Unilateral segmental odontomaxillary hypoplasia: an unusual case report

  • Facial asymmetry is not an uncommon occurrence in day to day dental practice. It can be caused by various etiologic factors ranging from facial trauma to serious hereditary conditions. Here, we report a rare case of non-syndromic facial asymmetry in a young female, who was born with this condition but was not aware of the progression of asymmetry. No relevant family history was recognized. She was also deficient in both deciduous and permanent teeth in the corresponding region of maxilla. Hence, the cause of this asymmetry was believed to be a segmental odontomaxillary hypoplasia of left maxilla accompanied by agenesis of left maxillary premolars and molars and disuse atrophy of corresponding facial musculature. This report briefly discussed the comparative features of segmental odontomaxillary hypoplasia, hemimaxillofacial dysplasia, and segmental odontomaxillary dysplasia and justified the differences between segmental odontomaxillary hypoplasia and the other two conditions.

  • The author examined 515 full mouth roentgenograms stored in the Dept. of Oral Radiology, College of Dentistry, Seoul National University. For evaluating the efficiency of the routine full mouth roentgenogram, each of abnormal conditions such as impacted teeth, missing, caries, crown, filling and apical lesions was observed. The results obtained were as follows; 1. Among 14.420 teeth examined, missing teeth were 174, impacted 16, caries 161 and treated 1,162. 2. The incidence of impacted third molar was close to 18.7 percent and the incidence of missing third molar was about 33.4 percent. 3. Among carious and treated teeth, 178 teeth (1.2%) were needed observation of root apex. And of these teeth, 119 teeth 0.8% were observed with apical lesion. 4. There is a considerable relation between the incidence or size of apical lesion and the accuracy of endodontical treat. 5. Among the teeth with apical lesion, upper and lower first molars were most frequent (about 41%), upper and lower canine were rare (about 1.7%).

Evaluation of dental panoramic radiographic findings in edentulous jaws: A retrospective study of 743 patients "Radiographic features in edentulous jaws"

  • PURPOSE. The aim of this study was to determine the frequency of significant panoramic radiographic findings and eventual treatment requirements before conventional or implant supported prosthetic treatment in asymptomatic edentulous patients. MATERIALS AND METHODS. A total of 743 asymptomatic edentulous patients were retrospectively evaluated using a digital panoramic system. We analyzed the radiographic findings, including impacted teeth, retained root fragments, foreign bodies, severe atrophy of the posterior maxillary alveolar bone, mucous retention cysts, soft tissue calcifications and radiopaque-radiolucent conditions. RESULTS. Four-hundred-eighty-seven (65.6%) patients had no radiographic finding. A total of 331 radiographic findings were detected in 256 (34%) patients. In 52.9% (n=175) of these conditions, surgical treatment was required before application of implant-supported fixed prosthesis. However, before application of conventional removable prosthesis surgical treatment was required for 6% (n=20) of these conditions. CONCLUSION. The edentulous patients who will have implant placement for implant-supported fixed prosthesis can frequently require additional surgical procedures to eliminate pathological conditions.

  • Oral health in youth is very important because this period influences life for adult remarkably. Therefore, more effective oral health education and recognition of early treatment for oral disease are required sincerely. For more effectual education for high-school students, it is necessary to consider their circumstance like obesity and smoking. This study will analyze the correlation oral health behavior with obesity and subjective oral health concerns. In addition to, the actual condition of smoking for high-school girls and oral health condition will be verified. A survey was progressed for high-school girls in Busan city and BMI(body mass index) was used for classification of weight. The statistical significance was analyzed using SPSS 13.3 for Windows. Results of this study is following. 1. The frequence of daily toothbrushing was significantly different with obesity although difference of oral health knowledge was not significant. The frequency of toothbrushing of overweight students were lesser than the others. This results may be caused by low-level for oral health concerns and by passive life style of fat person. This habit has possibility of causing serious periodontal disease like periodontitis and tooth caries. 2. The correlation of obesity and time of brush change, using for supplement, and recent dental institution was insignificant. However, concerns of oral health were showed significantly according to obesity. Under-weight and standard-weight students were more concerned about oral health than over-weight. This results can be related to frequence of toothbrushing for fat subject. This relevance of two factors was confirmed by logistic regression analysis. 3. Approximately 70% students have smoking experience responded to uncomfortableness of periodontal conditions. On the other hand, non-smoking students felt uncomfortable in 50%. This results indicated that smoking have an effect on oral health condition. Further study which identify periodontal conditions practically has to go on for verification of direct correlation smoking and oral health.

Relationships between Intake of Sweet Drinks, Sweet Foods and Oral Health Status in Rural Adult Inhabitants (일부 지역사회 주민들의 감미식품 섭취와 구강 건강상태와의 관련성)

  • Objectives: This study aimed to investigate the relationship between consumption of sweet drinks and foods with accompanying oral hygiene health behaviors, and their condition of oral hygiene rural residents who are expected to have relatively poor oral hygiene compared to their urban counterparts. Methods: The 384 subjects, who are aged over 40 and have taken dental examination by dentists in health branches in 6 Myouns, Kongju city, Chungchongnamdo Province during the period between June 1st through 31st, 2004, were asked about dental health behaviors and subjective symptoms using questionnaires. Results and Conclusions: Based on dental health behavior according to sweet beverage and food, the dental health behavior was desirable in both sexes for 'almost daily drink' concerning sweet beverages and 'almost never eat' concerning sweet food, with no statistical significance. As for oral conditions, the groups who responded to 'almost daily drink/eat' for sweet beverages and food had the highest rate of non-treated teeth, lost teeth, DMFT and CPITN, with no statistical significance. Multiple logistic regression analysis with the consumption frequency of sweet foods as explanatory variables and 5 factors of oral conditions as dependent variables, revealed that there was no statistical significance in the oral conditions according to the consumption frequency of sweet drinks while, as for sweet drinks, there was a significant difference to the group who answered as 'occasional gum bleeding' in the case of 'almost daily eat.' responders(OR= 2.33, 95% CI=1.42-3.81, p<0.05).

Participation of central GABAA receptors in the trigeminal processing of mechanical allodynia in rats

  • Here we investigated the central processing mechanisms of mechanical allodynia and found a direct excitatory link with low-threshold input to nociceptive neurons. Experiments were performed on male Sprague-Dawley rats weighing 230-280 g. Subcutaneous injection of interleukin 1 beta ($IL-1{\beta}$) ($1ng/10{\mu}L$) was used to produce mechanical allodynia and thermal hyperalgesia. Intracisternal administration of bicuculline, a gamma aminobutyric acid A ($GABA_A$) receptor antagonist, produced mechanical allodynia in the orofacial area under normal conditions. However, intracisternal administration of bicuculline (50 ng) produced a paradoxical anti-allodynic effect under inflammatory pain conditions. Pretreatment with resiniferatoxin (RTX), which depletes capsaicin receptor protein in primary afferent fibers, did not alter the paradoxical anti-allodynic effects produced by the intracisternal injection of bicuculline. Intracisternal injection of bumetanide, an Na-K-Cl cotransporter (NKCC 1) inhibitor, reversed the $IL-1{\beta}$-induced mechanical allodynia. In the control group, application of GABA ($100{\mu}M$) or muscimol ($3{\mu}M$) led to membrane hyperpolarization in gramicidin perforated current clamp mode. However, in some neurons, application of GABA or muscimol led to membrane depolarization in the $IL-1{\beta}$-treated rats. These results suggest that some large myelinated $A{\beta}$ fibers gain access to the nociceptive system and elicit pain sensation via $GABA_A$ receptors under inflammatory pain conditions.

Assessment of the efficiency of a pre- versus post-acquisition metal artifact reduction algorithm in the presence of 3 different dental implant materials using multiple CBCT settings: An in vitro study

  • Purpose: The aim of this study was to assess artifacts generated in cone-beam computed tomography (CBCT) of 3 types of dental implants using 3 metal artifact reduction (MAR) algorithm conditions (pre-acquisition MAR, post-acquisition MAR, and no MAR), and 2 peak kilovoltage (kVp) settings. Materials and Methods: Titanium-zirconium, titanium, and zirconium alloy implants were placed in a dry mandible. CBCT images were acquired using 84 and 90 kVp and at normal resolution for all 3 MAR conditions. The images were analyzed using ImageJ software (National Institutes of Health, Bethesda, MD) to calculate the intensity of artifacts for each combination of material and settings. A 3-factor analysis of variance model with up to 3-way interactions was used to determine whether there was a statistically significant difference in the mean intensity of artifacts associated with each factor. Results: The analysis of all 3 MAR conditions showed that using no MAR resulted in substantially more severe artifacts than either of the 2 MAR algorithms for the 3 implant materials; however, there were no significant differences between pre- and post-acquisition MAR. The 90 kVp setting generated less intense artifacts on average than the 84 kVp setting. The titanium-zirconium alloy generated significantly less intense artifacts than zirconium. Titanium generated artifacts at an intermediate level relative to the other 2 implant materials, but was not statistically significantly different from either. Conclusion: This in vitro study suggests that artifacts can be minimized by using a titanium-zirconium alloy at the 90 kVp setting, with either MAR setting.

Evaluation of the nutrient quotient for the elderly (NQ-E) using congregate meal services according to their oral health conditions

  • BACKGROUND/OBJECTIVES: The purposes of this study were to evaluate the nutritional status and dietary habits of the elderly using the nutrition quotient for the elderly (NQ-E) and to analyze the differences in the NQ-E according to their levels of oral health. SUBJECTS/METHODS: The survey was administered to 123 elderly people receiving congregate meal services in Seoul. The questionnaire comprised 3 domains: oral health status, general characteristics, and the NQ-E for the elderly. RESULTS: The respondents were divided into 2 groups based on the average score of their levels of oral health (the group with high oral health scores: 4.42 points and the group with low oral health scores: 2.89 points). As a result of evaluating nutritional status using the NQ-E, it was found that the average NQ-E score was 58.7 points, with 46.0 points in the balance domain, 47.0 points in the diversity domain, 72.9 points in the moderation domain, and 61.8 points in the dietary behavior domain. The NQ-E score (62.3 points) of the group with high oral health scores is significantly higher than the NQ-E score (54.7 points) of the group with low oral health scores (P < 0.001). Concerning the NQ domain scores, the elderly with good oral health status had "favorable" results in terms of balance and dietary behavior, and the elderly with poor oral health status had "favorable" results only in terms of balance. CONCLUSIONS: Overall, several dietary areas needed improvement in general. Those with poor oral health conditions urgently needed to improve related factors to minimize the risk of increasing imbalanced nutrition and comorbidities due to insufficient nutrition and undesirable eating habits.

A qualitative study on the present conditions and problems of oral health care in senile dementia patients (치매 노인의 구강건강관리실태 및 문제점에 관한 질적 연구)

  • Objectives: The purpose of this study was to investigate the present conditions and problems of oral health care in senior citizens with dementia using a qualitative research method, through focused group interviews. Methods: Data was collected for approximately one month from May 2019. The subjects were divided into two groups: care workers and family caregivers. Fifteen participants were included in the study. Results: In-depth interviews with the care workers revealed the following three categories: characteristics of senile dementia patients, oral health care in senile dementia patients, and oral health care education. In-depth interviews with the family caregivers revealed the following four categories: characteristics of senile dementia patients, oral health care in senile dementia patients, oral health care education, and burden of care. The central themes common to both the care workers and family caregivers were the challenges owing to the characteristics of senile dementia patients, poor health condition of the senile dementia patients, difficulty in oral health care of the senile dementia patients, the desire to receive oral health care education and related information, and to access the information more easily. Additional central themes specific to the care workers were, the applicability of the intervention programs, variability between the facilities, and the problems of oral health care education. An additional central theme specific to the family caregivers was the burden of care. Conclusions: It is necessary to provide oral health care education and information to care workers and family caregivers of senile dementia patients, and to manage and support the dental health professionals ready to care for senile dementia patients. In addition, support to the family caregivers should not be limited only to the financial aspects, but also consider the psychological and emotional difficulties.

Resveratrol Impaired the Morphological Transition of Candida albicans Under Various Hyphae-Inducing Conditions

  • The ability of the human fungal pathogen Candida albicans to undergo the morphological transition from a single yeast form to pseudohyphal and hyphal forms in response to various conditions is known to be important for its virulence. Many studies have shown the pharmacological effects of resveratrol, a phytoalexin polyphenolic compound. In this study, we investigated the antifungal activity of resveratrol against C. albicans. Both yeast-form and mycelial growth of C. albicans were inhibited by resveratrol. In addition, normal filamentation of C. albicans was affected and yeast-to-hypha transition under serum-, pH-, and nutrient-induced hyphal growth conditions was impaired by resveratrol.
