장부상통의 원리를 이용한 동씨기혈(董氏奇穴)의 임상활용에 관한 문헌고찰(文獻考察) (A Philological study on the clinical application of Tong's acupuncture by the principle of mutual communication between organ and bowel.)

  • Objectives & Methods : This study aims to investigate the application of the principle of the mutual communication between organ and bowel(臟腑相通) on Tong's acupuncture. We classified the frequently used Tong's acupuncture points according to the principle of the mutual communication between organ and bowel. Results & Conclusions : Considering the locations and indications of the most frequently used points among the 740 master Tong's acupuncture points, we could reach the following conclusions; 1. The chief virtues of 66 master Tong's acupuncture points could be explained by the principle of mutual communication between organ and bowel. 2. Among the relationships of mutual communication between organ and bowel, the most frequently applied relationship was relationship which applied to 16 master Tong's acupuncture points, and the , , , , and followed. 3. Considering the body regions, 13 out of 66 master Tong's acupuncture points which the relationship of mutual communication between organ and bowel was applied to, were located at the 1-1 region, and the same number at the U region, the others were at the 7-7, 3-3, 2-2, 4-4, 10-10, 6-6, 5-5 regions in the order of the number of the points, and none at the 9-9 region.

순환정지 후 장기기증의 의료윤리적 문제 (Medical ethical problems of organ donation after circulatory death)

  • It is necessary to expand organ donations of the deceased, as more ethical problems are emerging from the increase in organ donations from the living. As the legal and ethical discussions on donation after circulatory death (DCD) as a field with the potential to expand such domestic organ donation are being held, the need for a social consensus is increasingly highlighted. Organ DCD refers to the donation of an organ from a patient whose spontaneous blood circulation has stopped due to cardiac death. In this study, we aimed to examine whether there are legal and ethical obstacles or medical uncertainties for expanding the practice of DCD. By law, death is recognized as cardiac death, and brain death is recognized as an exception for organ transplantation. Although circulatory death precedes brain death, this paper presents a discussion to begin the process of reaching an ethical and legal agreement on whether or not circulatory death can be recognized as death. Successful implementations of DCD, including Category III DCD, in other countries are currently being reviewed to introduce Category III DCD in Korea.

Organ dose conversion coefficients in CT scans for Korean adult males and females

  • Dose monitoring in CT patients requires accurate dose estimation but most of the CT dose calculation tools are based on Caucasian computational phantoms. We established a library of organ dose conversion coefficients for Korean adults by using four Korean adult male and two female voxel phantoms combined with Monte Carlo simulation techniques. We calculated organ dose conversion coefficients for head, chest, abdomen and pelvis, and chest-abdomen-pelvis scans, and compared the results with the existing data calculated from Caucasian phantoms. We derived representative organ doses for Korean adults using Korean CT dose surveys combined with the dose conversion coefficients. The organ dose conversion coefficients from the Korean adult phantoms were slightly greater than those of the ICRP reference phantoms: up to 13% for the brain doses in head scans and up to 10% for the dose to the small intestine wall in abdominal scans. We derived Korean representative doses to major organs in head, chest, and AP scans using mean CTDIvol values extracted from the Korean nationwide surveys conducted in 2008 and 2017. The Korean-specific organ dose conversion coefficients should be useful to readily estimate organ absorbed doses for Korean adult male and female patients undergoing CT scans.

  • The model of an isolated organ system has been developed to simulate the kinetic behavior of drug levels in an acting organ or site. The model is developed from basic considerations of drug distribution with hemodynamical and pharmacokinetical meanings. Model: It is considered a situation in which non-metabolic drug substance is injected into the arterial inflow of an isolated organ at constant rate. The volume of distribution and the concentration of drug in the venous outflow can be mathematically expressed as a function of time.

융복합 교육시대에 간호대학생의 장기이식 인식에 관한 모색 (A Study on the recognition of organ donation of Nursing students in convergence and integration time)

  • 본 연구는 간호대학생의 장기이식 인식에 관해 모색해보고자 생명의료윤리의식과 장기기증태도와의 관계를 확인하였다. 202명의 간호대학생을 대상으로 2017년 4월부터 7월까지 자료를 수집하였다. 연구결과 대상자의 생명의료윤리의식은 2.98점, 장기기증태도는 2.67점이었고 뇌사는 2.67점으로 가장 낮았다. 두 변수간의 관계는 양의 상관관계이었고(r=.18, p<.01) 생명의료윤리의식의 영향요인은 장기기증태도, 성별, 생명윤리의식에 관한 교육 수혜경험이었으며 설명력은 9.1%이었다. 본 연구 결과를 바탕으로 장기이식의 인식고취를 위해 뇌사 및 장기기증 태도 함양에 초점을 둔 간호윤리교육이 필요함을 제안한다.

축방향 CT 스캔과 나선형 CT 스캔에서 영·유아의 장기흡수선량 비교 평가 (A Comparative Evaluation of Organ Doses in Infants and toddlers between Axial and Spiral CT Scanning)

  • 성인에 비해 방사선에 민감하고 검사건수가 증가하고 있는 영 유아의 CT 스캔 시의 장기흡수선량을 평가하기 위해 스캔부위를 머리부위와 가슴부위로 구분하고 64 MDCT를 이용하여 축방향 스캔과 나선형 스캔으로 비교했다. 스캔부위에 상관없이 나선형 스캔 시의 선량이 축방향 스캔 시 보다 유의하게 낮은 것으로 나타났다. 축방향 스캔과 비교해서 나선형 스캔 중 피치 1.355를 사용했을 때가 나머지 두 피치(0.525, 0.988)를 사용했을 때보다 가슴부위 스캔의 평균 장기흡수선량이 유의하게 높게(평균 -12.03%) 나왔다. 나선형 스캔 시 장기흡수선량이 축방향 스캔보다 평균 20.54% 낮게 나왔다. 결과적으로 인체모형을 통한 장기흡수선량을 평가한 본 연구는 몬테카를로 시뮬레이션 결과값을 실증하고, CT 검사를 받는 영 유아의 장기흡수선량의 보다 정확한 평가에 기여할 것이다.

간호대학생의 생명의료윤리의식, 간호전문직관이 장기이식 태도에 미치는 영향 (Impacts of Biomedical Ethics Consciousness and Nursing Professionalism on Attitudes toward Organ transplantation of Nursing Students)

  • 본 연구는 간호대학생의 생명의료윤리의식, 간호전문직관이 장기이식 태도에 미치는 영향을 파악하기 위해 시도되었다. 연구 대상자는 G, P시에 소재하고 있는 4년제 간호대학에 재학중인 간호대학생 202명으로 자료수집기간은 2021년 10월 12일부터 10월 22일까지였다. 자료는 SPSS 20.0 통계프로그램의 빈도분석, 평균과 표준편차, t-test와 One way ANOVA, Pearson's correlation coefficient, Stepwise multiple regression으로 분석하였다. 본 연구대상자의 생명의료윤리의식은 3.39±0.44, 간호전문직관은 3.84±0.56, 장기이식 태도는 3.67±0.25였다. 장기이식 태도에 영향을 미치는 요인으로 생명의료윤리의식의 신생아의 권리, 안락사영역, 간호전문직관의 사회적 인식, 간호의 전문성, 간호계의 역할영역이 장기이식 태도를 52.4% 설명하는 것으로 나타나 본 연구결과를 토대로 간호대학생의 장기이식 활성화를 위한 간호교육 자료로 활용 할 수 있을 것이다.

Organ dose reconstruction for the radiation epidemiological study of Korean radiation workers: The first dose evaluation for the Korean Radiation Worker Study (KRWS)

  • The Korea Institute of Radiological and Medical Sciences has started a radiation epidemiological study, titled "Korean Radiation Worker Study," to evaluate the health effects of occupational exposure to radiation. As a part of this study, we investigated the methodologies and results of reconstructing organ-specific absorbed doses based on personal dose equivalent, Hp(10), reported from 1984 to 2019 for 20,605 Korean radiation workers. For the organ dose reconstruction, representative exposure scenarios (i.e., radiation energy and exposure geometry) were first determined according to occupational groups, and dose coefficients for converting Hp(10) to organ absorbed doses were then appropriately taken based on the exposure scenarios. Individual annual doses and individual cumulative doses were reconstructed for 27 organs, and the highest values were observed in the thyroid doses (on average 0.77 mGy/y and 10.47 mGy, respectively). Mean values of individual cumulative absorbed doses for the red bone marrow, colon, and lungs were 7.83, 8.78, and 8.43 mSv, respectively. Most of the organ doses were maximum for industrial radiographers, followed by nuclear power plant workers, medical workers, and other facility workers. The organ dose database established in this study will be utilized for organ-specific risk estimation in the Korean Radiation Worker Study.
