  • The objective of this study was to determine the influence of drug lipophilicity on the extent of ocular and systemic absorption following topical solution instillation in the pigmented rabbit. ${\beta}-Blockers$ of various lipophilicity were chosen as model drugs, $25\;{\mu}l$ of a 15 mM drug solution in isotonic pH 7.4 buffer was instilled, and ocular tissue and plasma drug concentrations were monitored. Ocular absorption was apparently increased in all eye tissues, but non-corneal absorption ratio was decreased by increasing of drug lipophilicity. Systemic bioavailability was ranged from 61% for atenolol to 100% for timolol, and at least 50% of the systemically absorbed drug reached the blood stream from the nasal mucosa. Occluding the nasolacrimal duct for 5 min reduced the extent of systemic absorption of timolol and levobunolol, but did not do so for atenolol and betaxolol. Taken together, the ocular absorption of topically applied ophthalmic drugs would be modest for lipophilic drugs. By contrast, the systemic bioavailability would be modest for drugs at the extremes of lipophilicity, and the nasal contribution to systemically absorbed drug diminished with increasing of drug lipophilicity.

  • This review on ocular toxicology concentrates on the effects on ocular health and contact lens wear induced by systemically used drugs, including alcohol. Many systemically administered drugs produce ocular adverse effects. Fortunately, relatively few are capable of causing significant, irreversible visual impairment. The visual symptoms of acute intoxication are as follows : Drop in vision/visual acuity, diplopia, poor dark adaptation, increase in time for glare recovery, early cataract, decreased depth perception, blue-yellow or red-green colour defect and visual hallucinations. Blinking pattern, tear production, and discoloration of contact lenses can be affected by some systemic or local ocular medications. The cornea, conjunctiva, or eyelids may react to some systemic medications and to some preservatives used in contact lens solutions. The hydrogel contact lens act as a drug reservoir that emits the drug over time. I discuss management of contact lens-induced infectious and inflammatory conditions.

  • Antioxidative activities of methanol extracts of 7 natural products which have been orientally used or studied as ophthalmic drugs were investigated. All of 7 extracts assayed - Rubus coreanus, Dendrobium monile, Eriocaulon sieboldianum, Ligustrum japonicum, Cassia tora, Cicadae Periostacum and Polygonatum odoratum var pluriflorum had some electron-donating ability to OPPH radicals, in which R. coreanus and D. monile showed strong activity as similar as ascorbic acid or BHT. At the concentration of $200{\mu}g/m{\ell}$, methanol extract of L. japonicum showed superoxide dismutase-like activity the same as 0.1 mM ascorbic acid and methanol extract of R. coreanus inhibited effectively the production of ABTS radical produced by $H_2O_2$. Furthermore, methanol extract of C. tora showed the highest nitrite scavenging ability. The inherent antioxidative activities of these natural products appear to be responsible for its ophthalmic effects.

  • Two patients presented with amaurosis fugax[AMF]. despite treatment with antiplatelet drugs and anticoagulants due to previous transient ischemic attacks. Angiography demonstrated severe stenosis in the cavernous and petrous internal carotid artery[ICA] respectively, with reduced flow in the ophthalmic artery[OA]. Endovascular stent placement in both patients resulted in normalization of the vessel lumen of the stenotic vessel segments. In addition, complete restoration of OA flow was noted immediately after stenting. Both patients showed no further episode of AMF over a follow-up period of 38 and 23 months respectively. Our clinical and angiographic findings suggest that hemodynamic Insufficiency in retinal vasculature caused by a stenosis of the cavernous or petrous ICA can be treated successfully by endovascular stent.

  • The separation system of pharmacy and clinic has begun on the purpose of preventing drug misuse and abuse since July 1st of 2000. The system revealed some conflicts between doctors. pharmacists and consumers. Consequently pharmaceutical law and related policies undergone some change. Now in an early period of the implementation of the system, the necessity to examine relevance of those policies and law enforcement to medical doctors' prescriptions pattern evolves. This study tries to verify the pattern through a field study. For the purpose, 930 prescriptions collected in May of 2001, from a pharmacy located in Gangnam-gu in Seoul, were analysed. The prescriptions were issued from several clinics: 459 prescriptions from otorhinolaryngological clinic(ENT), 177 from internal medicine clinic(IM), 130 from ophthalmic clinic(Opt), 52 from obstetric and gynecologic clinic(OB & GY), and 112 from miscellaneous clinics. ENT, IM, Opt. OB & GY are situated in a clinic building of 40m distance. The general findings are following: 1) $88.8\%$ of the total patients came from 5clinics in nearby single clinic building. 2) Average prescribing days were 6.2 days and the average number of used drugs were 4.0 drugs, i.e. 2-4 times of WHO criteria 1-2 drugs. 3) Use of antibiotics in the oral administration drugs rated $71.8\%(WHO: \;22.7\%)$ 4) Use of injection rated $31.3\%(WHO:\;17.2\%)$ 5) $96.2\%$ of the patients use multiple antibiotics in the injection and oral administration together. 6) The patients had multiple disease : ENT patients 1.7 disease and 1M patients has 2.7 disease in average and several regular prescribing types evolved particularly in the ENT prescription. With this result we found that drugs. especially antibiotics are still abused a lot, and there were significant differences in the number of used drugs and prescrbing days between the clinics. It implies somes differences of the preparation work and time for pharmacists. And preparation can be done in advance by pharmacists' own efforts through noticing regular prescribing types. The study suggests the followings: 1) Patient counseling should be done to minimize the incidence of adverse events. 2) The enforcement of the standardized differential preparation price system should be reconsidered. 3) Preparation of typical regularly appeared prescription in advance. which is regarded as 'a prearranged work between doctors and pharmacists' and has been prohibited should be reconsidered. 4) Drug utilization review program should be established to prevent drugs abuse. especially antibiotics abuse.

  • Purpose: The purpose of this study was to identify clinical nurses' knowledge of pharmacology, their need on pharmacology education contents, and perceived connectivity between knowledge and clinical practice. Methods: Subjects consisted of 114 clinical nurses. They responded to self-administered questionnaires about knowledge of pharmacology and need in pharmacology, and perceived connectivity between the knowledge and clinical practice. Results: The mean score of knowledge of pharmacology was $5.2{\pm}1.7$. The mean score of need on pharmacology in clinical practice was $7.9{\pm}1.9$, and those of satisfaction, application, and confidence with clinical performance by applying the knowledge to clinical practice were $7.4{\pm}1.9$, $6.8{\pm}2.0$, and $7.5{\pm}2.1$ respectively. Knowledge of pharmacology was positively correlated with perceived connectivity between knowledge and clinical practice. Scores of need of dose calculations was the highest among 14 units of pharmacology education contents in undergraduate courses while that of development of new drugs was the lowest. Scores of need of coagulation modifier drugs and thrombolytic agents were the highest among 16 units of pharmacology education contents by system specific drug while those of dermatologic and ophthalmic drugs were the lowest. Conclusion: The results indicate that knowledge of pharmacology is important in promoting perceived connectivity with clinical practice by applying knowledge to clinical practice.

  • The most common cause of blindness is cataract. The possible causes, radiation, sugar, drugs, trauma, nutrition, congenital, secondary to eye disease etc. Generally, the opacification of the lens are the result fa oxidation of the lens fibers. The basis for lens transparency is the structural integrity of lens fibers. Opacities were occur if there is a significant amount of high molecular weight protein aggregates. If Ca ions combined with lens ${\alpha}$-crystallin proteins, the lens fibers were aggregated by high molecular weight proteins and the lens were opacified. If Ca ions detached from lens ${\alpha}$-crystallin proteins, the lens fibers were aggregated by low molecular weight proteins and the lens were re-cleared. We need to find out the variety of factors can initiated the process of age-related cataract. And to understanding the mechanism how the various kind, of diabetic cataracts occur.

  • Carboplatin intra-arterial chemotherapy(IAC) has an advantage of increased uptake during the first passage of the drugs through tumor capillaries. Although not common, this type of therapy is known to cause neurological complications, myelosuppression, and ototoxicity. However, the incidence of ocular toxicity is reported to be rare. Eleven of our patients with glioma(Grade II Astrocytoma : 3, Grade III Astrocytoma : 1, Grade IV Astrocytoma : 5, Gliofibroma : 1, Oligodendroglioma : 1) underwent IAC regimen with carboplatin($300mg/m^2$) which were administrated after blood-brain barrier disruption. Of there, 3 patients had ocular complications after supra-ophthalmic IAC injection of carboplatin but fully recovered following steroid therapy. Although our results from IAC seem to be favorable for these patients, we suggest that its complications, such as ocular toxicity, need to be carefully considered prior to treatment.

  • One-year-old male Cocker Spaniel dog was referred for the third eyelid enlargement and inflammation in the left eye (OS). It gradually swelled for 2 weeks after the cherry eye repair by conjunctival mucosa pocket procedure at a private animal clinic. Routine ophthalmic examinations including neuro-ophthalmic examination, Schirmer tear test, intraocular pressure and corneal fluorescein staining were all normal. No lesions were found on slit lamp biomicroscopy and indirect ophthalmoscopy except for third eyelid swelling in the OS. Ultrasonography revealed cystic structure within the OS nictitating membrane. Fluid from the cyst was aspirated and there were no microorganisms or neoplastic changes. Surgical intervention was performed under general anesthesia. On the day of the surgery, there was a deep corneal ulcer in the OS, which had not existed before. Ventral palpebral surface of the third eyelid was incised horizontally to the shaft of the T-shaped hyaline cartilage. And then, a full thickness of the cystic wall was incised and marsupialized. Additionally, a direct suture was performed on the ulcerated cornea. Topical and systemic antibiotics and anti-inflammatory drugs were prescribed. One month after the surgery, the third eyelid swelling and the discharge were improved. Marsupialization of the nictitating membrane cyst relieved the swelling of the third eyelid and inflammation. It could be a simple but effective surgical intervention for the cystic complication of conjunctival mucosa pocket procedure in dogs.

  • Purpose: This study has been compared the degree of corneal edema arising from the material of the lens after wearing the recommended wearing time for the contact lens. Methods: For the study, 43 adults of 77 eyes who did not use drugs and have not any disease participated. Three types of lenses including narafilcon A, HEMA, and silicone hydrogel material regular replaceable lenses were used. Central corneal thickness was measured before and after 8 hours from wearing lenses using Pentacam. The degree of corneal edema was determined by the difference before and after wearing. Results: The average corneal edema of narafilcon A, HEMA material lens, and silicone hydrogel material regular replaceable lens were $2.36{\pm}6.15{\mu}m(Mean{\pm}SD)$, $23.61{\pm}10.71{\mu}m$, $18.25{\pm}8.64{\mu}m$, respectively. The central corneal thickness after wearing narafilcon A was not statistically significant with the central corneal thickness before wearing the lens. Central corneal thickness before and after wearing HEMA material lens and silicone hydrogel material regular replaceable lens were statistically significant. Conclusions: The amount of corneal edema induced by silicone hydrogel material regular replaceable lens was no statistically significant difference to the amount of corneal edema induced by HEMA material lens, but there was significant difference to narafilcon A. Therefore, it is judged to be careful not to exceed the recommended wearing time depending on the composition of the contact lens material.
