Orthognathic surgery(2-jaw or 1-jaw surgery) is very famous one of cosmetic techniques. However, primary purpose of orthognathic surgery is to improve the occlusion of jaw and secondary purpose is to improve the esthetic result. Unfortunately, many patients don't only confuse often primary and secondary purpose of orthognathic surgery but they think the esthetic result is more important than the occlusion. Therefore, oral and maxillofacial surgeon has to fully understand cosmetic needs of patient and reflect that in the treatment plan. Patients with prominent mandibular angle want to have the narrower face of so called 'V-line' shape. Various techniques like the angle shaving, ostectomy of the lateral cortex around the mandibular angle and masseter musclectomy can be used for improving the mandibular angle hypertrophy. These techniques also can be applied in orthognathic surgery at the same time. We operated patients of orthognathic surgery, especially, with wide lower face and post-operative results were satisfactory in all cases. So, we propose mandibular angle management for improving the esthetic result of orthognathic surgery.

We observed 469 cases (male 217 cases, female 252 cases) with dento-facial deformity for 10 years from Jan 1994 to Dec 2003. The ratio of male to female was 1:1.2 and the mean age was 23.3 years (male 24.0 years, female 22.6 years) ranged from 11 to 43 years. The most dominant group was related to mandibular prognathism (80.4%). A rate of 83.8% demonstrated mandibular protrusion according to Slavicek's classification (n=160), and 85.4% were classified as skeletal Class III type according to Sugawara's classification (n=151). Surgical method were divided into 355 cases of one jaw surgery (single method), 26 cases of one jaw surgery (combined method), and 77 cases of two jaw surgery. Sagittal split osteotomy were performed on 316 cases (69.0%). The average operation time and blood loss in SSRO were $4.1{\pm}2.2$ hrs. and $138.8{\pm}222.6$ ml (n=152).

The patient's appearance can be dramatically enhanced as a result of orthognathic surgery. However, esthetic contouring of the mandibular body portion is often overlooked in the surgery. Restoring a more beautiful jaw line is important and directly affects surgical results. From December 2010 to February 2012, we performed mandibular body contouring for the slimming of the third lower part of the face in 37 patients who had undergone either 1-jaw surgery or 2-jaw surgery. The third lower part of the facial contour was improved in all the patients after at least 3 months follow-up. Mandibular body contouring is a simple method that can be additionally used for the slimming of the third lower part of the face in patients who require orthognathic surgeries. It makes the lower face look more attractive from both the anterior and lateral perspectives.

Objectives: Contemporary biometric technologies have been gaining traction in both public and private security sectors. Facial recognition is the most commonly used biometric technology for this purpose. We aimed to evaluate the ability of a publicly available facial recognition application program interface to calculate similarity scores of presurgical and postsurgical photographs of patients who had orthognathic surgery. Materials and Methods: Presurgical and postsurgical photographs of 75 patients who had orthognathic surgery between January 2018 and November 2020 in our department were used. Frontal photographs of patients in relaxed and smiling states were taken. The patients were classified into three groups: Group 2 had one-jaw surgery, Group 3 had two-jaw surgery to correct mandibular prognathism, and Group 4 had two-jaw surgery to correct facial asymmetry. For comparison, photographs of 10 participants were used as controls (Group 1). Two facial recognition application programs (Face X and Azure) were used to assess similarity scores. Results: The similarity scores in the two programs showed significant results. The similarity score of the control group, which did not undergo orthognathic surgery, was the highest. The results for Group 2, Group 3, and Group 4 were higher in the order of Group 2, Group 3, and Group 4. Conclusion: In this study, all orthodontic patients were recognized as the same person using the face recognition program before and after surgery. A significant difference in similarity results was obtained between the groups with both Face X and Azure and in both relaxed and smiling states.

To evaluate the stability after orthognathic surgery in cleft lip and palate patients using rigid fixation, 20 patients underwent primary repair in childhood and later developed a jaw deformity and malocclusion that required orthognathic surgery were reviewed. Two groups, one of 10 patients performed Le Fort I osteotomy with sagittal split ramus osteotomy and one of 10 patients with sagittal split ramus osteotomy of the mandible, were evaluated. Each group had unilateral cleft only and all alveolar cleft sites had been grafted with autogeneous bone before the orthognathic surgery. The amount of surgical movement and relapse were compared in both horizontal and vertical dimensions. Two-jaw surgery group was more stable than mandibular surgery only group in mandibular position (p< 0.05). Statistically significant relapse was observed in mandibular skeletal point in mandibular surgery group (p<0.05). There was no statistically significant relapse in the skeletal point of two-jaw surgery group. However, the correlation between the horizontal surgical movement and relapse was detected (r = 0.88). This correlation indicates the need of overcorrection. The presence of scar tissues and relatively deficient maxillary bone could be attributed to this close relation between the surgical change and relapse.

Background: This study are to identify the symptomatic changes and condylar stability after 2 jaw surgery without preceding treatments for Temporomandibular joints(TMJ) in class III patients with the TMJ symptoms; and to assess therapeutic effect of 2 jaw surgery and the necessity of preceding treatment for alleviation of TMJ symptoms. Methods: 30 prognathic patients with preexisting TMJ symptoms were divided into 2 groups according to presence or absence of preceding treatments before the surgery. We evaluated symptomatic changes on both TMJ by questionnaires and clinical examinations. And we reconstructed 3D cone beam computed tomography images before 2 jaw surgery, immediately after the surgery, and 6 months or more after the surgery with SimPlant software, and analyzed the stability of condylar position on 3D reconstruction model. Significances were assessed by the Wilcoxon signed rank test on SPSS ver. 20.0. Results: Both groups had favorable changes of TMJ symptoms after orthognathic surgery. And postoperative position of condyle had good stability during follow-up period. Conclusion: 2 jaw surgery without preceding treatments for TMD can have therapeutic effect for TMD patients with class III malocclusion.
Surgery first approach in orthognathic surgery is to proceed the orthognathic surgery without preoperative orthodontic treatment. This approach has many advantages, which include a shorter total treatment period, a high level of patient satisfaction due to immediate post-surgical facial improvement, easy postoperative orthodontic treatment due to early normalization of skeletal muscle, and the rapid tooth movement reflecting the regional accelerated phenomenon. However instability due to transient occlusal interference after surgery make worse of long-term skeletal stability. Especially increasing of vertical occlusion caused by interference of interbicupid and molar happen postsurgical skeletal change. Until now, there is no common consensus about treatment protocol of surgery first approach in orthognathic surgery. The purpose of this paper is to introduce our treatment protocol of the surgery first approach and to evaluate indication and limitation with case analysis.

Orthognathic surgery is designed to correct problems of the jaw and face and restore facial harmony. The limitations of orthognathic surgery occur at all steps of the surgical workflow: preoperative planning, simulation, and operation. Many studies have shown the accuracy and advantages of 3 dimensional computer-assisted program for orthognathic surgery. The purpose of this paper is to introduce the accuracy of the maxillary repositioning in patients who underwent orthognathic surgery using a 3 dimensional computer assisted surgery program. The reliability of computer guided orthognathic surgery using splint and surgical guide need to be improved further. The 3 dimensional computer assisted analysis seems to be more precise to interpret than two-dimensional analysis. High-precision planning of orthognathic surgery has predictable results. Three-dimensional computer assisted orthognathic surgery has the following advantages : planned surgical movement is possible, splints guide with CAD/CAM technology; and increase predictable results .Computer assisted simulation surgery ensures accuracy during surgery, thereby facilitating predictable results. It may provide solution that enables surgeon to perform planned surgery more accurately.
The purpose of this study was to investigate the degree of relapse following orthognathic surgery and the relationship between preoperative state and the methods of orthognathic surgery in skeletal Class III patients. Thirty-one patients were selected(17 men, and 14 women) for this study, who had received orthognathic surgery(20 one jaw surgery, and 11 two jaw surgery). The mean age was 22.5 years. Their lateral cephalograms, that were taken preoperatively, immediate postoperatively and follow-up over one year, were traced and analysed. The results were as follows : 1. In two jaw surgery, mandibular length, lower facial height and lower lip length were 4.24mm, 4.64mm and 4.13mm longer than in one jaw surgery, respectively But in two jaw surgery, overjet was 3.13mm shorter than in one jaw surgery. 2. In one jaw surgery, mandible was moved back $8.95\pm4.45mm$ at B point. In two jaw surgery, maxilla was moved forward $5.15\pm3.46mm$ and mandible was moved back $7.24\pm9.11mm$ at B point. 3. Between postoperation and follow-up over one year, A point, A' point, Pn and Sn were moved backward 1.02 $\pm$ 2.14mm, $1.73\pm1.63mm,\;1.05\pm1.48mm\;and\;1.55\pm1.37mm$ in two jaw surgery, respectively. 4. Between postoperation and follow-up over one year, in one jaw surgery, B point was moved forward $2.58\pm4.22mm$ and B' point was moved forward $1.95\pm4.39mm$. In two jaw surgery, B point was moved forward $0.65\pm2.88mm$ and B' point was moved forward $0.19\pm3.32mm$. In one jaw surgery, relapse rate was $28\%$ at B point and $24\%$ at B' point, whereas in two jaw surgery, relapse rate was $8\%$ at B point and $3\%$ at B' point.

But, because of the rate of increase of an blood-born infection like AIDS via transfusion, nowadays an autologous blood transfusion is interesting to us. We made a comparative study of an amount of blood loss &transfusion using hemoglobin value after classifying the orthognathic surgeries from Feb. '97 to Mar. '98 in single-jaw and doublejaw surgery. And we intended to set a standard against of a routine preoperative cross-matching deciding the amount of predictive homologous blood transfusion according to operative method. Simultaneously, we studied the realization &effectiveness of autologous blood transfusion with some cases, so would like to present. Results: 1. Single-jaw operation can be performed without blood transfusion or with homologous blood transfusion through only blood typing & screening. 2. We commonly transfuse two units of blood with double-jaw operation and an autologous blood transfusion has much more advantage than an homologous blood transfusion. 3. We can reduce charge associated with blood transfusion through precisely preoperative evaluation of patients and proper type of blood transfusion.
