Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery in Adulthood: Challenges and Outcomes

  • Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is an extremely rare, potentially fatal, congenital anomaly with a high mortality rate in the first year of life. It occurs rarely in adulthood and may appear with malignant ventricular arrhythmia or sudden death. We report a case of a 49-year-old woman with ALCAPA who presented with dyspnea on exertion. Management was coronary artery bypass grafting to the left anterior descending artery and obtuse marginal arteries, closure of the left main coronary artery ostium, and reestablishment of the dual coronary artery system.

  • Mucocele of the paranasal sinuses is a benign, slow-growing, expansile lesion. Maxillary sinus mucoceles are usually associated with painless bulging of the cheek; however, orbital expansion is rarely observed. Maxillary sinus mucoceles can be classified as primary or secondary according to their etiology. An impediment to sinus ostium ventilation is thought to be the cause of primary mucocele, while sequestering of residual mucosa after surgery in the wound and long-term retention of tissue fluid have been suggested to lead to the formation of secondary mucocele. This report presents 2 cases of primary and secondary mucoceles, with a focus on radiographic features. As primary and superiorly positioned secondary maxillary sinus mucoceles are uncommon and their close proximity to the orbit predisposes the patient to significant morbidity, the authors expect that this report will contribute to a better understanding and diagnosis of maxillary sinus mucocele involving the orbit.

  • An adequate location of the reimplanted coronary arteries is one of the most important prognostic factors for an arterial switch operation for treating TGA (transposition of great arteries). We report here on a case of malposition of the reimplanted coronary artery in a 14-year-old boy who had undergone an arterial switch operation. He had chest pain and dyspnea on exertion. The imaging study showed that the ostium of the left main coronary artery, which was between the neo-pulmonary artery and the neo-aorta, was compressed by the great arteries. We excised the left main coronary artery with a button incision from the aorta and reimplanted it on the left lateral side of the aorta. The patency of the left main coronary artery os after the operation was good and it was not compressed by the surrounding arteries.

  • World Health Organization reported that heart-related diseases such as coronary artery stenoses show the highest occurrence rate which may cause heart attack. Using Computed Tomography angiography images will allow radiologists to detect and have intervention by creating 3D roadmapping of the vessels. However, it is often complex and difficult do reconstruct 3D vessel which causes very large amount of time and previous researches were studied to segment vessels more accurate automatically. Therefore, in this paper, Region Competition, Geodesic Active Contour (GAC), Multi-atlas based segmentation and Active Shape Model algorithms were applied to segment aortic root from CTA images and the results were analyzed by using mean Hausdorff distance, volume to volume measure, computational time, user-interaction and coronary ostium detection rate. As a result, Extracted 3D aortic model using GAC showed the highest accuracy but also showed highest user-interaction results.   • Objective : In order to investigate anti-inflammatory effect of Alum objectively which is traditional remedy applicated by external preparation frequently, especially in Rhinologic field. Materials and Method : We studied the histopathologic and hematologic features, serum transaminase activities in the experimentally induced maxillary sinusitis in rabbits. 9 rabbits was divided into normal, control anti sample group(each 3 rabbits). We inoculated $10^{10}$ P. aeruginosa for experimental maxillary sinusitis to control group and sample group. Sample group was treated with aim solution(l0g/100cc) lcc via both nasal cavity (each 0.5cc) after 24hrs everyday for 14days. Results and Conclusion : 1. We confirmed that maxillary sinusitis was well induced by P. aeruginosa without occlusion of maxillary ostium. 2. There was no abnormal findings in serum transaminase(AST/ALT) activities even though application of Alum solution on nasal mucosa for 14days continuously. 3. Alum has evident anti-inflammatory effect of recovering mucosal surface injury, reduction of goblet cell, lymphocyte infiltration, edema and expansion of glandular tissue, dilatation and congestion of blood vessels and so on. 4. Alum has the effect that recover glandular tissue injury and decrease goblet cell increase by the result of dermal PAS staining increase and epidermal AB staining decrease in the qualitative analysis of epidermal and dermal mucopolysaccharide

A RADIOLOGIC STUDY OF THE MAXILLARY SINUSITIS AS SEEN BY WATERS′ AND SPECIALIZED PANORAMIC PROJECTIONS (워터스촬영법과 파노라마촬영법을 이용한 상악동염에 관한 방사선학적 연구)

  • The purpose of this study was to evaluate the specialized Orthopantomograms and standard films compared with Waters' views in the diagnosis of maxillary sinusitis. The author analysed the types of mucosal thickening, the types of bony wall change, the relation between the types of chief complaints and mucosal thickening on the Waters' views of 156 patients with 206 maxillary sinuses demonstrating radiographic changes. And the pathologic change of antral floor was observed on 187 standard films and total amount of agreement in the diagnosis of maxillary sinusitis was evaluated at 156 specialized Orthopantomograms and 187 standard films. The results were obtained as followings. 1. Among 206 mucosal thickenings classified by 6 types, generalized increased radiopacity was 69 cases (33.5%) and the entire thickening of mucosa except an ostium area was 66 cases (32.0%), 2. The bony wall change was seen in 128 cases (62.1%) and the incidence of indistinct antral wall was the highest as 55 cases (43.0%) among 5 types. 3. Among 148 chief complaints, pain was 71 cases (48.0%), swelling was 19 cases (12.8%) and discomfort was 18 cases (12.2%). 4. The most frequent change of antral floor was indistinct antral floor as 25 cases (13.4%) and elevation of antral floor was 23 cases (12.3%). And the first molar presented the most frequent involvement of antral floor as 47 cases (57.3%). 5. In 156 specialized Orthopantomograms, 101 cases (49.0%) presented apparent increased radiopacity and 72 cases (35.0%) showed suspicious increased radiopacity in comparison with Waters' view. In 187 standard films, 47 cases (25.1%) presented apparent increased radiopacity and 53 cases (28.4%) showed suspicious increased radiopacity in comparison with Waters' views.

  • It is a well-known clinical fact that contrast-enhanced magnetic resonance angiography exaggerates vertebral arterial ostial stenosis and sometimes shows pseudostenosis. Considering the clinical significance of a lesion in the posterior circulation ischemia, the importance of an accurate imaging diagnosis of ostial stenosis should not be underestimated. We were able to differentiate pseudo stenosis of the ostium from true stenosis using thin-slab maximum-intensity-projection(MIP) images which are thought to be helpful for minimizing standard full thickness MIP images.

  • Sporadic reports of hypernatremic dehydration in breastfed newborn infants have appeared in medical literature for at least 3 decades. We report the first case of hypernatremic dehydration resulting from inadequate breast-feeding in Korea. A 14-day old baby, born to a mentally retarded mother, was transferred to our hospital with a body weight loss of 460 g since birth(17%) and a serum sodium(Na) level of 179 mEq/L, after initial hydration at another hospital. On admission, a cardiac murmur was heard and an enlarged liver was palpated. Cardiac ultrasonogram revealed ventricular septal defect and ostium secundum atrial septal defect. During hydration, a seizure-like attack developed. Serum Na decreased to 135 mEq/L on the 5th day of admission. Brain ultrasonography and brain magnetic resonance image revealed no remarkable abnormalities. Electroencephalography was normal. She suffered from prerenal azotemia, hyperglycemia and disseminated intravascular coagulation at admission but was treated successfully. Heart failure was also controlled with dobutamine, diuretics and digoxin.

Right coronary artery atresia in Marfan's syndrome: A case report (Marfan 증후군에 동반된 우관상동맥 폐쇄증 - 1 례 보고 -)

  • A case of Marfan’s syndrome with atresia of right coronary artery is reported. A 45-year-old woman, who was diagnosed as Marfan’s syndrome 1 year ago, came to the hospital complaining of acute chest pain. The patient showed arachynodactyly, pectus carinatum, and long and slender extremities. In echocardiography there were severe aortic regurgitation measured grade IV and aortic dilatation of ascending aorta maximally 5.9 cm in diameter. Mitral regurgitation was mild, but there were also moderate left ventricular dilation and moderately decreased ejection fraction of left ventricle. At operation, atresia of right coronary artery was found. We performed Bentall type operation with SJM 27mm valved conduit for left coronary artery, and Piehler’s modification for right coronary artery bypass using 6mm PTFE graft. The atretic portion of right coronary artery from the suspected right coronary ostium to distal coronary flow was about 4 cm in length. The combination of right coronary artery atresia and Marfan’s syndrome is very rare. The author describes the rare case, which is treated with combined technique of Bentall and Piehler modification for reconstruction of coronary circulation.

  • The ultrastructure on the dorsal vessel of 5-day-old cabbage butterfly, Pieris rapae L., was carried out using the transmission and scanning electron microscope. The results are as follows. 1) The aorta. The aorta is simple tubular type and consists of the inner and outer membrane of the myocardium and thick myocardium is located between them. However the inner membrane with $0.26{\mu}m$ thickness and outer membrane with $0.08{\mu}m$ are composed of fibrous materials, the former is composed of low and high densed fibrous materials and the latter appears homogeneous layer. The myocardium consists of typical striated muscles. The sarcomere with $1.6{\mu}m$ length and in cross section, each thick filaments are surrounded by $7{\sim}8$ thin filaments. The intercalated disc is joining the end of the two muscle cells, desmosomes and septate junctions are appeared between the neighboring muscle cells. 2) The heart. The heart composing of myocardium enclosed by its inner and outer membrane as the aorta has a series of well formed segmental chamber. The arrangement of myofilaments, cell adhensions and membrane elements are observed as same as at the aorta. The inner membrane of the heart is deeply invaginated into the myocardium than the outer membrane and a lot of well developed mitochondria with rod shape are aggregated in the folds. The longitudinally and transversely oriented tubule system formed by invagnation of the sarcolemma into the muscle bundle is built up dyad with the sarcoplasmic reticulum as the aorta. The slit is formed by deeply invagination of the inner membrane of myocadium toward the muscle layer and then the inner and outer membrane of myocardium are fused. Therefore, the ostium is formed between the myocardium and situated at the lateral side of the myocardium.
