Arthroscopic Removal of Ossicles Associated with Osgood-Schlatter's Disease (관절경을 이용한 오스굿씨 병(Osgood-Schlatter's Disease)의 골편 제거술)

  • Osgood-Schlatter's disease is generally treated conservatively. However, surgical treatment is necessary for some patients with recurrent or persistent pain that does not respond to conservative treatment. Most authors recommend the excision of the loose ossicles present around the distal end of the patellar tendon. The authors report the technique of arthroscopic removal of the ossicles for Osgood-Schaltter's disease instead of the conventional technique through the incision at the tibial tuberosity. The advantages of this technique are less damage to the patellar tendon, early postoperative recovery, making no incisional scar in front of the tuberosity which causes the scar discomfort in kneeling, and more cosmetic result.

  • Sesamoid bones and accessory ossicles are normal anatomic variants with varying morphological appearances and incidences. They are usually small osseous fragments with well-corticated margins located adjacent to the joint space and bone. Patients with sesamoid bones and accessory ossicles are usually asymptomatic and commonly encountered in clinical practice. These sesamoids and accessory bones are occasionally painful because of fractures, dislocations, degenerative changes, avascular necrosis, accessory bone infections, or abnormalities of the adjacent tissue, such as nerve entrapment, tenosynovitis, or soft tissue impingement. This article aimed to illustrate the imaging features of symptomatic sesamoids bones and accessory ossicles at various anatomic locations and describe their clinical features and radiological differential diagnosis.

  • In this study, we developed a finite element model of the human middle ear has been developed to calculate itsfor sound transfer characteristics calculation. We usedThe geometric data forof ossicles, obtained byfrom micro-CT scanning, was used in order to develop the middle- ear FE model. A right- side temporal bone of a Korean cadaver was used for the micro-CT scanning. The developed FE model includes three ossicles, the tympanic membrane, ligaments, and muscles. We calculated theA sound transfer function from the tympanic membrane to the stapes footplate was calculated. The sound transfer function calculated vias of the FE model shows good agreement with measured responses over the 10- kHz frequency band. To measureidentify the sensitivityies of the middle- ear function due to material property variation, we studied several parameters studies have been fulfilled using the middle ear FE model. TAs a result the stiffness property of the incudostapedial joint had the greatest influence onwas the most influential to the middle- ear sound transfer function among the parameters.

  • In this paper, a new type of implantable middle ear hearing aid, which consists of external loop coil, a small magnet and a simple external device, is proposed. The internal device of proposed type consists of only a small magnet and the external device does not need to be positioned behind the ear or in the ear canal. The proposed type is excellent in cosmetic sides and very convenient to use, because the external device can be hidden in upper garment and collar of clothes. Also, purposely this type doesn't need to be small on the size of battery, which means it has longer battery life. Therefore, the battery is not necessary to be charged frequently. It also can solve the difficulty of gap calibration at surgical operation which conventional Implantable middle. Therefore, the battery is not necessary to be charged frequently. We investigate the performance of proposed implantable middle ear bearing aid and we analyze that proposed type is appropriate for mild and severe hearing impaired person and the result of experiment showed the accuracy of our analysis. For the validation of our analysis we used the temporal bone at the experiment and confirm that ossicles can be vibrated when the proposed system In startled in the body.

  • Recently, various hearing aids are developed to overcome hearing loss. There are available hearing aids, such as air conduction hearing aid, implantable middle ear hearing aid and so on. But air conduction hearing aid is inconvenience caused by howling, and ossicle chain driving type implantable middle ear hearing aid has some week point due to problem of possible nercobiosis of coupling spot along incus long process. In recent years, in order to improve these shortcomings round window (RW) driving hearing aid has been paying attention. In this paper, the physical cochlear model is proposed for a performance evaluation of the RW driving hearing aids of a transducer. In order to verify an experiment proposed on a performance of physical cochlear model, the transducer which has ossicles characteristics is used. By measuring and comparing the frequency characteristics of transducer with ossicles and human temporal bone, performance of physical cochlear model was verified. As from the result of experiment, it is expected that an implemented cochlear model is useful for evaluating characteristics of RW transducer.

  • Bekesy measured the sound transformation system of the middle ear 49 years ago. According to his reports, a ratio between the size of ear drum and the size of oval window is 17 : 1, and the lever function of the ossicles is physiologically 1.3 : 1. Therefore, the hearing might be aggravated to 27.5㏈ in the case of the vanishment of 3 ossicles. In 1952, Wullstein reported 5 types of tympanop-lasty and the fourth type among them was especially named for the sound-protection. The oval window is only exposed by the sound pressure and the round window is not exposed. According to the application by this idea, the post-operative hearing might be improved until 27.5㏈. Mean while, in 1942, Onchi verified through his experiment that the results of Bekesy's measurement was not completely conformed to Onchi result. Bekesy measured the sound pressure on the stapes plate of the oval window, on the other hand, Onchi measured the sound pressure on the surface of the perilymph of the oval window after removing the stapes plate(Fig. 1).(Figure omitted) The difference of their experiment is recognized that the impedance of the stapes plate exists or not (Fig. 1). Both Audiogrums are compared as Fig. 2. The result of IV type of tympanoplasty is success ful in 54% as the Table 1. (Table Omitted) The reason of unsatisfactory is caused by the thick and unmovable window-membrane and by the closing of air passage to the round window. The closing of the air passage to the round window is occurred by the adhesion between the grafting membrane and the surface of promontorium. In order to preserve this adhesion, I produce to transplantate the mucous membrane of the lip to the bone surface of tympanic cavity after removing the granulation tissue of the tympanic cavity and to form a membranous canal for the sake of air passage (Fig. 3). (Figure Omitted) The post-operative hearing by this method is shown as Fig. 4, 5. In other words, the post-operative sound pressure entered into the cochlea directly, by way of the oval window only, not by way of the round window, as a theorie of the sound protection. A Study on the Development of a Human Middle Ear Implant (인체 중이 임플란트 개발에 관한 연구)

  • This paper describes the design of the ossicular replacement prosthesis for the people who have problem of hearing because of middle ear disease. Also the simulation results of the designed product for verification by finite element analysis software (ABAQUS) are presented. New model is applied to middle ear FE analysis which was generated in previous study. The full replacements of ossicular prosthesis for ossicles (malleus, incus and stapes) are made of Hydroxyapatite and Titanium. Although other existing prosthesis models consider only sound amplification effect, current type has damper system which is operating on the audible frequencies. High frequency sound transmitted to inner ear can be reduced and the prosthesis and inner ear can be prevented from damage.

Newly recorded sea star Henricia hayashii(Asteroidea: Spinulosida: Echinasteridae) in the East Sea, Korea

  • Henricia specimen was collected from the East Sea of Korea using fishing nets. The specimen was identified as Henricia hayashii (Djakonov 1961), belonging to the family Echinasteridae of the order Spinulosida. This species can be distinguished from other Henricia species by slender arms (R/r=7.4), constricted arm base, six to thirty-five slender spinelets, open-meshed, reticulated abactinal skeleton comprising semi-triangular, rod-like form, and small ossicles present inside the papular areas. This species superficially resembles H. reniossa in terms of its body size but differs in the shape and arrangement of the abactinal and actinal plates. To date, two genera of Echinasteridae, Aleutihenricia and Henricia, including a total of 14 species, have been reported in Korea. Herein, the morphological characteristics of H. hayashii are described, and photographs are provided.

A newly recorded sea star of genus Henricia (Asteroidea: Spinulosida: Echinasteridae) from the East Sea, Korea

  • Henricia specimens were collected using fishing nets from 14 November 2012 to 8 June 2018 from the East Sea of Korea. The specimens were identified as Henricia sanguinolenta (O.F. $M{\ddot{u}}ller$, 1776), belonging to the family Echinasteridae of the order Spinulosida, which is known for its wide distribution throughout the world. This species can be distinguished from other Henricia species by long arms (R/r=6-6.8), thick arm base, coarser stubby abactinal spines with more than ten apical points, and visible longitudinal series of actinal ossicles. Previously, eight species of the genus Henricia were reported in the Korean fauna. The morphological characteristics of this species were prepared and described with illustrations.
