Effect of Otitis Media and Cerumen Occlusion on Body Temperature Measured by Thermometers (중이염과 귀지가 고막 체온에 미치는 영향)

  • Purpose : To examine the effect of unilateral otitis media and unilateral cerumen occlusion of the ear canal on thermometers. Methods : One hundred eighty six children with unilateral otitis media, fifty children with unilateral cerumen occlusion, and fifty children with neither otitis media nor cerumen were enrolled. Temperature was measured in both ear canals using thermometers. After 15 minutes, second temperature was measured again in both ears. Unilateral otitis media was graded by video otoscope for 7 grades. Differences in temperatures between affected ears and unaffected ears were analyzed. Results : No temperature difference between the normal and cerumen groups was observed. The mean temperature of the otitis media ear canal was $0.13{\pm}0.20^{\circ}C$ higher than that of the intact ear canal ($36.99{\pm}0.54^{\circ}C$ vs $36.86{\pm}0.52^{\circ}C$; P<0.001). There was no statistically significant temperature difference between grades. Conclusion : Unilateral otits media can affect estimation of body temperature measured by thermometers.

External Auditory Canal Atresia with Otitis Media in a Dog

  • A 5-year-old, intact male, poodle dog with right external auditory canal obstruction and subaural mass was presented. Physical examination revealed that right external auditory canal opening was absent and right head tilt was identified. Aspiration in right subaural mass revealed a small amount of dark brown exudate. Streptococcus canis and Staphylococcus spp. were identified on the microbial culture. Radiography of the skull was revealed absence of air-filled ear canal at the right external ear canal (EEC) level. Computed tomography (CT) revealed well capsulated, hypoattenuated mass in the right EEC region. On the contrast enhanced CT images, rim enhancement around the mass and ear canal obstruction were identified. Fluid attenuated material filled with right bulla. Mild thickening of the right tympanic bulla wall with mild lytic lesion of the ventral wall were found. Based on the images findings, the case was tentatively diagnosed as right external auditory canal atresia with otitis media. Total ear canal ablation and lateral bulla osteotomy was performed. The entire ear canal was removed, numerous hair in the canal and the thickening wall were founded. Right ear canal was sent for histopathological evaluation and found to otitis externa. The patient was followed up for two weeks and there were no complications. This report described the CT diagnosis of right EEC atresia with otitis media rarely reported in small breed dogs.

  • Objectives : It has been known that immune reaction plays an important role in the pathogenesis of otitis media (OM). We investigated the change of middle ear mucosal inflammation induced by lipopolysaccharide (LPS) following administration of Taglisodog-eum (TSE) in experimental animals. Materials and Methods : Otitis media was induced by injecting 1 mg/kg of Pseudomonas aeruginosa LPS transtympanically in 60 rats. These rats were divided into two groups, the LPS group (n=30, remained naive after OM elicitation) and the TSE group (n=30, treated with TSE after OM elicitation) and 6 additional rats were kept as a normal control group (n=6, remained naive until tissue collection). The rats were killed at the 1st, 3rd, and 7th days after challenge. The temporal bones in each group were harvested and examined histopathologically by hematoxyline-eosin stain. Middle ear mucosa were taken at the 1st, 3rd, and 7th days after challenge. The levels of spicing variants of TNF-${\alpha}$ transcription were evaluated by Realtime-PCR. Results : TSE suppressed LPS-induced TNF-${\alpha}$ mRNA expression and thickness of the submucosal layer and infiltration of inflammatory cells in rat middle ear epithelium. Conclusion : The results suggest that TSE may be effective in decreasing inflammation with particular application to mucosal metaplasia in OM.

  • The author measured IL-8 and $TGF-{\beta}1$ levels of 84 ears - 48 ears of them had treated by antibiotics, 36 of them by Kamihyunggyeyungyotang(KHY) - of pediatric recurrent otitis media with effusion using ELISA assay, and compared them. The results were obtained as follows. 1. The level of IL-8 in KHY group was significantly lower than that in antibiotics group(p<0.05). 2. The level of $TGF-{\beta}1$ in KHY group was lower than that in antibiotics group. According to above results, KHY is considered to be used for treating recurrent otitis media with effusion by controlling the production of interleukin-8 and transforming $growthfactor-{\beta}1$.

Comparison of Nursing Activity Costs of Chronic Otitis Media Surgery Patients among Time, RBRVS, and CP (상대가치, 소요시간, 표준임상경로지에 의한 만성중이염수술 간호원가 비교분석)

  • Purpose: The purpose of this study was to compare the cost using different methods of costing nursing activities for patients with chronic otitis media having surgery. Method: Data were collected from 30 patients who had mastoidectomy and tympanoplasty. To compare the cost, the researchers used three different costing methods; consumed time, resource based relative value scale (RBRVS), and critical pathway (CP). Results: Twenty-six nursing activities for surgical patients with chronic otitis media were found. Total cost was 83,843.7 won using RBRVS. The costliest activity was recording at 9,734.4 won, followed by confirmation of doctors' orders at 9,302.4 won, and injection with infusion pump at 9,072.0 won. There was a difference in nursing activities performed according to the length of hospital stay, and the cost was highest on the surgery day at 13,417.8 won. Comparatively, the total cost was 72,014.4 won using CP. Conclusions: Nursing activities are performed in various forms according to the disease and patient's condition, and different nursing activities are executed according to the length of hospital stay. In order to measure the load of nursing activities and distribute it appropriately, it is necessary to analyze the cost of nursing activities by the process of nursing services performed.

A Case of Chronic Suppurative Otitis Media treated with Oriental medicine (한방치료로 호전된 만성 중이염 치험 1례)

  • Objective : The purpose of this study is to report the effect of Oriental medicine on Chronic suppurative otitis media. Methods : We used herbal medication, acupuncture and cupping therapy were for treatment. To evaluate the results of the treatment, we observed the patient's condition and otoscopic findings. Results : The patient's objective and subjective symptoms showed noticeable improvement after treatment. Conclusions : We suggest that chronic suppurative otitis media can be treated by Oriental medicine effectively.

  • Objectives : The purpose of this study is to investigate the trend of Tranditional Chinese Medicine for Otitis Media with Effusion(OME) in Chinese journals. Methods : Chinese National Knowledge Infrastructure(CNKI) and Wanfang med online were used to search Chinese studies which were published from January, 2010 to April, 2020. Results : Among Chinese studies, Exterior-releasing medicinal(解表藥) and Heat-clearing medicinal(淸熱藥) were the most frequently used. The herbs which used the most frequently are Bupleuri Radix(柴胡), Acori Graminei Rhizoma(石菖蒲). All of studies have reported that Tranditional Chinese Medicine is effective for Otitis Media with Effusion. Conclusions : In analysis of selected studies, Tranditional Chinese Medicine is more effective than Western Medicine Treatments. Recurrence rates and side effects of OME can be reduced by cotreatment of Tranditional Chinese Medicine and Western Medicine Treatments.

The Clinical Study on the Relationship between State of the Tympanic Membrane and Treatment Period in the Acute Otitis Media by Video Otoscope (video otoscope로 관찰한 급성 중이염 소아 환자의 고막상태와 치료기간과의 연관성에 대한 연구)

  • Objective: In children with acute otitis media, we investigated the relationship between state of tympanic membrane and treatment period by video otoscope. Methods: Ten children(6 boys, 4 girls) with acute otitis media were assigned to one of three groups. In the grade I, there were redness in the tympanic membrane. In the grade II, there were bulging, effusion with exudate, dark color in the tympanic membrane. In the grade III, there were effusion with purulent exudate or cholesteatoma with serous exudate. And we investigated the treatment period in each group. Results: Overall, the $mean{\pm}S.D.$ on the treatment period in the grade I was shorter than grade II. It was $12.6{\pm}5.18$ in the grade I versus $26{\pm}2.83$ in the grade II. That in the grade II was shorter than grade III. It was $26{\pm}2.83$ in the grade II versus $148{\pm}43.03$ in the grade III. Conclusions: Although we couldn't have enough cases in each group. we could predict the conclusion that there was the relationship between treatment period and state of tympanic membrane.

A Case of Chronic Suppurative Otitis Media with Perforated Eardrum Treated with Bojungikgi-tang (보중익기탕(補中益氣湯)으로 호전된 고막천공을 동반한 만성 화농성 중이염 치험 1례)

  • Objectives : This study is to report the treatment of chronic suppurative otitis media with Bojungikgi-tang and acupuncture without western medicine. Methods : A 57-year-old female patient visited my korean medical clinic on November 19, 2019 with complaints of ear fullness and yellowish effusion in the left ear. The patient had perforated eardrum with yellowish effusion. For the first month she took Bojungikgi-tang 3 times a day and received acupuncture 5-6 days a week without western treatment. The next month, only acupuncture treatment was performed twice a week. The condition of the eardrum and otorrhea was observed through an otoscope, and the degree of ear fullness and head heaviness were evaluated by visual analogue scale. Results : During the treatment, the eardrum was reperforated once, but it was regenerated, the otorrhea disappeared, and all other symptoms improved. Afterwards, it was confirmed through follow-up that the eardrum was maintained in a regenerated state. Conclusions : These results show that the combined treatment of herbal medicine and acupuncture is effective for chronic suppurative otitis media.

Treatment of Otitis Media in Cleft Palate (구개열 환자에 있어 중이염의 치료)

  • Otitis media with effusion is known to be very common among children with cleft palate and is the common cause of acquired hearing loss in childhood. The purpose of the present study was to examine the anatomic variances, incidence and treatment of middle ear disease in children with cleft palate. In Korea, before 4-year-old age, the prevalence of otitis media was as high as 91.7%. Common treatments for otitis media were conservative or surgical treatment. The medical treatment options include the use of decongestants, antihistamines, antibiotics and mucolytics. Surgical treatment options include grommet insertion, myringotomy. Unlike the case for children without clefts, cleft patient has a higher incidence of recurrence, so surgical treatment between the age of 3 month and 6 month was recommended. The effect of palatoplasty on middle ear disease has remained controversial. Early and periodic evaluation on the symptomatic infection or significant of hearing loss was recommended.
