Chronic Gastric Volvulus with Laparoscopic Gastropexy after Endoscopic Reduction: A Case Report

  • Gastric volvulus is an uncommon clinical entity. There are three types of gastric volvulus; organoaxial, mesenteroaxial and combined type. This condition can lead to a closed-loop obstruction or strangulation. Traditional surgical therapy for gastric volvulus is based on an open approach. Here we report a successful case of a patient with chronic gastric volvulus with a laparoscopic treatment. A 79-year-old woman came to the emergency department with epigastric pain accompanied by nausea for 2 weeks. Abdominal computed tomography revealed markedly distended stomach with transposition of gastroesophageal Junction and gastric antrum. Barium meal study revealed presence of the antrum was folded over 180 degrees that was located above gastroesophageal junction. We attempted an endoscopic reduction, but it was unsuccessful. The patient got laparoscopic anterior gastropexy. Based on our result, laparoscopic gastropexy can be considered as a good choice of the treatment for gastric volvulus.

  • 피드포워드 증폭기는 선형화 성능이 우수하고 선형화 대역폭이 넓은 장점이 있다. 그러나 피드포워드 증폭기는 다양한 소자들로 구성되는 개루프 시스템이기 때문에 주변 환경의 변화에 의해서 성능 저하가 일어나기 쉽다. 따라서 피드포워드 증폭기가 허용하는 범위 내에서 원하는 성능을 유지하기 위해서는 선형화 루프의 이득과 위상을 조정하기 위한 제어 방법이 필요하다. 본 논문에서는 steepest descent 알고리즘을 이용한 새로운 적응형 제어 방법을 제안하였다. 제안한 방법은 기존의 제어 방법에 비해 수렴 속도가 빠르고 구현하기가 쉬운 장점이 있음을 시뮬레이션을 통해 확인하였다.

  • This paper presents the information transmission between on-board and ground-site in MAGLEV. considering safety and high speed operation and density operation, information transmission between them is necessary. Therefore it is necessary for transmission system to ensure high speed transmission, low error rate, massive information, and reliability of information. To provide above conditions, 1.1km signal line assembly was constructed and Frequency Shift Keying(FSK) modulation and Open System Interconnection(OSI) based high-level data link control(HDLC) protocol are applied. To modulate digital signal for transmission from ground-site to on-board, carrier frequency of 70kHz is used and 90khz is used for transmission from on-board to ground-site. Transmission speed is 2400bps for consideration of train speed, quantity of information, and data error rate. And this paper introduces information monitoring considering user interface and presents the method for an effective data transmission in MAGLEV which is now being tested and intends to provide for an intelligent train operation system in future.

  • Usually, the thrust is calculated by magnetic equivalent circuit modelling method for thrust capacity and accuracy progress of position control about a Linear Pulse Motor of which position precision is good and open-loop control is possible within Linear Motors. Analytical thrust deviation exists to calculating magnetic flux density by using Permeance Modelling Method, Finite Element Method, and Velocity Electric Motive Force Method. For calculating accuracy thrust by using these every method, the thrust is calculated and compared by Lorentz Force Method, Magnetic Coenergy Method, and Maxwell correspondence force Method. And that becomes important factor at the comparison of each capacity and parameter of Motor. So this study wants to compare and analyze measurement data and calculating data of the static force of Linear Pulse Motor. and then we can get more accuracy method, calculating the static thrust of Linear Pulse Motor(LPM).

  • In this paper, Neuro-Fuzzy Controller(NFC), a fuzzy system realized using a neural network, is to adopt for the multivariable system. In the multivariable system, the interactive effects between the variables should be taken into account. A simple compensator, using the steady-state information can be obtained for open-loop stable systems, is presented to cope with this problem. However, it should be supposed that the plant is unknown to the control system designer, but an estimate of the DC gain has been obtained by carrying out experiments on the plant. Also, if the variables are not combinated completely, it is difficult to design the controller. Therefore, we design a neuro-fuzzy controller which controls a multivariable system with only input output informations, and compare its performance with that of a PI controller. In the proposed controller, the construction of the membership functions and rule base, which is highly heuristic, can be achieved using a training process. This allows the combination of knowledge of human experts and evidence from input-output data.

Sensorless Sine-Wave Controller IC for PM Brushless Motor Employing Automatic Lead-Angle Compensation

  • This paper presents an advanced sensorless permanent magnet (PM) brushless motor controller integrated circuit (IC) employing an automatic lead-angle compensator. The proposed IC is composed of not only a sensorless sine-wave motor controller but also an isolated gate-driver and current self-sensing circuit. The fabricated IC operates in sensorless mode using a position estimator based on a sliding mode observer and an open-loop start-up. For high efficiency PM brushless motor driving, an automatic lead-angle control algorithm is employed, which improves the efficiency of a PM brushless motor system by tracking the minimum copper loss under various load and speed conditions. The fabricated IC is evaluated experimentally using a commercial 200 W PM brushless motor and power switches. The proposed IC is successfully operated without any additional sensors, and the proposed algorithm maintains the minimum current and maximum system efficiency under $0N{\cdot}m$ to $0.8N{\cdot}m$ load conditions. The proposed IC is a feasible sensorless speed controller for various applications with a wide range of load and speed conditions.

오버헤드셔틀시스템의 동특성해석 및 잔류진동제어 (Dynamics Analysis and Residual Vibration Control of an Overhead Shuttle System)

  • This paper discusses the dynamics and control problem of an overhead shuttle system (OSS), which is a critical part of the automated container terminal at a port. The main purpose of the OSS is efficient automated transport function of containers, which also requires high precision and safety. A major difference between the OSS and the conventional container crane is the configuration of the cables for hoisting the spreader. A mathematical model of the OSS is developed here for the first time, which results in an eight-pole system. Also, open loop control methods (trapezoidal and notch-type velocity profiles) are investigated so that the command input to the overhead shuttle produces the minimum possible sway of the payload. Simulation results show that the vibration suppression capability of the OSS is superior to the conventional overhead container crane, which is partially due to the cable configuration.

소형화된 구형 SIR을 이용한 대역통과필터 설계 및 제작 (A Design and Fabrication of Bandpass Filter using Miniaturized Square SIR)

  • 본 논문에서는 소형화된 구형 SIR(Stepped Impedance Resonators)을 이용하여 IMT-2000에 적용할 수 있는 중심주파수 1.95GHz에서 60MHz의 대역폭을 갖는 4-극 준-타원 함수 대역통과필터를 설계 및 제작하였다. 시뮬레이션은 NUFDTD(Nonuniform Finite Difference Time Domain)을 사용하여 3가지 기본 결합구조에 대한 결합계수를 구하였다. 소형화된 구형 SIR은 기존의 마이크로스트립 구형 개방 루프 공진기 필터 보다 50% 이상의 크기를 감소됨을 알 수 있었다. SIR을 대역통과필터에 적용시 제 3고조파 내에서의 스퓨리어스 응답 특성은 최대 19dB의 우수한 특성을 보였으며, 실험한 결과와 시뮬레이션 결과가 잘 일치하였다.

  • In clinical practice, recurrent Achilles ruptures have been noted to occurr at the original ruptured site. However, reports of new developed fresh rupture of the Achilles tendon in other sites are is extremely rare. Our report is about one uncommon case of a traumatic calcaneal tuberosity avulsion fracture following augmented repair, which was performed using the Krackow locking loop technique. We performed open reduction and intra-osseous fixation using a suture anchor. This procedure was done through the primary longitudinal incision for the calcaneal avulsion fracture fragment. After 6 months of follow-up, our patient has achieved a complete functional recovery and he can normally perform daily and work-related tasks without pain.

  • 본 논문은 STS(Space Time Spreading) 다이버시티 기능을 지원하는 시스템을 FPGA(Field Programmable Gate Array)를 이용하여 구현하고, 이 시스템을 구성하고 있는 각 모듈의 기능과 실제 설계 방법을 소개한다. 본 논문에서 구현한 개루프 전송 다이버시티 시스템인 STS 시스템의 성능을 개선하기 위해서는 페이딩 환경에 따라 변화하는 통신채널의 정확한 검출이 필수적이다. 이를 위하여 파일럿 패널의 정확한 검출을 위한 최적의 망각인자(Forgetting factor)를 제안한다. 본 논문에서 구현한 STS 시스템과 컴퓨터 시뮬레이션을 통하여 CDMA2000 1x 신호환경에서 STS 시스템 적용시 도플러 주파수 80 Hz일 경우에 0.7의 값을 가지는 망각인자를 사용하여 각 구간의 페이딩을 검출함으로써 파일럿 신호의 전력이 충분하지 않을 경우에도 적분 구간을 많이 늘릴 필요 없이 파일럿 채널을 검출할 수 있음을 알 수 있었다.
