를 위해 보건기관을 가장 많이 이용하고 있으며 보건교육, 건강상담, 순회진료, 가정간호서비스 등을 많이 기대하고 있다. 따라서 지역주민의 보건기관 이용률을 높이기 위해서는 도시지역과 농촌지역의 보건관련 인구학적 특성과 주민의 요구에 적합한 보건사업을 개발하여 추진하고 보건사업의 질적 향상을 꾀하여야겠다.
