의용소방대용 근무복 소재의 역학적 특성 및 쾌적성에 관한 연구 (A Study on the Mechanical and Comfort Performances of the Working Uniform for a Volunteer Fire Brigade Member)

  • The purpose of this study was to investigate mechanical and comfort performances of the material for the improved working uniform for a volunteer fire brigade member, to get basic data for its evaluation and to help its material and design development. The results were as follows; The material of the newly developed working uniform was thinner and lighter than the material of the current used working uniform. It had better breathability, air permeability and heat transmission rate than the current one. It also had superior anti-flammability In all mechanical properties(tensile, bending, shearing, compression and surface properties), it showed better performances than the current one. It elongated and bended easier and more in both warp and weft directions. The elastic recovery and shape stability after elongation were also higher. The recovery and resistance to shearing and the resiliency and recovery after compression were also better than the current one. It was more flexible, softer and smoother in primary hand value, and was more suitable for the material for winter suit which needs softness and fullness in total hand value. The material of the improved working uniform showed lower thermal insulation value and higher evaporative resistance value compared to the material of the currently used working uniform from the sweating thermal manikin test. It was shown that the physical performances and the hand value of the textile material used in the newly developed working uniform for a volunteer fiber brigade member was improved compared to the one used in the currently used working uniform.

취학 전 아동의 손 기민성과 시각-운동 기술과의 상관성에 대한 연구 (Correlations Between Hand Dexterity and Visual-Motor Skills of the Preschooler)

  • Objective : The purpose of this study was to investigate the correlation between hand dexterity and visual-motor skills of preschoolers. We searched for the correlation between two test results, which were the 9-Hole Peg Test performance time and the visual motor integration test(VMI) performance score. Then, we also compared the 9-Hole Peg Test performance times with the VMI performance scores according to sex and age. Method : The participants were fifty normal children ranging in age from four to six years living in Busan. We used two test methods which have high reliability and validity. One was the 9-Hole Peg Test to evaluate hand dexterity, and the other was the VMI to the evaluate visual-motor skills of these young children. Results : First, over the entire range of the participating children, there was a high correlation between the performance times from the 9-Hole Peg Test and the VMI performance scores(r=-.682). Second, there was not a statistically significant difference between the performance times from the 9-Hole Peg Test and the VMI scores according to sex. Third, there were significant differences between the hand dexterity and visual-motor skills according to the children's age(p<.001). Conclusion : This study proved that there is high correlation between hand dexterity and visual-motor skills of the preschooler. Occupational therapists in preschool service should consider that visual-motor skills need to be compatible with hand dexterity in both evaluation and therapy. In addition, hand dexterity and visual-motor skills improved according to advances in age, so we must give graded tasks to proper age groups through concrete analysis of activity. By doing this, children can get the better therapeutic effects.

수부의 고압 분사 손상 (High-pressure Injection Injuries in the Hand)

  • Purpose: High-pressure injection injury is caused by accidental injection of the high-pressure injection devices in industry. The initial benign appearance of the wound fools patients into delays in an adequate treatment. And it can result in disastrous outcomes such as necrosis and amputation. To avoid the poor prognosis, the injuries require a prompt surgical intervention. The purpose of this article is to recognize the poor outcome of the highpressure injection injury and to introduce an adequate treatment in need. Methods: We have 4 cases of the high-pressure injection injuries in the hand from April, 2005 to March, 2009. Average age is 39 years (30 - 49 years old), 2 cases are the palm of dominant hand, 1 case is the thumb of dominant hand, and 1 case is the palm of non-dominant hand, respectively. We followed up these patients for 20 months on average. In 3 cases, the immediate, aggressive surgical intervention was carried out, but the other one was delayed in early adequate treatment. The wounds were covered by local advancement flap, anterolateral thigh free flap, conservative treatment with antibiotics and dressing. Results: No pathogens after culture were found nor any findings of fracture in imaging study. Conservative treatment, local advancement flap and anterolateral thigh free flap for the open wound resulted in a desirable aesthetic outcome. In a long-term follow up, functional capability of the patient was also satisfactory. Conclusion: Upon initial evaluation, most high-pressure injection injuries present as innocuous wounds with very few symptoms and result in delaying the proper management. And the majority of high-pressure injection injuries will produce significant morbidity to the hand, amputation. And the initial aggressive surgical debridement was needed to prevent the poor outcome. The key to success in treating high-pressure injection injuries of the hand is the prompt aggressive surgical intervention.

혼방 및 연사방법에 따른 아크릴 니트소재의 객관적 감성평가 (Objective Sensibility Evaluation of the Acrylic Knitted Fabrics from Various Blended and Twisted Yarns)

  • We performed the objective sensibility evaluation on knitted fabrics by the following procedures: setting acrylic fabrics with knitted fabrics as basis, knitted five kinds of blended spun yarns and four kinds of twisted filament yams made by different twisting methods(the amount and direction of twist) then, measuring mechanical properties in the use Kawabata Evaluation System, obtaining hand values and total hand values. The results are as follows: First, A(F)/W acrylic/wool spun knits obtain high scores in bending, compressing, shear properties, MMD, and thickness among five kinds of acrylic-blended knit fabrics. A(S)/W acrylic/wool blended knit represented prominent values at compressing properties and thickness and so wool-blended yams demonstrated superior characters comparing other blended yarns. To contrast, acrylic/rayon blended knits showed low scores in bending properties, shear properties and thickness, so that it affects to total hand values. On the one hand, among the four kinds of acrylic filament knitted fabrics, they do not exhibit any notable dynamic differences such as tensile properties of knitted fabrics by the twist number and direction of filament yarns, bending, shear, compressing properties, weight and thickness except surface properties. Second, fabrics showed the most high score at FUKURAMI (fullness and softness) among the hand values. A(S)/W acrylic/wool blended knits obtaining the lowest values at SAHRI (crispness) outrank at total hand values, so that it was the predominant knitted fabric in objective sensibility evaluation. In total hand values, five kinds of acrylic blended knits got a higher score than four kinds of acrylic filament knits, and the amount and direction of twist did not influence on total hand values among the four kinds of acrylic filaments.

다중 도플러 레이다와 머신러닝을 이용한 손동작 인식 (Hand Gesture Classification Using Multiple Doppler Radar and Machine Learning)

  • 본 논문에서는 사람의 손동작을 이용해 전자기기를 제어할 수 있도록 다중 도플러 레이다와 머신러닝의 일종인 SVM (Support Vector Machine)을 이용한 손동작 인식 기술을 제안하였다. 하나의 도플러 레이다는 간단한 손동작만을 인식할 수 있는데 반해, 다중 도플러 레이다는 레이다 위치에 따라 각각 다른 도플러 효과가 발생되므로, 이를 이용하여 다양한 손동작을 인식할 수 있다. 또한, 머신러닝 기법을 이용하여 손동작을 분류하면 손동작 인식의 성공률을 높일 수 있다. 다중 도플러 레이다와 머신러닝을 이용한 손동작 인식 시스템의 구현 가능성을 확인하기 위하여 두 개의 도플러 레이다, NI DAQ USB-6008, MATLAB을 이용한 실험 장치를 구성하였다. 구현된 실험 장치를 이용하여 Push, Pull, Right Slide 및 Left Slide의 4가지 손동작 인식 실험을 수행하였고, SVM 모델을 적용하여 손동작 인식의 높은 정확도를 확인하였다.

수부 및 상지 재건을 위한 전외측 대퇴부 천공지 유리피판의 다양한 이용: 119예의 후향적 분석 (Versatile Applications of Anterolateral Thigh Perforator Flap in the Reconstruction of Upper Extremity Defects: Retrospective Analysis of 119 Cases)

  • Purpose: The perforator flaps have established their role in the reconstruction of various soft tissue defects. For the last five years, we have extensively used anterolateral thigh (ALT) flap for the reconstruction of the complex tissue defects of the hand and upper extremity and report the clinical results and our experiences with the versatile applications of this flap. Materials and Methods: From March 2003 through May 2008, 119 free ALT perforator flaps were transferred for reconstruction of the complex tissue defects of the elbow, forearm, wrist and hand after crushing or degloving injuries as well as severe scar contractures. There were 95 females and 24 males. The mean age of the patients was 37 years and mean size of the flap was 170 $cm^2$. In 20 cases, the flap was vascularized by septocutaneous and in 99 cases by musculocutaneous perforators. Intra-muscular dissection length averaged 3.4 cm. The total length of pedicle averaged 8.4 cm and the average arterial diameter was 0.84 mm. End-to-end arterial anastomosis was performed in 103 cases and end-to-side in 16 cases. Results: Flap survival rate was 98.3%(117/119) and there were 6 cases of partial necrosis. Donor site was closed primarily in 41 cases and skin grafts were applied in 78 cases. Conclusion: The reliability and versatility of ALT flap makes it one of the foremost choices for the reconstruction of complex tissue defects of the upper extremity.

손 자세 인식을 이용한 MPEG-U 기반 향상된 사용자 상호작용 인터페이스 시스템 (MPEG-U based Advanced User Interaction Interface System Using Hand Posture Recognition)

  • 최근 손과 손가락을 인식하는 기술은 HCI(human computer interaction)에서 자연스럽고 친숙한 환경을 제공하기 위한 기술로 주목 받고 있다. 본 논문에서는 깊이 카메라를 이용하여 손과 손가락의 모양을 검출 및 인식하는 방법을 제안하고, 그 인식 결과를 활용하여 다양한 기기와 상호연동 할 수 있는 MPEG-U 기반 향상된 사용자 상호작용 인터페이스 시스템을 제안한다. 제안하는 시스템은 깊이 카메라를 이용하여 손을 검출한 후, 손목의 위치를 찾아 최소 손 영역을 검출한다. 이어서 검출된 최소 손 영역으로부터 손가락 끝점을 검출 한 후, 최소 손 영역의 중심점과 손가락 끝점간의 뼈대를 만든다. 이렇게 만든 뼈대의 길이와 인접 뼈대간의 각도차를 분석하여 손가락을 판별한다. 또한, 제안하는 시스템은 사용자가 MPEG-U에서 정의하는 다양한 심벌들을 손 자세로 취하였을 때 제안 방법을 이용하여 손 자세를 인식하고, 인식 결과를 상호연동 가능한 MPEG-U 스키마 구조로 표현한다. 실험에서는 다양한 환경에서 제안하는 손 자세 인식 방법의 성능을 보인다. 또한, 제안 시스템의 상호연동성을 보이기 위해 인식 결과를 MPEG-U part2 표준에 맞는 XML 문서로 표현하고, MPEG-U 참조 소프트웨어를 이용하여 그 표현 결과에 대한 표준 부합성을 검증한다.

손의 피부색 추출과 무게중심을 이용한 팩맨 게임 (Pacman Game Using Skin Color Extraction and Center of Gravity of Hand)

  • 팩맨(Pacman)은 이전에 오락실에서 선택을 받던 세계적으로 유명한 게임 중의 하나이다. 본 논문에서는 팩맨 게임을 간단한 손동작만으로 수행할 수 있도록 한다. 우선 피부색 추출을 위해 이진 RGB 영상을 구하고, 이 영상에서 휘도를 뺀 이진 YCbCr 영상으로 변환하도록 한다. 다음으로 이진 RGB 영상과 이진 YCbCr영상의 곱 연산을 수행하여 손의 영역을 추출하도록 하였다. 마지막으로, 손 영역의 무게 중심은 최소 경계 사각형과 손의 픽셀 영역 중심을 구하고, 이 두 중심의 평균으로 손 영역의 중심점을 구하는 방법을 사용하였다. 즉, 손의 좌표 중심점을 이용하여 방향키를 대신할 수 있도록 한 모션을 이용한 게임을 제시하였다. 또한 실험을 통하여 평균 움직임과에러율을 나타내었고, 에러의 종류에 따른 원인도 규명하였다.

Case Series Report on the Effect of Hand Acupuncture - Focusing on 29 Patients with Headache -

  • Objectives : The purpose of 29 case series is to report the possibility that a hand acupuncture is effective in relieving headache. Methods : After approval from the Institutional Review Board (IRB), I analyzed 29 cases medical records of outpatients with headache, who visited ${\bigcirc}{\bigcirc}$ oriental medical hospital from December 2008 to December 2010, who have taken a hand acupuncture's treatment without other intervention, and who were diagnosed with one disease of international classification of headache disease second version (ICHD-2). The data was analyzed with Wilcoxon signed rank test to determine whether a hand acupuncture's treatment differed between before and after treatment's Visual Analogue Scale(VAS) according to types of headache and syndrome differentiation. Statistics program was used SPSS 18.0. Differences were considered significant at P<0.05. Results : The VAS of patients with headache was reduced after treatment of hand acupuncture from $6.57{\pm}2.04$ to $2.90{\pm}2.04$ for overall headache, from $6.32{\pm}2.05$ to $2.47{\pm}2.03$ for tension-type headache(P<0.001), from $7.10{\pm}2.18$ to $3.70{\pm}1.77$ for migraine(P<0.001), and from $6.00{\pm}1.41$ to $2.50{\pm}3.54$ for headache unspecified. A hand acupuncture produced a decrease in VAS of both ascendant hyperactivity of liver yang(P<0.001) and phlegm turbidity according to syndrome differentiation(P=0.002). No adverse event were encountered in any of the patients. Conclusions : It is suggested that a hand acupuncture may be effective in relieving headache, and could be applicable to first choice of acupuncture treatment for headache.

Clinical outcome in patients with hand lesions associated with complex regional pain syndrome after arthroscopic rotator cuff repair

  • Background: Complex regional pain syndrome (CRPS)-related hand lesions are one of the complications following arthroscopic rotator cuff repair (ARCR). This study aimed to investigate the clinical outcomes of patients with CRPS-related hand lesions following ARCR. Methods: Altogether, 103 patients with ARCR were included in this study (mean age, 63.6±8.2 years; 66 males and 37 females; follow-up period, preoperative to 12 months postoperative). Clinical assessment included the Japanese Orthopaedic Association (JOA) score, University of California, Los Angeles (UCLA) score, Constant score, 36-item short form health survey (SF-36) score, and Quick Disabilities of the Arm, Shoulder, and Hand (QuickDASH) score from preoperative to 12 months postoperatively. The patients were either assigned to the CRPS group or non-CRPS group depending on CRPS diagnosis until the final follow-up, and clinical outcomes were then compared between the groups. Results: Of 103 patients, 20 (19.4%) had CRPS-related hand lesions that developed entirely within 2 months postoperatively. Both groups showed significant improvement in JOA, UCLA, and Constant scores preoperatively to 12 months postoperatively (p<001). Comparisons between the two groups were not significantly different, except for SF-36 "general health perception" (p<0.05) at 12 months postoperatively. At final follow-up, three patients had residual CRPS-related hand lesions with limited range of motion and finger edema. Conclusions: CRPS-related hand lesions developed in 19.4% of patients following ARCR. Shoulder or upper-limb function improved in most cases at 12 months, with satisfactory SF-36 patient-based evaluation results. Patients with residual CRPS-related hand lesions at the last follow-up require long-term follow-up.
