위암 수술후 화학요법 환자의 삶의 질과 영향요인 (A Study on Relationships between the Quality of Life and conditioned Variables in the Gastric Cancer Patients Receiving Chemotherapy after Surgery)

  • Purpose : This study was conducted to determine the relationships between conditioned variables and the quality of life in the gastric cancer patients receiving chemotherapy after surgery. Method : 56 gastric cancer patients receiving chemotherapy included Cisplatin and 5-Fu were recruited from one university hospital located in Seoul. Questionnaires for this study were Quality of life Index developed by Padilla et al. at 1983, Rhode's nausea and vomiting scale and tool for side effects after chemotherapy at home developed by authors. Result : Quality of life in this study was correlated significantly with age, stage of cancer, nausea and vomiting, and level of side effects of chemotherapy. Quality of life of woman was lower than that of man. Conclusion : It is suggested that nurses be reinforce nurse's discharge education for the patients about increasing oral intake and preventing side effect after discharge from hospital and pay more attention to the change of the quality of life and relevant variables among the gastric cancer patients.

항암화학요법제 투여 중인 암환자의 대체요법 이용실태 및 의사결정요인 (A study on the use of Alternative Therapies for Cancer Patients Undergoing Chemotherapy)

  • Purpose: This study was conducted to examine utilization of alternative therapies and understand decisional criteria related to use of alternative therapies among chemotherapy patients. Method: Data were collected from 165 cancer patients through the use of questionnaire and interviews at the chemotherapy treatment rooms of two general hospitals located in Seoul. Result: Sixty point six percent of subjects after cancer diagnosis have experience of using alternative therapies and 39.4% were still using alternative therapies. On the average the patients used alternative therapies for 9.4 months and they paid 483,000 Won per month for alternative therapies. Decisional criteria related to use of alternative therapy were curability, social group influence about alternative therapy, confidence in alternative therapy, present symptoms, and who decide on the use of alternative therapy. These variables in the alternative therapy group were statistically different from those of the hospital treatment only group. Conclusion: Utilization rate of alternative therapy among chemotherapy patients is high and cost burden is heavy. Thus, they need to be educated about utilization of alternative therapy by nurse and medical personnel.

암병동간호사와 일반병동간호사의 건강상태와 직무스트레스 비교 연구 (A Comparative Study regarding Health Condition and Work Stress of Nurses Working in Cancer Ward and General Ward)

  • The health of a nurse is very important because her mental and physical health can influence toward nursing patients directly, Especially, Cancer patients are growing annually. Also, terminal cancer patients' nursing and dead place are increasingly using hospital and the period of nursing for cancer patients are increasing. Nursing for cancer patients are different with acute disease. Therefore, I analysed comparing nurses' health working in cancer and general ward so that nurses working at cancer ward could be developed as a professional nurse, and I wanted to establish the foundation of nursing administration and reasonable manpower management to supply good quality of nursing to patients. In my research, I selected 117 nurses working in cancer ward and 134 nurses working in general word to analyse the stress rate and nurse's health comparing nurses working in general ward and cancer ward. The survey was conducted of nurses working in cancer ward nurses in 2 university hospitals, nurses working in a cancer hospital, and general ward nurses working in 3 public hospital. Also, the data was collected from Sep. 13, 2001 to Sep. 28, 2001. As health measuring tool, I used Cornell Medical Index(CMI) which are developed to fit Koreans by Ko Ungrin and Park Hang-bas (1980) using Cornell Medical Services which were designed by Weiser, Brosman, Mittelman, Wechler, Wolff in Cornell University(1945). As working stress measuring tool, I used Questionaries which were designed by Kim Mae-ja and Ku Mi-ok(1984) and then developed by Bae In-sook(1996). For managing the data, I used frequency, percentage, ${\chi}^2$ verification, t-test, and F-test (ANOVA). And in the case of significant data(p<.05). I did Duncan's test for post verification. The mutual relation between health condition and working stress rate have been conducted using Pearson's Correlation Coefficient. Followings are the results of my research. 1. Two groups showed significant differency at age after testing homogeneous character between two groups (${\chi}^2$ =9.919, p=.007). 2. Comparing two group's health condition, cancer ward(average 19.35${\pm}$18.34) were higher than general ward(14.42${\pm}$10.59) and showed statistical significant differency(p=.009). And, comparing two group's mental condition, cancer ward(9.00${\pm}$9.79) were higher than general ward(7.13${\pm}$6.35) and statistically no differency. 3. After comparing two group's working stress rate, the rate of cancer ward nurse's working stress(3.36${\pm}$.50) is higher than general ward nurse (3.32${\pm}$.48). There are no significant differency. However, in the detailed verification test, there were significant differency at inappropriate compensation (t=3.254, p=.001) and medication issue (t=2.170, p=.031). 4. After comparing health condition at general points, physical health condition showed significant differency at age(p=.020), the number of children (p=.015), religion (p=.015), position(p=.005), career(p=.008), working satisfaction(p=.003), activity after office hour(p=.045); and mental health condition showed significant differency at position(p=.010), career (p=.017), working satisfaction (p=.003). 5. After comparing the working stress rate according to general points, there were significant differency at working satisfaction (F=5.285, p=.006), predicted nursing(F=3.822, p=.023). 6. At the relation of health condition and working stress rate between two groups. physical and mental condition showed significant relation with working stress rate. i.e, if a nurse's health condition is not good, she are feeling much more stress than others. After considering all the factors in my research, I found that the health condition and stress rate of cancer ward nurses is much higher than general ward nurses. Considering that cancer ward nurses is necessary to care for increasing cancer patients with mental and physical nursing, the less stress for cancer ward nurses is very important to develop nursing quality and working efficiency by keeping good health condition, specializing cancer ward nurses. Therefore, we need following studies to find the factors which are effecting to cancer ward nurses' health and specialization. Also, we need to improve managing working condition to decrease working stress by improving working condition.

고위험 유전성 유방암을 지닌 한국계 미국 여성의 질병경험 (Experiences of Korean-American Women with High Risk Hereditary Breast Cancer)

  • Purpose: This micro-ethnographic study aimed to understand coping experiences of Korean-American (K-A) women after diagnosis with breast cancer due to a hereditary gene mutation. Methods: Participatory observation and in-depth interviews were performed at one breast cancer screening center in Southern California, in 2005 with eleven first generation K-A immigrant women. All transcribed interviews and field notes were analyzed using ethnographic methodology. Results: K-A women's experience varied based on acculturation risk factors including: limited English speaking ability; disrupted family relationships, individualistic family values, or intergenerational communication barriers; lack of Korean speaking nurses; and Korean physicians' who lacked knowledge about hereditary breast cancer risk. These risk factors led to isolation, loneliness, lack of emotional and social support. In comparison to Korean homeland women in a similar medical situation, these K-A immigrants felt disconnected from the healthcare system, family support and social resources which increased their struggling and impeded coping during their survivorship journey. These women were not able to access self-support groups, nor the valuable resources of nurse navigator programs. Conclusion: Professional oncology associations for nurses and physicians have a moral obligation to support and promote knowledge of hereditary cancer risk and self-help groups for non-native speaking immigrants.

전문간호사 양성을 위한 간호대학원 교과과정 및 운영방안 개발 (A Study for Curriculum Development for Advanced Nurse Practitioner Program)

  • The traditional nursing roles have become increasingly blurred. Nurses are now working in different ways and at higher levels of practice that enable nurses to adapt their roles and take on new responsibilities. The advanced role of nurses requires a different kind of master-level prepared education. Method & Result: This article describes an curriculum development process in preparing registered nurses for their advanced nurese' roles in the area of acute adult health nursing, geriatric nursing, pediatric nursing, neonatal intensive care nursing and oncology nursing. Several important issues to be solved regarding introduction of APN were also discussed. Conclusion: The curriculum that was proposed in the study will equip nurses to meet the challenges of future healthcare provision and will be a model to other areas of nursing practice and curriculum development.

Nurse Attitude-Related Barriers to Effective Control of Cancer Pain among Iranian Nurses

  • Background: Many cancer patients still experience pain worldwide. There are many barriers for effective control of cancer pain and many of these are related to health care providers. There is a need for further investigation of these barriers. The aim of this study was to investigate nurse-related barriers to control of cancer pain among Iranian nurses. Materials and Methods: In this descriptive study 49 nurses from two hospitals affiliated to Tabriz and Ardebil Universities of Medical Sciences participated using a census sampling method. A demographic and profession related checklist and Barriers Questionnaire II (BQ-II) were used for data collection. Results: The results showed negative attitudes of participants regarding control of cancer pain. Participants believed that cancer pain medications do not manage cancer pain at acceptable levels; patients may become addicted by using these drugs; cancer pain medications have many uncontrollable effects; and controlling cancer pain may distract the physicians from treating disease. Conclusions: Iranian nurses have negative attitudes toward pain control in cancer patients especially about effectiveness of pain medication and their side effects. Educational intervention to reduce these misconceptions is needed.

데이터 마이닝을 이용한 입원 암 환자 간호 중증도 예측모델 구축 (An Analysis of Nursing Needs for Hospitalized Cancer Patients;Using Data Mining Techniques)

  • Back ground: Nurses now occupy one third of all hospital human resources. Therefore, efficient management of nursing manpower is getting more important. While it is very clear that nursing workload requirement analysis and patient severity classification should be done first for the efficient allocation of nursing workforce, these processes have been conducted manually with ad hoc rule. Purposes: This study was tried to make a predict model for patient classification according to nursing need. We tried to find the easier and faster method to classify nursing patients that can help efficient management of nursing manpower. Methods: The nursing patient classifications data of the hospitalized cancer patients in one of the biggest cancer center in Korea during 2003.1.1-2003.12.31 were assessed by trained nurses. This study developed a prediction model and analyzing nursing needs by data mining techniques. Patients were classified by three different data mining techniques, (Logistic regression, Decision tree and Neural network) and the results were assessed. Results: The data set was created using 165,073 records of 2,228 patients classification database. Main explaining variables were as follows in 3 different data mining techniques. 1) Logistic regression : age, month and section. 2) Decision tree : section, month, age and tumor. 3) Neural network : section, diagnosis, age, sex, metastasis, hospital days and month. Among these three techniques, neural network showed the best prediction power in ROC curve verification. As the result of the patient classification prediction model developed by neural network based on nurse needs, the prediction accuracy was 84.06%. Conclusion: The patient classification prediction model was developed and tested in this study using real patients data. The result can be employed for more accurate calculation of required nursing staff and effective use of labor force.

중년남성의 배우자 사별경험 (Experiences of Spousal Bereavement in Middle Aged Men)

  • Death of spouse is the most heartbreaking stressful and inevitable tragic life event. In middle aged men who belong to the social middle class and accomplished their occupational success, experiences of spousal bereavement are great shock. The aim of this study was to find out how they overcome their mental and physical pain and to obtain the basic materials to develop suitable nursing care programs for them. The methodological approach of this study is Giorgi's phenomenological analysis meaning unit. This method also makes theme focal meaning, situated structural description and create general structural description grasped by participator's experience through situated structure description. This study performed from November 2002 to May 2004, and participators were four men. Data collected through in-depth personal interviews. which had been tapped and analysed the Giorgi's method. Finally, the five focal meaning below have been abstracted. Theme 1. Physical symptom loss of appetite, fatigue, insomnia. outbreak of illness, weight loss. Theme 2. life of spiritless lack of desire, sense of emptiness, unstableness, prosaic life. wandering. indifference of appearances, avoidance of meeting people. Theme 3. life of retrospction reflection for his wife, yearning, grief, muttering to himself, never-to-be forgotten wife, leading a lonely life. Theme 4. negative emotion reproaching, feeling hurt, marriage of daughter, feeling heavy, getting angry, sexual desire, awareness of his sinfulness. loneliness Theme 5. social support and adjustment getting his wife off his mind, curring favor with children, support and consolation by his daughter-in-law, appreciation for hospice nurse, considering remarriage, taking care of himself, good relation with his children. The result of this study showed that middle aged men bereaved of their wife by cancer need other's concern. And we have to study further to understand their experience. Until now hospice nursing is concentrated on cancer patients But from now we have to provide their families suitable nursing care programs to adjust themselves to social life before and after death of patients.

웃음요법이 방사선요법을 받는 유방암 여성의 우울, 불안, 스트레스에 미치는 효과 (The Effect of Laughter Therapy on Depression, Anxiety, and Stress in Patients with Breast Cancer Undergoing Radiotherapy)

  • Purpose: The purpose of this study was to investigate the of laughter therapy on depression, anxiety, and stress among patients underwent radiotherapy. Methods: Participants in the study were comprised of 60 breast cancer outpatients who received radiotherapy. Thirty-one of the patients were assigned in the experimental group and the other 29 patients made up the control group. Laughter therapy was consisted of the delivery of information and active motion resulting in laughter. We provided laughter therapy 4 times for 2 weeks. Each session lasted 60 min. The experimental group received laughter therapy intervention as a group. An average attendance frequency per person was 2.7. To evaluate the effect of laughter therapy, the degrees of depression and anxiety were measured by Hospital Anxiety and Depression (HAD) and stress was measured by the Brief Encounter Psychosocial Instrument (BEPSI). Data analysis included a 2-test, t-test, and Fisher's exact test using SPSS 12.0 and SAS 8.0. Results: There was significant decrease in the degrees of depression, anxiety, and stress in the experimental group compared to those in the control group (p<.01, p=.04, and p<.01, respectively). Conclusion: This study provides evidence that laughter therapy is an effective intervention in improving depression, anxiety, and stress in breast cancer patients.

항암화학요법 후 퇴원한 암 환자의 증상관리를 위한 전화상담 가이드라인 개발 (Developing Telephone Guidelines for Symptom Management of Cancer for Outpatients with Cancer after Chemotherapy)

  • Purpose: This study was done to provide oncology nurses systemic guidelines designed to perform telephone counseling in outpatient settings treating patients with chemotherapy. Methods: Symptoms which are frequently questioned were identified through case reports recorded by 4 clinical nurse specialists from January 4 to December 31. 2010. Fifteen clinical experts and two hemato-oncologists reviewed the developed draft. Their opinions were synthesized to arrive at consensus on the ideal guidelines. This process confirmed content validity. The two hemato-oncologists also verified the face validity. Results: The six symptoms which were most frequently asked about were identified through 4,644 case reports and concerned pain, fever, skin alteration, nausea/vomiting, stomatitis and diarrhea. Evidence-based telephone triage guidelines for the 6 major symptoms in patients receiving chemotherapy were developed. Conclusion: These guidelines will help oncology nurses from novice to expert incorporate evidence-based telenursing into their practice.
