Application of Homograft in the Surgical Correction of Complex Congenital Cardiac Malformations (선천성 복잡 심기형 환자의 외과적 교정술시 동종이식편의 적용에 관한 연구)

  • We have been used cryopreserved homograft valves for right ventricular outflow tract[RVOT reconstruction since November 1993. The homograft valves were harvested from the hearts of brain dead patients or hearts of transplant recipients. There were 12 male and 10 female patients. Their ages ranged from 5 months to 13 years[mean age,39.2 $\pm$ 37.4 months and the weight ranged from 5 to 48kg [mean weight, 13.7$\pm$ 9. l kg . The diagnoses included pulmonary atresia with ventricular septal defect [n=14 , tetralogy of Fallot[n=4 , truncus arteriosus[n=3 , and double outlet right ventricle with pulmonic stenosis[n=l .Monocuspid homograft patches were used for RVOT widening or REV[reparation l`etage ventriculaire operations in 4 patients. We also used homograft as valved conduits for RVOT reconstruction in 17 patients and left ventricular outflow tract reconstruction in anatomically corrected transposition in 1 patient. Among them size-reducing technique [converting a tricuspid valved conduit into a bicuspid valved conduit were applied to six patients for the correction of size mismatching. The mean follow-up period was 10.6 $\pm$ 5.4 months. There was one operative death[4.5% due to bleeding and one reoperation for removal of vegetation on the homograft leaflet. Postoperative echocardiography documented no significant homograft insufficiency and RVOT obstructions.In short-term, the homograft valves provide excellent hemodynamic characteristics, even though further studies are necessary to evaluate the long-term results.

  • This paper presents experimental results carried out on the purge characteristic of the anodic dead-end mode fuelcell and how to improve the cell performance by pulsation effects. The dead-end mode fuelcell has some merits that a fuel supplying device is not needed and the cell power is higher than that in the open mode fuelcell. However, the purge is necessary for preventing the porous media from being flooded by liquid water formed in the channel. At this time, the un-reacted fuel is discharged with the liquid water together in purge process. The discharged fuel can make the fuel efficiency lower. Therefore, the number of purge times should be decreased for the better fuel efficiency. In this study, the outlet of the anode channel was equipped with a purge solenoid valve and a pulsation generator. The purge times was decreased when the current density decreased and operation pressure increased without the pulsation effects. In addition, when the pulsation effects such as various frequencies or amplitudes were applied, purge times was alleviated up to 40%.

  • This study investigates the flow characteristics of fuel droplets between twin spray for the 4-hole injector used a 4-valve gasoline engine. The injectors for this study were the three types of 4-hole gasoline injector in which orifice diameter was 0.24mm. The spray behavior of twin spray was investigated by means of visualization employed stroboscope. A PDPA system was employed to simultaneously measure the size and velocity of fuel droplets. The 3 dimensional mean velocities. droplet size distributions, SMD and joint probability density function of velocity and droplet size are analyzed at the center of the spray and the center region of twin spray. As a result, the configurations of injector exit such as orifice interval and length of outlet, are very important factors that affect the flow characteristics of fuel droplets at the center region of twin spray.

Temperature Control of Oil Cooler with Hot-gas Bypass (토출가스 바이패스제어에 의한 산업용 냉각기의 온도제어)

  • This paper presents precise temperature control of oil outlet in an oil cooler with hot-gas bypass control as an industrial refrigerator. The control system was designed for obtaining precise temperature control performance even though abrupt disturbances based on flow rate control of hot-gas bypass. PID controller was adopted in feedback control system. We showed that the gain of PID could be easily determined by using gain-tuning methods without any numerical model. Through some experiments, excellent control performances such as overshoot within 1.7%, steady state temperature error within ${\pm}0.1^{\circ}C$ were established by a simple PI controller. We expect that the system can control the target temperature within error of $0.33^{\circ}C$ under abrupt disturbances.

A Study on the Structural Safety Evaluation of Light Weight Damper for Offshore Rigs (해양시추선용 경량수밀댐퍼의 구조안전성 평가에 관한 연구)

  • In this study, The watertight damper was designed to improve conventional DN 350A butterfly valve. The FSI(Fluid-Structure Interaction) analysis has performed to investigate the safety factor for the watertight damper. When watertight damper of disk was closed, the disk of pressure value is constant. However depending on the opening angle of disk, the flow velocity and pressure are changed. The maximum velocity was appeared at the end of disk on the small outlet area of duct. When the opening angel of disk is $90^{\circ}$, the maximum velocity was appeared at the center of ending disk. So we were found the opening angle of disk is bigger, the flow rate is increased and velocity is also increased from the result of FSI analysis. We can find the least deformation and stress when the opening angel of damper is $90^{\circ}$. When the $45^{\circ}$ opening angle of disk, the largest deformation and stress was found and the minimum safety factor 1.3 was calculated. As a result, we found that the structure of watertight damper is safe enough irrespective of opening angel.

  • Coarctation of aorta, a well known congenital cardiovascular defect, can be recognized in the most instances by simple finding of physical examination. This condition shortens life if untreated, but it can be corrected surgically to render the patient functionally normal. It seems relatively rare in Asian. During Six-Year period from January, 1982 through June, 1988, twenty four consecutive operations for the coarctation of the aorta were performed at Yonsei Medical Center. The patients included 14 males and 10 females in the range, 2 months and 34 years old. Associated Cardiac anomalies were patient ductus arteriosus, ventricular septal defect, mitral valve regurgitation, aortic stenosis, double outlet right ventricle, corrected transposition of great vessel, etc. The preoperative main symptoms were frequent URI and dyspnea. Congestive heart failure was the most common symptom at the group younger than 2 year olds. Operative techniques for the coarctation of the aorta were prosthetic patch aortoplasty in 18 patients, resection & vascular graft interposition in 4, resection and end to-end anastomosis in 2. There was no operative death. Four patients had persistent or paradoxical hypertension, and one had postoperative paraplegia.

  • The performance characteristics of heat exchangers which consist of a gas cooler, an evaporator and an internal heat exchanger have been investigated at various operating conditions of $CO_2$ air conditioning system by experiments. The heat exchangers were designed for use in the vehicle $CO_2$ air conditioning system, when considering the characteristics of heat transfer and high pressure as $CO_2$ refrigerant. This paper studied the performance of heat exchangers at various compressor speeds and expansion valve openings, and quantified the heat transfer rates and pressure drops. Heat transfer rates at the gas cooler and the evaporator were 6.9 kW and 5.2 kW, respectively, when the compressor speed was 4000 rpm and refrigerant vapor quality at the evaporator outlet was 0.98. Therefore, this paper carried out that the heat exchangers were analyzed to achieve superior performance for the vehicle transcritical $CO_2$ cycle.

  • The development of sail gas has increased the production of ethane as well as natural gas. The decline in the market price for ethane has led to a change in the petroleum-based ethylene production process into an ethane-based ethylene production process and an increase in the ethane/ethylene trade volume. Large-scale ethane/ethylene carrier have been needed due to an increase in long-distance trade from the US, and cargo type change have leaded to consider a liquefaction process to re-liquefy Boil-Off gas generated during the voyage. In this paper, the liquefaction system of Liquefied Ethane Gas carrier was evaluated with Low-GWP (Low-Global Warming Potential) refrigerant and process parameters, Boil-Off Gas pressure and expansion valve outlet pressure, were optimized. Low-GWP refrigerants were propane (R290), propylene(R1270), carbon dioxide(R744) was considered at two type of liquefaction process such as Linde and cascade cycle. The results show that the optimal pressure point depends on the individual refrigerant and the highest liquefaction efficiency of carbon dioxide (R744) - propane (R290) refrigerant.

EGR Effects on Exhaust Gas of Heavy-Duty Turbo Charge Engine with Low Pressure Route System (저압방식을 적용한 대형과급기관의 배기가스에 관한 EGR효과)

  • The efforts of EGR on performance and emissions were investigated in this study. The engine used for the tests was a six-cylinder, 11 liter, and turbo-charged, heavy-duty diesel engine with a low pressure route ECR system. The volume of recirculated gas was controlled by a manually operated valve which was installed between the turbine outlet and compressor inlet. The experiments were performed at various engine speeds and loads while the ECR rates were set at 4% and 8%. Exhaust emissions with EGR system were compared with the baseline emissions.

Twenty three experiences of Fontan operation (Fontan 술식의 치험 23례에 관한 고찰)

  • The Fontan principle of redirecting systemic venous blood into the pulmonary arteries via a conduit was initially proposed for Tricuspid Atresia, but it and its modifications have now gained a much wider application than initially intended. From September 1978 to July 1983, Atriopulmonary anastomosis has been performed in 23 patients, 2 months to 19 years of age, Seoul National University Hospital. The diagnoses were Tricuspid Atresia [TA] in 13, Univentricular Heart [UVH] in 9 and one case of Double Outlet of Right Ventricle [DORV]. Previous procedures included two Glenn shunts and one Blalock-Taussl8 shunt. Among these 23 patients, 10 patients had Right Atrial to Pulmonary Artery conduit, with a valve in 7 and without in 3. The remaining 13 patients had direct anastomosis between RA to PA. There were 14 early deaths, 7 of 13 TA patients, 6 of 9 UVH patients and one DORV, and the total hospital mortality was 60%. But there were no later deaths, 9 of these 23 patients survived operation and are presently alive. The Fontan procedure can be done with an acceptable low mortality with good functional results for Tricuspid Atresia and other complex lesion in foreign hospital, but till now our results revealed much higher risks. For our good operative results and effective patients selection, we must clarify the exact condition of pulmonary arterial system and accumulate much more experiences and technique.
