This study was attempted to explore the relationship between locus of control and the discomfort of the patients at the initial stage of the orthodontic treatment. Locus of control was measured by 'Locus of Control(LOC) scale for children' and 'Orthodontic Locus of Control(OLOC) scale for children'. The discomfort was measured by 'discomfort index card' in 52 children and adolescent patients who initiate orthodontic treatment. In addition, locus of control of the patients' mothers was measured by 'Orthodontic Locus of Control(OLU) scale for parents'. The results were as follows : 1. The test-retest reliability of 'Locus of Control(LOC) scale for children' and 'Orthodontic Locus of Control(OLOC) scale' was in the moderate to high range. 2. Out of 52 patients, 47 showed moderate to severe discomfort following placement of an initial archwire. The patients showed the most severe discomfort on the first day, and most of the discomfort was manifested within the first 3 days, then decreased until the 7th day. There was no significant difference in the discomfort according to sex and age. 3. The discomfort of the patients was the highest in the morning session when a day was divided into 4 sessions, i.e., morning, afternoon, evening, and night. 4. In the score of 'Locus of Control(LOC) scale for children' and 'Orthodontic Locus of Control(OLOC) scale for children', the group of internal locus of control expressed more discomfort than the group of external locus of control. And there was no significant correlation between locus of control of the patients and that of their mothers. 5. There was no significant difference in the score of locus of control according to sex and me. However, the score of boys tended to be lower than that of girls and the score of primary school students higher than that of middle and high school students.
Objective: This study was performed to explore the effect of different bracket, archwire, and ligature combinations on resistance to sliding (RS) and rotational control in first-order angulation. Methods: Three types of brackets (multi-level low friction [MLF], self-ligating, and conventional brackets) coupled with four nickel-titanium archwires (0.012, 0.014, 0.016, and 0.018-inch diameter) and two stainless steel ligatures (0.20 and 0.25 mm) were tested in different first-order angulations ($0^{\circ}$, $2^{\circ}$, $4^{\circ}$, $6^{\circ}$, $8^{\circ}$, $10^{\circ}$, $15^{\circ}$, $20^{\circ}$) by using an Instron universal mechanical machine in the dry state at room temperature. RS value was evaluated and compared by one-way ANOVA. Results: Under the same angulation, the RS values showed the following order: conventional brackets > MLF brackets > self-ligating brackets. The RS was the highest for conventional brackets and showed a tendency to increase. The RS for MLF brackets coupled with thinner archwires and ligatures showed a similar tendency as the RS for the self-ligating bracket. In contrast, the RS for MLF brackets coupled with thicker archwires and ligatures increased like that for conventional brackets. MLF brackets showed the greatest range of critical contact angles in first-order angulation. Conclusions: The RS in first-order angulation is influenced by bracket design, archwire, and ligature dimension. In comparison with self-ligating and conventional brackets, MLF brackets could express low friction and rotational control with their greater range of critical contact angles.
Objective: The purpose of this study was to evaluate the effects of chewing gum and low-level laser therapy in alleviating orthodontic pain induced by the initial archwire. Methods: Patients with 3-6 mm maxillary crowding who planned to receive non-extraction orthodontic treatment were recruited for the study. Sixty-three participants (33 females and 30 males) were randomly allocated into three groups: laser, chewing gum, and control. In the laser group, a gallium aluminum arsenide (GaAlAs) diode laser with a wavelength of 820 nm was used to apply a single dose immediately after orthodontic treatment began. In the chewing gum group, sugar-free gum was chewed three times for 20 minutes-immediately after starting treatment, and at the twenty-fourth and forty-eighth hours of treatment. Pain perception was measured using a visual analog scale at the second, sixth, and twenty-fourth hours, and on the second, third, and seventh days. Results: There were no statistically significant differences between the groups at any measured time point (p > 0.05). The highest pain scores were detected at the twenty-fourth hour of treatment in all groups. Conclusions: Within the limitations of the study, we could not detect whether low-level laser therapy and chewing gum had any clinically significant effect on orthodontic pain. Different results may be obtained with a higher number of participants or using lasers with different wavelengths and specifications. Although the study had a sufficient number of participants according to statistical analysis, higher number of participants could have provided more definitive outcomes.
There are varieties of severe malocclusions, which can be treated orthodontically, but with a great deal of effort. Anterior openbite, in particular, is one malocclusion thought to be more difficult to treat, and therefore, most of them have to be corrected by means of surgical intervention. To solve these problems, numerous studies pertinent to treatment modalities have been introduced with controversies on the effectiveness of treatment. Suggested treatment modalities for anterior openbite are based directly or indirectly on the neuromuscular and morphological features and on the etiologic and/or the environmental factors. Even though the vertical relationship of the face is increased due to the growth variation, the normal occlusal relationship can be achieved by the adequate dentoalveolar compensatory mechanism, but in the case of inadequate or negative dentoalveolar compensation, openbite is likely to be present. If the skeletal dysplasia is too severe to be solved by orthodontic treatment alone, combined treatment with surgery should be done to restore the function and the esthetics of the orofacial complex. In many cases, however, orthodontic alteration of the dentition pertinent to the given skeletal pattern with the proper diagnosis and treatment planning can bring satisfactory results. The treatment changes with the Multiloop Edgewise Archwire(MEAW) therapy occurred mainly in the dentoalveolar region and showed a considerable similarity to the natural dentoalveolar compensatory mechanism. In other words, the MEAW technique allows orthodontists to produce the natural dentoalveolar compensation orthodontically. Even if an openbite is corrected by the orthodontic dentoalveolar compensation suitable for the skeletal pattern, relapse may still occur by the persisting etiologic factors which originally prohibited the natural dentoalveolar compensation. The etiologic factors should be determined at the time of initial diagnosis and should be controlled during treatment and retention.

Purpose: The purpose of this study was to investigate the differences of displacement pattern depending on type of sliding jig and application method during maxillary molar distalization with temporary anchorage devices (TADs). Materials and Methods: Maxilla with normal tooth size and arch shape was selected to create a 3-dimensional finite element model, which included the bracket, orthodontic main archwire, removable sliding jig (R-jig). The orthodontic mini-implant anchorage was set 8 mm superiorly from main archwire, buccally between the second premolar and first molar. The base experimental design was Condition 1, which was composed $0.019{\times}0.025$ inch stainless steel (SS) of wire size of R-jig, 200 gm force, un-tied state. And the other designs varied to wire size of R-jig, magnitude of force. The results are as follows. Results: As the wire size of R-jig was increased, the deformation of R-jig was decreased. However, the displacement of second molar wasn't different each other. As the force to second molar was increased, the more displacement of second molar was observed, and the more distal tipping movement, vetical displacement was observed. Conclusion: R-jig can get distal teeth movement in orthodontic treatment without side effects.
In the correction of dental Class III molar relationship in skeletal Class II patients, uprighting of the mandibular posterior segments without opening the mandible is an important treatment objective. In the case reported herein, a C-tube miniplate fixed to the lower labial symphysis and connected with a nickel-titanium reverse-curved archwire provided effective uprighting of the lower molars, without the need of orthodontic appliances on the mandibular anteriors. Using this approach, an appropriate magnitude of force is exerted on the molars while avoiding any negative effect on the mandibular anteriors.
The aim of this study were to measure and compare the stress level on three type brackets and each other material (stainless steel, ceramic) with tipping and torquing forces by using the finite element analysis and to design biomechanically favorable brackets. For this study, three kinds of brackets were selected(A:Transcend-RMO, B:Signature-Unitek, C:PAW: plain archwire appliance-applied for a patent in Yonsei Udiversity). The slot size of bracket was 0.022inch and the size of archwire was 0.0175x0.025inch and taper shaped archwire was used in PAW. Loading force in tipping was 4.27N and torquing force was 32.858N applied by archwire torsion with 19.7degree and 11.3 degree in C type bracket. The conclusions were that (1) The finite element method proved to be a useful tool in the stress analysis of orthodontic bracket subjected to various forces. (2) With tipping, the stresses were concentrated at the gingival wall of the wire slot where it meets the mesial bracket surface and the incisal wall of the wire slot where it meets the distal bracket surface and with torquing, the stresses were concentrated at the junction of the gingival or incisal wall and base of the slot. (3) The maximum stress value was higher in torquing force than tipping force and therefore it is desirable to design on the basis of torquing force. (4) It was considered that the change in material might be affect on the diminish of stress value in the place of stess concentration. (5) The maximum stress value was highest on PAW bracket when the tipping and torquing force was applied and therefore it would be desirable to use mechanically favorable material on PAW bracket.
The Purpose of this study was fourfold - to evaluate the general laws of friction applied to orthodontic conditions, to compare archwire materials under these controlled conditions, to compare ligation method, and to measure the effect of the artificial saliva on friction with these materials Three wire alloys (Cobalt-chromium, Nickel-titanium, Beta-titanium) in two size wires (.016" , .016" ${\times}$.022" ) were examined respect to the bracket (.018" ${\times}$.025" standard), and two ligature material (stainless steel, elastomeric) in dry and wet conditions The results were as follows, 1. The order of frictional force against alloy materials was Co-Cr (lowest), Ni-Ti, and ${\beta}$-Ti(highest) - with the exception of elastomeric ligation under wet conditions. 2. S.S. ligation gave rise to significantly greater friction than elastomeric ligation did. 3. Testing in the presence of saliva, rather than in dry conditions, decreased the frictional force for S.S. ligation with .016" Co-Cr, Ni-Ti, ${\beta}$-Ti. but, increased the frictional force for S.S. ligation with .016" ${\times}$ .022" Co-Cr, Ni-Ti, ${\beta}$-Ti. 4. .016" ${\times}$.022 " wire generated more friction than .016" wire.
The purpose of this study was to amount of the frictional forces with the brackets and wires, ligation methods, dry/wet, offsets, interbracket distances, velocity and to compare them each other by different conditions. This study tested 0.018'x0.025' slot sized 8 types of orthodontic bracket systems and 0.016', 0.016'x0.022' sized stainless steel, NiTi, Cu-NiTi orthodontic wires. One cuspid bracket were positioned on the slide glass and archwire was engaged into bracket and ligated with elastomeric modules. The values of frictional forces were measured with the instron universal testing machine. The results were as follows; 1. Polycrystalline ceramic bracket had the highest mean frictional forces and followed and by ceramic reinforced plastic bracket, metal bracket, plastic bracket with metal slot, monocrystalline ceramic bracket, single bracket, self-ligating bracket, friction free bracket in descending order. The self-ligating bracket showed low frictional forces in the round wires and high frictional forces in the rectangular wires. 2. Stainless steel wires had the least frictional forces and followed by NiTi, Cu-NiTi wires in descending order. Round wires had lower frictional forces then that of rectangular wires. 3. The stainless steel ligation method had significantly greater mean frictional forces them the elastomeric module ligation method. 4. Artificial saliva statistically increased the frictional forces in stainless steel wire, NiTi wire and Cu-NiTi wire. 5. There was a statistically significant difference with offset change 6. There was no statistically significant difference with interbracket distance in stainless steel wires but a significant difference in NiTi wires as the interbracket was decreased. 7 There was no statistically significant difference with velocity change. From the above findings, self-ligating bracket, stainless steel wires and the elastomeric module ligation method might be effective than any other materials to reduce the frictional forces in the orthodontic treatment and can be correlated to clinical situations seen in orthodontic patient care.
Objective: To evaluate the therapeutic effects of a preformed assembly of nickel-titanium (NiTi) and stainless steel (SS) archwires (preformed C-wire) combined with temporary skeletal anchorage devices (TSADs) as the sole source of anchorage and to compare these effects with those of a SS version of C-wire (conventional C-wire) for en-masse retraction. Methods: Thirty-one adult female patients with skeletal Class I or II dentoalveolar protrusion, mild-to-moderate anterior crowding (3.0-6.0 mm), and stable Class I posterior occlusion were divided into conventional (n = 15) and preformed (n = 16) C-wire groups. All subjects underwent first premolar extractions and en-masse retraction with preadjusted edgewise anterior brackets, the assigned C-wire, and maxillary C-tubes or C-implants; bonded mesh-tube appliances were used in the mandibular dentition. Differences in pretreatment and post-retraction measurements of skeletal, dental, and soft-tissue cephalometric variables were statistically analyzed. Results: Both groups showed full retraction of the maxillary anterior teeth by controlled tipping and space closure without altered posterior occlusion. However, the preformed C-wire group had a shorter retraction period (by 3.2 months). Furthermore, the maxillary molars in this group showed no significant mesialization, mesial tipping, or extrusion; some mesialization and mesial tipping occurred in the conventional C-wire group. Conclusions: Preformed C-wires combined with maxillary TSADs enable simultaneous leveling and space closure from the beginning of the treatment without maxillary posterior bonding. This allows for faster treatment of dentoalveolar protrusion without unwanted side effects, when compared with conventional C-wire, evidencing its clinical expediency.
