Hypopharyngeal carcinoma is an aggressive malignancy that usually presents at a late stage, thereby resulting in an overall poor prognosis fir these unfortunate patients. The most effective treatment of pyriform sinus carcinoma combines surgery and radiation. Total laryngopharyngectomy is necessary in advanced tumors, but may not be appropriate in limited lesions. Therefore several procedure for partial laryngopharyngectomy have been proposed. Supracricoid hemilaryngopharyngectomy is the one of these procedure. This report presents one case of supracricoid hemilaryngopharyngectomy in a patient with pyriform sinus medial wall cancer and analyses aerodynamic study before and after operation.
Between March 1989 and December 1994, one-stage repair was performed for correction of the intracardiac malformations associated with aortic coarctation in 34 patients or interrupted aortic arch in 8 patients via median sternotomy. There were 26 male and 16 female patients, and their body weight ranged from 1.8 to 8kg[mean weight, 4.0$\pm$l.4kg . The age at the operation ranged from 7 days to 18 months [mean age, 3.1$\pm$3.8 months . The repair of aortic coarctation or interrupted aortic arch was performed using extended end-to-end anastomosis in most of the patients[86%, 36/42 , and six patients underwent ductal tissue excision and patch aortoplasty. Intracardiac defects were corrected concomitantly through the right atrium unless the anatomy dictated otherwise. Obstructive outlet septurn was resected whenever necessary. There were seven early deaths[16.8% , and three late deaths with a mean follow-up period of 25 months [range from 1 to 65 months . Three patients were reoperated upon residual subaortic stenosis,stenosis at the RPA origin, and subacute bacterial endocarditis respectively. None showed any significant residual or anastomotic stenosis postoperatively. One stage repair of the aortic coarctation and interrupted aortic arch associated with intracardiac defect leaves no native coarctation shelf tissue or residual hypoplasia in the repaired segment, has low incidence of recurrent or residual stenosis, minimizes reoperation and incisions, and manages arch hypoplasia easily.We conclude that surgical results of one-stage repair for the intracardiac malformation associated with aortic coarctation or interrupted aortic arch are resonable.
The lack of seed sludges for Ammonium Oxidizing Bacteria (AOB) and slow-growing ANaerobic AMMonium OXidation (ANAMMOX) bacteria is one of the major problem for large-scale application. In this study, $24m^3$ of single-stage SBR (Sequencing Batch Reactor) was operated to remove nitrogen from reject water using AOB and ANAMMOX bacteria cultivated from activated sludge in the field. The ANAMMOX activity was found after 44 days of cultivation in the ANAMMOX cultivation reactor, and then $0.66kg\;N/m^3/d$ of the nitrogen removal rate was achieved at $0.78kg\;N/m^3/d$ of the nitrogen loading rate at 153 days of cultivation. The AOB cultivation reactor showed $0.2kg\;N/m^3/d$ of nitrite production rate at $0.4kg\;N/m^3/d$ of nitrogen loading rate after 36 days of operation. The cultivated ANAMMOX bacteria and AOB was mixed into the single-stage SBR. The feed distribution was applied to remove total nitrogen stably in the single-stage SBR. The nitrogen removal rate in the single-stage SBR was gradually enhanced with an increase of specific activities of both AOB and ANAMMOX bacteria by showing $0.49kg\;N/m^3/d$ of the nitrogen removal rate at $0.56kg\;N/m^3/d$ of the nitrogen loading rate at 54 days of operation.
Squamous cell carcinoma is a neglected disease entity in orthopedic oncology. The purpose of this study was to analyze overall survival and the role of surgery on survival and to evaluate the significance of possible prognostic factors. From Oct, 1986 to Aug, 1996, 57 patients were enlisted and 42 patients ere eligible. Inclusion criteria included more than one year follow-up and no distant metastasis at the first visit. Staging and survival followed AJC classification and Kaplan-Meier plot. Stage II included 17 cases and stage III, 25 cases. Thirty-eight patients underwent operations, chemotherapy, and/or radiotherapy, and the remaining four had operations only. The chemotherapeutic regimen was adriamycin-cisplatin. The average follow-up period was 45 months. The ten-year actuarial survival rate of whole patients was 65.4%. Location of primary lesion, stage, pathologic grading, and intensity of chemotherapy in the same stage showed a significant difference in survival. Nine out of 42 patients had local recurrence. Seven patients had inadequate wide margins and two had intralesional margins. Average period of recurrence from operation was 13(4-35)months. The operation itself had no impact on survival but a surgical margin of no less than 3cm from the lesion was important for local control. Pathological grade and staging were significant variables for long term survival. Acral lesion had a significantly higher chance of regional and distant metastasis but actual survival showed no difference. In stage II, aggressive chemotherapy could delay or reduce the chance of regional or distant metastasis.

Generally, the systems engineering techniques apply to the LRT's power systems are not optimize the whole life cycle cost of the power systems because systems engineering management activities are concentrate in performing the key-technology oriented at the construction stage of the dedicated power systems for light rail transit. Through this study, All the stakeholders can be utilize a this advanced systems engineering techniques which is fully considered the life cycle cost through the considering in whole system life cycle (such as concept, design, operation, maintenance and dispose stage as well as construction stage) and adopted by KSX ISO/IEC 15288 system life cycle processes.
A novel push-pull voltage converter structure, using a switched capacitor type voltage doubler, is proposed. The circuit is constructed with a two-stage push-pull voltage doubler that has a stable operation with small output ripple. The two-stage voltage doubler creates the output voltage 4Vdd. The high clock signal is cross-coupled to the input of the second stage with the opposite phase to reduce two switching transistors and capacitors. Simulation results verify that even with a reduced number of transistor and capacitor, there is no circuit performance loss. Adding one capacitor and two switching transistors the circuit can be changed to eight times of Vdd maker.
A switching power stage is a very nonlinear system because it has two or more operation modes in one switching cycle. To model a switching power stage, the state space averaging method has been developed. Though it allows a unified treatment of a large variety of switching power stages, the model it yields is always very nonlinear. So, it is required to linearize the averaged model. But it is well known that a controller for a nonlinear plant designed by the linearization frequently fails in showing satisfactory control performance. Hence it is very natural to try to design a nonlinear controller for a switching power stage. In design of a switching power system, nonlinear control approaches such as adaptive control and fuzzy control have been widely studied so far. In this research, a recently developed control method, time delay control is briefly studied and a design example for a ZVS PWM half bridge converter is given. The performance of the time delay controller is compared to its conventional counterpart, PI controller by computer simulations.
The lung cancer in patients under 40 years old is rare and reported to be more rapidly fatal than in older persons. We reviewed the records of 18 cases who younger than 40 years with histologically proven lung cancer between 1959 and 1992 at department of Thoracic Surgery in National Medical Center. There were 12 male and 6 female patients. Two male and all female patients had never smoked. The 17 cases had respiratory symptoms for a mean duration of 3 months. The squamous carcinoma was found in 7 cases[38.9%], small cell carcinoma in 4 cases[22.2%], mixed cell carcinoma in 4 cases[22.2%], adenocarcinoma in 2 cases[11.1%] and bronchoalveolar cell carcinoma in 1 case. Among the 6 resected tumors, one case was stage I, two cases were stage II and three cases were stage IIIa. The operation was considered complete and curative in 6 patients and noncurative in 6 pations because of an exploratory thoracotomy. We conclude that lung cancer in young persons is virulent and that diagnosis is frequently delayed.
Ac-dc PWM single-stage converters that integrate the PFC and dc-dc conversion functions in a single switching converter have been proposed to try to minimize the cost and complexity associated with implementing two separate and independent switch-mode converters. In this paper, two simple ac-dc single-stage PWM full-bridge converters are considered - one current-fed, the other voltage-fed. The operation of both converters is explained and their properties are noted. Experimental results obtained from simple lab prototypes of both converters are presented, then compared and discussed.
This paper considers an assembly-type flowshop scheduling problem in which each job is assembled with two types of components. One type of the components is outsourced with positive lead time but the other type is fabricated in-house at the first stage. The two types of the components should be prepared at the first stage before starting the assembly operation for each job at the second stage. The objective is to schedule the jobs so that the makespan is minimized. Some solution properties and lower bounds are derived and incorporated into a branch and bound algorithm. Also, an efficient heuristic is developed. The performances of the proposed branch and bound algorithm and heuristic are evaluated through computational experiments.
