Endosseous implants have been used to provide anchorage control in orthodontic treatment without the need for special patient cooperation. However these implants have limitation like space requirement, cost, equipments. Recently titanium micro-implant for orthodontic anchorage was introduced. Micro-implants are small enough to place in any area of the alveolar bone, easy to implant and remove, and inexpensive. In addition, orthodontic force application can begin almost immediately after implantation. The mandibular first, maxillary first, mandibula second, and maxillary second molars were the four most commonly missing teeth in adult sample. In case of posterior molar teeth missing, deflective contacts in any position, over time, has produced pathologic change of occlusal scheme because of extrusion of opposing teeth. This case had interocclusal space deficiency by mandibular right molars missing over time. The micro-implants had been used for intrusion of maxillary right molars for interocclusal space. The micro-implant would be absolute anchorage for orthodontic movement. Therefore, the micro-implant would be effective method for correction of occlusal plane.
본 연구는 경희대학교 치과대학 교정과와 Biosstech$^{\circledR)$이 개발한 교정용 골유착성 티타니움 임프란트를 이용하여 임프란트 매식 후 가해지는 교정력이 임프란트의 골유착에 미치는 영향과 임프란트 제거 후 손상된 치조골의 치유를 알아보기 위해 시행되었다. acid-etched type, sand-blasted type, sand-blasted & acid-etched type, 3종류의 교정용 임프란트 24개를 2마리 실험 동물의 구강 내에 식립하였다. dog1의 상하악 좌측에 식립한 6개 임프란트에는 식립 후 교정력을 주지 않았고, 우측에 식립한 6개 임프란트에는 식립 즉시 200-300gm의 교정력을 가하였다. dog2에서는 임프란트 식립후 4주의 치유기간을 기다린 후, 골 내에 고정되어 있는 모든 임프란트에 4주 동안 200-300gm의 교정력을 가하였다. 4주, 8주의 관찰 기간이 지난 후 실험 동물을 각각 희생시켜 임프란트와 주위 조직을 포함하는 비탈회 표본을 제작하여 광학 현미경하에서 검경하였다. 이 실험을 통해 다음과 같은 결과를 얻었다. 1. 골내 고정에 성공한 임프란트들의 식립 4주 후 조직 소견에서, 식립 즉시 교정력을 가한 임프란트와 교정력을 가하지 않은 임프란트 사이에 조직학적 차이가 없었다. 두 군 모두에서 미성숙 소주골이 임프란트 식립시 손상된 부위로 생성되고 있었으며 부분적인 골유착이 보였다. 2. 임프란트 식립 부위의 골손상이 완전히 치유되기 전에 가해진 교정력은 골유착의 진행을 방해하지 않았다. 4주의 치유기간 후 4주간 교정력을 가한 임프란트의 조직소견에서 골조직과 임프란트 계면 사이의 접촉은 더 넓은 부위로 진행되었고 나사골을 채우고 있는 신생골은 기저골의 구조와 형태를 닮은 층판골로 성숙되어 있었다. 3. 식립 후 임프란트 주위 연조직 염증이 심했던 임프란트는 대부분 탈락되거나 동요가 있었으며, 실험 동물 희생시 동요가 있었던 임프란트의 조직 소견에서 임프란트 주위는 연조직으로 둘러 쌓여 골조직과 분리되어 있었다. 4. 골내 고정에 실패한 임프란트의 식립 부위에서는 임프란트 제거 2주 후 염증 소견없이 왕성한 골형성을 나타내는 조직학적 소견이 관찰되었다.
고정원의 조절은 교정치료에 있어서 매우 중요한 요소로 이를 보강하기 위한 많은 노력이 있어왔다. 골융합성 임프란트의 경우 확실한 고정원으로서 가능성이 인정되고 있고, 또 임상에서 많이 시도되고 있다. 그러나 임프란트를 매식하기 위해서는 무치악이 존재해야 하거나 하악구치 후방부위에 식립해야 하는 등 장소의 제약이 있고, 값이 비싸며, 골융합을 위하여 기다리는 시간이 필요하다는 등의 단점으로 인하여 보편화되고 있지는 않다. 최근 몇몇 임상가에 의하여 수술용 titanium microscrew 나 miniscrew를 교정치료시의 고정원으로 사용하려는 시도가 있었는데, 이것은 골융합성 임프란트보다 수술이 간단하며, 가격이 저렴하고, 치조골 어느 부위이든지 식립할 수 있다는 장점이 있다. 저자는 titanium microscrew implant를 사용한 skeletal cortical anchorage를 이용하여 통상적인 교정치료 동안 협조도가 고갈된 환자를 치료하였다. 6개월간의 titanium microscrew로 부터 가해진 교정력에 의하여 상악 전치부는 4 mm후방 치체이동과 압하이동을 얻었다. 통상의 교정치료에서 고정원역할을 하는 상악 구치부도 1.5 mm후방이동 되었다. titanium microscrew는 치료기간 동안 움직임없이 잘 유지되었다. 비록 과학적으로 밝혀져야할 임상적인 문제가 있기는 하나, skeletal cortical anchorage는 확실한 고정원으로서의 역할을 할 수 있을 것으로 생각된다.

Objective: The aim of this study was to compare the initial stability as insertion and removal torque and the clinical applicability of novel orthodontic zirconia micro-implants made using a powder injection molding (PIM) technique with those parameters in conventional titanium micro-implants. Methods: Sixty zirconia and 60 titanium micro-implants of similar design (diameter, 1.6 mm; length, 8.0 mm) were inserted perpendicularly in solid polyurethane foam with varying densities of 20 pounds per cubic foot (pcf), 30 pcf, and 40 pcf. Primary stability was measured as maximum insertion torque (MIT) and maximum removal torque (MRT). To investigate clinical applicability, compressive and tensile forces were recorded at 0.01, 0.02, and 0.03 mm displacement of the implants at angles of $0^{\circ}$, $10^{\circ}$, $20^{\circ}$, $30^{\circ}$, and $40^{\circ}$. The biocompatibility of zirconia micro-implants was assessed via an experimental animal study. Results: There were no statistically significant differences between zirconia micro-implants and titanium alloy implants with regard to MIT, MRT, or the amount of movement in the angulated lateral displacement test. As angulation increased, the mean compressive and tensile forces required to displace both types of micro-implants increased substantially at all distances. The average bone-to-implant contact ratio of prototype zirconia micro-implants was $56.88{\pm}6.72%$. Conclusions: Zirconia micro-implants showed initial stability and clinical applicability for diverse orthodontic treatments comparable to that of titanium micro-implants under compressive and tensile forces.
This article reports the orthodontic treatment of a patient with skeletal mandibular retrusion and an anterior open bite due to temporomandibular joint osteoarthritis (TMJ-OA) using miniscrew anchorage. A 46-year-old woman had a Class II malocclusion with a retropositioned mandible. Her overjet and overbite were 7.0 mm and -1.6 mm, respectively. She had limited mouth opening, TMJ sounds, and pain. Condylar resorption was observed in both TMJs. Her TMJ pain was reduced by splint therapy, and then orthodontic treatment was initiated. Titanium miniscrews were placed at the posterior maxilla to intrude the molars. After 2 years and 7 months of orthodontic treatment, an acceptable occlusion was achieved without any recurrence of TMJ symptoms. The retropositioned mandible was considerably improved, and the lips showed less tension upon lip closure. The maxillary molars were intruded by 1.5 mm, and the mandible was subsequently rotated counterclockwise. Magnetic resonance imaging of both condyles after treatment showed avascular necrosis-like structures. During a 2-year retention period, an acceptable occlusion was maintained without recurrence of the open bite. In conclusion, correction of open bite and clockwise-rotated mandible through molar intrusion using titanium miniscrews is effective for the management of TMJ-OA with jaw deformity.
Statement of problem : There have been previous studies about considerable variations in machining accuracy and consistency in the implant-abutment-screw interfaces. Purpose : The purpose of this study was to evaluate the machining accuracy and consistency of implant/abutment/screw combinations on two randomly selected implants from each of four manufactures. Material and methods : In this study, screws were respectively used to secure a cemented abutment, to a hexlock implant fixture ; teflon coated titanium alloy screw(Torq-Tite) and titanium alloy screw in Steri--Oss system, gold-plated gold-palladium alloy screw(Gold-Tite) and titanium alloy screw in 3i system gild screw ana titanium screw in AVANA Dental Implant system, and titanium screws in Paragon System. The implants were perpendicularly mounted in polymethyl methacrylate autopolymerizing acrylic resin block(Orthodontic resin, Densply International Inc. USA) by use of dental surveyer. Each abutment screw was secured to the implant with recommended torque value using a digital torque controller. Each screw was again tightened after 10 minutes. All samples were cross sectioned with grinder-polisher unit(Omnilap 2000 SBT Inc) after embeded in liquid unsaturated polyester (Epovia, Cray Valley Inc) Results : There were the largest gaps in the neck areas of screws in hexagonal extension implants which were examined in this study. The leading edge of the abutment screw thread (superior surface) was in contact with the implant body thread, and the majority of the contacting surfaces were localized to the middle portion of the mating threads. Considerable variation in the contacting surfaces was noted in the samples evaluated. Amounts of contact in the abutment screw thread were larger for assemblies with Gold-Tite screw, gold alloy screw. Torq-Tite screw than those with titanium screws. The findings of intimate contact between the screw and screw seat were seen in all samples, regardless of manufacturers. However, microgap between the head and lateral neck surface of the screw and the abutment could be dectected in all samples. The findings of intimate contact between the platform of the implant and the bottom of the abutment were consistent in all samples, regardless of manufacturers. However, microgaps between the lateral surface of external hex of the fixture and the abutment could be dectected in all samples. Conclusion : Considerable variations in machining accuracy and consistency were noted in the samples and the implant-abutment-screw interfaces were incomplete. From the results of this study, further development of the system will be required, including improvements in pattern design.
Transitional implants were developed to support provisional restorations and to allow for load-free osseointegration of conventional implants while a patient was provided with immediate esthetics and function and are usually placed simultaneously at the time of definitive implant placement. Transitional implants are placed in a non-submerged fashion in a single-stage surgery and are designed to be immediately loaded. They generally are made of commercially pure titanium or titanium alloy and are designed as 1-piece implants composed of root and crown replacement segments. Transitional implants can be used in a wide range of indications, such as basic use as temporary implant, to support and protect the primary implants during the healing phase, single crown in the edentulous anterior region of mandibular, anchorage for orthodontic treatment, support a surgical and radiographic template, and primary implant to extremely atrophied alveolar crests of the mandible and maxilla. This article describes the clinical use of transitional implants to support the provisional complete denture and single crown in the restricted edenturous central incisor region of mandible.
본 연구는 miniplate(Titanium C-tube, Martin Co., Germany)를 교정적 고정원으로 사용하여 교정력 및 악정형력을 즉시 가했을 때 miniplate의 안정성 및 주위 조직 반응을 관찰하여 고정원으로서의 유용성을 밝히고자 시행되었다. 네 마리의 비글 성견을 대상으로 하악의 좌우 협측 제3소구치와 제4소구치 사이에 두개의 titanium miniscrew(직경 2.0mm, 길이 7.0mm)에 의해 고정되는 miniplate를 식립하여 15주 동안 200-250gm의 힘을 가한 교정력 적용군과 8주 동안 400-450gm의 힘을 가한 악정형력 적용군으로 분류하였고 대조군은 제4소구치와 제1대구치사이에 식립하고 힘을 가하지 않았다. 동요도는 식립 직후와 희생 직전에 측정하여 비교하였고 희생 후 제작된 조직 절편을 방사선적으로 검사하여 주위 골의 흡수 여부를 관찰하였다. 실험 동물은 관류 고정하고 조직 절편은 레진 포매하여 비탈회 경조직 표본을 제작하였다 miniplate 제거 후의 조직의 치유와 연조직의 관찰을 위해 일부 조직편은 탈회 표본으로 제작하였다. 표본은 H&E 염색 후 광학 현미경하에서 검경하였고 위의 실험내용을 통해 다음과 같은 결과를 얻었다. 1. 교정력과 악정형력을 적용한 두 그룹 모두 miniscrew와 골간의 계면에는 직접적인 골유착이 일어났다. 힘의 크기에 따른 차이는 관찰되지 않았고, 힘의 적용기간이 증가함에 따라 골침착이 증가되었으며, 하중이 가해지지 않은 대조군이 실험군에 비해 골조직이 덜 침착되었다. 2. Miruplate와 miniscrew를 피개하고 있는 연조직은 염증소견 없이 모두 치유가 잘 일어났다. 3. 모든 실험군에서 동요도는 발생되지 않았고, 하중이 가해지지 않은 대조군의 경우 경미한 동요도를 보였으며, 교합력이 직접 가해진 대조군에서 고정원이 탈락될 정도의 동요도를 보였다. 4. Miniscrew 제거 후의 연조직 및 경조직은 정상조직의 형태와 구조로 치유되었다. 이상의 결과로 miniplate(Titanium C-tube, Martin Co., Germany)는 식립 즉시 악정형력 정도의 힘까지 수용할 수 있고, 인접 조직에 자극이 적은 골내 고정원으로서 충분한 가치가 있다고 판단되어 진다.

M.I.A.는 치료 전기간동안 안정되게 유지되어 교정치료의 고정원으로서의 가능성을 확인시켜 주었다. M.I.A.를 고정원으로 사용한 sliding mechanics를 통하여 골격성 II급 부정교합자를 치료하는 새로운 접근법은 환자의 협조도에 의존하지 않고 치료할 수 있고, 비교적 빠른 시기에 많은 안모의 변화를 가져와 환자의 협조도를 끌어 낼 수 있다. 그리고 상악 6전치를 동시에 후방견인하므로 치료기간을 줄일 수 있으며 호선의 교환이 적어 chair time이 짧다. 이런 결과로 미루어 볼 때 MIA는 치아이동의 고정원으로 역할을 할 수 있을 것으로 생각되고, MIA를 고정원으로 이용한 sliding mechanics를 통한 교정치료는 골격성 II급 부정교합의 치료에 있어서 쉽고 효율적인 치료법으로 생각된다.
Dental implants have been widely used in the treatment of esthetic and functional problems of the mouth due to alveolar bone loss, after tooth extraction. The success of implantation strongly depends on osseointegration. For osseointegration, implant material, methodology, and design have been investigated. For materials, two popular materials at present are titanium and hydroxyapatite. For methods, immediate implantation is being used recently. The purpose of this study is to evaluate osseointegration between the unthreaded cylindrical TPS implant and the HA-coated implant by a histomorphometric analysis. For this analysis, experimental periodontits was induced on the 3, 4 premolars of adult dogs by the ligation of orthodontic threads. Thereafter, each tooth was extracted. TPS. Implants and HA-coated implants were immediately inserted in the extraction socket. In control group, TPS implants were immediately inserted, and In experimental group, HA implants were immediately inserted. The dogs were sacrificed after 12 weeks, then the specimens were prepared for LM and histomorphometric analysis. The conclusion of this study is as follows l. In both control and experimental group, no inflammatory cells were observed. 2. The results of the histomorphometric analysis showed that the total osseointegration was 48.5% in control group, and 68.8% in expermental group. The experimental group was higher than the control group, and the difference was not statistically significant (p<0.05). 3. The results of the histomorphometric analysis showed that the osseointegration in the hole was 40.6% in control group, and 70.2% in experimental group. The experimental group was higher than the control group, and the difference was statistically significant (p<0.05). In both control and experinental group, no inflammatory cells were observed. 4. The results of the histomorphometric analysis showed that the osseointegration in the lower part was 52.1% in control group, and 73.3% in experimental group. The experimental group was higher than the control group, and the difference was statistically significant (p<0.05). 5. In experimental group, the bone to HA interface seemed to be mixed of bone and HA. We could not distinguish HA from the bone. The HA coating was detached from the titanium surface.

② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
