Difference of Functional Outcome and Related Factors in Patients With Stroke (일부 뇌졸중 환자의 기능변화 및 관련요인)

  • The purposes of the present study were to determine the difference of functional outcome, and to identify factors associated with functional difference in patients with stroke. The sample consisted of 56 stroke patients who had received physical therapy at the physical therapy unit of the Dongeui Medical Center in Busan city between January 2000 and June 2002. Stroke patients were evaluated by physical therapists 3 times; The first day in physical therapy (PT) (T1), one month after the first day in PT (T2), and two months after the first day in PT (T3). Functional status was assessed with the Functional Independence Measure (FIM) instrument, a validated instrument for documenting the severity of disability and assessing the outcome of rehabilitation treatment. Functional gain was calculated over T2-T1, T3-T1, and T3-T2. SAS statistical software was used for the analysis. The Student's t-test, paired t-test, analysis of variance (ANOVA/Tukey and Scheffe), and analysis of covariance (ANCOVA) were used to examine the functional difference in variables. Repeated measures ANOVA was also used to analyze the functional difference by time (T1, T2, and T3). Multiple regression analysis was performed to determine the effects of independent variables on the difference of functional outcome as defined by the FIM score. A total of 56 stroke patients were evaluated, their average age${\pm}$standard deviation was $61.6{\pm}9.3$ years (range: 40~81 yr). The functional status of patients who received physical therapy for about 2~3 months was significantly improved (mean FIM scores, $20.5{\pm}1.8$, $28.9{\pm}1.9$, and $8.41{\pm}1.1$ points for each time period, respectively) (p<.0001). Diabetes was significantly associated with the FIM score for T2-T1 (p<.05). The type of diagnosis was significantly associated with the FIM score for T3-T1 (p<.05). Gender, smoking, and the FIM score on admission were significantly associated with the FIM score for T3-T2 (p<.05). In conclusion, gender, smoking, diabetes, the type of diagnosis, and the FIM score on admission were significantly associated with improved FIM scores. We recommend that further research should explore the functional outcome by using larger sample sizes, longer follow-up periods, and more sensitive assessment instruments.

  • This paper presents how to establish an efficient e-Learning environment using open source software. A LMS with additional functionalities on the top of dotLRN. which is a open source project for LMS, is presented. Additional functionalities include modification of the language for Korean, adoption of SCORM educational standard, and management of learning outcome. This system had been serviced for Kongju cyber university for one year on stable basis. The scope of this paper covers introduction, characteristics review, and the learner's learning outcome analysis.

  • This study reviewed the all specialized vocational high school across the nation, through the data per school in the Elementary, Middle & High Schools information disclosure system(2012), and carried out an analysis on the specialized vocational high school's educational outcomes in using the data mining technique. The educational outcomes of specialized vocational high schools were defined as the employment rate, the university entrance rate, the aware records in various vocational techniques contests. As the first research question, this study investigate whether there was any significant differences in educational outcomes depending on school's general characteristics. And as the second research question, this study explored the factors influencing on educational outcome.

Difference in Length of Stay and Treatment Outcome of Pulmonary Tuberculosis Inpatients between Health Insurance Types (의료보장유형에 따른 폐결핵 입원환자의 재원기간과 치료결과 차이분석)

  • The purpose of this study is to identify patient and hospital characteristics with pulmonary tuberculosis and to analyze factors which were influencing length of stay and treatment. The Korean National Hospital Discharge In-depth Injury Survey database from 2006 to 2012 was used for analysis. Study subjects were 4,704 patients and analyzed by using frequency, chi-square and logistic regression through using STATA 12.0. To avoid selection bias, we used propensity score matching. Analysis results show that the length of stay and treatment of pulmonary tuberculosis was different between insurance types. Patients characteristic(female, comorbidity, admission by outpatient department, medical insurance type) and hospital characteristic(500-999 beds, over 1000 beds) significantly influence length of stay. Admission by outpatient department and over 1000 beds are significantly influence treatment. Based on these findings, it is necessary to clarify between length of stay and treatment outcome by medical aids beneficiaries and audit hospitals follow discharge guidelines in pulmonary tuberculosis patients.

  • Purpose: The aim of this study was to explore the relationship between nursing work environment and nursing outcome of clinical nurses. Also, the purpose of this study was to identify the nurses' workload. Method: The participants in this study were 246 nurses working at one hospital in a province. Data was collected using a structured questionnaire from October 2011 to November 2011. Open questions were subject to content analysis; closed questions were subject to descriptive statistics and Pearson correlation coefficient with SPSS 18.0 program were used. Result: The number of patients per nurse in day-duty and overtime work had a positive correlation. Nursing performance and the overtime work in night duty nurses had a negative correlation. The results of this study, with a much higher number of patients per nurse, showed an increase in overtime hours. In the nursing delivery system experience of nurses, 13 themes were identified and they were divided into five categories. Conclusion: Therefore, in order to improve nursing performance and increase job satisfaction, efforts must be made to reduce overtime, and nursing personnel placement to secure a substantial nursing staff.

  • The notion of outcome-based educational paradigm and its adaptability for higher education has become a recent growing and quite stirring trend. In the year 2017-18, this educational philosophy has been embraced by some of the higher educational institutions in Pakistan as well. This research attempts to investigate OBE and non-OBE systems in the context of students learning outcomes and academic attainment levels in engineering education in Pakistan. The study has been conducted on undergraduate students of MUET, Jamshoro, Sindh Pakistan. The students of the software engineering department are taken as the sample. Student cohorts are formed i.e., OBE and non-OBE (traditional/teacher-centered approach) cohorts. The summative assessments of semester exams are used for data analysis descriptive statistics and independent samples t-test is performed to set up the group statistic. The findings of this study show that, in terms of students' performance, the OBE system outperforms the traditional system and this transition in engineering institutions might be beneficial in the future.

Comparing Learning Outcome of e-Learning with Face-to-Face Lecture of a Food Processing Technology Course in Korean Agricultural High School

  • This study identified the effectiveness of e-learning by comparing learning outcome in conventional face-to-face lecture with the selected e-learning methods. Two e-learning contents (animation based and video based) were developed based on the rapid prototyping model and loaded onto the learning management system (LMS), which is http://www.enaged.co.kr. Fifty-four Korean agricultural high school students were randomly assigned into three groups (face-to-face lecture, animation based e-learning, and video based e-learning group). The students of the e-learning group logged on the LMS in school computer lab and completed each e-learning. All students were required to take a pretest and posttest before and after learning under the direction of the subject teacher. A one-way analysis of covariance was administered to verify whether there was any difference between face-to-face lecture and e-learning in terms of students' learning outcomes after controlling the covariate variable, pretest score. According to the results, no differences between animation based and video based e-learning as well as between face-to-face learning and e-learning were identified. Findings suggest that the use of well designed e-learning could be worthy even in agricultural education, which stresses hands-on experience and lab activities if e-learning was used appropriately in combination with conventional learning. Further research is also suggested, focusing on a preference of e-learning content type and its relationship with learning outcome.

Adjusting for Confounders in Outcome Studies Using the Korea National Health Insurance Claim Database: A Review of Methods and Applications

  • Objectives: Adjusting for potential confounders is crucial for producing valuable evidence in outcome studies. Although numerous studies have been published using the Korea National Health Insurance Claim Database, no study has critically reviewed the methods used to adjust for confounders. This study aimed to review these studies and suggest methods and applications to adjust for confounders. Methods: We conducted a literature search of electronic databases, including PubMed and Embase, from January 1, 2021 to December 31, 2022. In total, 278 studies were retrieved. Eligibility criteria were published in English and outcome studies. A literature search and article screening were independently performed by 2 authors and finally, 173 of 278 studies were included. Results: Thirty-nine studies used matching at the study design stage, and 171 adjusted for confounders using regression analysis or propensity scores at the analysis stage. Of these, 125 conducted regression analyses based on the study questions. Propensity score matching was the most common method involving propensity scores. A total of 171 studies included age and/or sex as confounders. Comorbidities and healthcare utilization, including medications and procedures, were used as confounders in 146 and 82 studies, respectively. Conclusions: This is the first review to address the methods and applications used to adjust for confounders in recently published studies. Our results indicate that all studies adjusted for confounders with appropriate study designs and statistical methodologies; however, a thorough understanding and careful application of confounding variables are required to avoid erroneous results.

Cost-Benefit Analysis of Electrical Safety Speed-call Service Using Electrical Fire Statistics Analysis and Outcome Analysis Logic Model (전기화재 통계 및 성과 분석 모델을 이용한 전기안전 긴급출동 고충처리 서비스의 비용 편익 분석)

  • Korea Electrical Safety Cooperation(KESCO) have provided the electrical safety speed-call service from 2007 year. Purpose of the service is to reduce discomfort of electricity use and to prevent electrical accident like as electrical fire and shock accident by providing emergency treatment service on fault of the residential electrical facilities notified in the specific house like as a lower-income group and a social welfare facility. But efficiency and economic evaluation of the electrical safety speed-call service is impossible because analysis on the quantitative effect of the service is difficult. This paper presents cost-benefit analysis method and result of the electrical safety speed-call service. The presented cost-benefit analysis method has a two-step process: the first step is to measure quantitative electrical fire prevention effect of the service by using electrical accident statistics and developing outcome analysis logic model of the service effect, and the second step is to analysis cost-benefit(B/C)of the service by calculating quantitative benefit analysis on the measured quantitative electrical fire prevention effect. The results showed that cost-benefit(B/C)of the electrical safety speed-call service is over 4 after 2010 year.

Evaluation of a Fungal Spore Transportation in a Building under Uncertainty

  • A fungal spore transportation model that accounts for the concentration of airborne indoor spores and the amount of spores deposited on interior surfaces has been developed by extending the current aerosol model. This model is intended to be used for a building with a mechanical ventilation system, and considers HVAC filter efficiency and ventilation rate. The model also includes a surface-cleaning efficiency and frequency that removes a portion of spores deposited on surfaces. The developed model predicts indoor fungal spore concentration and provides an indoor/outdoor ratio that may increase or decrease mold growth risks in real, in-use building cases. To get a more useful outcome from the model simulation, an uncertainty analysis has been conducted in a real building case. By including uncertainties associated with the parameters in the spore transportation model, the simulation results provide probable ranges of indoor concentration and indoor/outdoor ratio. This paper describes the uncertainty quantification of each parameter that is specific to fungal spores, and uncertainty propagation using an appropriate statistical technique. The outcome of the uncertainty analysis showed an agreement with the results from the field measurement with air sampling in a real building.
