Relations between the Questionnaire and Health State in each Sasang Constitution based on the Oriental medical doctors' diagnoses (전문가 진단을 통한 체질별 건강수준과 설문결과의 연관성 연구)

  • 1. Objectives: We report the relation between the questionnaire and the health state on the Sasang Constitution. 2. Methods: We carried out a clinical test, in which 2 Oriental medical doctors participated to diagnose health state in each Sasang Constitution with 239 subjects. The subjects were divided into 3 subgroups, which are allocated to healthy group, poor health group and disease group. 3. Results: We extracted 4 questions for Taeeumin, 5 questions for Soyangin and 3 questions for Soeumin from the whole 98 questions respectively. Soyangin and Taeeumin share a same question that shows significant difference by health status. For the statistical analysis, we used a Chi-square test. As a result, we found out that Taeeumin showed different distributions of answers in direct/indirect characteristic, perspiration, frequent urination, regular defecation. According to the health status Soyangin showed the different distribution of the answers in color of urination, frequent urination, mouth dryness, abnormal perspiration, discomfort of head. Soeumin showed the different distribution of the answers in cold and chilly feel of abdomen, digestion status and cause of digestion problems when body condition gets worse. 4. Conclusions: This is the first work which reports the relatedness between Questionnaire and Health State in each Sasang Constitutions by an objective clinical test. We found out that several questions have different distributions by health status in each Sasang Constitution. These extracted questions are categorized not only by Wansilmoobyung(완실무병), but also by other symptoms and body status.

Pattern Identification of 97 Functional Dyspepsia Patients and the Characteristics of Each Pattern Type (기능성 소화불량 환자 97명의 변증유형별 특성)

  • Objective: This study was designed to identify and explore the pathological patterns of functional dyspepsia (FD) patients. We also evaluated the usefulness of the Pattern Identification Questionnaire by comparing it with other assessment tools for FD. Methods: We recruited 97 FD patients based on the Rome III criteria for FD diagnosis. The pathological patterns of the subjects were determined by the Pattern Identification Questionnaire. Their dyspepsia-related symptoms were assessed using the Gastrointestinal Symptom Questionnaire (GIS) and the Pyeongwi-san (Pingwei-san) Patternization Questionnaire. Depressive symptoms were evaluated with the Beck Depression Inventory (BDI) and quality of life with the Functional Dyspepsia-Related Quality of Life (FD-QoL) Questionnaire. Tongue coating was measured by the Digital Tongue Diagnosis System (DTDS). Results: The male to female ratio was 1:1.1, and the forties and fifties age groups were largest in number. The spleen deficiency and phlegm-dampness pattern was the most common pattern found among the FD patients. No significant differences in the GIS, BDI, FD-QoL, and DTDS scores were found among the five pattern types. All pattern types showed significant correlation with GIS, Pyeongwi-san Patternization Questionnaire, and FD-QoL scores. Conclusions: Pattern Identification Questionnaire can not only identify the pathological pattern types of FD patients but also evaluate the severity of their symptoms. Compared to conventional assessment tools for FD, it could enable a more dynamic evaluation of FD patients reflecting the severity of dyspeptic symptoms and the quality of life. Further studies on the Pattern Identification of FD patients are anticipated in order to improve the diagnosis and therapy for Korean FD patients.

  • Objectives : This study was done to evaluate the effect of stopping drinking, using alcoholic liver disease questionnaire, Diagnosis System of Oriental Medicine (DSOM) and Health Related Quality of Life (HRQOL). Methods : 49 men who satisfied the requirement participated in this trial. They stopped drinking for 6 weeks. They were analyzed using DSOM, alcoholic liver disease questionnaire and SF-36. The data were classified by age (<47,$\geq$48) and alcoholic intake per day (<100g,$\geq$100g). For HRQOL, the SF-36v2 Health Survey was used and Quality Metric Health Outcomes Scoring Software 2.0 (QualityMetric, Lincoln, RI, USA) was applied for the analysis. Results : The alcoholic liver disease questionnaire had a partial correlation with DSOM. Generally stopping drinking decreased Heat (熱). Especially in the group drinking over 100g per day, the correlation was high. In the group over 48 years old, spleen (脾) was improved comparatively. In the group with low HRQOL (PCS<31.43, MCS<23.33) deficiency (虛) was improved. Conclusions : We found that stopping drinking can improve pathogenic factors of alcoholic liver disease and the alcoholic liver disease questionnaire be a useful diagnostic method on alcoholic liver disease by comparison with DSOM.

  • This study was designed to investigate job stress, job satisfaction and related factors of oriental medical doctors. The survey used administered questionnaire, was conducted from June 10 to June 30, 2010 in Jeonbuk province. Questionnaire was composed of general characteristics, socioeconomic status perception, job stress scale and job satisfaction scale. Job stress was related to sex, age, marriage, working form, working time, clinical career. Job satisfaction was related to age. Working time, working form and self-status perception influenced job stress. Self-status perception influenced job satisfaction. Especially, it was supposed that working time was most important factor to job stress.

In the Question patients Taeyang items is 50, Soyang items is 53, Taeumin items is 63, Soeumin items is 64. 2) The variable which got above 25 from the Department of Sasang Constitution Medicine specialist, In the Questionaire for doctors Taeyang items is 5, Soyang items is 8, Taeumin items is 7, Soeumin items is 12. items is 32. 3) The variable which got above 25 from the Department of Sasang Constitution Medicine specialist and got above 3 the selection frequency, the Questionaire for doctors Taeyang items is 4, Soyang items is 4, Taeumin items is 5, Soeumin items is 8. In the Questionaire for patients Taeyang items is 10, Soyang items is 17, Taeumin items is 24, Soeumin items is 23.

  • Background and purpose : We previously developed questionnaire of Yukmijiwhang-tang symptom on the Delphi method. But developed a questionnaire was not verified in the clinical. So, to ensure objectivity, quantification and validity, verification is needed for questionnaire items before applying a clinical. On this study, we looked at whether questionnaire items had been validity in the clinical. Methods : The previously developed questionnaire of Yukmijiwhang-tang symptom was applied to 200 patients who visited 12 local oriental medicine clinics. Item data were analyzed by factor analysis and reliability test. Results : The developed questionnaire showed remarkable reliability.(Cronbach's $\alpha$=0.869) In factor analysis, items were clustered to 6 factors. Conclusions : The results of this study indicate that the developed quetionnaire is reliable and valid for Yukmijiwhang-tang symptom. It can be used to evaluate Yukmijiwhang-tang symptom objectively.

Test-retest reliability of the questionnaire in the Sasang constitutional analysis tool (SCAT)

  • Background: The Sasang constitutional analysis tool (SCAT) is an integrated Sasang constitutional analysis system developed by the Korea Institute of Oriental Medicine. This study aimed to evaluate the reliability of a questionnaire for measuring personality and pathophysiological symptoms that is one of the components of the SCAT. Methods: In this study, data were collected from university students in their twenties. Tests were administered twice, with an interval of 4 weeks between tests. Test-retest data from 176 students were collected and used for analysis. Internal consistency reliability was analyzed by using Cronbach's alpha coefficient, and test-retest reliability was analyzed by using Spearman's rank correlation coefficient. Results: Cronbach's alpha coefficient was 0.788 for personality, 0.511 for eating habits, 0.718 for digestion, 0.667 for heat- or cold-wise penchant, and 0.612 for water ingestion. Spearman's rank correlation coefficients, which were used to assess correlations between test and retest results, ranged from 0.444 to 0.828. Conclusion: The internal consistency and test-retest reliability of the SCAT questionnaire were found to be satisfactory.

  • This study was performed to examine a clinical application of the QSCC II (Questionnaire for the Sasang Constitution Classification II) In this study, the subjects were 218 patients whose constitutions were differentiated at the Wonkwang Kwangju Oriental medical hospital. The results are as follows: 1. QSCC II were given to 218 patients after they were examined by the specialists in the Sasang constitutional medicine and received herbal formulas specific to their constitution. The constitutional analysis made by the specialists were then compared to the results of the questionnaire. The result showed only 51% agreement. 2. We made a frequency analysis of 121 questions and found that 85 questions had low statistical significance (P>0.2). 3. As a result, we have found that there is a limitation in differentiating the constitution by the questionnaire. Thus, in order to raise the statistical significance of the constitutional analysis, we hope that more suitable questionnaire will be developed for the future research.

Research on Oriental Medicine Diagnosis and Classification System by Using Neck Pain Questionnaire (경항통 설문지를 이용한 한의학적 진단 및 분류체계에 관한 연구)

  • Objectives : The purpose of this thesis is to help the preparation of oriental medicine clinical guidelines for drawing up the standards of oriental medicine demonstration and diagnosis classification about the neck pain. Methods : Statistical analysis about Gyeonghangtong(頸項痛), Nakchim(落枕), Sagyeong(斜頸), Hanggang (項强) classified experts' opinions about neck pain patients by Delphi method is conducted by using oriental medicine diagnosis questionnaire. The result was classified by using linear discriminant analysis (LDA), diagonal linear discriminant analysis (DLDA), diagonal quadratic discriminant analysis (DQDA), K-nearest neighbor classification (KNN), classification and regression trees (CART), support vector machines (SVM). Results : The results are summarized as follows. 1. The result analyzed by using LDA has a hit rate of 84.47% in comparison with the original diagnosis. 2. High hit rate was shown when the test for three categories such as Gyeonghangtong and Hanggang category, Sagyeong caterogy and Nakchim caterogy was conducted. 3. The result analyzed by using DLDA has a hit rate of 58.25% in comparison with the original diagnosis. The result analyzed by using DQDA has a accuracy of 57.28% in comparison with the original diagnosis. 4. The result analyzed by using KNN has a hit rate of 69.90% in comparison with the original diagnosis. 5. The result analyzed by using CART has a hit rate of 69.60% in comparison with the original diagnosis. There was a hit rate of 70.87% When the test of selected 8 significant questions based on analysis of variance was performed. 6. The result analyzed by using SVM has a hit rate of 80.58% in comparison with the original diagnosis. Conclusions : Statistical analysis using oriental medicine diagnosis questionnaire on neck pain generally turned out to have a significant result.

A Validation Study of the Sasang Constitution Questionnaire for Japanese(SSCQ-J) (일본인용 사상체질진단지의 타당화 연구)

  • Objectives This study was aimed to validate Sasang Constitution Questionnaire for Japanese (SSCQ-J). Methods Sasang Constitution Questionnaire for Patients (SSCQ-P) was developed by joint researches between the Society of Sasang Constitutional Medicine and Korea Institute of Oriental Medicine. We translated SSCQ-P into Japanese and modified some items of that for Japanese. By getting approval from the Institutional Review Board(IRB)of School of Medicine, Keio University, we conducted a questionnaire survey of patients who visited Oriental Medicine Center from early January until mid-February 2011. The total of 364 patients filled out that Questionnaire and gave an interview with a Sasang constitution specialist. Using this Questionnaire data, we made Sasang constitutional classification functions and calculated diagnostic accuracy rate of SSCQ-J using discrimination analysis. Results 1. Male group's diagnostic accuracy rate of SSCQ-J was 77.01% and female was 78.10%. 2. Diagnostic accuracy of SSCQ-J was a little higher than SSCQ-P Conclusions 1. SSCQ-J can be considered to have good discriminant power compared with SSCQ-P 2. Further research with SSCQ-J will be helpful in the comparison study on the usual symptoms between Korean and Japanese as well as development of good discriminant function.
