Prevalence of Operation Room-Related Syncope and Presyncope among Medical Students (의학과 학생들의 수술실 임상실습 중 실신 및 실신 전조증상 경험 빈도)

  • Attending the operation room is an essential part of surgical clerkships. Syncope or presyncopal attacks in the operation room may negatively affect students' learning and career development. This study set out to identify the prevalence of syncope and presyncopal attacks in the operation room during medical students' surgical clerkships. Data from 420 medical students (303 men and 117 women) in their 3rd year of clerkship were collected between 2014 and 2017. An anonymous questionnaire was distributed to assess the prevalence and degree of syncope and presyncopal symptoms. A total of 27% of the respondents had experienced syncope or presyncopal symptoms, 49.6% of the female students and 18.8% of the male students (p<0.001). Fifty students (43.5%) had been attending as observers at the time of the syncopal attack, while 65 students (56.5%) had been participating as assistants. Thirty-four students (29.6%) had recently eaten at the time of the syncopal attack, while 81 students (70.4%) had not recently eaten. Prodromal symptoms included the urge to sit down (21.2%), sweating (19.3%), nausea (16.9%), a feeling of warmth (13.3%), darkened vision (12.6%), yawning (11.7%), palpitation (11.0%), ear fullness (10.2%), black spots in one's vision (7.6%), and hyperventilation (7.1%). This study showed the prevalence of syncope and presyncopal symptoms in the operation room during surgical clerkships. For students' safety and effective clerkship learning, thorough proactive education on syncopal attacks is required.

A Study on the Risk Assessment and Reduction of Initial Construction Cost in a Biosafety Laboratory According to Improvement of Supply and Exhaust Method (급배기 방식 개선에 따른 생물안전 밀폐시설의 Risk Assessment와 초기 건설비 저감에 대한 연구)

  • In general, entire supply air of the BSL3 laboratory should be vented to the outside for its biosafety and the air conditioning system should always be operating to maintain a room pressure difference. In this regard, annual energy consumption is approximately five or ten times greater than the magnitude of the office building. In addition, to adjust room pressure difference to the set value efficiently, the supply and exhaust duct system are installed in each room of the BSL3 lab. Thus, initial construction cost is extremely high. In this study, multizone simulation is performed to estimate maintaining the appropriate room pressure difference in the case of changing model A (each room supply and exhaust system) to model B (each zone supply and exhaust system) for verification of the BSL3 lab biosafety. Also, in the case of these two models, the multizone simulation for three kinds of biohazard scenario is performed as part of risk assessment. The analysis of initial construction cost of two models is conducted for comparison. According to the studies, initial construction cost of model B is less than about 22% of existing model A. Moreover, biosafety of the BSL3 lab is still maintaining in the case of the two models.

  • Multipurpose Dam, it produces electric energy by converting the potential energy into kinetic energy utilizing its head and quantity of the water. However, in this process, since during the time when the turbine connected to the hydraulic turbine generator revolves, there occurs a ceaseless loud noise, and due to this condition, it is true state that those people who work at inside of the power plant are damaging as hard as they are unable to concentrate on their work. Not only this, because the hydro-electric power generator room that locates at middle section between the hydraulic turbine room and the office is very large space volume, also since it was constructed chiefly by the reflecting material, it is functioning of amplify the noise when operating the generator, the soundproof measure against this condition is necessitated. On such viewpoint, I have presented the problem point of the relevant Hydraulic turbine dynamo and Hydraulic turbine dynamo room, and after improve such problem point, this study has ever investigated the satisfying degree about the noise-reduction at before and after of the improvement of soundproof measure, using the Auralizational technique that can experience virtual acoustic field. It is considering that such result could be utilized usefully as the fundamental material hereafter for the acoustic performance of the hydro-electric power generator room in dam and when its construction.

A Study on the Reduction effect of Peak Cooling Load on the Sunshade effect of BIPV System (BIPV 시스템의 차양 효과에 따른 피크 냉방부하 절감효과에 관한 연구)

  • As the number of buildings that use the transparent permeation materials as the outer wall is on the increase, the coming amount of the light rays is a lot, and thus the increase in the cooling load and the radiant heat of high temperature may cause the residents to discomfort. In order to reduce such influences, this paper analyzed the installation effects of the sunshade BIPV. The inner temperature of the room installed the sunshade BIPV or otherwise was measured, and compared and analyzed the effects of reducing the cooling load by the incoming light rays. The sample space of the third floor of S university installed the sunshade BIPV has two rooms on the same conditions, and for five sunny days selected in August, the researcher measured the air temperature and the temperature of the fittings with closing the windows to minimize the movement of air without operating the coolers. The maximum cooling load measured by the incoming light rays in the room where the sunshade BIPV was not installed was examined as 459.13kcal/h. It can be understood as the effect of reducing the cooling load according to the incoming rays of the room with sunshade BIPV. Even though the effect of cooling load reduction is not so great in a room, the total reduction in cooling room for the 32 rooms installed the sunshade BIPV was estimated to be 40442.27kcal/day, which will be able to bring the maximum reduction effect of 17.1kW in energy and reduce the investment cost owing to the reduction in cooling load when initially designing the building.

  • The infusion of fluid and blood is necessary in the ward, operating room, recovery room, neonatal room, etc. for nutrition and blood supply to the patient, but air bubbles generated during infusion of fluid and blood circulate along the artery or vein. Serious illnesses occur and there is also a risk of death. In this paper, we propose a medical bubble detection system, a bubble detection system, a bubble detection alarm system, and a communication method in order to develop a safer fluid and blood injection system in the existing system, which is detected by a medical staff monitoring system or an ultrasonic bubble detection sensor In this study, infrared rays are transmitted to a tube through a tube for injecting fluid or blood into a patient, infrared rays transmitted by an infrared ray emitting section are received, and the amount of light is measured in real time. Based on the data, we study how to detect and analyze the presence of bubbles in fluid and blood.

The Effect of Informational Intervention Delivered by Electronic Patient Information Board in The Waiting Room on The Anxiety of Mothers Whose Children Underwent Elective Surgery (수술대기실 환자현황판을 이용한 수술진행 정보제공이 환아 어머니의 불안에 미치는 효과)

  • Purpose: The purpose of this study was to explore the effectiveness of informational intervention delivered by electronic patient information board in the OR waiting room for reduction of anxiety of mothers with child having operation. Method: Nonequivalent control group non-synchronized design was used in this study. The subjects was 52 mothers whose children underwent elective surgery in one Pediatric Hospital. Informational intervention in this study consisted of four critical stages. State anxiety by Spielberger, mean arterial pressure, and heart rates at preoperative and postoperative period were measured. The data were analyzed by using Chi-Square test and t-test with SPSS/PC 10.0 program. Results: 1) There was a significant difference in the state anxiety between the control and the experimental groups. 2) There was no significant difference in the mean arterial pressure between the two groups. 3) There was no significant difference in the heart rate between the two groups. Conclusion: The results of this study indicate that informational intervention delivered by electronic patient information board would be an effective intervention during the operation in reducing anxiety of mothers with child having operation.

A Flow Analysis of a Refrigeration Warehouse where an Unusual Death of an Operator Occurred by Deficiency of Oxygen

  • A numerical flow analysis of the case of a refrigeration warehouse where an unusual death of an operator occurred by deficiency of oxygen is performed by using STAR-CD program of the computational fluid dynamics (CFD) code. The refrigeration room of the warehouse for storing the fruits maintains an atmosphere of 95% nitrogen and 5% oxygen by volume. When the operator was found dead in the refrigeration room, the room was in normal operating conditions except for the fact that the auxiliary door had been left open. For the flow analysis, unsteady 3-dimensional natural convection with mass transfer is considered. The flow analysis result is compared with the oxygen concentration level measured against time during on-site investigation. The change in oxygen concentration level in the warehouse due to the opening of the auxiliary door is found to be negligible.

  • Fire protection is one of important issues to ensure safety and reduce risks of nuclear power plants (NPPs). While robust programs to shut down commercial reactors in any fires have been successfully maintained, the concept and associated regulatory requirements are constantly changing or strengthening by lessons learned from operating experiences and information all over the world. As part of this context, it is necessary not only to establish specific fire hazard assessment methods reflecting the characteristics of research reactors and educational reactors but also to make decisions based on advancement encompassing numerical analyses and experiments. The objectives of this study are to address fire simulation in the control room of an educational reactor and to discuss integrity of digital console in charge of main operation as well as analysis results through comparison. Three electrical fire scenarios were postulated and twenty-four thermal analyses were carried out taking into account two turbulence models, two cable materials and two ventilation conditions. Twelve supplementary thermal analyses and six subsequent structural analyses were also conducted for further examination on the temperature and heat flux of cable and von Mises stress of digital console, respectively. As consequences, effects of each parameter were quantified in detail and future applicability was briefly discussed. On the whole, higher profiles were obtained when Deardorff turbulence model was employed or polyvinyl chloride material and larger ventilation condition were considered. All the maximum values considered in this study met the allowable criteria so that safety action seems available by sustained integrity of the cable linked to digital console within operators' reaction time of 300 s.

  • This paper attempts to verify the possibilities of sharing facilities in adjacent elementary and middle school. To achieve these purposes, the educational theories are reviewed and several schools are surveyed. The outcomes from this study are as follows : (1)It is possible and effective to share some facilities in the adjacent elementary and middle schools. To achieve the successful sharing effects, it is necessary for the schools concerned to collaborate heartfully in managing school facilities and in operating school curriculum. (2)This study recommends some school facilities such as the dinning room and kitchen, multi-purpose auditorium, administrative part and nursing room. (3)It is more effective that the sharing facilities be located in the adjacent zone and two playground and main entrance be detached from the other territorial zone.

Room temperature operating nitrogen dioxide sensor based tellurium thin films (Te를 이용한 상온 동작형 NO2 센서 제작 및 감응 특성)

  • The characteristic of tellurium thin films was studied for detecting nitrogen dioxide gas at room temperature. The film was deposited on $Al_{2}O_{3}$ substrate by using thermal evaporator. The subsequent process was heat treatment by several conditions. (temperature, flowed gases) Surface and grain boundary was investigated using SEM. The results showed that resistance of the tellurium film decreases reversibly in the presence of nitrogen dioxide. The sensitivity of this device depends on the gas concentration and detect lower concentrations less than 10 ppm.
