Objective: The impact of early mechanical removal of cumulus cells on fertilization and embryonic development is not yet precisely known. This study aimed to investigate the effects of early and late cumulus cell removal on fertilization, polyspermy, embryonic development potential, blastocyst development, and clinical outcomes. Methods: A prospective study was conducted of patients who underwent in vitro fertilization between September 2019 and October 2020. Sibling oocytes were randomly allocated after insemination to early cumulus cell removal at 6 hours (group I) and late cumulus cell removal at 16-18 hours (group II). If total fertilization failure (TFF) was determined to have occurred at early cumulus cell removal, rescue intracytoplasmic sperm injection (ICSI) was performed. Fertilization, embryonic development, and pregnancy outcomes were compared. Results: A total of 912 oocytes were assigned to group I (458 oocytes) and group II (454 oocytes). Fertilization, polyspermy, embryo quality, and pregnancy outcomes were not significantly different between both groups. Rescue ICSI enabled fertilization of 79.2% of the TFF oocytes. Conclusion: Early cumulus cell removal at 6 hours had no significant difference in fertilization, polyspermy, embryo development, or obstetric and perinatal outcomes compared to late removal. Early cumulus cell removal combined with early rescue ICSI may have the potential to help couples with TFF.
Objective: This study compared the outcomes of single blastocyst transfer cycles, using day- 5 poor-quality blastocysts and day-6 high-quality blastocysts. Methods: We analyzed 462 frozen-thawed embryo transfer (FET) cycles performed at our center from January 2014 to December 2019. The cycles were divided into two groups: a day-5 poor-quality blastocyst transfer group (group A) and a day-6 high-quality blastocyst transfer group (group B). The clinical outcomes were tested. Results: In groups A and B, respectively, the clinical pregnancy rate (CPR; 61.65% vs. 67.17%, p=0.258), implantation rate (IR; 61.65% vs. 67.17%, p=0.258), and live birth rate (LBR; 69.51% vs. 77.83%, p=0.134) showed no significant differences. Moreover, when day-3 embryo quality was considered, the CPR, IR, and LBR were also similar in group A and group B (p>0.05). Conclusion: The clinical outcomes of day-5 poor-quality blastocysts and day-6 high-quality blastocysts were similar, suggesting that the developmental speed of the embryo might be more important than embryo quality for the clinical outcomes of single blastocyst transfer in FET cycles.
The purpose of this study was to evaluate the folate nutritional status of Korean pregnant women and to investigate the relation between folate levels of maternal-umbilical cord blood, placenta tissue, and pregnancy outcomes. The study subjects consisted of 25 pregnant women who have had normal term deliveries. Dietary folate intakes of the pregnants were estimated by semi quantitative frequency questionnaire and the serum and placenta tissue folate level was measured by microbiological analysis. The total folate intakes of the pregnant women was 655.6 ${\mu}$g/d, which was 131.1% of the Korean RDA for pregnants. Maternal serum folate level was 16.18ng/ml, which was significantly lower than that of umbilical cord blood (34.98ng/ml, p<0.05). Mean folate concentration of the placental tissue was 998.0ng/ml, which was the highest compared to maternal and umbilical cord serum level. Umbilical cord serum folate level and placental tissue folate level were highly influenced by maternal serum folate level. The umbilical cord folate levels of the infant group whose birth weight was higher than 3500g were significantly higher than the group whose birth weight was less than 3500g (p<0.05). The placental folate level was significantly higher in maternal group who showed desirable weight gain during pregnancy (11 - 14kg). In conclusion, the birth weigt was related to the umbilical cord folate level and the maternal weight gain was affected by the placental folate level.
A study was conducted on a group of 107 women, attending the three peripheral community clinics in Seoul and Kyunggi area for their prenatal care, to ascertain the relationship between maternal serum zinc concentration measured in pregnancy and several pregnancy outcomes. The serum zinc concentration was adjusted for estimated gestational age at the time of drawing blood. Mean daily zinc intake of the pregnant women estimated by a 24-hour recall method was 7.68$\pm$3.70$\mu\textrm{g}$/dl, 51% of RDA. and mean serum zinc concentration of the women was 94.03$\pm$36.99ug/dl. Adjusted maternal serum zinc level was significantly related to gestational maternal weight gain(p<0.05) and infant birth weight(p<0.05). Pregnant women with tgreater than 9kg weight gain during the gestation period had higher adjusted serum zinc levels than the subjects with less than 8kg weight gain. Adjusted serum zinc levels of mothers who delivered 3.0-3.5kg and greater than 4.0kg birth-weight infants were higher than those of mothers of low-birth-weight infants. Any association between maternal serum zinc level and gestational length, complications and morning sickness was not observed. There results suggest that the maternal serum zinc level may predict perhaps the author could be none specific.
This study was carried out to find the factors which are related to the weight gain during pregnancy of women and infant birth weight. The information of the general characteristics and pregnancy outcomes of the 506 women who had a delivery during Jan to Dec, 1997 in a hospital at Taegu area were collected from the medical records. The results are as follows. The mean age of the subjects was 29 years old and the average prepregnancy weight was 52.75kg. They gained 13.51kg of weight during the pregnancy. The weight gain during pregnancy was higher in prepregnancy BMI<20kg/m$^2$, the infant weight was heavier in groups that had over 14kg of weight gain during the pregnancy than other groups. The prepregnancy BMI was negatively correlated to weight gain during pregnancy(r=0.2825), and positively correlated to number of pregnancy(r=0.2146), number of living delivery(r=0.1409), and infant weight(r=0.1250). The baby weight was Positively correlated to weight gain during pregnancy(r=0.1392) and Apgar score(r=0.1627). The results showed that the prepregnancy BMI and weight gain during pregnancy may be the influential factors on the infant weight, thus we need to develop the specific nutritional management program according to the status of prepregnancy weight.
The purpose of this study was to analyze the intake of antioxidant vitamins and plasma TRAP concentrations of 60 maternal-infant pairs (30 in normal term delivery group, NT; 30 in preform group, PT) We also investigated the relationship between plasma TRAP levels of maternal-umbilical cord blood and pregnancy outcomes. Mean energy intakes of NT and PT pregnant women were 93.2% and 85.4% and their protein intakes were 113.3% and 110.9% of the recommended dietary allowance (RDA), respectively. The vitamin A intakes of NT and W pregnant women were 559.7 RE and 497.8 RE, which were less than RDA. While the vitamin E and C intakes of both NT and PT pregnant women were more than RDA. The maternal plasma TRAP level of PT was 1.41 mmol/l and that of was 1.50 mmol/l, which was significantly higher than TRAP level of PT (p < 0.05) . The umblical cord plasma TRAP levels of NT and PT were 1.44 mmol/l and 1.23 mmol/l, which indicates the significant difference between those two groups (p < 0.001) . In case of comparing the TRAP level of maternal and umbilical cord blood, there was no significant difference in NT pregnant women, however, in PT group maternal the TRAP level significant higher than that of umbilical cord (p < 0.001). The length of gestation and plasma TRAP level of maternal and umbilical cord showed a positive correlation. However, other parameters of pregnancy outcomes such as birth weight, weight gain, and Apgar score were not affected by the plasma TRAP levels. Based on these results, preform infants could have a risk of oxidative stress because of low plasma TRAP level.

Objective: It is well known that fresh blastocyst transfer results in better pregnancy outcomes with a smaller number of transferred embryos compared with cleavage stage embryo transfer. However, in terms of frozen-thawed blastocyst transfer, only a few studies are available. We aimed to evaluate clinical outcomes of frozen-thawed embryo transfer (FET) with blastocysts. Methods: Retrospective analysis of FET cycles with blastocysts (B-FET) between Jan 2007 and June 2009 was performed. Age-matched FET cycles with cleavage stage embryos (C-FET) during the same period were collected as controls. A total of 58 B-FET cycles were compared with 172 C-FET cycles and also compared with those of post-thaw extended culture blastocysts from frozen pronuclear stage embryos (22 cycles). Results: There was no difference in the patient characteristics of each group. The embryos' survival rates after thawing were comparable (>90%) and there was no difference in the implantation rate or clinical and ongoing pregnancy rate among the three groups. Conclusion: In FET, blastocyst transfers may not present better pregnancy outcomes than cleavage stage embryo transfers. A further large-scale prospective study is needed.

Objective: To study the clinical outcomes of single frozen-thawed blastocyst transfer cycles according to the hatching status of frozen-thawed blastocysts. Methods: Frozen-thawed blastocysts were divided into three groups according to their hatching status as follows: less-than-expanded blastocyst (${\leq}EdB$), hatching blastocyst (HgB), and hatched blastocyst (HdB). The female age and infertility factors of each group were evaluated. The quality of the single frozen-thawed blastocyst was also graded as grade A, tightly packed inner cell mass (ICM) and many cells organized in the trophectoderm epithelium (TE); grade B, several and loose ICM and TE; and grade C, very few ICM and a few cells in the TE. The clinical pregnancy and implantation rate were compared between each group. The data were analyzed by either t-test or chi-square analysis. Results: There were no statistically significant differences in average female ages, infertility factors, or the distribution of blastocyst grades A, B, and C in each group. There was no significant difference in the clinical pregnancy and implantation rate of each group according to their blastocyst grade. However, there was a significant difference in the clinical pregnancy and implantation rate between each group. In the HdB group, the clinical pregnancy and implantation rate were similar regardless of the blastocyst quality. Conclusion: There was an effect on the clinical outcomes depending on whether the blastocyst hatched during single frozen-thawed blastocyst transfer. When performing single frozen-thawed blastocyst transfer, the hatching status of the frozen-thawed blastocyst may be a more important parameter for clinical outcomes than the quality of the frozen-thawed blastocyst.
Objective: Delaying embryo transfer (ET) enables us to select among the embryos available for transfer and is associated with positive effects on implantation and pregnancy outcomes. However, the optimal day for ET of human cleavage-stage embryos remains controversial. Methods: A retrospective study of 3,124 in vitro fertilization/intracytoplasmic sperm injection cycles (2,440 patients) was conducted. We compared the effects of day 2 and 3 ET on rates of implantation and pregnancy outcomes between young maternal age (YMA; < 38 years old, n = 2,295) and old maternal age (OMA; ${\geq}38years\;old$, n = 829) patient groups. Results: The YMA and OMA groups did not differ in terms of patient characteristics except for the proportion of unexplained factor infertility, which was significantly greater in the OMA group, and the proportion of arrested embryos, which was significantly greater in the YMA group. However, the biochemical pregnancy, clinical pregnancy, ongoing pregnancy, abortion, and implantation rates per cycle were not significantly different between day 2 and 3 ET in the YMA group or the OMA group. Conclusion: We suggest that offering patients the opportunity to decide which day would be suitable for ET could be part of a patient-friendly protocol that takes into consideration an infertile woman's circumstances and work schedule by allowing ET to be performed on day 2 instead of the traditional transfer on day 3.

Objective : To evaluate the safety and efficacy of selective fetal reduction (SFR) and compare the outcome of twin pregnancy after SFR in multiple pregnancy induced by assisted reproductive technology (ART) with that of natural twin pregnancy. Methods : From September 1995 to March 2002 in Ajou University Hospital, SFR was performed in 79 patients whose gestational sacs were more than 3. Of these 79 patients, 47 patents resulted in twin pregnancy after SFR. SFR was performed using transvaginal intracardiac KCl injection at gestational age of $6{\sim}9$ weeks. Control group was composed of 264 patients with natural twin pregnancy, who delivered after intrauterine pregnancy at 24 weeks, from June 1994 through December 2002. We compared Obstetric and perinatal outcomes between SFR group and natural twin group. Results: Among 47 patients with twin pregnancy after SFR, 2 spontaneous abortion were occurred at intrauterine pregnancy at 8 and 19 weeks. Obstetrical and perinatal outcomes were available in 43 patients. Single intrauterine fetal death was occurred in 1 of 43 (2.3%) patients in SFR group. Incidence of preterm labor, premature rupture of membrane, preeclampsia and placenta previa were similar, but gestational diabetes mellitus (GDM) was occurred more frequently in SFR group (3 (7.0%) vs 4 (1.5%), p=0.02). Mean gestational age, mean birth weight, incidence of discordancy, use of intubation and ventilation, incidence of fetal anomaly, low (<7) Apgar score and intrauterine growth restriction were similar in both groups. Conclusion: Twin pregnancy after SFR has the increased incidence for GDM but other obstetric and perinatal outcome was similar compared with natural twin pregnancy. So SFR is a safe and effective procedure, so we suggest SFR is needed in multifetal pregnancy more than triplet.
