Review of Case Reports of Neuropsychiatry Disease in Oriental Medicine (한의학에서 신경정신과 질환의 증례논문 동향분석)

  • This study was aimed to review the trend of case reports of neuropsychiatry disease. The case reports were collected by retrieving Journal of Oriental Neuropsychiatry and analyzing period were from 1990 to 2010. We analyzed them for publish year, research institution, patients, sorts of diseases, pattern identification diagnosis, instrument, treatment period, sorts of treatments, and treatment with western medicine. We analyzed 142 case reports and there were many kinds of neuropsychiatry diseases and instruments that were evaluated the process. The reserchers used various treatment with oriental medicine. However, the number of case reports have decreased since the year of 2008. Most of the research institutions were university and general oriental medical hospital and the portion of private hospitals was only 2.11%. For the development of practical clinical trial of oriental medicine, we needs more qualifying case report. We hope more reserchers for psychiatry disease of oriental medicine will be interested in the publication of case report and this will serve to produce advance of oriental medicine.

The Study of Oriental Medicine Music Therapy Applied to the Acupuncture of Pathogenic Fire (화열병(火熱病) 침구치료 시 적용 가능한 한방음악치료에 관한 연구)

  • Objectives : The purpose of this study is to introduce oriental medicine music therapy during the acupuncture of pathogenic fire. Methods : We investigated the acupuncture points of pathogenic fire and the methodology of oriental medicine music therapy. Results : During the acupuncture of pathogenic fire, patients listen to proper music and after withdrawing of acupuncture, patients play the musical instrument. The patients with the case of fire due to affection by exopathogen, the method of dispelling superficial fire can be used through playing percussion instruments. The case of patients with the fire of five organ, the Five Phases music can be used properly. Conclusions : This study describes oriental medicine music therapy has a good possibility as a treatment of pathogenic fire with acupuncture.

  • Through the survey of related studies for last ten years, it was known that accurate sample data of sasang constitution are the most important condition in sceintific research of sasang constitutional medicine. So we can propose three step for scientific sasang constitution medicine: firstly, the construction of SCIM(Sasang Constitutional Information Bank); secondly, the development of instruments for constitution diagnosis; thirdly, the new diagnosis methods based on the organic definition of sasang constitution.

Study of Instruments for Assessment and Clinical Research Trends in Common Cold (감기 임상연구의 최신 동향 및 평가도구에 관한 연구)

  • Objectives: This study was conducted to help clinical studies for treating the common cold with Oriental medicine. We inspected tendencies of clinical studies of the common cold in western medicine and analyzed the assessing instruments for it by scrutinizing PubMed publications for the last five years. Materials and Methods: We inspected 32 theses and scrutinized their objectives, periods, participants, materials and methods, the instrument of assessment for the studies, and validity, reliability, criteria and symptom score for questionnaires, results and JADAD score. Results: 1. The median for total period of study was 7 months, the median for days of treatment was 7.5, and most studies were carried out during the winter/spring period, because of the enhanced risk of cold infections. 2. The studies focused on unorthodox medicines or new drug products, such as antipyretic analgesics (e.g. paracetamol), decongestant and bronchodilator (e.g. ephedrine), echinacea, antioxidants (e.g. zinc), probiotic bacteria and so on. 3. All theses except one had subjectscomposed of cold patients who were infected naturally. The median number of subjects for final analysis was 187. 4. 27 studies used survey as the instrument of assessment. Among these, only one thesis was verified for validity. 5. The mean of JADAD score was 4.41, suggesting most of theses of worth. 7 studies were assessed as ineffective. Conclusion: Recent studies of the common cold are focused on unorthodox medicines or new drug products, and it is necessary to provide an objective instrument for assessing common cold treatments.

A Correlation Study of Pattern Identification Instrument and Biomarkers for Polycystic Ovary Syndrome (다낭성난소증후군 변증도구와 생체지표 간의 상관성 연구)

  • Objectives: The purpose of this study was to investigate the correlation between Pattern identification instrument and biomarkers in polycystic ovary syndrome patients. Methods: Pattern identification instrument questionnaire, Sasang constitutional test, body composition test, pulse analyzing test, laboratory test were performed and the results were analyzed. Correlation analysis was performed between pattern identification and laboratory test results. Results: Testosterone, SHBG, $17{\alpha}$-OH-progesterone, AMH were highest in the Kidney-Yang Deficiency (腎陽虛) group. LH, FSH, LH/FSH ratio were the highest in the Endogenous Heat due to Yin Deficiency (陰虛內熱) group. DHEA-S was the highest in the Dampness-Heat in Liver Meridian (肝經濕熱) group. E2, Prolactin, Cholesterol, Triglyceride, HDL-Cholesterol, LDL-Cholesterol, Glucose (FBS), Insulin, HOMA-IR ratio, HbA1c were the highest in the Phlegm-Dampness (痰濕) group. Conclusions: In this study, we obtained basic data analyzing the correlation between pattern identification instrument and biomarkers in polycystic ovary syndrome patients. If further studies are performed, we expect to be able to obtain clues to study the mechanism of polycystic ovary syndrome.

  • Objective: The purpose of this study is to investigate how to use the Quality of Life(QoL) of obesity and to study the report cases which measure QoL on obesity. Method: This study investigate the definition of Health-related Quality of Life(HRQoL), the measurement of Health-related Quality of Life, and the papers of Health-related Quality of Life on obesity through the books and medicine journals of HRQoL and obesity. Conclusions: 1. The QoL includes role functional ability, the degree and quality of social and community interaction, psychological state, somatic feeling, and life satisfaction than personal health and social well-being. 2. The investigator needs to evaluate the reliability, validity and sensitivity of the scale, and the appropriateness of the instrument for the target population When he decides measurement. 3. The investigator have the well-drilled supporter or himself managed the data and study population to prevent missing data. 4. The investigator should decide which is needed on obesity a obesity-specific or broad-ranging instrument, or both.

  • The purpose of this study is to introduce oriental medicine music therapy related to the treatment of obesity. Obesity is caused by deficiency of the spleen, retention of phlegm, stagnation of liver Gi. According to the patients' state, the musical instruments should be chosen. In case of deficiency of the spleen, retention of phlegm, and stagnation of liver Gi, the obese patients beat pulsatile instruments to Earth-Gi rhythm, Fire-Gi rhythm, and Wood-Gi rhythm respectively. In addition to beating the musical instrument to the Five Phases rhythm, an oral sound therapy, relieving constraint therapy, and dispersing therapy can be performed. This study describes the theory of oriental medicine music therapy that has a good possibility as a new method of oriental medical treatment. It is necessary to study in measuring the effects after oriental medicine music therapy from now on.

Improvement and Limitations in the Sasang Constitution Diagnosis by the Instrument-based Pulse Diagnosis (맥진을 이용한 사상체질 판별 방법의 개선 및 의의)

  • Recently, there have been emerging research activities in classifying the Sasang constitution objectively by machine-based methods. The concordance rate of the classification by the pulse wave features was, however, only around 50% up to now. In this paper, we introduce a novel classification algorithm that can promote the accuracy substantially at the expense of the non-classifiable subgroup as a byproduct. For instance, with the pulse wave features alone, we show that female/male subject group in their 20s can be classified into the Sasang constitution group with the concordance rate of 68.4%/65.5% for a subgroup of 57/29(31%/15%) subjects out of 184/195, by leaving the other subjects as the non-classifiable group. Next, we show that the pulse diagnosis has been used only as a supportive tool in determining one's constitution, and consequently the accuracy of the concordance ratio by the pulse wave features alone cannot exceed a finite value, which we estimate to be about 60%.

Researches Trend of Korea Institute of Oriental Medicine in Sasang Constitution Field - from 2007 to 2010 (한국한의학연구원의 사상체질 연구동향 - 2007년부터 2010년까지의 논문을 중심으로 -)

  • Objective : The aim of this study is to investigate current status of Sasang constitutional medicine researches and to provide source in setting a development strategy and making a policy on the Sasang constitutional medicine. Method : This study analyzed trends in research as represented in the 142 articles of Korea Institute of Oriental Medicine in the field of Sasang constitution medicine from 2007 to 2010. Result : The main themes of studies were Sasang constitution clinical study (68, 47%) and developing instruments for measuring diagnosis (48, 33%), especially pulse diagnosis (25, 17%). Genetic researches and trends studies on Sasang constitutional medicine were respectively 17 (12%) and 8 (5%). As a result, Korea Institute of Oriental Medicine established Sasang clinical information bank, and discovered clinical character of Sasang constitution to develope various diagnosis tools include constitution survey form and pulse diagnosis instrument. Genetic researches also discover constitution related genes such as FTO and MC4R, and explore association between heredity and Sasang constitution. Conclusion : The above mentioned achievements of Korea Institute of Oriental Medicine were limited to diagnosis process, unquestionably studies for reproducibility and authenticity of constitution diagnosis were important mission of Sasang constitutional medicine. However, to develop the advance of Sasang constitutional medicine, not only constitution diagnosis but also constitutional treatment studies should be conducted.

Study on the Repeatability and Reproductivity of a Moire Body Shape Analyser (모아레를 이용한 체형분석의 반복성 재현성에 관한 연구)

  • Background : As each human has a look in the face of oneself, he or she has a look of him or herself in the shape of the body also. And for the shape of the body which gives a big clue in diagnosis in musculoskeletal disorders. Therefor many means are used and developed for diagnosis through body shape or posture analysis for musculoskeletal disorders. X-ray, CT, MRI has been used for diagnosis through image in this way to tell about the inside of the human body. On the other side, moire topography was used for information about the exterior of the human body, but yet only minimal information such as the number of contour lines in each side was available. Therefor there were a few studies to use moire topography or other methods to get information about the surface of the human body in numeric values. The instrument used in this study which is a laser projection moire, is another trial to get numeric data about the surface of the human body. The instrument is composed of laser projector and a computer software to recompose and analyse the image data into depth, height, angle and length. Objectives : The study was focused on whether the instrument is reliable for clinic use, and to seek the proper environment and posture for the examination, and among the data the software provides, which items are more reliable and useful. Methods : For reproductivity and repeatability, 4 testers tested 2 persons. And to how if the body shape changes according to the posture and which posture gives the most reliable data, the test was performed in 6 different positions. Results : Result, the instrument showed sufficient repeatability and reproductivity for clinical use. And among the items the software provides, the length of the back, the angle of the back in the sagittal and coronal plane showed reliable results. And there was difference in the results according to the posture, and Therefor, in following studies using this instrument or similar type of posture analysing instruments, the length of the back, the angle of the back in the sagittal and coronal plane could be reliable item to use.
