Cardiopulmonary arrest has long been accepted as an unquestionable definition of death. An advent of cardiopulmonary resuscitation and artificial ventilation along with the development of organ transplantation has prompted the emergence of the concept of brain death. The criteria for brain death are based mainly on the clinical examination of coma, apnea and total loss of brain stem function. Although organ transplantation by donor brain death has increased in Korea over recent years, there is still a substantial shortage of donor organs compared to the demand. Improvement of government policies and changes of social culture for organ donation are needed for the activation of organ transplantation by donor brain death. Pediatricians have an important role for the search of potential donors in cases of brain death and optimal medical care for successful organ transplantation.

Objective : The Act on Life-Sustaining Treatment (LST) decisions for end-of-life patients has been effective since February 2018. An increasing number of patients and their families want to withhold or withdraw from LST when medical futility is expected. This study aimed to investigate the status of the Act on LST decisions for patients with acute cerebrovascular disease at a single hospital. Methods : Between January 2017 and December 2021, 227 patients with acute cerebrovascular diseases, including hemorrhagic stroke (n=184) and ischemic stroke (n=43), died at the hospital. The study period was divided into the periods before and after the Act. Results : The duration of hospitalization decreased after the Act was implemented compared to before (15.9±16.1 vs. 11.2±18.6 days, p=0.127). The rate of obtaining consent for the LST plan tended to increase after the Act (139/183 [76.0%] vs. 27/44 [61.4%], p=0.077). Notably, none of the patients made an LST decision independently. Ventilator withdrawal was more frequently performed after the Act than before (52/183 [28.4%] vs. 0/44 [0%], p<0.001). Conversely, the rate of organ donation decreased after the Act was implemented (5/183 [2.7%] vs. 6/44 [13.6%], p=0.008). Refusal to undergo surgery was more common after the Act was implemented than before (87/149 [58.4%] vs. 15/41 [36.6%], p=0.021) among the 190 patients who required surgery. Conclusion : After the Act on LST decisions was implemented, the rate of LST withdrawal increased in patients with acute cerebrovascular disease. However, the decision to withdraw LST was made by the patient's family rather than the patient themselves. After the execution of the Act, we also observed an increased rate of refusal to undergo surgery and a decreased rate of organ donation. The Act on LST decisions may reduce unnecessary treatments that prolong end-of-life processes without a curative effect. However, the widespread application of this law may also reduce beneficial treatments and contribute to a decline in organ donation.
Purpose: This study was done to Identify attitude and factors influencing attitudes to organ transplants. Method: Study participants were 165 people over the age of 18 and living in Seoul. The instrument used for this study was the descriptive questionnaire developed by Ju (1995). Analysis of the data was done with SPSS Win 10.0 with descriptive statistics. t-test. ANOVA and stepwise multiple regression. Results : 1. The scores on attitude toward organ transplants ranged from 14 to 70. and had a mean score of 49.26. There were statistically significant differences in scores on attitude to organ transplants according to age (p= .03). marital status (p= 00), monthly income (p=.02) and experience of having education about organ transplants (p= .00). 2. The factors influencing attitudes to organ transplants were having the experience of education on transplants and marital status. These two variables explained 20.1% of the variance. 3. Reasons given for no written pledge to donate one's organs were 'fear on organ donation', 'not knowing the way of' how to do'. 'plan to give my organs to a family member when needed'. 'physical condition' and 'religious belief'. Conclusion: Experience with education for organ transplant and marital status were identified as important variables in attitudes to organ transplants.
Purpose: This study aimed to explore young adult donors' experiences of living donor liver transplantation. Methods: A phenomenological research method was used. The participants were two women and six men. Data were collected through individual in-depth interviews from November 25th, 2019 to June 10th, 2020 and analyzed using Colaizzi's phenomenological method. Results: Five theme clusters extracted from the young adult donors' experiences were painful decision of a liver donation, the agony of both mind and body that overpowers youth, the bitter and bare face of reality that a young donor encounters, feeling the power of love that fills up the space of the organ removed, and liver donation becoming priming water for maturity. Conclusion: The results of this study provide a deeper understanding of the lives of young adult donors who have experienced unexpected difficulties as well as self growth from the donation. It is expected that the results can be of use for developing and applying customized nursing interventions for management before and after liver donation among young adult donors.
본 연구는 치위생(학)과와 보건계열 학생들을 대상으로 2018년 6월에서 7월까지 생명윤리의식을 파악하여 생명윤리 교육과정 개발에 필요한 기초자료를 제공하고자 설문조사하였다. 생명권과 인공수정 인식도의 관한 상관관계는 성별(r=0.241), 종교(r=-0.176), 학과(r=-0.160)에서 유의하였고, 장기기증 및 장기이식에 관한 인식도에 관한 상관관계는 학년(r=0.129), 종교(r=-0.098), 생명윤리 교육여부(r=0.235)에서 유의하였다. 안락사의 인식도에 관한 상관관계는 종교(r=0.139), 생명윤리교육 여부(r=-0.157)에서 유의하였다. 결과적으로 보건계열 대학생들에게 바람직한 윤리관을 확립하기 위해서는 교육과정을 개발하고 생명의료 윤리의식에 영향을 미치는 요인을 다각적으로 분석하여 체계적으로 교육 프로그램을 운영해야 할 것이다.
Background: Extracorporeal life support (ECLS) can be applied in brain-dead donors for organ perfusion before donation, thereby expanding the donor pool. The aim of this study was to examine the benefits and early clinical outcomes of ECLS for organ preservation. Methods: Between June 2012 and April 2017, 9 patients received ECLS with therapeutic intent or for organ preservation. The following data were collected: demographics, purpose and duration of ECLS, cause of death, dose of vasoactive drugs, and need for temporary dialysis before organ retrieval. The early clinical outcomes of recipients were studied, as well as survival and graft function at 1 month. Results: ECLS was initiated for extracorporeal cardiopulmonary resuscitation in 5 patients. The other patients needed ECLS due to hemodynamic deterioration during the assessment of brain death. We successfully retrieved 18 kidneys, 7 livers, and 1 heart from 9 donors. All organs were transplanted and none were discarded. Only 1 case of delayed kidney graft function was noted, and all 26 recipients were discharged without any significant complications. Conclusion: The benefits of protecting the vital organs of donors is significant, and ECLS for organ preservation can be widely used in the transplantation field.
This is a study to search for the ethical basis for valid informed consent of organ donors. It is an admirable action that a person give his own body part or organ as a gift to another person. The organ for transplantation can be removed only when the donor consents voluntarily to donation. It is recently proposed as the need for organ transplantation is increased that organs can be harvested although the consent of deceased cannot be obtained. This may raise many moral issues because human beings all have an unalienable right to control their own bodies. The principle of autonomy is usually regarded as an ethical basis for informed consent. However, some people criticize that the principle of autonomy requires a person and his decision to be autonomous (but there are many patients who aren't autonomous due to their confusion or unconscious condition in a clinical situation). or this principle can foster indifference to patients needing help: thus respect for principles of care and beneficence is necessary. When we consider the complexity of making a decision about organ donation. the principle of autonomy should be replaced by the principle of respect for individual autonomy. as expressed by Childress (1990). This principle requires the care givers to respect the client's individual decisions. The elements of informed consent are threshold elements: competence to understand and decide. voluntariness in deciding: information elements: disclosure of material information. recommendation of a plan. understanding of disclosure and recommendation: and consent elements: decision in favor of a plan. authorization of the chosen plan. In cases of living donors. the elements of competence and voluntariness are more important than the others. So only an adult can give a recipient his own body part. but it should be forbidden to harvest from minors or protected adults (i.e. developmentally disabled person However. when organs are removed from a cadaver donor. we ought to respect the donor's decision. So we ought to try to seek donor cards or any documents expressing the donor's opinion about organ transplant. All health care givers ought to disclose donor information about organ transplantation clearly enough for the donor to understand it and to be able to weigh the harms and benefits. We are going to propose 'the subjective standard' as the ethical standard of disclosure. This standard will assure that patients have enough information to be able to decide autonomously from their own position. Care givers have to consider the method of disclosure because donors can be influenced by it positively or negatively, Establishment of the Hospital Committee is recommended. because medical professionals will have a chance to discuss the procedure of decision and the validity of harvesting a organ from a person.
우리나라의 경우, 이식 대기자에 비해 공여 장기 수가 부족하여 생체 장기기증에 대한 의존도가 높으며, 생체 장기기증 건수도 지속적으로 증가하고 있다. 특히, 생체 신장 이식 건수는 전 세계적으로 높기 때문에 생체 신장 공여자에 대한 이식 전 정신사회적 평가의 중요성이 부각되고 있다. 이전 연구에 따르면 이식 전 공여자 평가 시 사전 동의 여부 및 수술 위험성에 대한 인지 여부를 파악하는 것이 중요하며 이를 위해 다음과 같은 평가 도구들을 시행할 수 있다. ELPAT living organ donor Psychological Assessment Tool (EPAT), Live Donor Assessment Tool (LDAT), Living Donation Expectancies Questionnaire (LDEQ), Minnesota Multiphasic Personality Inventory-2 questionnaire (MMPI-2). 이식 전 정신사회적 평가 도구에 관한 문헌을 검토한 후, 한국의 생체 신장 기증자에 대한 법적 고려 사항을 살펴본 후 생체 신장 공여자에 대한 효과적인 이식 전 선별 평가 방법을 제안하려 한다.
목적: 본 연구는 병원 근무자의 연명치료 중단에 영향을 미치는 장기기증, 장기이식, 죽음, 호스피스 완화의료의 지식, 태도 및 인식을 알아본 서술적 조사연구이다. 방법: K도 J시에 위치한 일개 상급종합병원에 근무하는 228명의 대학병원 근무자를 대상으로 자가 보고식 설문지를 이용하여 자료를 수집하였다. 자료는 SPSS WIN 21.0 프로그램으로 t-test, ANOVA, and Pearson's correlation coefficient를 이용하여 분석하였다. 결과: 대상자의 생명의료윤리 의식관련 지식은 연령, 학력, 직종, 근무부서, 생명의료윤리 교육유무에 따라 차이를 보였다. 뇌사 장기기증 장기이식에 대한 지식은 인체조직기증 및 이식에 대한 태도, 호스피스 완화의료에 대한 지식, 호스피스 완화의료의 인식에서 유의한 양의 상관관계가 있었다. 인체조직기증 및 이식에 관한 태도는 호스피스 완화의료에 대한 지식, 호스피스 완화의료의 인식, 연명치료 중단에서 유의한 상관관계가 있었고, 죽음에 대한 인식은 호스피스 완화의료에 대한 지식, 호스피스 완화의료의 인식, 연명치료 중단에서 유의한 상관관계가 있었다. 호스피스 완화의료의 인식은 연명치료 중단에서 유의한 상관관계가 있었다. 연명치료 중단 태도에 영향을 주는 요인은 호스피스 병동 근무와 인체조직기증 및 이식에 관한 태도, 호스피스 완화의료의 인식으로 연명치료 중단 태도를 32.5% 설명하였다. 결론: 연명치료 중단 태도에 호스피스 병동 근무와 인체조직기증 및 이식에 관한 태도, 호스피스 완화의료 인식이 관련이 있었으며, 생명의료윤리에 기반을 둔 다양한 교육과정의 개발 및 적용에 대한 연구가 필요함을 제언한다.
