Inferomedially impacted zygomatic fracture reduction by reverse vector using an intraoral approach with Kirschner wire

  • Background In inferomedially rotated zygomatic fractures sticking in the maxillary sinus, it is often difficult to achieve complete reduction only by conventional intraoral reduction. We present a new intraoral reduction technique using a Kirschner wire and its clinical outcome. Methods Among 39 inferomedially impacted zygomatic fractures incompletely reduced by a simple intraoral reduction trial with a bone elevator, a Kirschner wire (1.5 mm) was vertically inserted from the zygomatic body to the lateral orbital rim in 17 inferior-dominant rotation fractures and horizontally inserted to the zygomatic arch in nine medial-dominant and 13 bidirectional rotation fractures. A Kirschner wire was held with a wire holder and lifted in the superolateral or anterolateral direction for reduction. Following reduction of the zygomaticomaxillary fracture, internal fixation was performed. Results Fractures were completely reduced using only an intraoral approach with Kirschner wire reduction in 33 cases and through an additional lower lid or transconjunctival incision in six cases. There were no surgical complications except in one patient with undercorrection. Postoperative 6-month computed tomography scans showed complete bone union and excellent bone alignment. Four patients experienced difficulty with upper lip elevation; however, these problems spontaneously resolved after manual tissue lump massage and intralesional steroid (Triamcinolone) injection. Conclusions We completely reduced infraorbital rim fractures, zygomaticomaxillary buttresses, and zygomaticofrontal suture fractures in 84% of patients through an intraoral approach alone. Intraoral Kirschner wire reduction may be a useful option by which to obtain effective and powerful reduction motion of an inferomedially rotated zygomatic body.

  • $Nb_3Sn$ superconducting wires were fabricated in order to investigate the effect of pre-heat treatment for internal tin process. 2 types of Sn reservoir were fabricated. One was arranged one large Sn reservoir in the center of wire, the other arranged several Sn reservoirs inthe wire. Diffusion of Sn is better in the strand divided Sn equally than in the strand had one large Sn reservoir during pre heat-treatment. Critical current was better in the wires divided Sn reservoirs uniformly after whole heat treatment.

  • This paper presents a dynamic modeling and a sliding mode controller for the high-speed/high-accuracy position control system. The selected target system is the wire bonder assembly which is used in the semiconductor assembly process. This system is a reciprocating one around the pivot point that consists of VCM(voice coil motor) as an actuator and transducer horn as a bonding tool. For the modeling elements, the sys-tem is divided into electrical circuit, magnetic circuit and mechanical system. Each system is modeled using the bond graph method and united into the full system. Two major aims are considered in the design of the controller. The first one is that the horn must track the given reference trajectory. The second one is that the controller must be realizable by using the DSP board. Computer simulation and experimental results show that the designed sliding mode controller provides better performance than the PID controller.

  • This paper presents an introduction and economical efficiency analysis of spring tension balancer in trolly wire system. In the overhead trolly wire systems, one of the most things is tension balancer. Because it can be automatically adjusted tension for expansion and contraction of overhead catenary system. In this paper, we introduced spring tension balancer and analyzed economical efficiency of it.

  • In electrification of the Honam line, the grounding method of the copper wire shield of 66kV feeder cable was disputed. This paper describes that the review of the proper earthing method(one-side earthing or both-side earthing) under consideration of the site condition and the acceptability of the induction voltage of the shield on the end of the cable.

  • This paper presents one method to resolve the end-to-end digital connectivity through 2-wire subscriber loop. This system which consists of the subscriber`s device and the line termination device makes use of the advantages of time compressed multiplexing and modified DPSK. Experimental results show that the transmission range of the lab test covers 2km and that of the field test covers 1.5km.

Using a Kirschner wire as an internal splint at nasal fractures accompanied fracture of frontal process of maxilla or perpendicular plate of ethmoid (상악골 전두돌기 골절 및 사골 수직판 골절을 동반한 비골 골절에서 K 강선을 이용한 내고정)

  • Purpose: In cases where nasal fractures involve frontal process of maxilla or perpendicular plate of ethmoid, 4 - 5 days of nasal packing may not provide sufficient support for avoiding displacement after packing removal. Therefore a single Kirschner - wire(K - wire) is used as an internal splint when nasal fractures involve the above two areas. Methods: Thirty five patients during the last 3 practical years were treated with a K -wire pinning according to the anatomic locations of nasal fractures. We performed a retrospective study using 13 nasal fractures out of total 35 patients. Among 13 cases, 10 patients involved frontal process of maxilla, and 3 patients were diagnosed as bilateral nasal side wall fractures accompanied with fractures of perpendicular plate of ethmoid. One patient of the last three cases had been augmented with dorsal silicone implant long before the trauma. We analyzed the anteroposterior displacement of key stone area and the width between both lateral walls by comparing immediate postoperative radiographs with 2 month follow - ups. To reduce the errors, the same measurements were taken by two different inspectors, and the mean of each inspector's measurements was compared. Patient satisfaction was analyzed using a questionnaire regarding the esthetic and functional outcomes. Results: Ten patients underwent a longitudinal K - wire fixation in submucoperiosteal plane underneath the frontal process of maxilla. And three patients underwent a transverse K - wire fixation from the one side of lateral wall to the perpendicular plate of ethmoid and to the other side of lateral wall. The mean postoperative anteroposterior displacement of the key stone area measured by two inspectors were 1.84% and 3.06%; mean narrowing of bony pyramid were 1.33% and 1.48%, respectively. Subjective satisfaction scores regarding the esthetic appearance and the maintenance of nasal shape compared with immediate post - operative state with the long term ones were not different (p>0.05). Conclusion: K - wire pinning after closed reduction is a reliable and useful method for the treatment of nasal fractures involving frontal process of maxilla or perpendicular plate of ethmoid. This is because it achieves longer intranasal support after reduction. This method also leaves conspicuous external scar, and minimal soft - tissue injury.

  • The method of conductor wiring is that a messenger wire is installed, the end of the wire is connected to the conductor and a engine puller pulls the conductor. The length of one section of wiring is $3{\sim}5km$ and one group messenger wire pulls simultaneously $2{\sim}4$ group conductor, while a tensioner maintains wiring tension to prevent the deflection of the conductor. However, there are many obstacles such as roads, power lines, communication lines, buildings, farms and crops. Therefore to prevent damage from conductor deflection a staging is used. The currently used staging is scaffolding lumber which is difficult to secure and it's construction efficiency is very low because it requires a lot of time and manpower. So this study developed a insulating defense tube and pipe connecting device, and a truss structure fabrication module using steel pipe which reduces construction time and cost through a compressive and dielectric strength test.

  • In this study, we developed a process technology to fabricate RFID tag antennas using a one-sheet inlay micro-pattern forming process by press-molding RFID tag antennas on insulation sheet layers, such as polymer films, using ultrasonic longitudinal vibration. In addition, a fine pattern applicable for RFID tag antennas was manufactured using a $25{\mu}m$ thick thin-plate square wire; this is in contrast to the method that uses a conventional round wire. The developed ultrasonic indentation process can be used to fabricate fine pattern of the RFID antenna using one piece of equipment. The simplified manufacturing process technology has a shorter manufacturing time and is more economical. The developed RFID tag antenna forming technique involves pressing the $25{\mu}m$ square wire directly on the thin sheet insulation sheet of maximum thickness $200{\mu}m$, using a 60 kHz ultrasonic tool horn.
