장루보유자의 불편함 (Distress of the Patients with Ostomy)

  • Purpose: The purpose of this study was to explore and describe the distress of the patients with ostomy. Methods: A descriptive research design was adopted for data collection and analysis. Six patients with ostomy participated in the study. The consent from the participants was obtained for ethical protection. Data were collected from July 31, 2009 to January 10, 2010 using in-depth interview technique. Krippendorff's content analysis method was utilized for data analysis. Results: Eight categories and 26 themes were extracted from the data which illustrated the lived distress of the patients with ostomy. The categories were "do not have any freedom to eat whatever I wish to eat", "uncontrolled defecation"tomy", "can not live with comfort", "easily take a pessimistic view", and "see how the wind blows in daily family life". Conclusion: Ostomates were affected by the distress of having an ostomy in their physical, psychological, social and spiritual life. Individualized and continued nursing education program has to be developed in hospital and community settings in order to improve the quality of life of the ostomates.

간호사들의 항암제 취급실태와 안전수칙에 대한 인지도와 실천도 (Cognition and Practice of Safe Handling of Cytotoxic Drugs in University Hospital Nurses)

  • Purpose : To provide basic data for establishing the policy of safe handling of cytotoxic drugs and for building training materials and clinical guidelines for nurses handling cytotoxic drugs. Methods: 255 nurses in Seoul and 257 nurses in D metropolitan area participated in the study. Questionnaires were based on the safe handling guideline for cytotoxic drugs issued by OSHA and ONA. Results: Nurses in Seoul scored higher in the cognition for safe handling guidelines of cytotoxic drugs in preparation, administration. as well as total score compared with nurses in D metropolitan area. The practical score also showed higher in nurses in Seoul in administration, disposal and total score. But both groups showed low level of practical score. The answers why they could not follow the guideline for cytotoxic drugs were "don't have", "busy", and "don't know", etc Conclusion: Handling of the cytotoxic drug is very common and important. The study findings suggest that hospitals especially in D metropolitan area should educate the nurses and supply equipments for safe handling of cytotoxic drugs. Thus, repetitive and continual education of all nurses including experts is needed. In addition, it is necessary to supply enough protective equipment for preventing exposure to the cytotoxic drugs.

암환자의 증상 심각성과 증상관련 지장이 수면장애에 미치는 영향 (Effects of Symptom Severity and Symptom Interference on Sleep Disturbance in Cancer Patients)

  • Purpose: The purpose of this study was conducted to identify the impact of Symptom Severity and Symptom Interference on Sleep Disturbance among cancer patients. Methods: This study was conducted from October 8th to October 25th, 2012. One hundred eight cancer patients were recruited from S city in Korea. The instruments used in this study were the Symptom Severity, Symptom Interference and the Sleep Disturbance scales for patients with cancer. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple regression with the SPSS/WIN 20.0 program. Results: The Sleep Disturbance for cancer patients showed a significant relationship with Symptom Interference and cancer stage. The significant factors influencing Sleep Disturbance were Symptom Severity and Symptom Interference. They explained 40.5% of the variance in stage IV. Conclusion: Patients with cancer experienced Symptom Severity and Symptom Interference which led to a negative effect on Sleep Disturbance. The results suggest that intervention programs to reduce Symptom Severity, Symptom Interference could improve Sleep Disturbance for cancer patients.

유방암 수술 환자의 감각 변화에 대한 연구 (Patients' Experiences of Sensations After Breast Cancer Surgery in Korean Women)

  • Purpose: The purpose of this study was to obtain information about the experiences of sensations after breast cancer surgery characterized by prevalence, frequency, and severity, distresses, and disturbances in ADL using the BSAS (Breast Sensation Assessment Scale) and to develop the standardized Korean BSAS. Methods: Thirty two women from 3 to 100 days after breast cancer surgery (BCS) completed Roberta's BSAS. The 18 sensations of BSAS were translated using the references of Lee's Korean Pain Rating Scale and English-English, English-Korean dictionaries and the consultation from two native Americans, one bilingual permanent resident of the States, and one nursing professor. The Korean-translated BSAS has high reliability in test-retest. Likert type 4-point scale and 100 mm VAS were used for assessment. Results: Certain sensations remained prevalent (tender, pull, pain), frequent (numb, nag, throb), severe (throb, shoot, numb), causing distress (throb, penetrate, shoot), and influencing on ADL (throb, penetrate, nag). The most frequently experienced sensation other than BSAS was itching. There was little difference in the prevalence of symptom experiences between sentinel lymph node biopsy and axillary lymph node dissection. Conclusion: The women after BCS are suffering from neuropathic sensations. The Korean- translated BSAS could be used in effectively assessing breast sensations after BCS in Korean women.

동태적 직무분석을 이용한 암 환자 케어 코디네이터의 직무 분석 (Dynamic Job Analysis of the Cancer Care Coordinator in a General Hospital)

  • Purpose: The purpose of this study was to explore roles of cancer care coordinator's by developing job description, job specification and job process map using dynamic job analysis. Method: The development process consisted of three stages of vertical job analysis and three stages of horizontal job analysis by modifying Song(1997)'s dynamic job analysis. Focus group interview was used to validate the content of the job analysis. Results: Cancer care coordinator's job description was categorized into six major categories, fourteen intermediate categories and one hundred forty specific jobs. Major categories are professional nursing practice, consultation and counsel, coordination and collaboration, education, research and leadership. Cancer care coordinator's job specification included master's degree with over five years of clinical experience preferably relevant clinical area, professional knowledge on pathophysiology of cancer, case management and cost control, competency for communication and counselling skills and clinical decision making. Cancer care coordinator's job process map was framed with time(horizontal) and activities(vertical). Conclusion: The Outcomes of this study will guide to develop possible areas of oncology advanced practice nurses in hospital setting and facilitate the use of oncology nurse practitioners by developing care coordinator roles in cancer care.

암환자가족이 지각하는 수술대기중 불안과 불확실성 및 의료인의 지지 (Uncertainty, Medical Staff's Support, and Anxiety Perceived by Family Members with Patients Undergoing Surgery for Cancer)

  • Purpose: This study was to assess relationships among the uncertainty, medical staff's support, and anxiety perceived by family members with cancer patients while the family members were waiting for their patients undergoing surgery. Method: The data were collected from the family members of cancer patients who were undergoing surgery in D University Hospital at B city from February 1 to April 12, 2005. The used instruments were the State Anxiety Scale of Spielberger's(1975) STAI, Mishel's Uncertainty in Illness Scale (MUIS)(1981), and Relationship Questionnaires (Lee, 1978). The collected data was analyzed by using t-test, ANOVA, Pearson's coefficients, and stepwise multiple regression. Results: As the result, the most influential variable explaining anxiety of family members was uncertainty $({\ss}=0.37)$, followed by perceived illness state $({\ss}=-0.27)$. These two variables simultaneously explained 29.3% of the variance in anxiety. Conclusion: We suggest to develop a nursing intervention program to reduce the uncertainty through the medical staff's support and o test its effects.

  • Purpose: The purpose of this study was to describe the experience of hospice nurses on spiritual care. Methods: Data was collected from 9 hospice nurses by using in-depth interview. The main questions include what they understand as spiritual care, when they feel the needs of spiritual care, how they perform spiritual care, and what is the outcome of spiritual care. The data was analyzed by grounded theory methodology developed by Strauss and Corbin. Results: The core category of experience of hospice nurses on spiritual care was identified as "Untie a knot of mind". In the process of spiritual care in hospice nurses was consisted of soothing, dwelling with, releasing, giving meaning, plunging, and going beyond a life. Conclusion: The result of this study was expected to give useful information to nurses and nursing managers about the real situation of performance of spiritual care. The findings of this study contributes to developing programs and supportive policies for encouraging spiritual care.

유방암의 위험요인 (Risk Factors of Breast Cancer)

  • Purpose: The purpose of this study was secondary analysis to explore about risk factors with breast cancer on a basis of primary literature. Methods: This study was searched articles by using CINAHL, MEDLINE, Riss4u, Internet website regarding breast cancer. This study searched for the journal published in Korea and foreign countries from 2000 to 2008, about risk factors of breast cancer. This study was reviewed 42 articles (5 experimental study, 35 survey, 1 qualitative study, 1 report) suitable for the research objectives. Results: Magnitude of risk breast cancer (++) was age, geographic region, family history, mutations in BRCA1, BRCA2 genes and in other penetrance genes, radiation, history of benign breast disease, late age of menopause, early age of menarch, nulliparity and older age at first birth, high mammographic breast density, high insulin-like growth factor 1 level. Magnitude of risk factor (+) was hormone replacement therapy, oral contraceptives use, obesity, tall stature, alcohol consumption, high prolactin level, high saturated fat and well-done meat intake, polymorphisms in low penetrance gene, high socioeconomic status. Conclusion: A breast cancer screening protocol according to magnitude of risk factors is needed for disease prevention. The nurses need to educate and counsel women with risk factors of breast cancer.

대장암 환자의 증상경험과 극복력이 삶의 질에 미치는 효과 (The Effect of Symptom Experience and Resilience on Quality of Life in Patients with Colorectal Cancers)

  • Purpose: This study aimed to investigate the influence of resilience and symptom experience on quality of life. Methods: Seventy five patients in an active treatment stage were recruited from 2 hospitals between October and December 2010. A survey including perceived health status, physical functional status, the symptom experience index, resilience, and the functional assessment of cancer therapy-colorectal (FACT-C) was completed. Results: The level of FACT-C was 86.61 (${\pm}18.91$), which was higher than for patients with colostomy, but lower than for patients in a rehabilitation stage. Participants without physical functional limitations showed higher level of both resilience and quality of life. Participants with bad health status showed a lower level of both resilience and quality of life and higher level of symptom experience. The quality of life was related to the level of resilience (${\beta}$=.373), symptom experience (${\beta}$=-.292), and perceived health status (${\beta}$=-3.961). Conclusion: Proactive nursing approaches to manage symptoms and to improve perceived health status would enhance the quality of life of colorectal cancer patients. Furthermore, various strategies to strengthen the level of resilience should be developed.

암환자의 피로와 기능상태의 관계 (Relationship between Fatigue and Functional Status in Patients with Cancer)

  • Purpose: The purpose of this study was to identify the relationship of fatigue and functional status in patients with cancer. Method: The data were collected from August, 15 to October, 10, 2005. The subjects were recruited from three university hospitals and two general hospitals in B, U, and S cities in Korea. Fatigue was measured using the Fatigue Scale for Cancer Patients, and Functional Status was measured using the Karnofsky Performance Status Index. Result: 1) The mean score of fatigue was $79.72{\pm}21.25$(range: 26-130), and the mean score of functional status was $79.33{\pm}13.02$. 2) There were significant differences in the scores of fatigue by sex, age, marital status, employment, major care-giver, pain, and period after diagnosis. 3) There were significant differences in the scores of functional status by employment, major care-giver, pain, stage of cancer, period after diagnosis, and type of cancer. 4) There was a significant negative correlation between fatigue and functional status (r= -.472, p= .000). Conclusion: Increase in fatigue was associated with decrease in functional status in patients with cancer. Nurses must assess fatigue and functional status when caring for the cancer patients, and provide nursing intervention to relieve fatigue and to improve functional status.
