SECONDARY CHEILORHINOPLASTY OF BILATERAL CLEFT LIP AND NOSE DEFORMITIES (양측성 구순 비변형 환자의 이차 구순비성형술)

  • The columella, nasal tip, lip relationship in the secondary bilateral cleft deformity remains an enigma and a great challenge for the cleft surgeon. A subset of patients with bilateral cleft lip still require columellar lengthening and nasal correction, despite the advances in preoperative orthopedics and primary nasal corrections. An approach to correct this deformity is described. This consists of 1) lengthening the columella, 2) open rhinoplasty, allowing definitive repositioning of lower lateral cartilages, ear cartilage grafting to the tip and columella when necessary, 3) nasal mucosal advancement, 4) alar base narrowing and 5) reconstruction of the orbicularis oris as required. In surgical repair of the cleft lip nose, the timing of the operation(during lip closure, before or after the puberty growth sput), and the operative technique play a key role in the final result. In this study, 13 cleft lip patients who had undergone a secondary cheilorhinoplasty at the Department of Oral and Maxillofacial Surgery, Pusan National University Hospital were evaluated to check the proper time and method of the operation.

Relationships among the Pain Belief, Pain Coping, and Pain Disability of Patients with Chronic Musculoskeletal Pain (근골격계 만성통증 환자의 통증신념과 통증정도, 대처양상 및 통증생활방해정도와의 관계)

  • Purpose: The purpose of this study was to understand the degree of pain belief in musculoskeletal patients, and to identify the correlation with chronic pain, pain coping and pain disability. Methods: A total of 203 inpatients or outpatients with chronic pain in orthopedics agreed voluntarily to participate in this study and answer a questionnaire. Data were analyzed using SPSS/WIN 17.0 program with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficient, and simple linear regression. Results: The degree of pain belief in this study showed statistically significant differences depending on their age, education, job, health status, and pain duration. Among the variables correlating with pain belief in this study, there were positive correlations between pain and pain disability, pain and passive coping, pain belief and passive coping, pain belief and pain, pain belief and pain disability. The strongest correlation was passive coping and pain disability. Conclusion: From the results of this study, we concluded that it is necessary to develop the nursing intervention which can help reducing negative pain belief in patients with chronic musculoskeletal pain. Also we need to enhance the ways of coping to active or chronic pains for controlling them effectively.

Finite element modeling of manufacturing irregularities of porous materials

  • Well-ordered porous materials are very promising in orthopedics since they allow tailoring the mechanical properties. Finite element (FE) analysis is commonly used to evaluate the mechanical behavior of well-ordered porous materials. However, FE results generally differ importantly from experimental data. In the present article, three types of manufacturing irregularities were characterized on an additive manufactured porous titanium sample having a simple cubic unit-cell: strut diameter variation, strut inclination and fractured struts. These were included in a beam FE model. Results were compared with experimental data in terms of the apparent elastic modulus (Eap) and apparent yield strength (SY,ap). The combination of manufacturing irregularities that yielded the closest results to experimental data was determined. The idealized FE model resulted in an Eap one order of magnitude larger than experimental data and a SY,ap almost twice the experimental values. The strut inclination and fractured struts showed the strongest effects on Eap and SY,ap, respectively. Combining the three manufacturing irregularities produced the closest results to experimental data. The model also performed well when applied to samples having different structural dimensions. We recommend including the three proposed manufacturing irregularities in the FE models to predict the mechanical behavior of such porous structures.

  • Examination with medical radiography is the most significant advances in medicine in the 21st century. Diagnostic imaging occupies an important position in all medical subjects, surgical and internal medicine, including radiology, orthopedics, and neurosurgery. However, the invention associated with conventional radiography devices has focused only on emphasizing the efficient aspects of the examination or obtaining clearer images without considering the age and disease-induced constraints of the patient's posture. The newly developed X-ray aids to solve this problem can effectively perform X-ray imaging of children's who have difficulty communicating and controlling. It is also thought to be a good way to provide accurate imaging information and minimize radiation exposure for children.

  • This is to investigate the physical properties of the commercially available Soft, Thomas, Minerva, and Philadelphia cervical braces which are widely used in orthopedics, neurosurgery, and rehabilitation medicine clinics as assisting devices for physical stabilization of cervical vertebrates, to use as a basic data for designing new type brace. Tensile strengths were observed by universal mechanical measuring device and Thomas brace required the highest stress to break by tensile stress. Durabilities against continuous frictional forces were also determined, and Minerva brace demonstrated the longest frictional time until being perforated. According to these results, poly ethlene is recommendable as a frame and preparation of pores in the material is favorable to provide ventilation to skin.

Involvement of NMDA Receptor and L-type Calcium Channel in the Excitatory Action of Morphine

  • We studied the excitatory action of morphine on the responses of dorsal horn neuron to iontophoretic application of excitatory amino acid and C-fiber stimulation by using the in vivo electrophysiological technique in the rat. In 137 of the 232 wide dynamic range (WDR) neurons tested, iontophoretic application of morphine enhanced the WDR neuron responses to N-methyl-D-aspartate (NMDA), kainate, and graded electrical stimulation of C-fibers. Morphine did not have any excitatory effects on the responses of low threshold cells. Morphine-induced excitatory effect at low ejection current was naloxone-reversible and reversed to an inhibitory action at high ejection current. NMDA receptor, calcium channel and intracellular $Ca^{2+}$ antagonists strongly antagonized the morphine-induced excitatory effect. These results suggest that changes in intracellular ionic concentration, especially $Ca^{2+},$ play an important role in the induction of excitatory effect of morphine in the rat dorsal horn neurons.

  • Ganglionic cyst is the most common benign tumor in the wrist and hand, and easily diagnosed with ultrasonography. Ganglionic cyst in sonography usually appears as hypoechoic or anechoic with well-demarcated cystic mass. We report a case of the hyperechoic mass at the volar aspect of the wrist, which confirmed as ganglionic cyst by operative and histological findings.

  • This study researched 1,408 physical therapy patients in hospital. orthopedic clinic. neurosurgical clinic, general surgical clinic, and general clinic located Taegu city on may 9. 1990. Also this study reviewed front doctor's prescripted progress note. The results obtained were as followed. 1) 4th decade of age $(22\%)$ are the highest, and 3rd, 2nd, 5th decade were followed. Many dorsopathies are not correlated with age, also they involved 2nd decade age and 3rd decade are sprain and strains of joints and adjacent muscle. 4th and 5th decade of age are rheumatism. 2) According to medical facility. They treated in hospital are other disorders of central nervous system. orthopedics are rheumatism, and in neurosurgical clinic. general surgical clinic, general clinic are dorsopathies. 3) Male and female both have a lot of dorsopathies, sprains and strains of joint and adjacents muscle occurred in males, female have a lot of rheumatism. 4 At physical therapy patients, $21.4\%$ patients are in patients and others are out patients. Among the $66.1\%$ of on patients, they treated in hospital as they have other disorders of central nervous system.

Correction of Class III malocclusion with alternate rapid maxillary expansions and constrictions using a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment: A case report

  • In this report, we describe the successful use of alternate rapid maxillary expansions and constrictions with a hybrid hyrax-mandibular miniplate combination and simultaneous orthodontic treatment for the management of severe Class III malocclusion due to maxillary hypoplasia in an 11-year-old girl. The devices were removed after 20 months of treatment, and the family was instructed about a careful control and retention program that should be followed in accordance with the patient's growth. The final result included the correction of Class III malocclusion with adequate function and excellent facial esthetics, which restored the patient's self-esteem and provided personal motivation. The outcomes showed good stability after 24 months of retention. The decrease in the duration of active treatment is the most important finding from the present case. Considering that facial esthetics in adolescence is a determining factor for the development of a personality and interpersonal relationships, we recommend the use of this protocol for growing patients, who will exhibit not only an improved physical appearance but also a better quality of life.

The Effects of Cervical Stabilization Exercise on Neck Pain, Range of Motion, and Deep Cervical Muscle Strength in Patients With Chronic Neck Pain (경추의 안정화 운동이 만성 목통증 환자들의 통증, 경추 가동범위 및 심부근 근력에 미치는 효과)

  • Background: This study was planned to assess and compare the effectiveness of cervical region stabilization exercise and strengthening exercise on pain and range of motion in patient with chronic neck pain. Methods: Fifty-one patient with chronic neck pain were randomly allocated into strengthening and stabilization exercise (SSE) group (n=27) and Strengthening exercise (SE) group (n=24). The program was carried for 12 sessions, 3 days/week in 4weeks. Pain intensity with visual analog scale (VAS), cervical range of motion, deep cervical flexor strengthening were evaluated before and after the intervention. Results: After the program, pain intensity decreased in group SE and SSE were found (p<.05). Range of motion and deep cervical muscle strength increased in group SE and SSE were found(p<.05). The intergroup comparison showed that significant difference in range of motion and deep cervical muscle strength (p<.05). Conclusions: The study suggest that both SSE program and SE program might be a useful treatment for patients with chronic neck pain. However SSE program might be superior in improving cervical range of motion and deep cervical flexor strength compare to SE program.
