가토에서 $TGF-{\beta}$ 길항제인 Decorin이 창상치유에 미치는 영향 (THE EFFECT OF DECORIN, $TGF-{\beta}$ ANTAGONIST, ON WOUND HEALING IN RABBITS)

  • There have been many trials to decrease the scar formation followed by wound treatment. $TGF-{\beta}$ plays a important role on wound healing in adult. Therefore the repression of $TGF-{\beta}$ expression will be helpful to decrease scar formation. Decorin is known to competitively inhibit $TGF-{\beta}$ expression. Decorin were subcutaneously administered in surgical wounds in rabbits to investigate the preventing effect of scar formation for clinical application. Histologic findings of wound healing progresses is similar with control and experimental group at 2week. $2.5{\mu}g$ decorin of administrated group was similar to those of control group at 4 and 8week. In wound healing process $10{\mu}g$ decorin of administrated groupsat showed that thickness of immature collagen fibers(scar) was decreased as compared with control group at 4, 8 weeks. $20{\mu}g$ decorin of administrated group showed similar histologic features to $10{\mu}g$ administrated group. The wounds of 8week experimental group(10, $20{\mu}g$) were completely recovered to the normal surrounding skin tissue including sweat gland and hair follicle. These results suggested that decorin can be of help to the prevention of local scar formation.

Inducing re-epithelialization in skin wound through cultured oral mucosal keratinocytes

  • Objectives: The purpose of this study was to investigate the wound healing effect of primary cultured oral mucosal keratinocytes (OMKs) and to assess their roles in skin wounds. Materials and Methods: OMK labeled with BromodeoxyUridine were scattered onto $1.5{\times}1.5$ cm skin defects of adult female nude mice (OMK group, n=15). For the control, culture media were placed on the wound (control group, n=15). Mice in both groups were sacrificed at three days (n=5), one week (n=5), and two weeks (n=5), and histomorphometric and immunoblot analyses with keratinocyte growth factor (KGF), interleukin (IL)-6, and IL-$1{\alpha}$ antibody were performed for the biopsied wound specimen. To verify the effect of the cytokine, rhIL-$1{\alpha}$ was applied instead of OMK transplantation, and the OMK and control groups were compared with regard to re-epithelialization. Results: Histomorphometric analyses demonstrated faster re-epithelialization in the graft group than in the control group at the third day, first week, and second week. Newly forming epithelium showed maintenance of the histological character of the skin epithelium. The graft group showed superior expression of KGF, IL-6, and IL-$1{\alpha}$ protein, compared with the control group. Similar faster re-epithelialization was observed after treatment with rhIL-$1{\alpha}$ instead of OMK transplantation. Conclusion: We successfully confirmed that the graft of primary cultured OMKs promoted regeneration of skin defects. The mechanism of accelerated wound healing by primary cultured OMKs was attributed to inducement of cytokine expression as required for re-epithelialization.

두경부 악성종양 환자에서 조기 방사선치료를 위한 구강관리법에 대한 실험적 연구 (The experimental study of oral care for early radiation therapy in the head and neck cancer patients)

  • Background: Teeth requiring extraction before radiotherapy in head & neck cancer patients should be removed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it was not uncommon for the radiotherapist and cancer patient to feel an urgent need to process with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy, were considered and applied based on the experimental study Materials and Methods: Eighteen dogs were processed for histopathologic wound healing. The effect of the primary endodontic treatment and extraction before early radiotherapy was examined. Results: No specific complication, such as, post-extraction wound infection, radiation osteitis and osteoradionecrosis, were encountered despite the early radiotherapy. Conclusion: Based on the experimental study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for the early radiotherapy in head and neck cancer patients.

  • Behcet Disease is a multisystem inflammatory disorder of unknown orign. It is characterized by recurrent oral ulcer, genital ulcer, skin lesions and ocular inflammation, and which may involve the joints, skin, central nervous system and gastrointestinal tract. Because Behcet Disease dose not have any specific symptoms and laboratory findings, the diagnosis is made on the basis of the criteria proposed by the the International Study Group for Behcet Disease. Behcet Disease is affecting both arteries and veins, and clinically manifest large vessel involvement occurs in between 7 and 49% of patients. Superior vena cava thrombosis is a rare but well-recognized manifestation of Behcet disease. We report a case of a Behcet Disease with superior vena cava thrombosis in a patient presenting delayed facial wound healing.

  • Alstonia scholaris (Family: Apocynaceae) has been indicated for the treatment of various diseases, one among which is wound healing. The purpose of this study is to investigate the wound healing effect and antioxidant role of Alstonia scholaris leaves in animal models. The ethanol and aqueous extracts of Alstonia scholaris (EEAS, AEAS respectively) were tested against excision, incision and dead space wound models to evaluate the wound healing activity. In excision wound model, treatment was continued till the complete healing of wound, in incision and dead space wound models, the treatment was continued for 10 days. For topical application, 5% w/w ointment of EEAS and AEAS were prepared in 2% sodium alginate. For oral administration, EEAS was suspended in distilled water using Tween 80 and AEAS was dissolved in distilled water. The wound healing was assessed by the rate of wound contraction, period of epithelialisation, skin breaking strength, granulation strength, dry granulation tissue weight, hydroxyproline, collagen and histopathology of granulation tissue. Malondialdehyde level was also estimated to evaluate the extent of lipid peroxidation. AEAS and EEAS significantly promoted wound healing activity in all the wound models studied. Increase in the rate of wound contraction, skin breaking strength, granulation strength, dry granulation tissue weight, hydroxyproline and collagen, decrease in the period for epithelialisation and increased collagenation in histopathological section were observed with EEAS and AEAS treated groups. EEAS and AEAS also significantly decreased the levels of lipid peroxidation. The present study is suggestive that EEAS and AEAS promote wound healing activity.

흰쥐의 전층피부상처 동물모델에서 소풍산(消風散)이 VEGF 및 TGF-β1발현에 미치는 영향 (Sopung-san Extract Enhances healing potential on Full-thickness Skin Wound in Rats: Role of VEGF and TGF-β1)

  • Wounds are commonly created during almost every kind of surgery, trauma and skin diseases. Delayed wound healing affects a plenty of patients and requires prolonged treatments that seriously reduce the quality of life for patients. Skin damage involving large areas or great severity can lead to disability or even death. Wound healing involves a complicated series of actions, of various tissues and cell lineages, concerning inflammation, migration, proliferation, reepithelialization, and remodeling. Sopung-san is reported to have anti-inflammatory effect and has been used for various skin diseases such as allergic dermatitis and atopic dermatitis. In this study, the hypothesis that oral treatment with Sopung-san could enhances healing potential on rat full thickness skin wounds was tested. Twenty young male Sprague-Dawley rats were used for the studies. A full-thickness skin wound was made on the dorsal skin of the rats. Either Sopung-san water extract (SPS) or saline (Control) was orally administrated every day. The wound area was measured and the percentages of wound contraction, wound healed and wound epithelization were calculated. Wound tissue samples were excised following injection for histopathological and immunohistological examination. Wound area in rats of SPS group significantly was decreased compared to Control. SPS group showed significant promotion of wound healing compared to Cotrol group in the percentages of wound contraction, wound healed and wound epithelization. Histopathological examination revealed that SPS induces neo-vascularization potential in wound healing process. SPS treatment in rats significantly accelerated cutaneous wound healing in the neo-vascularization process by increasing VEGF and $TGF-{\beta}1$ synthesis. The results suggest that Sopung-san affects key cellular processes responsible for wound repair and point to a unique potential for this molecule in the therapy of skin wounds, particularly as an angiogenic agent.

Development of Hair Keratin Protein to Accelerate Oral Mucosal Regeneration

  • Background: In this study, we investigated the potential use of keratin for oral tissue regeneration. Keratin is well-known for its effectiveness in skin regeneration by promoting keratinization and enhancing the elasticity and activity of fibroblasts. Because of its structural stability, high storability, biocompatibility, and safety in humans, existing research has predominantly focused on its role in skin wound healing. Herein, we propose using keratin proteins as biocompatible materials for dental applications. Methods: To assess the suitability of alpha-keratin protein as a substrate for cell culture, keratin was extracted from human hair via PEGylation. Viabilities of primary human gingival fibroblasts (HGFs) and human oral keratinocytes (HOKs) were assessed. Fluorescence immunostaining and migration assays were conducted using a fluorescence microscope and confocal laser scanning microscope. Wound healing and migration assays were performed using automated software to analyze the experimental readout and gap closure rate. Results: We confirmed the extraction of alpha-keratin and formation of the PEG-g-keratin complex. Treatment of HGFs with keratin protein at a concentration of 5 mg/ml promoted proliferation and maintained cell viability in the test group compared to the control group. HOKs treated with 5 mg/ml keratin exhibited a slight decrease in cell proliferation and activity after 48 hours compared to the untreated group, followed by an increase after 72 hours. Wound healing and migration assays revealed rapid closure of the area covered by HOKs over time following keratin treatment. Additionally, HOKs exhibited changes in cell morphology and increased the expression of the mesenchymal marker vimentin. Conclusion: Our study demonstrated the potential of hair keratin for soft tissue regeneration, with potential future applications in clinical settings for wound healing.

양서류 피부 상처회복과정에 대한 조직화학적 분석 (Histochemical Analysis of the Cutaneous Wound Healing in the Amphibian)

  • 상처회복과정은 염증, 재상피화, 그리고 기질의 재형성등을 포함한 포괄적인 생물학적 반응이다. 생물학적 분류체계에서, 회복능력은 매우 다양한데, 일반적으로 하등동물이 고등동물에 비교해 단순하며 뛰어난 것으로 알려져 있다. 따라서, 하등동물들을 이용한 실험모델은 상처회복과 치유의 기작을 연구하는데 자주 이용되고 있다. 양서류를 이용한 피부 상처회복과정 동안의 조직화학적 특성들을 규명하기 위하여 한국산 무당개구리(Bombina orientalis)를 이용하였다. 1일, 10일 그리고 16일의 조직에서 점액물질이 활발하게 분비되는 것을 확인하였으며, PAS (periodic acid shiff)와 Alcian blue (pH 2.5) 반응에 강하게 염색되었다. PTAH(phosphotungstic acid hematoxylin) 염색을 통하여 상처 후 10일 조직에서 상처주변의 진피층에서 콜라겐의 합성이 증가하는 것을 확인하였다. 또한, 세포의 유사분열의 시점을 확인하기 위한 MG-P (methyl green pyronin)염색에서 초기에는 3시간과 6시간에 강한 염색반응을 관찰하였고, 이는 23일과 27일 조직에서도 동일한 패턴으로 이어졌다.

방사선조사가 구루병 백서의 발치창 치유에 미치는 영향 (Effect of irradiation on wound healing after tooth extraction in the rachitic rats)

  • Purpose: To observe the histopathological changes following irradiation on the wound healing after tooth extraction in the rachitic rats. Materials and Methods: In order to carry out this study, the rats were divided into four groups: Group 1 (normal diet/non-irradiation group), Group 2 (normal diet/irradiation group), Group 3 (rachitogenic diet/non-irradiation group), and Group 4 (rachitogenic diet/irradiation group). Rachitic changes were induced with rachitogenic diet No. 2 (high calcium, low phosphorus, and Vitamin D deficient diet) for 5 weeks. After the extraction of both maxillary first molars of the rats in Group 2 and 4, the head and neck of the rats were irradiated with single absorbed dose of 10 Gy. The rats were sacrificed at the 1st, 5th, 10th, and 15th day after tooth extraction. The specimens including the extraction wound were sectioned, stained with the hematoxylin-eosin and Masson's trichrome method and examined under the light microscope. Results: In the Group 2, the amount of newly formed bone trabeculae on the periphery of extraction socket and osteoblastic activity were reduced. In the Group 3, epithelial fusion was not revealed on the 5th day after toothe extraction and growth rate of osteoid formation was reduced. In the Group 4, necrotized tissue at the outer surface of extraction socket and destructive changes on the alveolar bones were noted on the 10th day. Epithelial fusion was not revealed and large amounts of osteoclast were noted on alveolar bone on the 15th day. Conclusion: The healing process of wound after tooth extraction was retarded by irradiation and especially in the rachitic rats.

Triamcinolone acetonide가 설 창상치유에 미치는 영향에 관한 실험적 연구 (An Experimental Study on the Effects of Triamcinolone Acetonide on the Wound Healing of Rat Tongue)

  • In order to study the effects of traimcinolone acetonide on the wound healing of tongue, eight two healthy albino rats were asesthetized with pentothal. The wound, approximately 1mm in depth and 2mm in diameter, was created on the midlateral aspect of tongue with the tip of small rongeur fourcep. Forty one animals were employed in one of the following groups:(I) Control group: Received 0.5ml/100gm B.W. of saline suspension intralesionally at weekly interval until sacrificed.(II) Experimental group: Received $100\mu\textrm{g}/100gm$ B.W. of trimacinolone acet- onide suspension intralesionally at weekly interval until sacrificed. Biopsy specimens of wounds were taken at 6,18,24 hours, 2,3,5,7,14 and 28 days after wounding. The specimens were sectioned, and observed with light microscope and electron microscope. The results were as follows : 1. In the triamcinolone acetonide treated wound, the epithelial and fibroblastic regeneration were dealayed when compared with controls. 2. After triamcinolone acetonide treated, edema was decreased at the early stage of wound healing. 3. In the triamcinolone acetonide treated wound, the fibroblasts exhibited fine morphological changes and reduction of collagen formation. It seems that triamcinolone acetonide inhibits fibroblast function.
