A Study for the Syndrome Differentiation of Retroverted Uterus Using Ultrasound (초음파 자궁형상진단을 이용한 자궁후굴증의 변증 진단 연구)

  • Objectives: We have studied the syndrome differentiation of ultrasound of retroverted uterus(RU) as the inspection in traditional Korean medicine. Methods: Eighty-four patients, who visited H clinic for infertility treatment, were recruited and evaluated by questionnaires and ultrasound, from April, 2012 to May, 2012. We used ultrasound(Logiq C3; GE Health care) for examination of RU. For statistics, we used Fisher's extract test to evaluate the RU and symptoms, and Chi-square test to evaluated the RU and syndrome differentiations, and Student T-test for the relationship between RU and each score of syndrome differentiation, using PASW Statistics(version 18.0.0) and GraphPad Prism(version 5.01). Results: 1. RU was correlated with moderate or severe dysmenorrhea and premenstrual lower abdominal pain. 2. In syndrome differentiation, blood stasis questionnaire was correlated with RU. 3. RU was related with dysmenorrhea, mass of low abdomen, pressure pain, clod of menstrual blood, dyspareunia. Conclusions: The ultrasound of RU might be related with the syndrome of blood stasis.

A Proposal for Standardization of Tongue Diagnosis Based on Diagnostic Criteria of Tongue Coating Thickness (설진의 표준화를 위한 제언 : 설태 후박의 진단기준을 중심으로)

  • Background : In oriental medicine, the status of the tongue is an important indicator to diagnose the condition of one's health, such as physiological and the clinicopathological changes of internal organs. A tongue diagnosis is not only convenient but also non-invasive, and therefore widely used in Oriental medicine. However, the tongue diagnosis is greatly affected by examination circumstances, patient's posture, and doctor's diagnosis criteria. Objectives : This study was designed to assure the necessity for standardization of tongue diagnosis based on diagnostic criteria of tongue coating thickness (TCT). Methods : Thirty tongue photographs were acquired and analyzed by digital tongue diagnosis system (DTDS) which measured the percentage of TCT on the tongue surface. Fifteen oriental medical doctors evaluated TCT in 30 photographs. Afterward, the 15 assessors were trained for diagnostic criteria of TCT and evaluated the photographs again. The intraclass correlation coefficient (ICC) was used to obtain the agreement rate among the 15 assessors and the agreement rate between assessors' TCT scores and DTDS values. Results : The agreement rate among the 15 assessors after training was higher than before training. The agreement rate between assessors' TCT scores and DTDS values after training was also higher than before training. Furthermore, the difference of the agreement rate between before and after training was significant (p<0.05). Conclusions : The standardization of diagnostic criteria of TCT increased the agreement rate among the assessors and the agreement rate between assessors' TCT scores and DTDS values. Therefore, the standardization of diagnostic criteria is expected to contribute to the objectification and quantification of the tongue diagnosis system.

Medical Certificate as an Evidence of Personal Injury (진단서의 증명력: 상해진단서를 중심으로)

  • Medical certificate is a document to demonstrate a patient's health status, made up and signed by a physician, dentist, or oriental physician who attended the patient. It serves as an evidence in many official process including civil or criminal law suit, especially for one's personal injury. The Korean legal system also acknowledges and protects the evidentiary function of medical certificate by mandating physicians etc. to issue medical certificate in good faith and only when they personally attended the patient, and by criminally punishing them when they do not comply with these legal requirements. There are some reasons, however, that medical certificates often do not reflect the true health status of the patient: When physicians attend the patient and collect information regarding the health status of the patient, their priority is and should be the most cost-effective way to meet the health needs of the patient. It does not necessarily correspond to the accurate examination of the health status of the patient. Even when the patient's report on the history of the illness or the injury seems suspicious, physicians might have to avoid disproving it because that kind of attitude might harm the rapport between the physician and the patient. All these can distort the perception of the physicians and this distortion can be reproduced in the medical certificate they made up. Some of these problems might be resolved or at least enhanced by introducing new form of medical certificate which would guide physicians to reveal the nature, factual and theoretical grounds, and the limit of their findings more accurately. Others, however, would not be able to address, because it stems from the conflict between the physician's primary duty, duty to be loyal to the patient's life and health, and his secondary duty to serve as a public or neutral witness on the health status of the patient, and when both values or duties conflict with each other, they should choose the duty to the patient sacrificing the duty to the public or the court.

Incidence and Related Factors of the Metabolic Syndrome in a Korean Medicine Hospital (한방병원 건강검진 수진자의 대사증후군 발생과 관련요인 연구)

  • The purpose of this study was to investigate incidence and related factors of the metabolic syndrome in a Korean medicine hospital. The 716 subjects were analyzed using biochemical data and survey who took medical examination in Daejeon Korean Medicine Hospital for general health check-up. This investigation was conducted from February in 2008 to July in 2010. The metabolic syndrome was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied by the WHO Western Pacific Region, IASO and IOTF: The Asia-Pacific Perspective in 2000. Incidence of metabolic syndrome was 12% (14.6% in men, 8.2% in women). The groups that have two metabolic risk factors were 21.9% in men and 7.5% in women. The incidence increased with ageing. The mean of metabolic syndrome`s triglyceride was in hypertriglyceridemia, and that of their BMI in men was in primary obese and that of their AST, ALT, ${\gamma}$-GTP means were in abnormal liver function. Smokers in men have metabolic syndrome 10 times more than non-smokers in men. Exercisers that do the exercise once or twice a week in women have metabolic syndrome 0.2 times more than non-exerciser in women. Women that have family history of stroke, were associated with metabolic syndrome by $x^2$-test. Men that have family history of hypertension, have metabolic syndrome 4 times more than otherwise men. Men that have family history of diabetes mellitus, have metabolic syndrome 3 times more than otherwise men.

A study on the validity to make a diagnosis of Soeumin by QSCC II (Questionnaire for the Sasang Constitution Classification II) (사상체질분류검사지(四象體質分類檢査紙)(QSCC) II 에 의한 소음인(少陰人) 진단(診斷)의 타당성(妥當性) 연구(硏究))

  • The purpose of this study is to examine validity to Soeum scales by QSCC II. For this study, the objects are 336 patients who were made a diagnosis of Soeumin and 422 patients of other constitutions in Oriental Medicine & Western Medicine Cooperative Health Examination Center KyungHee University Medical Center. A systematic analysis has been made and the results are as follows. 1. We made a frequency analysis in 758 examinees' questions and concluded that 35 questions, among which 30 questions were existing and 5 were new, had statistical significance in making a diagnosis of Soeumin by more than 10% in comparison with answer frequency of other constitutions. 2. Because confidence coefficient of 0.8011 revised Soeum scales(35) rises up as compared with that of 0.6319 existing Soeum scales(51), I think revised Soeum scales reflect trait of Soeumin well and it is liable to shorten question time by controling questions. 3. Soeumin's discrimination probability in Soeumin was 55.4% by QSCCII, but that rises up to 64.3% by discriminant function made by revised Soeum scales. So there is statistical significance in discriminant function. 4. When questions in QSCCII are divided into body-spirit(體形氣像), looks-attitude(容貌詞氣), temper- talent(性質才幹), continuous mind(恒心), avarice(心慾), physiological symptoms(生理的 證) and pathological symptoms(病理的 證), response frequency and constitution discrimination probability falls in questions about avarice and pathological symptoms.

A Study on the Character of Postpartum Women by Factor AA in 7-Zone-Diagnostic System (7구역 진단기의 Factor AA를 통한 산모의 특성 연구)

  • Purpose: This research was designed to study that VEGA DFM 722 is proper index in postpartum women. Methods: The subjects were 81 postpartum women who admitted for postpartum care in Oriental Hospital of Woosuk University from 20, May 2005 to 28, October 2005 and 83 general women who taken Medical Health Examination from 29, January 2005 to 22, July 2006. The women took the test of VEGA DFM 722 and CBC. The SPSS 12.0 for windows was used to analyse the data and the independent samples t-test and paired samples t-test were used to verify the results. Results: The following results were obtained 1. The heights of zone 1, 2, 3 of postpartum women group significantly higher than that of general women group. The height of zone 6 of postpartum women group significantly higher than that of general women group. But, there were no significant differences in the heights of zone 4, 5, 7. 2. The heights of zone 1, 2, 3 of second test significantly lower than that of first test in postpartum women. But, there were no significant differences in the heights of zone 4, 5, 6, 7. Conclusion: The results suggest that postpartum women has upper heat and lower cold compared with general women. And after oriental postpartum care, disparity in upper heat and lower cold was decreased in postpartum women.

The Analysis on Related Factors of the Aged Examines Who Get Diagnosed in Their Climacteric Period According to Sex (생애전환기 건강진단 노인수검자의 성별에 따른 관련 요인분석)

  • Objectives & Method:Based on the statistic from January 2008 to December 2008 include 66-year-old 375 people (159 men, 216 women) who got examined in National Health Insurance Corporation, this study set a goal to establish a new health index by analyzing the relation between the cardio-cerebra vascular disease and risk factor such as obesity, smoking, exercise, alcohol consumption, blood pressure, blood sugar and cholesterol. Results:1) Examine results, health status according to gender;People who are diagnosed as abnormal health status have higher risk for both men and women. As for the detailed diseases, high blood pressure was highest followed by hyperlipidemia and obesity. In case of the women, 96.3% in bone mineral density was highest. 2) Correlation of disease and health risk factors based on gender;Correlation of disease according to gender in the obesity(p<0.001), lipid abnormalities(p<0.001), kidney diseases(p<0.001), chest diseases(p<0.05), osteoporosis(p<0.001) showed a statistically significant difference. The analysis showed that the men have more obesity and lipid abnormalities as well as thoracic disease than the women, but the women have more kidney disease than the men. As for the osteoporosis examine which is conducted only for the women, most of the women were abnormal even though it cannot compare the result according to gender. Obesity(p<0.001), alcohol(p<0.001), smoking(p<0.001) and LDLcholesterol(p<0.001) showed statistical correlation between gender and health risk factor. And only high blood pressure(p<0.001) showed a statistical correlation between gender and risk of cardio-cerebra vascular disease. 3) Risk of cardio-cerebra vascular disease related to health risk factor characteristic;Obesity assessment(p<0.001), alcohol(p<0.05), smoking(p<0.05), blood pressure(p<0.001), glucose(p<0.001) showed correlation between health risk factor and stroke. Drinking(p<0.001), smoking(p<0.05), exercise(p<0.001), blood pressure(p<0.001), LDL cholesterol(p<0.001) showed health risk factor and myocardial infarction. Obesity assessment(p<0.001), smoking(p<0.05), blood pressure(p<0.001), glucose(p<0.001) and showed correlation between health risk factor and diabetes. Obesity assessment(p<0.001), blood pressure(p<0.001) showed correlation between health risk factor and high blood pressure. Conclusion:Therefore, we have to take one step farther after the health examination. In order to prevent and reduce the risk of disease, people have to understand their health status exactly first, and correct their habit with recognizing the amount of risk related to cardio-cerebra vascular disease.

The Quality of Life in COPD Patients according to Gender: Based on the 4th Korea National Health and Nutrition Examination Survey (제4기 국민건강영양조사에 기초한 만성 폐쇄성폐질환자의 성별에 따른 삶의 질 분석)

  • Purpose: In this study, we performed a comparative analysis on the quality of life (QoL) in male and female chronic obstructive pulmonary disease (COPD) patients based on the 4th Korea National Health and Nutrition Examination Survey (KNHANES IV) from 2007 to 2010. Methods: We extracted the socio-demographic and clinical data of 1,218 COPD patients including 874 men and 344 women from the KNHANES IV database. Descriptive statistics and correlation test were used to analyze the data. In order to find factors associated with QoL of COPD patients, we conducted multivariate linear regression analysis. Results: Infrequency analysis, the educational level and income were lower in the female COPD patients than in the male ones. The QoL indexes including mobility, self-care, activities of daily living, discomfort, anxiety, and depression were significantly worse in the female of COPD patients than in the male ones (p<.01).Conclusion: Our study indicates that QoL of female COPD patients should be improved on the educational, economic, and healthcare aspects.

The Quality of Life of Skin Disease Patients, Particularly Acne Patients (여드름을 중심으로 한 피부질환 환자의 삶의 질)

  • Background : Quality of life(Qol) is a broad concept that incorporates all aspects of an individual's existence. There is many study about Qol of the patient with dermatic disease in the west, but in korea there is few study, particularly for oriental medical academic world. Objective : The purpose of this study is to Investigate the influence of dermatic disease, particularly acne on the Qol by using the korean version of skindex-29. Method : We measured the Qol of Kyungwon university student participating in Dongseo health examination by using the Korean version of Skindex-29. A total of 535 students were enrolled In this study. Results are reported as 3 scales scores (functions, emotions, and symptoms) and a composite score (average scale score). Result : 1) There were no statistically significant gender-related differences in Qol scores in patient group with acne. 2) There were no significant correlation between Age Duration and Qol socres in patient group with acne. 3) Acne group marked higher Qol scores than contact dermatitis and normal group (lower than Acne with atopic dermatitis group) within emotional scales. 4) Acne group marked higher Qol scores than scar, tinea cruris, and normal group (lower than Acne with atopic dermatitis group) within functional and total scales. 5) Acne group marked higher Qol scores than tinea cruris and normal group (lower than Atopic dermatitis group) within symptom scales. 6) In acne group that feel the necessity of medical treatment, group that be treated by occidental medical method marked lower Qol scores than group that don't be treated within functional, symptom, and total scales. 7) In acne group that don't be treated, group that feel the necessity of medical treatment marked higher Qol scores than group that don't feel within emotional, functional, and total scales. 8) Acne group that don't fee the necessity of medical treatment and be treated marked high scores than normal group within all scales. Conclusion : Acne is sometimes thought of as unimportant, but Acne significantly affects patient's Qol. Occidental medical treatment can help Acne patients to improve Qol. Even if someone who suffering from acne feel that he don't need to be treated, he had lower Qol than healthy controls. And we can expect that proper occidental medical treatment help him. But few study have discuss whether oriental medical method can improve Qol of acne patients. From now on we expect interesting study that measure effect of oriental medical therapy on Qol of Acne patients and compare with occidental medical therapy by using Qol mesure instrument.

Design of discriminant function for thick and thin coating from the white coating (백태 중 후태 및 박태 분류 판별함수 설계)

  • Introduction: In Oriental medicine, the status of tongue is the important indicator to diagnose one's health, because it represents physiological and clinicopathological changes of inner parts of the body. The method of tongue diagnosis is not only convenient but also non-invasive, so tongue diagnosis is most widely used in Oriental medicine. By the way, since tongue diagnosis is affected by examination circumstances a lot, its performance depends on a light source, degrees of an angle, a medical doctor's condition etc. Therefore, it is not easy to make an objective and standardized tongue diagnosis. In order to solve this problem, in this study, we tried to design a discriminant function for thick and thin coating with color vectors of preprocessed image. Method: 52 subjects, who were diagnosed as white-coated tongue, were involved. Among them, 45 subjects diagnosed as thin coating and 7 subjects diagnosed as thick coating by oriental medical doctors, and then their tongue images were obtained from a digital tongue diagnosis system. Using those acquired tongue images, we implemented two steps: Preprocessing and image analyzing. The preprocessing part of this method includes histogram equalization and histogram stretching at each color component, especially, intensity and saturation. It makes the difference between tongue substance and tongue coating was more visible, so that we can separate tongue coating easily. Next part, we analyzed the characteristic of color values and found the threshold to divide tongue area into coating area. Then, from tongue coating image, it is possible to extract the variables that were important to classify thick and thin coating. Result : By statistical analysis, two significant vectors, associated with G, were found, which were able to describe the difference between thick and thin coating very well. Using these two variables, we designed the discriminant function for coating classification and examined its performance. As a result, the overall accuracy of thick and thin coating classification was 92.3%. Discussion : From the result, we can expect that the discriminant function is applicable to other coatings in a similar way. Also, it can be used to make an objective and standardized diagnosis.
