안면신경마비의 치험례 (Case Report of Facial Nerve Paralysis)

  • Facial nerve paralysis(or Bell's palsy) which commonly occurs unilaterally, gives rise to paralysis of facial expression muscle. This condition is classified into symptomatic facial nerve paralysis due to intracranial tumor, post operative trauma, etc. and idiopathic facial nerve paralysis. To explain the etiology of idiopathic facial nerve paralysis, many hypothesis including ischemic theory, viral infection, exposure to cold, immune theory etc. were suggested, but there is no agreement at this point. The method to evaluate the facial nerve paralysis, when it occurs, consists of three stage scale method, image thechnics like CT and MRI, laboratory test to examine the antibody titers of viral infection, neurophysiologic test to evaluate the degree and prognosis of paralysis. Treatment includes medication, stellate ganglion block(SGB), surgery, physical therapy and other home care therapy. In medication, systemic steroids, vitamins, vasodilating-drug and ATP drugs were used. SGB was also used repeatedly to attempt the improvement of circulation and to stimulate the recovery of nerve function. Physical therapy including electric acupuncture stimulation therapy(EAST) and hot pack was used to prevent the muscle atrophy. When No response was showed to this conservative therapies, surgery was considered. After treating two patients complaining of Bell's palsy with medication(systemic steroids) and EAST, favorable result was obtained. so author report the case of facial nerve paralysis.

Association between stress and chewing ability of adults older than 65 years

  • Objectives: Stress is the cause of several illnesses, in older people, stress may also cause various social problems. The oral health of older adults is closely related to the quality of life, and chewing ability is particularly important for their general health. The purpose of this study was to investigate the relationship between stress, the number of teeth remaining, and the chewing ability, which reflects the oral health status among older adults. Methods: This study evaluated the stress level and chewing ability of adults older than 65 years using the 6th (2014-2015) Korea National Health and Nutrition Examination data. The total number of remaining teeth was determined based on the data of the teeth conditions. Results: There was an association between stress and chewing ability among older adults. The odds ratio of chewing function increased by 2.67 times (crude OR=2.67; 95% CI=1.88-3.79) with increased stress. After adjusting, the odds ratio increased to 2.74 times (adjusted OR=2.74; 95% CI=1.88-3.98). Conclusions: Reducing stress may facilitate effective oral health management and improve the overall quality of life in older adults. The findings of this study may help in the discovery of various approaches s to reducing stress in older adults and provide relevant information for oral health education.

한국 노인의 치과보철 상태에 따른 건강 관련 삶의 질에 관한 연구 (A study of the EQ-5D index according to the dental prostheses status of the elderly in Korea)

  • Purpose: This study aimed to obtain basic data on oral health improvement by investigating the EQ-5D index according to the oral prosthesis status of elderly persons aged 65 years or older in Korea using the Seventh Korea National Health and Nutrition Examination Survey. Methods: In this study, 3,426 elderly persons aged 65 years or older were included to analyze the relationship between the dental prostheses status and EQ-5D index in the collected data. Results: Compared to the "complete denture" in oral prosthesis, EQ in case of no dental prostheses or fixed dental prostheses is comparable to removable prosthetics such as "no dental prostheses," "one fixed bridge," and "two or more fixed bridges." Removable partial denture and complete denture required for the maxillary and mandibular EQ-5D index were significantly higher in "not necessary" than in "complete denture necessary" in all the elderly persons. Conclusion: The condition of the dental prosthesis and the need for a removable partial denture and complete denture for the elderly's EQ-5D index were confirmed, and it was found that the fixed dental prostheses were more effective than the removable dental prostheses. Therefore, oral health education programs for the elderly should be developed to promote dental function and oral health maintenance.

구강내하악지수직시상골절단수술 전후 전류역치검사(CPT)를 이용한 지각신경의 변화에 대한 연구 (Evaluation of Sensory Nerve Function Before and after Intraoral Vertico-Sagittal Ramus Osteotomy Using Current Perception Threshold(CPT) Test)

  • The design of osteotomy plane in orthognathic surgery has been developed to diminish the nerve injury. Intraoral Vertico-Sagittal Ramus Osteotomy (IVSRO) is the one of the best way to minimize untoward results, which is designed not to expose the lingula. We evaluated the nerve damage before and after with current perception threshold (CPT) test which is modem and numerically expressible way of nerve damages. Sixty patients underwent IVSRO since 1998 were evaluated. They were divided into 2 groups; one group underwent IVSRO only, and the other underwent IVSRO plus genioplasty. The both groups were evaluated with CPT test 1 week before surgery, and 1, 3 and 6 months after surgery. The CPT test was performed on A-beta, A-delta and C fiber respectively. 111e result showed that the recovery of sensory function of damaged nerve fibers was observed at the period of three to six months after surgery. There was no impairment of nerve function after only the IVSRO . But there were sensory disturbances in cases of additional genioplasty group. We thought that one of major factors on nerve damages were exposure of nerve and traction injury during genioplasty.

Dec2 inhibits macrophage pyroptosis to promote periodontal homeostasis

  • Purpose: Macrophages play crucial roles as early responders to bacterial pathogens and promote/ or impede chronic inflammation in various tissues. Periodontal macrophage-induced pyroptosis results in physiological and pathological inflammatory responses. The transcription factor Dec2 is involved in regulating immune function and inflammatory processes. To characterize the potential unknown role of Dec2 in the innate immune system, we sought to elucidate the mechanism that may alleviate macrophage pyroptosis in periodontal inflammation. Methods: Porphyromonas gingivalis lipopolysaccharide (LPS) was used to induce pyroptosis in RAW 264.7 macrophages. Subsequently, we established an LPS-stimulated Dec2 overexpression cellular model in macrophages. Human chronic periodontitis tissues were employed to evaluate potential changes in inflammatory marker expression and pyroptosis. Finally, the effects of Dec2 deficiency on inflammation and pyroptosis were characterized in a P. gingivalis-treated experimental periodontitis Dec2-knockout mouse model. Results: Macrophages treated with LPS revealed significantly increased messenger RNA expression levels of Dec2 and interleukin (IL)-1β. Dec2 overexpression reduced IL-1β expression in macrophages treated with LPS. Overexpression of Dec2 also repressed the cleavage of gasdermin D (GSDMD), and the expression of caspase-11 was concurrently reduced in macrophages treated with LPS. Human chronic periodontitis tissues showed significantly higher gingival inflammation and pyroptosis-related protein expression than non-periodontitis tissues. In vivo, P. gingivalis-challenged mice exhibited a significant augmentation of F4/80, tumor necrosis factor-α, and IL-1β. Dec2 deficiency markedly induced GSDMD expression in the periodontal ligament of P. gingivalis-challenged mice. Conclusions: Our findings indicate that Dec2 deficiency exacerbated P. gingivalis LPS-induced periodontal inflammation and GSDMD-mediated pyroptosis. Collectively, our results present novel insights into the molecular functions of macrophage pyroptosis and document an unforeseen role of Dec2 in pyroptosis.

측두하악관절의 활액성 연골종증 4예 (The synovial chondromatosis of the temporomandibular joints: review of the 4 cases)

  • Synovial condromatosis of the temporomandibular joint (TMJ) is characterized by the presence of loose bodies (joint mices). It can be confused with temporomandibular disorder clinically, and be with chondrosarcoma histologically. The purpose of this clinical report was to review the clinical, radiological, arthroscopic findings, histological feature and the results of surgical treatment of TMJ synovial chondromatosis. Four patients presented with pain of TMJ and limited mouth opening. The dynamic magnetic resonance imaging (MRI) disclosed a characteristic morphologic changes and displacement of the meniscus with limited translation of the condyle head. Bone scans showed progressive resorptive changes with hot-uptake of the radioisotope. The synovial loose bodies in the joint spaces were removed and sent to pathology for diagnosis as the synovial chondromatosis. The follow-up examination with computed tomography (CT) and MRI revealed no evidence of recurrence and good in function until postoperative 18 months. Diagnostically, the distension of the lateral capsule and fluid findings in the joint on the MRI are very suggestive tool for this synovial chondromatosis, but they are not always detected on the preoperative MRI. Arthroscopic approaches are very useful to inspect the joint spaces and to remove the loose bodies without interruption of the whole synovial membranes.

  • The kidneys play a vital role in the maintenance of normal body fluid volumes and in the composition of the extracellular fluid compartments. There are normally more than 2 million functioning glomeruli that regulate total body water and solute concentrations. As renal failure progresses, there is a decrease in the number of functioning nephrons. Chronic renal failure(CRF) is the consequence of a multitude of diseases that cause permanent destruction of the nephron. Azotemia is an elevation in blood urea nitrogen(BUN) and serum creatinine levels subsequent to a decreased glomerular filtration rate(GFR), which results in uremia. This loss of renal function can cause functional and metabolic abnomalities of body. For this problem, oral & maxillofacial surgeons have demanded to routinely treat patients with CRF. However, there has not been a reported case of orthognathic surgery by bilateral sagittal split ramus osteotomy(BSSRO) in patients with CRF, which can cause multiple complications in healthy patients. We report developmental mechanism of complication associated with CRF and preop. and postop. care of orthognathic surgery by BSSRO in Cl III patient with severe chronic renal failure.

Treatment of Oral Leukoplakia with Diode Laser: a Pilot Study on Indian Subjects

  • Background: To evaluate the safety, convenience and effectiveness of 940nm diode laser for treatment of homogenous leukoplakia. Materials and Methods: Ten patients having homogenous leukoplakia which were diagnosed clinically were selected from an Indian dental educational institution for the study. Toludine blue staining was applied locally over the lesion. The area where there was increased uptake of stain was excised using a 940 nm EZLASE TM diode laser (BIOLASE-USA). Results: Although various treatment modalities have been tried and the search continues for novel treatment modalities for complete removal of homogenous leukoplakia, from results of our preliminary pilot study it is clear that the use of 940 nm diode laser as a treatment modality for homogenous leukoplakia is a good substitute. Healing was perfect without any complication within a duration of 1 month. Pain intensity was also mild and absolutely zero on the VAS scale after 1 month follow up. Conclusions: 940 nm diode lasers are safe and can be effectively used as a treatment modality of homogenous leukoplakia, without any complication and without compromising health and oral function of patients. Considering recurrence factor, long term follow up for patients is a must.

Porphyromonas gingivalis Lipopolysaccharide Regulates Migration of Vascular Smooth Muscle Cells

  • Porphyromonas gingivalis, a foremost periodontal pathogen, has been known to cause periodontal diseases. Epidemiologic evidences have indicated the involvement of P. gingivalis in the development of cardiovascular diseases. In this study, we show that the P. gingivalis lipopolysaccharide increases the mRNA expression and protein secretion of interleukin-6 in vascular smooth muscle cells. We demonstrate that P. gingivalis LPS activates the extracellular signal-regulated kinase 1/2 (ERK1/2), p38 mitogen-activated protein kinase (MAPK), and Akt, which mediate the IL-6 expression in vascular smooth muscle cells. Also, P. gingivalis LPS stimulates the vascular smooth muscle cell migration, which is a critical step for the progression of atherosclerosis. Moreover, neutralization of the IL-6 function inhibits the migration of vascular smooth muscle cells induced by P. gingivalis LPS. Taken together, these results indicate that P. gingivalis LPS promotes the expression of IL-6, which in turn increases the migration of vascular smooth muscle cells.

경남 일부지역 노인복지시설 입소자의 구강건강 실태조사 (Study on actual condition of oral hygiene among admitted aged person at the some of Gyeongnam welfare facilities)

  • In Korea, respect for the aged and filial devotion is treated as basic ethics for human life, and family takes care of the aged person mainly. Nowadays, family support on the aged person is prioritized than the others. However, number of aged person is growing, and family, which is used to protect the aged, becomes nuclear through industrialization. In addition, social advancement of female induces weakened supporting function of family, and all these issues generate the problem of protection for the aged as significant social problems. Author conducted oral inspection and questionnaire for the aged in some welfare facilities in Gyeongnam location from December 2005 to February 2006 to improve quality of life and oral hygiene of the aged. Through gathered data, the actual condition of oral hygiene management on the aged person in welfare facility was evaluated as fundamental data for project development on oral hygiene of aged person. Through the analysis of inspected data, the following conclusions are derived. 1. Management status of oral hygiene is mostly not good, and toothbrushing per day is 'one time' for the most cases. 2. In free-of-charge facility, monthly allowance is 'under 50 thousand won' for the most cases. 3. Time for visiting oral treatment facility is on 'when toothache is occurred' for the most cases. For treatment content, 'prosthetic dentistry' takes 36.8% in charged facility, and 'tooth extraction' is 27.0% for free-of-charge facility. 4. Average DMFT index of the aged in charged facility is 16.81, and free-of-charge facility is 21.71. 5. Average number of functional teeth in charged facility is 15.22, and free-of-charge facility shows 7.29. 6. Average number of remained teeth in charged facility is 15.71, and the umbe in free-of-charge facility is 9.04. 7. Average number of extractable teeth in charged facility is 0.48, and for free-of-charge facility, the number goes up to 1.70.
