A comparative study of metal artifacts from common metal orthodontic brackets in magnetic resonance imaging

  • Purpose: This study was performed to compare the metal artifacts from common metal orthodontic brackets in magnetic resonance imaging. Material and Methods: A dry mandible with 12 intact premolars was prepared, and was scanned ten times with various types of brackets: American, 3M, Dentaurum, and Masel orthodontic brackets were used, together with either stainless steel (SS) or nickel titanium (NiTi) wires. Subsequently, three different sequences of coronal and axial images were obtained: spin-echo $T_1$-weighted images, fast spin-echo $T_2$-weighted images, and fluid-attenuated inversion recovery images. In each sequence, the two sequential axial and coronal images with the largest signal-void area were selected. The largest diameters of the signal voids in the direction of the X-, Y-, and Z-axes were then measured twice. Finally, the mean linear values associated with different orthodontic brackets were analyzed using one-way analysis of variation, and the results were compared using the independent t-test to assess whether the use of SS or NiTi wires had a significant effect on the images. Results: Statistically significant differences were only observed along the Z-axis among the four different brands of orthodontic brackets with SS wires. A statistically significant difference was observed along all axes among the brackets with NiTi wires. A statistically significant difference was found only along the Z-axis between nickel-free and nickel-containing brackets. Conclusion: With respect to all axes, the 3M bracket was associated with smaller signal-void areas. Overall, the 3M and Dentaurum brackets with NiTi wires induced smaller artifacts along all axes than those with SS wires.

In vitro physical, chemical, and biological evaluation of commercially available metal orthodontic brackets

  • Objective: This in vitro study was undertaken to evaluate the physical, chemical, and biological properties of commercially available metal orthodontic brackets in South Korea, because national standards for these products are lacking. Methods: Four bracket brands were tested for dimensional accuracy, (manufacturing errors in angulation and torque), cytotoxicity, composition, elution, and corrosion: Archist (Daeseung Medical), Victory (3M Unitek), Kosaka (Tomy), and Confidence (Shinye Odontology Materials). Results: The tested rackets showed no significant differences in manufacturing errors in angulation, but Confidence brackets showed a significant difference in manufacturing errors in torque. None of the brackets were cytotoxic to mouse fibroblasts. The metal ion components did not show a regular increasing or decreasing trend of elution over time, but the volume of the total eluted metal ions increased: Archist brackets had the maximal Cr elution and Confidence brackets appeared to have the largest volume of total eluted metal ions because of excessive Ni elution. Confidence brackets showed the lowest corrosion resistance during potentiodynamic polarization. Conclusions: The results of this study could potentially be applied in establishing national standards for metal orthodontic brackets and in evaluating commercially available products.

다양한 교정용 브라켓 표면에 부착하는 타액단백질에 관한 연구 (Adherence of Salivary Proteins to Various Orthodontic Brackets)

  • 본 연구의 목적은 다양한 교정용 브라켓의 표면에 형성되는 타액성 피막의 조성을 확인하고, 전타액, 악하선타액 및 이 하선타액에서 유래하는 타액성 피막의 성분을 비교하는 것이다. 네 가지 서로 다른 종류의 교정용 브라켓을 본 연구에 사용하였다. 이들은 $022{\times}028$ Roth Prescription의 상악 소구치 브라켓으로 조성은 다음과 같다: 스테인레스 스틸, 단결정 사파이어, 다결정 알루미나 및 플라스틱 브라켓. 교정용 브라켓을 각각 전타액, 이하선타액 및 악하선타액에 2시간 배양하여 타액성 피막을 형성시켰다 브라켓 피막의 타액성분은 sodium dodecyl sulfate-polyacrylamide gel electrophoresis, Western transfer method 및 면역검출법을 통해 확인하였다. 이 결과 low-molecular weight salivary mucin, ${\alpha}-amylase$, secretory IgA (sIgA), acidic proline-rich proteins, cystatins 등이 모든 브라켓의 타액성 피막에 존재하였으며, 치아우식증의 원인균인 Streptococcus mutans의 부착을 촉진시키는 타액단백질인 high-molecular weight mucin은 어떤 브라켓에도 부착하지 않았다. 그러나, 비록 동일한 타액단백질이 모든 브라켓에서 발견되었지만, 타액단백질 부착 양상은 타액의 종류 및 브라켓의 종류에 따라 양적 및 질적으로 다르게 나타났다. 특히 sIgA는 이하선타액에서 유래한 브라켓 피막에 더 많이 부착하였고, cystatins의 경우는 플라스틱 브라켓에서 유래한 브라켓 피막에 더 많이 존재하였다 본 연구는 다양한 타액단백질이 교정용 브라켓에 부착하며, 타액단백질이 타액의 출처 및 브라켓의 종류에 따라 교정용 브라켓의 표면에 선택적으로 부착함을 나타내었다.

Effects of ultrasonic instrumentation with different scaler-tip angulations on the shear bond strength and bond failure mode of metallic orthodontic brackets

  • Objective: To evaluate the effects of ultrasonic instrumentation with different scaler-tip angulations on the shear bond strength (SBS) and bond failure mode of metallic orthodontic brackets. Methods: Adhesive pre-coated metallic brackets were bonded to 72 extracted human premolars embedded in autopolymerizing acrylic resin. The teeth were randomly divided into 3 groups (n = 24 each) to undergo no treatment (control group) or ultrasonic instrumentation with a scaler-tip angulation of $45^{\circ}$ ($45^{\circ}$-angulation group) or $0^{\circ}$ ($0^{\circ}$-angulation group). SBS was tested in a universal testing machine, and adhesive remnant index (ARI) scores were recorded. The Kruskal-Wallis test and Mann-Whitney U-test were used for statistical analysis. Results: The control group had a significantly higher mean SBS value than the treated groups, which showed no significant differences in their mean SBS values. The ARI scores were not significantly different among the groups. Conclusions: Ultrasonic instrumentation around the bracket base reduces the SBS of metallic orthodontic brackets, emphasizing the need for caution during professional oral hygiene procedures in orthodontic patients. The scaler-tip angulation does not influence the SBS reduction and bond failure mode of such brackets.

Orthodontic appliances and MR image artefacts: An exploratory in vitro and in vivo study using 1.5-T and 3-T scanners

  • Purpose: The aim of this study was to assess the artefacts of 12 fixed orthodontic appliances in magnetic resonance images obtained using 1.5-T and 3-T scanners, and to evaluate different imaging sequences designed to suppress metal artefacts. Materials and Methods: In vitro, study casts of 1 adult with normal occlusion were used. Twelve orthodontic appliances were attached to the study casts and scanned. Turbo spin echo (TSE), TSE with high readout bandwidth, and TSE with view angle tilting and slice encoding for metal artefact correction were used to suppress metal artefacts. Artefacts were measured. In vivo, 6 appliances were scanned: 1) conventional stainless-steel brackets; 2) nickelfree brackets; 3) titanium brackets; 4) a Herbst appliance; 5) a fixed retainer; and 6) a rapid maxillary expander. The maxilla, mandible, nasopharynx, tongue, temporomandibular joints, and cranial base/eye globes were assessed. Scores of 0, 1, 2, and 3 indicated no artefacts and minor, moderate, and major artefacts, respectively. Results: In vitro, titanium brackets and the fixed retainer created minor artefacts. In vivo, titanium brackets caused minor artefacts. Conventional stainless-steel and nickel free brackets, the fixed retainer, and the rapid maxillary expander caused major artefacts in the maxilla and mandible. Conventional stainless-steel and nickel-free brackets caused major artefacts in the eye globe (3-T). TSE with high readout bandwidth reduced image artefacts in both scanners. Conclusion: Titanium brackets, the Herbst appliance, and the fixed retainer caused minor artefacts in images of neurocranial structures(1.5-T and 3-T) when using TSE with high readout bandwidth.

복합레진과 Glass Ionomer Cement수복물에 대한 Bracket의 접착전단강도 (THE SHEAR BOND STRENGTH OF TWO ADHESIVES BONDED TO COMPOSITE RESIN AND GLASS IONOMER CEMENT RESTORATIONS)

  • If the bond strength is sufficient to resist orthodontic force, orthodontic brackets can be bonded to restorations. Orthodontic brackets were bonded to composite resin and glass ionomer cement restorations with no-mix adhesive or glass ionomer cement. The shear bond strength of adhesives bonded to restorations was studied in vitro. Orthodontic brackets were bonded to 10 extracted natural teeth, 40 composite resin restorations and 40 glass ionomer restorations. The surfaces of composite resin restorations were roughened or applied with bonding agent (Scothbond) after surface roughening. The surfaces of glass ionomer cement restorations were conditioned with acid etching or applied with Scotchbond to etched surface. The adhesive was no-mix resin or glass ionomer cement. The shear bond strength was measured. The results were as follows: 1. Orthodontic brackets could be bonded to composite resin restorations effectively as they could be bonded to acid etched enamel with no-mix adhesive. The shear bond strength was sufficient to resist orthodontic force and was not affected by bonding agent greatly. 2. The shear bond strength of no-mix adhesive bonded to acid etched glass ionomer cement restorations was sufficient to resist orthodontic force. However. the fracture risk of glass ionomer cement restorations was increased during debonding. The bonding agent couldn't increase the shear bond strength greatly. 3. The shear bond strength of glass ionomer cement bonded to glass ionomer cement restorations was lower than that of no-mix adhesive. The shear bond strength was sufficient to resist orthodontic force and was greatly decreased by bonding agent. 4. The shear bond strength of glass ionomer cement bonded to composite resin restorations was too low to resist orthodontic force.

Comparison of shear bond strength of orthodontic brackets using various zirconia primers

  • Objective: The aim of this study was to compare the shear bond strength (SBS) of orthodontic brackets bonded to zirconia surfaces using three different zirconia primers and one silane primer, and subjected to thermocycling. Methods: We designed 10 experimental groups following the surface treatment and thermocycling. The surface was treated with one of the following method: no-primer (NP), Porcelain Conditioner (PC), Z-PRIME Plus (ZP), Monobond Plus (MP) and Zirconia Liner Premium (ZL) (n=20). Then each group was subdivided to non-thermocycled and thermocycled groups (NPT, PC, ZPT, MPT, ZLT) (n=10). Orthodontic brackets were bonded to the specimens using $Transbond^{TM}$ XT Paste and light cured for 15 s at $1,100mW/cm^2$. The SBS was measured at a 1 mm/min crosshead speed. The failure mode was assessed by examination with a stereomicroscope and the amount of bonding resin remaining on the zirconia surface was scored using the modified adhesive remnant index (ARI). Results: The SBS of all experimental groups decreased after thermocycling. Before thermocycling, the SBS was ZL, $ZP{\geq}MP{\geq}PC>NP$ but after thermocycling, the SBS was $ZLT{\geq}MPT{\geq}ZPT>PCT=NPT$ (p > 0.05). For the ARI score, both of the groups lacking primer (NP and NPT) displayed adhesive failure modes, but the groups with zirconia primers (ZP, ZPT, MP, MPT, ZL, and ZLT) were associated with mixed failure modes. Conclusions: Surface treatment with a zirconia primer increases the SBS relative to no-primer or silane primer application between orthodontic brackets and zirconia prostheses.

  • The purpose of this study was to evaluate and compare the shear bond strengths and failure sites of metal brackets bonded with chemically cured adhesive and light-cured adhesive. 10 brackets were bonded on prepared enamel surfaces with $Transbond^{circledR}$ (Unitek/3M; U.S.A.) light-cured orthodontic adhesive and another 10 brackets were bonded with $Ortho-one^{\circledR}$ (Bisco:U.S.A.) chemically cured orthodontic adhesive. 24 hours after bonding, the Instron universal testing machine was used to measure the shear bond strengths. The failure sites were examined under streoscopic microscope. The results were as follows: 1 . The mean shear bond strength of metal brackets bonded with light-cured adhesive was lower than that of metal brackets bonded with chemically cured adhesive, but the difference was not statistically significant (p < 0.05). 2. Regardless of the type of adhesives, the brackets were failed primarily at the bracket base-adhesive interface. 3. Bonding of metal brackets with light-cured adhesive is considered to be clinically acceptable.

수종 교정용 접착제의 전단 접착강도 비교 (A COMPARISON OF SHEAR BOND STRENGTH OF VARIOUS ORTHODONTIC ADHESIVES)

  • Bonding of brackets is one of the essential factors for successful orthodontic treatment' so bond strength of orthodontic adhesives are very important. The purposes of this research were to compare shear bond strength of various orthodontic adhesives and to evaluate failure sites. One-hundred twenty extracted human first premolars were prepared for bonding and premolar brackets were bonded to prepared enamel surfaces with Super C Ortho, Mono-$Lok^2$, Transbond, and Super C Ortho after applying Fluorobond. After bonding of brackets, teeth specimens were divided into 3 groups. In group 1 specimens were stored at humidor $37^{\circ}C$ in 1 hour, in group 2 specimens were stored at humidor $37^{\circ}C$ in 24 hours, thermocycled 10 times and in group 3 specimens were stored at humidor $37^{\circ}C$ in 24 hours, thermocycled 1800 times. Then the universal testing machine Instron 6022, Instron Co., U.S.A. was used to test the shear bond strength of brackets to enamel. After debonding, brackets and enamel surfaces were examined under stereoscopic microscope to determine the failure sites The results were as follows : 1. Shear bond strength was significantly highest of using Super C Ortho after applying Fluorobond and Super C Ortho In group 1, was highest of using Super C Ortho in group 2, and was highest of using Mono-$Lok^2$ in group 3. 2. According to time and temperature change, in using Super C Ortho the group 2 had significantly highest strength and group 3 had lowest strength, in using Mono-$Lok^2$ the group 2 and had higher strength than group 1 and in using Super C Ortho after applying Fluorobond shear bond strength decreased constantly, 3. The failure sites were tooth-resin interface in Super C Ortho after applying Fluorobond, Mono $Lok^2$ and Transbond and were at almost same ratio bracket base-resin interface and tooth-resin interface in Super C Orth.

  • Use of ready-made orthodontic appliance can lead to inefficiencies in the final stages of the orthodontic treatment. Because patients' teeth have anatomic variations, brackets that have been designed to fit on average tooth surface may result in positional discrepancies when leveling and alignment is completed. As a result, additional steps such as rebonding, wire bending and use of auxiliaries may be needed. Even in patients who have normal tooth anatomy and proper tooth size relationships, precise bracket placement is crucial in order to efficiently control the tooth positions. Digital models can provide advantages in clinical orthodontics as virtual tooth setup could be performed, and clinicians can easily visualize the predicted final occlusion. Through this setup model, customized brackets with individualized prescription and archwires that optimally fit with the patients' dental arches can be produced using CAD/CAM technology. Also, the brackets can be accurately placed with an aid of 3D-printed jigs. The purpose of this article is to introduce the commonly used labial and lingual customized orthodontic appliance systems using digital technology.
