A Basie Health Survey of the Yonsei Community Health Service Area, Seoul (연세지역(延世地域)에 대(對)한 보건기초조사(保健基礎調査))

  • Introduction In order to improve medical education through the introduction of a concept of comprehensive health care of a community, an area surrounding the University Campus was chosen for the Community Health Service Project. It has been on operation for last 4 years with its major emphasis on family planning services, and maternal and child health care. The major objectives of this survey at the area are to obtain: 1) The demographic data, 2) The health need and trend of medical care, 3) The attitude and practice in maternity care to be used for further improvement of the planning and the services of the project. Population and Survey Method Out of three Dongs of the Community Health Service Area, only two Dongs namely Changchun and Yonhee were selected for the survey. Total number of households and population in the area studied was 3,683 and 21,857 respectively. An interview was performed with questionnaire schedule which was recorded by interviewers. This includes the degree of utilization of health services provided by the Community Health Service Program such as family planning, prenatal care during their last pregnancy, delivery history and complications of the delivery as well as the incidence of illnesses in general. Prior to the interview, all interviewers were trained for interviewing technique for two days. The survey was carried out during the period from October December 1967. Results 1) Demographic Data : 41.3% of the population studied were children under age 15 and only 3.5% were over 60 years of age. Crude birth rate and crude death rate of this area studied during the period of November 1966-October 1967 were 20.5 and 7.7 respectively. Infant mortality rate during the same period was 35.9. 50.4% of the 2,832 households fell into the category of middle class, 39.8% to the lower class and 9.5% to the upper class in economic condition. 19.8% of 2,832 householders had no formal education, 22.7% primary school, and 57.5% middle or higher school education. 2) Health Status and Utilization of the Community Health Service: Those who suffered from many illnesses during the month of October, 1967 were 690(4.6% of 14,891 persons). Classification of these patients into the type of disease shown respiratory diseases 27.4%, gastrointestinal diseases 18.1%, tuberculosis 10.9%, skin and genitourethral diseases 4.5% and gynecologic patients 4.5%. Only 55.9% of the patients received medical care at hospital or doctor's clinic. But among TB and gynecologic patients, 70.7% and 72.4% were treated at medical facilities. 10.6% of 2,832 householders interviewed has ever utilized the Community Health Service Program provided by the Yonsei Medical School, Classifying these clients into the type of service, 35.9% utilized the wellbaby clinic, 31.0% the family planning clinic, 14.7% the home delivery care, and the rest utilized other services such as the premarital guidance cinlic and the sanitary inspection service. 3) Maternity Care: 23.6% of 2,151 deliveries were done at medical facilities such as hospital, private clinic, while 76.4% were done at home. Acceptance rate of prenatal care was 32.6% as whole, but 49.6 of 774 women who had the prenatal care service had their deliveries at medical facility. 45.1% of total deliveries were attended by medical and or paramedical personnel. 75.8% of the deliveries of those received prenatal care were attended by medical and or paramedical personnel while only 27.8% of the deliveries of those who did not have prenatal care attended by medical and or paramedical personnel. 49.8% of deliveries of the upper class, 29.8% of the middle class and 9.9% of the lower class were attended by medical and or paramedical personnel. 6.2, 3.3% and 24.8% of mothers reported about their xeperience of edema, coma and fever during the period of trimester of pregnancy and puerperium. 4) Family Planning: The rate of practice of family planning was 27.9%. 31.7% of them were by IUD, 2.9% by oral pill, 15.2% by sterilization and the rest by traditional methods. Those women who had 3 to 4 children had highest(30.2%). Practice rate among the various methods of family planning, oral pill was the most popular method to whom had 2 or less children. In relation between the practicing rate of family planning and living standard, the upper, middle and lower class practiced 37.5, 29.4 and 19.9% respectively.

Suggestion of Learning Objectives in Social Dental Hygiene: Oral Health Administration Area (사회치위생학의 학습목표 제안: 구강보건행정 영역)

  • The purpose of this study is to propose learning objectives in social dental hygiene by analyzing and reviewing learning objectives in oral health administration area of the existing public oral health. This study is a cross-sectional study. The subjects of the study selected with convenience extraction were 15 members of the social dental hygiene subcommittee of the Korean Society of Dental Hygiene Science. Data collection was conducted by self-filling questionnaire. The research tool is from 48 items of A division in the book of learning objectives in the dental hygienist national examination, and this study classified each of them into 'dental hygiene job relevance', 'dental hygiene competency relevance', 'timeliness', and 'value discrimination of educational goal setting' to comprise 192 items. Also, to collect expert opinions, this study conducted Delphi survey on 7 academic experts. Statistical analysis was performed using the IBM SPSS Statistics ver. 23.0 program (IBM Co., Armonk, NY, USA). Recoding was performed according to the degree of relevance of each learning objective and frequency analysis was performed. This study removed 18 items from the whole learning objectives in the dental hygienist national examination in the oral health administration area of public oral health. Fifteen revisions were made and 15 existing learning objectives were maintained. Forty-five learning objectives were proposed as new social dental hygiene learning objectives. The topics of learning objectives are divided into social security and medical assistance, oral health care system, oral health administration, and oral health policy. As a result of this study, it was necessary to construct the learning objectives of social dental hygiene in response to changing situation at the time. The contents of education should be revised in order of revision of learning objectives, development of competency, development of learning materials, and national examination.

Types of malocclusion and oral health effect index(OHIP-14) according to recognition of orthodontic treatment (부정교합 종류에 따른 교정치료의 인식과 구강건강영향지수(OHIP-14))

  • The purpose of this study was to examine the influence of type of malocclusion and orthodontic treatment awareness on quality of life among orthodontic patients in the region of Busan as well as to develop an educational program tailored to the type of malocclusion as a way to improve quality of life. A survey was conducted for approximately 6 months from December, 2015, and the answer sheets from 472 respondents were analyzed. The most common painful area was the teeth, and this case was most predominant in the respondents with level 2 malocclusion, who differed from others in that regard (p<0.001). Regarding the relationship between satisfaction with orthodontic treatment and quality of life, respondents who were more satisfied currently and who were neither quite confident nor quite unconfident were ahead of their counterparts in quality of life. Concerning the reason for receiving orthodontic treatment, quality of life was lower among patients who started to receive treatment due to pronunciation problems (p=0.013), chewing difficulty (p<0.001), and temporomandibular joint click sound (p<0.001). With regard to influential factors on oral health-related quality of life, time for starting to receive orthodontic treatment was most influential (p<0.001), followed by current satisfaction (p<0.001), changes in confidence (p=0.003), self-rated teeth status (p=0.008), and type of occlusion (p=0.019). Therefore, accurate analysis of the oral health status of orthodontic patients and customized oral health education are required to improve quality of life even during the period of orthodontic treatment.

National Oral Health Screening for Infants and Children: A Survey on its Perception, Requirements and Accuracy of Parents and Dentists (영유아 구강검진에 대한 부모와 치과의사의 인식과 요구도 및 정확도 조사)

  • This study aimed to investigate parents' and dentists' perceptions of the national oral health screening for infants and children (OHS), and evaluate the dentists' accuracy of analyzing the results of OHS. A questionnaire survey was conducted, face-to-face or online, for 90 parents of children who received OHS in the Department of Pediatric Dentistry at Yonsei University Dental Hospital and 100 dentists working at local clinic or university dental hospital from May to October 2021. Most parents and dentists were aware of the importance of OHS, and approximately 96.7% of parents were satisfied with OHS. The requirements of parents and dentists about OHS were different. The reasons for having difficulties in explaining parents after OHS and the opinions on appropriate period and number of OHS were disagreed between pediatric dentists and general dentists. Regardless of dentists' major, work experience, elapsed period after taking online education program, the accuracy of the examination result was low. In this study, various opinions of parents and dentists on OHS were collected. Efforts should be made to enhance the oral health of infants and children by considering the requirements of parents and dentists and improving the accuracy of examination results.

A Study of the Mask and Hand Contamination in Dental Clinic (치과에서의 마스크 및 손의 미생물 오염정도 비교)

  • Purpose: The purpose of this study was to observe the degree of mask contamination in dental hygienist for general and oral bacteria and to identify areas of mask contamination after treatment. Methods: Masks were collected with every fifty dental hygienists who currently working in the department of preventive dentistry, prosthodontics, and orthodontics in Busan. The mask bacteria were collected in specific upper and side parts of the mask. Hand germs were collected using sterile cotton swabs, and then placed in a sterile conical tube. These were transferred to the laboratory. Hand germs and mask bacteria were incubated with nutrient broth (NB) and brain heart infusion broth (BHI) for 24 hrs and each cultured with NB and BHI plate at $37^{\circ}C$ for 48 hrs. Collected data were analyzed using the SPSS Window 20. Results: The number of bacteria was observed in the order of the department of preventive dentistry ($10.1{\times}10^5CFU/ml$), prosthodontics ($14.7{\times}10^5CFU/ml$), and orthodontics ($23.3{\times}10^5CFU/ml$) in the hand. In general bacteria, the difference of contamination was seen by the parts of the mask, but there was no significant difference. However, the oral bacteria were observed highly contaminated upper part of the mask in preventive dentistry. The mask contamination according to the medical departments was observed. Especially, the contamination of mask in preventive dentistry was significantly higher than other departments in oral bacteria. Conclusion: This study suggested that correct mask replacement and recognition of contamination areas can contribute to the prevention of infectious disease. and it would be necessary to increase hand hygiene performance to prevent cross-infection with masks. Also, this study may give an idea for making guidelines for mask management and supporting to establish clear criteria for the education program of personal protective equipment.

Study on Influence of Water Fluoridation Program on the Economic Benefit (상수도수불화사업의 경제적 편익에 대한 조사연구)

  • To find out influence of water fluoridation program on the economic benefit effect, this study analyzed comparatively dental treatment indicators obtained from dental clinics at Sangdang-gu in Chenongju City, fluoridated community and Manan-gu in Anyang City, non-fluoridated community, from January 1, 2003 to the end of June, and the results are as follows: 1. If water fluoridation program is carried out in Anyang, non-fluoridated community, the number of treatment from caries and disease of tissues around dental pulp and root apex from age 6 to 18 for the first half of year 2003 is expected to decrease by 1,272, 636 for low estimation, and 1,908 for high estimation. And the treatment cost is expected to decrease by 40,888,000 Wons, 20,444,000 Wons for low estimation, and 61,333,000 Wons for high estimation. 2. Assuming that water fluoridation program is spread to all over the country, the number of treatment per year is expected to decrease by 2,492,018, 1,246,009 for low estimation, and 3,738,027 for high estimation. And the treatment cost is expected to decrease by 80,105,075,000 Wons, 40,052,537,000 Wons for low estimation and 120, 157,612,000 Wons for high estimation respectively. 3. For Yeong-un water supply management office which has 48,925 water supply personnel, the benefit/cost rate was 2.9 times, and for Jibuk water supply management office which has 239,751 water supply personnel, the benefit/cost rate was 9.0 times. As the water supply personnel increased, the cost increase of water fluoridation program was little and as the scale got large, the economic effect was very large. Improving national oral health is thought to be associated with expense retrenchment of oral health insurance financial. So it may need to extend using tap water to all the area of the country and additionally establish confidence through an active public relations and education of water fluoridation program.

A Study on the Status of Recognition and Practical Application of Oral Hygiene Devices : with outpatient as the central figure (치과병·의원 내원환자의 구강위생용품에 대한 인지도 및 사용실태에 관한 연구)

  • This study was carried out to investigate recognition level and practical application status of oral hygiene devices through making a survey of Seoul inhabitants. And survey was implemented in order to propose oral hygiene device for household health care activities. The following results were obtained by analyzing personal interviews of 347 commuting patients at two university hospitals and seven dental clinics. 1) The average length of toothbrush head was estimated as 22.3 mm and average changing cycle was 2.3 months. The user ratio of flat-headed brush was estimated as 51.9% and 46.7% were using fluoridated toothpaste. 2) Generally the ratio of toothpick users was higher than other device users. But the user ratio of dental floss was higher than toothpick in case of patients under orthodontic treatment. 3) The patients under orthodontic treatment were not familiar with handling orthodontic toothbrush. Though 45.8% among orthodontic patients recognized this type of toothbrush, only 25.4% of them knew how exactly to use it. 4) It was shown very low user ratio of oral hygiene devices that the patients who had periodontal problem, hypersensitive trouble, halitosis, implant or denture 5) The patients who had halitosis showed the highest user ratio of toothpick. The patients who had separation of teeth showed the highest user ratio of dental floss. The patients who're under orthodontic treatment showed the highest user ratio of interproximal brush and motorized brush. 6) In response to the most interesting dental disease, it's surveyed as follows; 60.3% of dental caries, 24.0% of periodontal disease, 14.8% of false occlusion and 0.9% of oral cancer. 7) Regarding the motivation of using those oral hygiene devices, 45% responded that it was because of recommendation by dental clinics. Among the negative answers, 38.6% responded that it was because of no selection guidance. 31.3% answered that they didn't use hygiene device because it's inconvenient. 12.0% answered that it's difficult to buy and expensive. 7.8% responded that they didn't feel significant improvement. 4.8% answered that dental hospitals and clinics didn't even introduce those hygiene devices. Therefore efficient campaign for those hygiene devices over all Korea nation should be developed and education program must be prepared for each case of patients in every dental hospitals and clinics.

Maincause of influencing oral health impact profile(OHIP) and self-esteem of orthodontic patients (교정환자의 자아존중감과 구강건강영향지수(OHIP)에 영향을 미치는 요인)

  • Objectives : This study aimed to determine self-esteem and oral health impact profile (OHIP) of orthodontic patients to get data necessary for the process of consultation, education, and treatment for those patients who hoped to get orthodontic therapy in order to see effects of orthodontic therapy on personal life in consideration of general characteristics of those patients under orthodontic therapy. methods : A self-administered questionnaire was used among 232 outpatients under orthodontic therapy from October to November 2009, obtaining the following results. Results : 1. As for general characteristics of respondents, those under 19 years of age constituted 46.6%; the unmarried comprised 85.8%; and students constituted 53.4%. 2. Most respondents were getting orthodontic therapy for beauty (41.8%) and dental health (40.5%). Family (54.3%) contributed to their decision to get orthodontic therapy; parents or siblings (62.5%) were bearing the treatment expenses. 3. Females were getting orthodontic therapy for an aesthetic purpose (50.3%), and male for the purpose of dental health (44.6%) (P<0.01). 4. OHIP was slightly higher among males ($4.10{\pm}0.61$) than among female ($4.00{\pm}0.78$), and was highest among those 19 years old and under ($4.17{\pm}0.67$); the older they were, the lower it was significantly (P<0.05). Most of them were earning 3 to 4 million won ($4.22{\pm}0.72$); the married ($4.05{\pm}0.74$) constituted higher percentage. As for occupation, students ($4.13{\pm}0.66$) comprised the highest percentage. 5. Self-esteem was higher among males ($3.83{\pm}0.47$) than among females ($3.80{\pm}0.50$), and was highest among those 25 to 29 years old ($3.92{\pm}0.46$). Most of them were earning more than 5 million won ($3.91{\pm}0.42$), which was statistically significant (P<0.01). The married ($3.91{\pm}0.54$) constituted higher percentage than the unmarried ($3.80{\pm}0.48$); office managers ($3.95{\pm}0.55$) comprised the highest percentage. 6. As for correlation between variables effecting OHIP of orthodontic patients, there was negative correlation in age (r=-0.225) and positive correlation in occupation (r=0.122). As for correlation between variables effecting self-esteem of orthodontic patients, there was negative correlation in occupation (r=-0.130) and positive correlation n income (0.126), while there was positive correlation (r=0.202) between OHIP and self-esteem of orthodontic patients. Conclusions : Younger orthodontic patients who were students showed higher OHIP, and professionals earning more showed higher self-esteem. Orthodontic patients with higher OHIP showed higher self-esteem. Therefore, it is necessary to develop a program based on OHIP and self-esteem in consulting and treating orthodontic patients; since it is considered that orthodontic therapy will improve appearance, enhance self-confidence, and have positive effects on interpersonal relationships and quality of living, researches are necessary on constant changes in psychological properties.

Effects of Stress Perception Level on Dietary Habits and Oral Health Behaviors in Adolescents (청소년의 스트레스 인지수준이 식습관 및 구강건강행태에 미치는 영향)

  • The purpose of this study was to assess the relationship that adolescent stress perception level has with dietary habits and oral health behaviors in high school students. A survey of high school students in some areas of the Jeollabuk-do province of South Korea was conducted. Five hundred fourteen survey responses were used in the final analysis. As a result of the questionnaire survey, the following conclusions were obtained. The highest stress type was indicated to be academic stress (mean${\pm}$standard deviation [SD], $3.09{\pm}0.89$). The next was shown to be home (family) stress (mean${\pm}$SD, $2.85{\pm}0.84$). The possibility of using a dental clinic was indicated to be less in girls than boys (p<0.001). Regarding subjective oral health behavior, the possibility of visiting a dental clinic was low in those who thought that their own oral health condition was not good or moderate (p<0.05). Also, it was shown that the higher stress led to the higher possibility of visiting a dental clinic (p<0.01). Students with higher grades had a in the upper ranks were indicated to have high possibility of having a regular meal (p<0.01). Higher stress led to the significantly higher possibility of eating cariogenic food (p<0.01). Students with median grades had a high possibility of eating cariogenic food (p<0.01), while students with higher grades had a low possibility of eating cariogenic food (p<0.05). These resultss show that stress perception level influences dietary habits and oral health behaviors. Thus, there is a need to develop a program in high scholls to promote the physical and mental health of students to relieve stress. Substantial and systematic oral health education is thought to be likely needed to develop desirable dietary habits.

A Study on the Oral Health Awareness and Behavior of Higher Graders in Elementary School (초등학교 고학년 학생의 구강보건 인식 및 태도에 관한 조사)

  • In order to develop the program of oral hygiene education for elementary school senior-students, a questionnaire survey of elementary fourth, fifth and sixth students who are under continual control program of oral hygiene. (1) For tooth brushing time, after dinner is more than before. 73.7% of respondents brushed the teeth twice or more a day. (2) 62.1% of respondents took the cariogenic food twice or more a day. (3) 90.1% of respondents had visited a dental clinic. (4) Concerning fear for dental treatment, only 14.4% had a feeling of fear. (5) Regarding the experience with a preventive treatment, 39.7% had an experience with pit and fissure sealing, and 24.2% had an experience with the application of fluorides. (6) 46.2% of respondents had a preference for the fluoride mouth rinsing program, and 38.4% wanted that program to keep going on. As to the reason to favor the fluoride mouth rinsing program, 38.2% preferred it because of prevention of dental caries, and 43.0%, the largest percentage, didn't favor it because they found it too much trouble to do. (7) Comparing with the research result in 2000, we proved positively the educational effect of brushing frequency, positive understanding and preference to water school fluoridation program, and got the negative data of experience of visiting to oral clinic and preventive treatment.
