메틸말론산혈증은 선천성 유기산대사질환 중 하나로 증상의 발현시기 및 임상 증상이 매우 다양하며, 장기간의 합병증으로 세뇨관 간질 신염과 만성 신기능 저하, 췌장염, 기저핵 손상, 지능저하가 발생 할 수 있다. 연구자들은 이러한 메틸말론산혈증의 세뇨관 간질신염을 동반한 활동저하 환자에서 파미드로네이트 치료를 통해 고칼슘혈증과 골다공증의 호전을 경험하였기에 보고하는 바이다.
Isoflavones have been a central subject in research on the natural phytoestrogens found in Leguminosae. Their effects on bone formation and remodeling are important in that they can act like estrogen by binding on estrogen receptors on the target cell surface. We, therefore, believed that isoflavones may help in the treatment of patients with estrogen deficiency disease such as estrogen replacement therapy (ERT) for osteoporosis. As commonly known, osteoporosis is one of the hormonal deficiency diseases, especially in menopausal women. When estrogen is no longer produced in the body a remarkable bone remodeling process occurs, and the associated events are regulated by growth factors in the osteoblast lineage. In the present study, we investigated whether isoflavones (Isocal) extracted from Sophorae fructus affect the growth factors IGF-I and TGF-$\beta$ that have been known to be related with bone formation. In the study, we found that the active control (PIII) effectively enhanced the level of nitric oxide (NO) and growth factors, and thereby inhibited osteoclastogenesis. The most efficient concentration was $10^{-8}$% within five days, whereas the comparative control (soybean isoflavone) was not as effective even at a lower concentration. In conclusion, the products which contain enriched glucosidic isoflavone and nutrient supplements such as shark cartilage and calcium can be used for osteoporosis therapy by enhancing the production of IGF-I and TGF-$\beta$. Furthermore, the NO produced through endothelial constitutive NO synthase (ecNOS) may playa role in inhibiting bone reabsorption.
우리나라는 급격한 경제성장과 의료기술의 발달로 고령화 사회가 되면서 골다공증 발병이 증가하고 있다. 또한 서구화된 식생활로 암질환이 증가하였고 항암치료과 방사선 치료 등으로 인해 골다공증이 발생하게 된다. 골다공증은 골밀도의 감소로 인한 것으로 대퇴근위부의 형태학적 구조 변화와도 밀접한 관계를 가지고 있으며, 대퇴 근위부의 형태학적 구조 변화는 고관절 골절의 위험을 나타내는 요인이다. 본 연구는 T-score변화에 따라 정상, 골감소증, 골다공증으로 분류하여 각 군별로 대퇴 근위부의 형태학적 변화를 관찰하고 상관관계를 분석하였다. 조사대상은 부산지역의 D종합병원의 남녀 350명을 대상으로 연령, 성별, T-score변화로 질환을 구분하여 분석하였다. 결과는 연령, 성별에 따라 골다공증의 발병빈도는 유의한 차이를 보였고, T-score변화에 따른 질환 분류와 대퇴근위부 형태학적 특성의 7가지 파라미터로서 Cortical ratio calcar, Cortical ratio shaft, Hip/shaft Angle, Strength index, Section modulus, CSMI, CSA는 유의한 차이를 보였으며 골다공증과 높은 상관관계가 있었다. 그러므로 연구 결과는 T-score변화에 따른 대퇴근위부의 형태학적 변화를 상관관계 분석함으로써 골다공증을 진단하는데 대퇴 근위부의 형태학적 변화가 지표로서 활용가능하리라 사료되며, 향후에 대퇴근위부의 형태학적 파라미터가 골다공증 진단의 정확성을 높이고 예측인자로서 임상활용이 가능하리라 판단된다.
Objective : The purpose of this study is to investigate the results of korean traditional conservative therapy for thoraco-lumbar compression fracture and it's recovery rate and different result in accordance with sex, age, medical history. Method : We studied about 35 cases who were admitted to Jaseng Hospital of Oriental Medicine with diagnosis of 'Thoraco lumbar compression fracture' with X-ray examination. Result : Out of 35 patients 31 patients discharged with satisfying results. Most of the compression fracture occurred in age of $60{\sim}70$. And the single fractured patients recovered more easily than the multi-level fractured patients. It didn't affect the results whether the patient had another vertebral disease(such as Herniated intervertebral disc or osteoporosis) or not. And most of the patients who had vertebral compression fracture visited the western medicine hospital first. Conclusions : We analyzed the tendency of the patients who had vertebral compression fracture, and concluded that the conservative Korean traditional therapy is an effective means of treatment for the patients who have thoraco-lumbar compression fracture.
Several workers reported the relationship between osteoporosis and age-related reductions in the BV/TV (bone volume fraction) of vertebral trabecular bones. However, there were few micro finite element (micro-FE) models to account for the treatments of the osteoporotic trabecular bone. In the present study, micro-FE models of osteoporotic and hormone-treated bone models were constructed to analyze the effect of specimen location and boundary condition on mechanical characteristics of hormone treatment model for osteoporotic trabecular bone. Top and bottom sections of specimens were also investigated individually to study the effect of specimen location. Hormone-treated models were allowed to have the same relative BV/TV (13.4%) as that used in models of previous researchers. The present study reported the elastic and plastic characteristics of the osteoporosis and hormone-treated bone models. In the present study, in-situ boundary condition was applied to the simulated compression tests for in-vivo condition of vertebral trabecular bone. The present study indicated that the hormone therapy was likely to improve the mechanical characteristics of osteoporotic bones and the mechanical characteristics of vertebral trabecular bone specimen were dependent on the captured location and boundary condition.
3-Hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (Statins) are potent inhibitors of cholesterol biosynthesis. Cholesterol-lowering therapy using statins significantly reduces the risk of coronary heart disease. Various discovery of statins as bone anabolic agents has spurred a great deal of interest among both basic and clinical bone researchers. In-vitro and some animal studies suggest that statins increase the bone mass by enhancing bone morphogenetic protein-2 (BMP-2)-mediated osteoblast expression. Clinical and animal test results of statins focusing on the prevention and treatment of bone fractures was collected. Three independent literature searches were performed by using from January 1, 2002 to September 2008 for clinical and animal test results. Search term included statins, HMG-CoA reductase inhibitors, pleiotropic effects, fracture, osteoporosis and clinical and animal test. No consensus has been reached whether clinical use of statins has beneficial effects on bone health, partly due to lower statin concentrations because of first-pass metabolism by the liver. Experimental use of statins as stimulators of bone formation suggests that they may have widespread applicability in the field of orthopaedics. With their combined effects on osteoblasts and osteoclasts, statins have the potential to enhance resorption of synthetic materials and improve bone ingrowth. In conclusion, The use of statins in the prevention and treatment of bone fractures requires further study. But observational studies suggest that statins for decreasing bone fractures including osteoporosis have to be considered local direct administration like transdermal or subcutaneous type over oral adminstration.
Hemiplegia-induced immobilizatoin and reduction of mechanical loading in chronic stroke limbs are common cause of disuse osteoporosis. The purpose of this study was to investigate the effects of asymmetrical weight bearing on the loss of bone mineral in the individual with chronic stroke. Sixteen hemiplegic patients with strokes were evaluated. The measurements of bone mineral density (BMD) were evaluated with the quantitative ultrasound system on the calcaneus region of the paretic and non-paretic side. Plantar pressure was measured using the Mat-Scan system. The paretic side showed significantly smaller values in the T-score of BMD, and peak value of plantar pressure, which included forefoot, midfoot, and hindfoot, than the non-paretic side (p<.05). Results from the pearson correlation analysis showed statistically significant correlation between the BMD difference and the peak-pressure difference of midfoot pressure (p<.05). This finding indicated that BMD loss depended on decrease of body weight born on the paretic leg.
The purpose of this study is to examine the changes in bone mineral density in long-stay patients of a geriatric hospital in relation to physical therapy, thereby providing basic data for preventing the onset or deterioration of osteoporosis in inpatients of geriatric hospitals or relevant facilities. The subjects of this study were 133 elderly patients who had been in H geriatric hospital in Yongin-si for more than four years. Their bone mineral density T-scores at admission and those after four years were measured to compare and analyze the changes in bone mineral density in relation to physical therapy. According to the results, the decrease in T-score for the group without physical therapy was greater by .40 than that for the group with physical therapy, indicating a statistically significant difference between the two groups(p<.01). Accordingly, physical therapy proved to be effective in preventing bone mineral density loss and maintaining the current density.
This systematic review was conducted to assess the efficacy and safety of bisphosphonates for prevention and treatment of osteopenia or osteoporosis in men with non-metastatic prostate cancer receiving androgendeprivation therapy. We searched for randomised controlled trials (RCTs) of bisphosphonates compared with placebo from Pubmed, Embase, the Cochrane Library, and ISI - Science Citation Index. Meta-analyses of prespecified outcomes (bone mineral density, fractures, and adverse events) were performed using Review Manager. Ten RCTs with a total patient population of 1,017 were identified. There was generally more improvement in bone mineral density of the lumbar spine for patients who received bisphosphonate treatment than placebo or other medical treatment at 12 months (WMD 6.02,95%CI 5.39 to 6.65). Similar effects were also observed for total hip, trochanter or femoral neck bone mineral density. However, there was no significant reduction in fractures. Fever and gastrointestinal symptoms were the most common adverse events (10.4% vs. 1.2%; 0.10% vs. 0.03%). Currently, our meta-analysis suggested that oral and intravenous bisphosphonates caused a rapid increase in spine and hip or femoral BMD in non-metastatic prostate cancer patients receiving androgen-deprivation therapy. Fever and gastrointestinal symptoms were common with the use of bisphosphonates. These short-term trials (maximum of 12 months) did not show fracture reduction. In future, more efficient performance of higher quality, more rigorous, large sample, long-term randomised controlled trials (>12 months) are needed where outcomes are detailed.

Bisphosphonates are used effectively for many medical conditions, such as multiple myeloma, Paget's disease, osteoporosis, etc. However, recently, osteonecrosis of the jaw was observed in patients receiving long-term bisphosphonate therapy, including oral administration. This osteonecrosis is refractory, and complete recovery is not guaranteed despite a standard treatment protocol being established by many associations related to oral and maxillofacial surgery. The treatment outcome of oral bisphosphonate-related osteonecrosis of jaw (BRONJ) is reported with a review of the relevant literature.
