Clinical significance of saccade test, smooth pursuit test, and optokinetic nystagmus test in nystagmography (안진검사에서 단속운동검사, 시추적검사, 시운동성 안진검사의 임상적 의의)

  • Background: Saccade test, smooth pursuit test, and optokinetic nystagmus test are clinically useful tests to accurately diagnose vertigo. However, there have only been a few studies regarding a correlation between the anatomical site of the lesion and the abnormality of eyeball movement in patients with vertigo. Methods: The medical records of 97 patients with vertigo between January 2006 and June 2008 were reviewed retrospectively. We classified many kinds of abnormalities regarding the saccade test, smooth pursuit test and optokinetic nystagmus test into several categories and analyzed the localizing lesion of vertigo. Results: According to the saccade test, both total saccade abnormality (S-total) and slow velocity of saccade (S-type 3) were shown to be significantly higher in the central lesion of vertigo. According to the smooth pursuit test, symmetrical unidirectional smooth pursuit abnormality (SP-type 2) was observed to be significantly higher in the peripheral lesion over vertigo. Moreover, according to the optokinetic nystagmus test, total optokinetic nystagmus abnormalities (OKN-total) were shown to be significantly useful findings in the diagnosis of the central lesion of vertigo. The coexisting abnormalities of all three tests (S+SP+OKN abnormalities) were shown to be significantly higher in the central lesion of vertigo. Conclusion: These results suggest that all these tests, saccade test, smooth pursuit test, and optokinetic nystagmus test, are very useful to distinguish between the central lesion and the peripheral lesion of vertigo. However, these tests are not beneficial in localizing the central lesion of vertigo.

Effect of GABA Antagonist in the Monocular Optokinetic Nystagmus of the Chicken (닭의 Monocular Optokinetic Nystagmus에서 GABA Antagonist 효과)

  • Chicken monocular head and eye optokinetic nystagmus (OKN) were observed by coil recordings after intravitreal administration of GABA antagonists (picrotoxin and bicuculline) into the opened and closed eye. Before injection of drugs the chicken displayed an OKN for T-N stimulation being more efficient in evoking this visuomotor reflex than for N-T stimulation. The injection of GABA antagonist into the opened eye provoked a decrease or disappearance of the head and eye OKN. On the other hand, the injection of GABA antagonist into the closed eye, the head and eye OKN augmented. Thus, GABA antagonist abolished the directional asymmetry of the head and eye OKN, indicating the involvement of GABAergic mechanisms in the inhibition of the N-T component of the monocular OKN.

Involvement of GABAergic Mechanism in the PIasticity Phenomenon of Chicken (닭의 Plasticity 현상에서 GABAergic 기작의 관련)

  • In monocular vision, bead and eye optokinetic nystagmus (OKN) display directionnal asymmetry, in lower vertebrates such as chickens, T-N stimulation being more efficient in evoking this visuomotor reflex than N-T stimulation. The N-T component of monocular OKN is significantly weaker in chickens. Coil recordings and observation showed that in adult chickens, prolonged monocular visual deprivation by unilateral eyelid suture provoked significant and progressive increase of the N-T component in chickens. This plasticity phenomenon involved the eye and head OKN in chickens. The administration of THIP, a GABA agonist, abolished reversibly the increase of the N-T component in chickens. This fact suggests that the GABAergic system could be involved in determining this plasticity phenomenon observed in adult lower vertebrates.

  • The ultimate role of ocular movements is to keep the image of an object within the fovea and thereby prevent image slippage on the retina. Accurate evaluations of eye movements provide very useful information for understanding the functions of the oculomotor system and determining abnormalities therein. Such evaluations also play an important role in enabling accurate diagnoses by identifying the location of lesions and discriminating from other diseases. There are various types of ocular movements, and this article focuses on saccades, fast eye movements, smooth pursuit, and slow eye movements, which are the most important types of eye movements used in evaluations performed in clinical practice.

  • The purpose of this study was to develop a sinusoidal rotatory chair systim for evaluating the vestibular function in patients suffered from vertigo with vestibualr disorders. The sinusoidal rotatory chair system is composed of a rotatory chair systim and softwares. Maximum velocity of the rotatiry chair was upto 60 degree per second and frequency range was 0.01 to 0.64 Hz. To evaluate the vestibular function in vertigo patients, vestibuloocular reflex was measured by sinusoidal rotation of the whole body about vertical axis in the darkness, and optokinetic nystagmus, visual vestibuloocular reflex , and visual supression test were also performed. Eye movement was measured by means of a electronystgmograph amplifier through Ag-AgCI surface electrodes. Gain, phase, and symmetry were obtained from analysis of the eye movement ineuced by vestibular or visual stimulation. In healthy adults, sinusoidal rotation of the shole body produced nystagmus, of which gain was directly proportional to the velocity of stimulation. The gain of visual vestibuloocular reflex or optokinetic nystagmus, and it was suppressed markedly by visual suppression test. These results suggest that the sinusoieal rotatory chair systim which was developed by this study can evaluate the vestibular function quantitatively, also this system is very useful to diagnose and to dstimate the vestibualr function during recovery from vertigo

Functional Exploration of Optokinetic System by a Full Visual Field Stimulation

  • In the present study, we described a test to explore the function of optokinetic system which subjected to a full visual field stimulation using two different stimulus images patterns. Our results were interesting in a point of view that the stimulation image pattern had non- neglisible influence on the optokinetic response and that in a bidimensionnel image such as the randomly distributed spots images pattern the linearity of system was assured upto the stimulus velocity of about 50 deg/sec for normal subject. As for measuring human optokinetic after nystagmus, the regular stripes pattern was rather desirable than the randomly distributed spots pattern in this study.

  • Optokinetic nystagmus(OKN) testing is an useful tool for assessment of human visual function. Specially, this method has been used for assessing visual function in infants and malingering patients. OKN drum and OKN tape are the common devices currently used for clinical cases. In this paper, we present newly the automated and computerized method for the quantitative and objective assessment of visual acuity in OKN testing.

  • A new control method for precision robust speed control of a PMSM(Permanent Magnet Synchronous Motor) using load torque observer is presented. With this system we can obtain more reliable eye moving singal(nystagmus) Until now rotating chair system which is called 2D-optokinetic stimulator is used to make dizzincess. However an inclined rotating chair system witch is 3D-optokinetic stimulator is needed to obtain the precise dizziness data. This 3D-optokinetic sitimulator include unbalanced load aused by unbalanced center of mass. For this case new compensation method is considered to obtain robust speed control using load torque observer. To reduce the effect of this disturbance we can use dead-beat observer which has high gain. The application of the load torque observer is published in [1] for position control. A problem of using speed information such as amplifying effect of noise is reduce by moving average process. The experimental results are depicted in this paper to show the effect of this proposed algorithm.

  • A control of the body posture and movement is maintained by the vestibular system, vision, and proprioceptors. Especially, vestibular system has a very important function that controls the eye movement through vestibuloocular reflex and contraction of skeletal muscles through vestibulospinal reflex. However, postural disturbance caused by loss of vestibular function results in nausea, vomiting, vertigo and loss of craving for life. Lose of vestibular function leads to abnormal reflex of eye movements named nystagmus. Analysis of the nystagmus is needed to diagnose the vertigo, which is performed by means of electronystagmography (ENG). The purpose of this study is to develop a computerized system for data processing and an algorithm for the automatic evaluation of the slow component velocity (SCV) of nystagmus Induced by optokinetic(OKN) stimulation system. A new algorithm using recursive least square method (RLSM) to detect SCV of nystagmus is suggested in this paper. This method allows a fast and precise evaluation of the nystagmus, through artifact rejection techniques. The results are depicted in this paper.
