  • In this paper, we present a new cぉnet coding method, so called MLC (Multi-Level Codes), for error detection and correction in digital wireless communication. MLC coding method we the same coding procedure wed in the convolutional coding but it is distinguished from the existing convolutional coding in point of generating the code word by using multi-level information data (M-ary signal) and in point of speed of coding procedure Through computer simulation, we analyze the performance of the coding method suggested here compared to convolutional coding method in case of modulo-operation and in case of non-binary coding Procedure respectively under various channel environments.

심낭 삼출액을 동반한 교액성 심장외막염 환자에 적용된 Waffle Procedure -1례 보고- (Waffle Procedure in Chronic Constrictive Epicarditis Patient with Pericardial Effusion . -A Case Report-)

  • 교액성심낭염(constrictive pericarditis) 환자에서 심낭절제술(pericardiectomy)을 시행했음에도 불구하고 호전되지 않을 때 일반적으로 불완전 심낭절제술(Incomplete parietal pericardiectomy), 심근섬유위축(myocardial fiber atrophy), 제한성심근병증(unexpected restrictive cardiomyopathy) 등을 생각해볼 수 있다. 그리고 교액성심 장외막염(constrictive epicarditis)의 가능성도 생각해야한다. 본원에서는 심낭삼출액(pericardial effusion)을 동반한 교액성심낭염 환자에서 심낭절제술을 시행한 후 수술 후에도 인상적으로 임상적, 혈동학적으로 호전이 없는 교액성심장외막염 환자를 접하고 2차적으로 수술(Waffle procedure)을 시행하여 좋은 결과를 얻었기에 보고하고자한다.

후두 미세수술과 연관된 구강 및 후두 합병증 (Oropharyngeal Complications Associated with Laryngomicrosurgery(LMS))

  • Introduction : Laryngomicrosurgery(LMS) is frequent procedure applying to benign and early malignant larygeal disease such as vocal cord polyp, nodule and early glottic cancer. LMS has been known as safe procedure and short time consuming treatment. So few reports about complications in LMS was done. In this study, complications and problems from LMS were investigated and reported. Method : From 2000, January to 2001, December, 180 patients who were treated with LMS in Asan medical center were studied by retrograde chart review. Results : In these patients, total 9 patients(5%) were suffered from complication. 4 patients (2%) had teeth injury and 4(2%) were suffered from foreign body sensation in tongue and 1(1%) had hypoglossal nerve injury. Main mechanism of complications is thought by pressure injury by laryngoscope blade. No definite correlation between procedure and complication was observed. Conclusion : There are few neural complications with LMS such as lingual and hypoglossal nerve injury. Before operation of LMS, warning and informing of complications by mechanical stress must be done. Gentle procedure and short operation time are necessary to avoid these problems. And patients who have risk factors of oral complications such as dental disease or dental prosthesis must have dental evaluation and treatment before LMS procedure.

일부 K-DRG 환례의 의료기관 유형별 수술 및 처치 진료비의 변이 분석 모형 (Statistical Model for Analysing Variations in Inpatient Procedure and Operation Costs of Some Selected K-DRGs by Type of Hospitals)

  • Analysis of practice variations has been one of important issues in trying to contain costs as well as to manage quality in health care. This study was conducted to provide statistical model for analysing variations in inpatient costs by type of hospitals. Four K-DRGs including Cesarean section, appendectomy, cataract extraction, and pediatric pneumonia with CC class 0 were selected, and means and dispersions of inpatient procedure and operation costs were simultaneously compared between type of hospitals. The results indicated that joint modelling of means and dispersions by gamma distribution was a very useful analytic tool for identifying factors which might have relationship with variations in inpatient costs. This model can be expanded to test the significance of several independent variables in analysing cost variations. In surgical conditions, means and unit variations of procedure and operation costs showed consistent pattern which was tertiarty hospital, general hospital, and hospital in descending order. Different findings were identified in pediatric pneumonia, from which mean and unit variation of procedure and operation cost was the highest in general hospital. The practical implication of this difference could not be drawn from this study. It will be done by further sophisticated researches. In order to develop health policy for cost containment and quality management in Korea, it is essential to find out manageable factors affecting variations in practice patterns which include characteristics of population, providers, regions, and so on. The statistical model presented in this study will give health services researchers useful insights for future investigations in analysing cost variations.

Implementation of DYLAM-3 to Core Uncovery Frequency Estimation in Mid-Loop Operation

  • The DYLAM-3 code which overcomes the limitation of event tree/fault tree was applied to LOOP (Loss of Off-site Power) in the mid-loop operation employing HEPs (Human Error Probabilities) supplied by the ASEP (Accident Sequence Evaluation Program) and the SEPLOT (Systematic Evaluation Procedure for Low power/shutdown Operation Task) procedure in this study. Thus the time history of core uncovery frequency during the mid-loop operation was obtained. The sensitivity calculations in the operator's actions to prevent core uncovery under LOOP in the mid-loop operation were carried out. The analysis using the time dependent HEP was performed on the primary feed & bleed which has the most significant effect on core uncovery frequency. As the result, the increment of frequency is shown after 200 minutes duration of simulation conditions. This signifies the possibility of increment in risk after 200 minutes. The primary feed & bleed showed the greatest impact on core uncovery frequency and the recovery of the SCS (Shutdown Cooling System) showed the least impact. Therefore the efforts should be taken on the primary feed & bleed to reduce the core uncovery frequency in the mid-loop operation. And the capability of DYLAM-3 in applying to the time dependent concerns could be demonstrated.

Damus-Kaye-Stansel 술식을 받은 Taussig-Bing 기형의 환자에서 관상동맥 이식을 통한 완전 교정술 - 1예 보고- (Total Repair through Arterial Switch Operation in a Patient with Taussig-Bing Anomaly Undergoing the Modified Damus-Kaye-Stansel Procedure -1 case-)

  • 환아는 대동맥 축착을 동반한 Taussig-Bing기형으로 벽속 주행(intramural course)이 의심되는 관상동맥, 대혈관 크기의 현저한 차이, 호전되지 않는 폐렴과 심부전, 잠재적 대동맥하 협착으로 생후 52일에 대동맥궁 재건을 포함한 고식적 Damus-Kaye-Stansel (DKS) 술식을 시행받았다. 생후 45개월에 시행된 완전 교정술은 좌심실에서 폐동맥(신대동맥)으로의 심실중격결손을 폐쇄하고 DKS 술식해체 후관상동맥 전이를 통한 동맥전환술에서 자가 신폐동맥의 판막 기능을 유지한 채 우심실 유출로를 재건하였다. 완전 교정술 후 19개월째 외래 관찰 중으로 심장약은 복용하지 않으며 심초음파 검사에서 특이 소견은 관찰되지 않고 있다. 저자들은 DKS 술식을 받은 Taussig-Bing 기형 환자에서 인조 판막도관의 삽입이 필요한 Rastelli 술식을 피하고 DKS 술식해체과 동맥전환술로 양대혈관 판막을 모두 보존하는 완전 교정술을 시행하여 좋은 성적을 보였기에 보고하는 바이다.

Long-term Surveillance Comparing Satisfaction between the Early Experience of Nuss Procedure vs. Ravitch Procedure

  • Background: Long-term surveillance comparing satisfaction between the early experience of Nuss procedure vs. Ravitch procedure. Materials and Methods: A total of 100 patients that underwent surgical correction of a pectus excavatum between 2001 and 2004 and were followed for ${\geq}2$ years were included. Surveillance on the degree of satisfaction was performed using five-levels of the Likert scale and self-assessment scoring. Results: Nuss or Ravitch surgery was performed in 63 and 37 patients, respectively. The Nuss procedure required a shorter operation time and shorter hospital stay than the Ravitch procedure (p<0.001). The surveillance demonstrated that 17.6% of the Nuss group and 35.7% of the Ravitch group were not satisfied with the outcome of the surgery (p=0.072). The most common causes of dissatisfaction were redepression in the Nuss group (n=5) and incomplete correction in the Ravitch group (n=7). The multivariate analysis showed that reoperation and a high postoperative pectus index were significant risk factors for a low satisfaction score. Conclusion: The Nuss procedure had several advantages over the Ravitch procedure in the immediate postoperative period. However, the long-term satisfaction was determined by a complete correction without recurrence or need for re-intervention rather than by the operation type.

FASCAM을 이용한 적 진지 지역 화력 억제 분석 절차 연구 (Analysis Procedure for Fire Power Suppression on Enemy Artillery Base Using FASCAM)

  • FASCAM is a newly introduced mean against enemy fire power and supposed to be scattered on base areas in front of a tunnel so as to deter maneuver and shooting of enemy artilleries. However, for its characteristics different from conventional munitions, only a few studies have been undertaken on operations system using this new measure. In this research, we propose a procedure to describe and analyze mathematically an operation system for suppression of enemy fire power using FASCAM. Two suggested simulation methods, impact point generation and density integration, both can describe the actual operation rationally and each has advantages of its own respectively. Further, we demonstrated an example analysis to derive an optimal suppression plan based on the proposed procedure.

  • In this paper, we proposed new outworn aircraft management procedure. ROKAF has both good management skill and information system, AMMIS, regarding aircraft maintenance based on all kinds of aircraft's defects. To optimize and secure aircraft's operation, management of the outworn aircraft is very important for ROKAF. With respect to these outworn aircraft's defects and maintenance, we analyzed defects occurrence pattern of outworn aircraft by using AMMIS data and Market Basket Analysis, and found the specified association rules for each defect. By using these association rules, we developed new management procedure for outworn aircraft based on the results of affinity analysis. The management procedure in this paper will also be used to optimal operation and maintenance of other aircraft and weapon systems.

  • Many production methods are tried to enhance the productivity efficiency. Parallel connected floating cranes are one of the examples to lift mega-blocks quickly and efficiently. However, a general operation manual to operate parallel connected floating cranes and a method to consider risks during lifting operation are not confirmed. And if each floating crane is operated by itself, it is very hard to cooperate. Therefore, Synchronized operation system is installed to control parallel connected floating cranes simultaneously and to be informed of each floating cranes data. And weighting factor is calculated by considering all hazards during the operation and the general operation manual is confirmed based on the factor. This paper introduces the procedure for lifting operations by parallel Connected Floating Cranes using synchronized operation system, and its applications to lifting operation of a heavy cargo such as barge lifting test, floating dock installation and 900 ton goliath crane replacement operation, etc.
