Clinical Effect of Herbal Medicine Treatment on Insomnia in a Public Health Center (일개 보건소에서 시행한 불면증에 대한 한약치료의 효과분석)

  • Objectives: We conducted a public health promotion program in Korean medicine for participants suffering from chronic insomnia in 2016~2017. A significant improvement effect was obtained through the 8-week herbal medicine treatment program. The object of this study is to report the results of this public health promotion program. Methods: Seventy patients diagnosed with chronic insomnia participated in the program, 49 of whom completed the program. Participants were treated with the herbal medicine for insomnia treatment for 8 weeks. A sleep diary was used to assess the total sleep time, sleep latency, and number of times the participants woke. The treatment satisfaction was then evaluated. Results: Through the 8-week herbal medicine treatment program, the total sleep time was significantly increased by $109.59{\pm}70.77$ minutes, the sleep latency was significantly decreased by $63.53{\pm}47.79$ minutes, and the number of times the participants woke was significantly decreased by $1.54{\pm}1.19$ times compared to the baseline (all, p=0.000). Among the 24 patients who were examined for the treatment satisfaction, the average score was 4.33 points of a total of 5 points. Six patients had adverse events. Except for one patient, the symptoms of other patients were mitigated by changing or discontinuing the herbal medicine. Conclusions: For patients with chronic insomnia, the 8-week herbal medicine treatment program was effective, with few side effects and high treatment satisfaction. This study has significance as this is the first report in Korea on the effect of a herbal medicine treatment program for chronic insomnia through Korean medicine public health promotion.

A Study on the Efficacy of Growth Excercise Program in Rural Elementary School Students (농촌지역 초등학교 학생들의 성장도 및 성장운동프로그램 운영 보고)

  • Objectives : The purpose of this study is to investigate the efficacy of growth exercise program based on Korean medicine. Methods : We recruited fifty two elementary students in a rural area, and measured height, weight, BMI, WHR, and bone age. Questionnaire on sleep, exercise, symptoms based on Korean medicine was performed. Students exercised TaeGuk(太極) Health Gigong(氣功), a growth exercise program 11 times in 6 weeks. Results : Height, weight, BMI and WHR of the subject students were lower than those in the Korean CDC(Center for Disease Control and Prevention) growth chart. There was significantly negative correlation between time of sleep and bone age(p=0.028), but no correlation between time of exercise and bone age(p=0.668). According to diagnosis based on Korean medicine, many students were in weakness of spleen and lung, and bone age according to deficiency of organ showed no significant difference(p=0.774). Satisfaction, impact on time of exercise after program, ideal time for program and impact on reparticipation of program were strong in the questionnaire after growth exercise program(TaeGuk(太極) Health Gigong(氣功)). Conclusions : Korean medicine promotion of health has a salutary effect on elementary school students in rural area by collecting growth and health data and educating easy exercise based on Taichi.

Comparison Study on Yang-Saeng Grades between Hypertension Group and Control Group in Hong-Cheon County (일부 한국인 고혈압환자의 한방양생 실천수준 및 생활습관 -강원도 홍천군민을 중심으로-)

  • Objective : The purpose of this study was to investigate the prevalence and the relationship between hypertension and the grades of 'Yang-Saeng'(health promotion methods in oriental medicine) in hypertension group and control group of Hong-Cheon county in Korea. Methods : To accomplish the purpose, we analyzed the Hong-Cheon survey data which was collected from 1,739 subjects from october 1. 2006 to October 20. 2006. Results : Hypertension group marked lower mentality score($21.0{\pm}3.7$) than that of control group($21.8{\pm}3.2$). (p<0.05) Hypertension group marked higher grades in the other Yang-Saeng categories{Eating($31.6{\pm}5.0$ ; $29.9{\pm}4.9$), Daily life($31.5{\pm}4.3$ ; $30.6{\pm}4.2$), Seasonal life($29.1{\pm}3.4$ ; $28.3{\pm}3.1$), Sexual behavior($13.9{\pm}2.3$ ; $13.5{\pm}2.1$)}. (p<0.05) Conclusion : The result of this study suggest that mental program could be needed more during operating health promotion program for hypertension group.

A Study of School Doctor Program for Dysmenorrhea in Korean Medicine (월경통 한의 교의 사업에 관한 연구)

  • Objectives This study aims to investigate the effect of the Korean medicine in treating dysmenorrhea as a part of school health program (SHP) and satisfaction rate of the participated students in this program. With this study, we tried to contribute to the development of health promotion program of Korean medicine. Methods As part of the 'Korean Medicine Health Promotion Project', we conducted a dysmenorrhea program at one of many high schools in Eumseong-gun. From April to June 2018, we conducted the promotion program a total of 7 times and analyzed the questionnaire regarding satisfaction and the monthly experience of menstruation. Results The study participants' grade distribution was $2^{nd}$ grade 32, $3^{rd}$ grade 23 with a total 55 students. Mean dysmenorrhea VAS was $6.07{\pm}2.10$, and the mean number of visits to the dysmenorrhea program was $3.61{\pm}1.47$. There were no significant differences in the average number of visits between grades. Overall satisfaction was $4.06{\pm}0.97$, teacher satisfaction was $4.29{\pm}1.10$, and lecture satisfaction was $4.18{\pm}0.95$. Dysmenorrhea decreased but was not statistically significant. Conclusions Korean medicine is appropriate for the health promotion program. With cooperation and support from local schools, it will be very possible to carry out a successful business. It is also necessary to organize the program according to the viewpoint of youth and to interact with the students.

Clinical Effect of Korean Medical Treatments on Stroke in a Public Health Center (일개 보건소에서 시행한 중풍에 대한 한의 진료의 효과분석)

  • Objectives: In 2019, we conducted a 12-week public health promotion program on the use of Korean medical treatments for elderly people who are at potential risk for stroke. A significant improvement effect was obtained through the program. The objective of this study was to report the results of this public health promotion program. Methods: Fifty people participated in the program. The participants were received herbal medicine and acupuncture treatments for stroke prevention for 12 weeks. Systolic blood pressure, diastolic blood pressure, blood sugar, weight, and body fat were evaluated twice. A questionnaire was used to assess the European Quality of Life-5 Dimensions (EQ-5D) score, the stroke danger signs, the recognition of stroke signs, and treatment satisfaction. Results: During the 12-week Korean medical treatment program, systolic blood pressure was significantly decreased by 119.40±19.32 mmHg, diastolic blood pressure was decreased by 79.80±8.92 mmHg, and weight was decreased by 58.24±9.70 Kg. The EQ-5D score was increased by 13.82±1.2 and the stroke danger signs score was increased by 26.76±3.59. Sex and EQ-5D score, stroke danger signs score, weight, and body fat were correlated. Age and weight were correlated. Conclusions: For elderly people with the potential risk for stroke, the 12-week Korean medical treatment program was effective and their satisfaction with the treatment was high. This study has significance because this is an unusual report in Korea on the effect of a Korean medical treatment program for stroke using a Korean medicine public health promotion.

Reliability Study on YangSaeng Survey Using by Hong-Cheon Data (홍천군 데이터를 이용한 한의학 설문지의 문항간 신뢰성 분석에 관한 연구)

  • Background : 'Yangsaeng' means health preservation concept in TKM(Traditional Korean Medicine). Health promotion using by traditional medicine was increased recently, but, has little measurement tools in evaluating traditional health care program. This study accomplished to analyze internal consistency reliability by Hong-Cheon survey data Method : Internal consistency reliability analysis within survey items Result : There was little reliability in mind and sexual life yangsaeng factor(Cronbach's alpha value 0.325, 0.273), higher reliability in lifestyle yangsaeng factor(0.718). And reliability was various within items according to age and health insurance type.

  • Public health oriental medical doctor has played a great role in providing oriental medical treatment and oriental medical health program with public health medical services, the basic infrastructure, however, is not sufficient. In this study, the researcher surveyed the treatment working or service condition of public health oriental medical doctor. 1. The payment, allowance of doctors are fixed upon the law and guideline according to the financial status of local government. The branch of public health center has more support like an official residence with expenses. 2. The public health center mainly has assitants and ratio of full-timer is more than the branch public health center without any assistants if any, they are temporary employee 3. The public health center has 5.22 beds while the branch has 3.14 beds. The daily average number of patient for public health center is 15.01 while the branch has 8.7 More than half of outpatients are over sixties with musculoskeletal disease. 4. Regarding the traveling clinic, the public health center put into more operation than the branch. The 3rd year serving public doctor gives negative about the traveling clinic much more than 1st and 2nd year serving public doctor. The treatment service condition of public doctor of the public health center is better than the branch doctor, but we are on the point of role-changing as health promotion and preventive treatment to bring up motivation, education and competence strengthening for execution the local-bound health program.

Effects of Oriental Nursing Intervention Program for Health Promotion of the Elderly in the Community (지역사회 재가노인의 건강증진을 위한 한방간호중재 프로그램의 적용효과)

  • Purpose: This study was carried out to examine the applied effects of an oriental nursing intervention program on the physical health status, activities of daily living (ADL), depression, life satisfaction, and Yangseng of the elderly living in the community. Methods: This was a quasi-experimental study using a non-equivalent control group pretest-posttest. Study participants were total 79 elderly (experimental: n=39, control: n=40) who aged 65 or over with normal cognitive status and available communication in D community, Seoul, South Korea. They were recruited by convenient sampling, and assigned randomly. The oriental nursing intervention program composed Pal-Dan-Gum as Qi-gong therapy and Kyunglak exercise. This program consisted of 20 sessions total for 10 weeks. Measures were CMI(Cornell Medical Index), Instrumental ADL (Activities of Daily Living), GDS(Geriatric Depression Scale), geriatric life satisfaction scale, and Yangseng scale. Data were analyzed by SPSS WIN21.0 using descriptive statistics, x2-test, and independent t-test. Results: There were statistically significant differences in the mean scores of physical health status (t=10.295, p<.001), ADL (t=7.571, p<.001), depression (t=-15.434, p<.001), life satisfaction (t=21.257, p<.001), and Yangseng (t=9.527, p<.001) between intervention and control groups. Conclusions: Oriental nursing intervention program was effective for improving the physical health status, ADL, depression, life satisfaction, and Yangseang of the elderly living in the community. This intervention program can be implied for health promotion of elderly living in the community.

Development of Korean Medicine Healthy Aging Program for Public Health Center (보건소 한의약 건강노화 프로그램 개발)

  • Objectives : The aim of this study was to introduce the development process of Korean medicine health aging program for the older that is suitable for applied at public health center. Methods : For the development of the draft of health promotion program, we conducted a literature review of previous studies through searching international and domestic databases. The opinions of several experts were also colleted. Results : The details of 8-week of Korean medicine healthy aging program are as follows: 8-week of taking herbal medicines (Yukmijihwang-tang or Palmijihwang-tang, or Jaeumganghwa-tang) based on counseling with Korean medicine doctor, qigong, acupuncture on head, abdominal hotpack, taking nuts, and health education Conclusions : This healthy aging program can help to develop the healthy lifestyle habits and increase the general health status of community seniors.

  • With the start of the residentship of Korean preventive medicine in 2008, the residency training programs should be established and also operation plan for the residentship of Korean preventive medicine should be setup. This residency training program need to contain the future oriented study objective to be an expert of the preventive medicine and could be updated the changes in health service needs of the population. Specially in the part of Health Promotion(YangSaeng) in Korean Medicine, strengthening in medical practice, not just in knowledge is urgently required. And we should have more concentrations on preventive service for the individual, as in clinical preventive medicine. Training residents by the systematic and well scheduled programs, not just teacher..and assistant..in the academic facilities. These approach may not only contribute for the better future of the preventive medicine, but also improve in disease prevention and health promotion.
