국소 병변의 다제 내성 폐결핵 환자에서 폐절제술의 역할 (The Role of Resectional Surgery for the Treatment of Localized Multi-drug Resistant Pulmonary Tuberculosis)

  • 배경 : 다제 내성 폐결핵은 적극적인 내과적 치료에도 불구하고 높은 치료 실패율을 보이므로 최근 보조적 치료 수단으로 폐절제술이 다시 등장하여 시행되고 있다. 그러나, 폐절제술에 대한 적응증이 아직 확립되지 않은 상태이므로 본원에서 폐절제술을 시행 받은 다제 내성 폐결핵 환자들의 치료 효과를 비교 분석하여 적응증의 정립에 지표를 제시하고자 본 연구를 시행하였다. 방법 : 1996년 5월부터 2000년 2월까지 INH와 RFP에 동시 내성을 보이는 다제 내성 폐결핵 환자 중 폐결핵 자체에 대한 보조적 치료로써 폐절제술을 시행한 환자 13명을 대상으로 하여 진료 기록을 중심으로 후향적 으로 조사하였다. 결과 : 남자 5명, 여자 8명으로 연령은 $37.5{\pm}12.4$세 (24~63세)였다. $2.4{\pm}0.8$ 회의 화학치료 과거력을 가지고 있었으며, 수술 전에 내성 약제수는 $5.2{\pm}1.9$제, 감수성 약제수는 $5.4{\pm}1.5$제였다. 다제 내성 결핵을 진단 받고 수술까지의 기간은 $109.7{\pm}132.0$개월이 본원에서 재치료 받은 후 $10.5{\pm}8.8$ 개월이었다. 대부분의 환자 (92.3%)에서 주병변에 공동을 동반하고 있었으며, 수술 전 $FEV_1$$2.37{\pm}0.83$ ml 였다. 늑막폐정제술은 2명에서, 폐엽절제술은 분엽절제술이나 설상절제를 포함하여 11명에서 시행 받았다. 수술 후 한 환자 (7.7%)에서 폐렴이 발생하였고, 사망은 없었다. 균음전은 수술 후 $41.5{\pm}58.9$ 일만에 되었다. 모든 환자에서 수술 후 $5.1{\pm}0.8$ 제의 약제를 약 24개월까지 투여 받았는데, 한 환자(7.7%)에서 수술 32개월 만에 재발 하였다. 결론 : 주병변이 국한되어 있는 다제 내성 폐결핵 환자는 내과적 재치료에 실패하는 경우에는 보조적인 치료로써의 폐절제술을 적극적으로 고려해야 할 것이다.

발효유 산물인 LHFM의 인지기능 개선 효과 (Cognitive-enhancing Effects of a Fermented Milk Product, LHFM on Scopolamine-induced Amnesia)

  • 본 연구팀의 선행 연구 결과, L. helveticus IDCC3801의 발효유 주정 침전물이 BACE의 활성 억제를 통한 amyloid beta($A{\beta}$) 생성 저해효과를 갖고 있음을 확인하였다(15). 이러한 결과를 바탕으로 L. helveticus IDCC3801의 발효유 주정 침전물의 산업화를 위한 공정 개선연구를 진행하였고, 대량생산을 통해 LHFM을 확보하였다. 본 연구는 LHFM의 인지기능 개선 기능성 입증 연구로써 $A{\beta}$ 저해효과 외에 신경세포 보호효과 및 기억력 개선 효능을 확인하고 어떠한 작용 기전으로 개선 효과가 나타나는지 조사한 것으로 scopolamine으로 기억 손상을 유발한 mouse에서 학습 능력과 기억력 개선 효과, 신경세포 보호 효과, AChE 저해와 ACh 증가 효과 등을 평가하였다. LHFM은 신경세포 사멸을 유도하는 고농도의 glutamate가 존재하는 환경에서 농도 의존적으로 신경세포의 사멸을 억제하여 신경세포를 보호하였다. 또한 LHFM은 in vitro에서 농도 의존적으로 강력하게 AChE의 활성을 저해하였다. 이러한 in vitro 결과들을 근거로 scopolamine으로 기억 손상을 유발한 mouse에서 Morris water maze와 Y-maze 행동 시험을 통해 LHFM의 학습능력과 기억 개선 효과를 조사하였다. LHFM은 Morris water maze에서 실험 2일째부터 escape latency시간을 유의적으로 감소 시켰으며 5일째에서는 정상 mouse와 동등한 수준으로 감소되어 우수한 학습 효과를 보였다. 실험 6일째 platform을 치운 후 시행한 probe trial에서도 platform이 있던 위치에 머무는 회수가 scopolamine을 단독으로 처리한 것보다 유의적으로 높아 장기기억의 개선에도 효과가 있었다. Y-maze 시험에서도 LHFM은 scopolamine 단독 처리군과 비교하여 뚜렷하게 변경 행동력을 증가시켜 여러 행동 시험을 통해 LHFM의 인지기능 개선 효과를 확인하였다. 행동실험 후 hippocampus를 적출하여 ACh 및 AChE의 활성을 측정한 결과, donepezil과 마찬가지로 LHFM은 AChE의 활성을 유의적으로 억제하였으며 또한 ACh 함량도 증가시켰다. 결론적으로 LHFM은 신경세포 보호 효과와 신경전달물질인 ACh의 뇌 조직 내 농도를 높여 인지기능 개선 효과를 보이는 것을 확인하였다.

채소류의 수확 후 원재료 및 데침 처리에 의한 비타민 B5 및 B6 함량 변화 (Vitamin B5 and B6 Contents in Fresh Materials and after Parboiling Treatment in Harvested Vegetables)

  • 국내 다소비 채소류 39종에 대해 원재료 및 데침 조리 시 함유된 수용성 비타민 $B_5$$B_6$의 함량변화를 자외부 흡광 검출기(HPLC/DAD) 및 형광검출기(HPLC/FLD)로 분리 검출하였다. 각 성분의 분석법을 밸리데이션하여 신뢰도 높은 분석법임을 확인한 후 조제분유를 이용하여 내부 분석품질을 관리하였고, 비타민 $B_6$의 경우에는 국제 정도 관리를 통하여 정밀한 분석 수행능력을 평가 받았다. 검증된 분석법으로 채소류 39종의 원재료 및 데침 조리 시 비타민 $B_5$$B_6$ 함량 변화를 분석한 결과, 비타민 $B_5$의 경우 근채류에서는 마늘, 엽채류에서는 포항초, 경채류는 두릅, 과채류의 숙과용 호박이 각각의 분류에서 가장 높은 함량을 나타내었으며, 그 중 경채류인 두릅($2.462mg{\cdot}100g^{-1}$)이 가장 높은 함량을 나타내었다. 비타민 $B_6$의 경우에는 근채류에서 마늘, 엽채류에서는 근대, 경채류는 마늘쫑, 과채류의 적색토마토가 각각의 분류에서 가장 높은 함량을 나타내었고, 그 중 엽채류의 근대($0.127mg{\cdot}100g^{-1}$)가 가장 높은 함량으로 평가되었다. 대부분의 채소류에서 데침 처리 시 원재료에 비해 $B_5$$B_6$의 함량이 감소하거나 검출되지 않는 양상을 나타내었으며, 특히 비타민 $B_5$는 두릅, 비타민 $B_6$는 노란감자 및 근대에서 각각 약 20배 및 4배 대폭 감소하는 경향을 나타내었다. 이 결과는 채소작물의 이용 및 가공, 국민 식생활 정보 및 학교 조리 급식 등 영양적 부분과 국민 건강을 위한 중요한 기초자료로 이용될 수 있을 것이다.

상향 개화형 백색 오리엔탈나리 'Pacific Wave' 육성 (Breeding of Oriental Lily 'Pacific Wave' with Upward-facing and White Petals)

  • 'Pacific Wave'는 농촌진흥청 국립원예특작과학원에서 2007년에 육성된 절화용 오리엔탈나리 품종이다. 수출용 상향개화성 오리엔탈나리 품종을 육성하기 위하여 1999년 백색의 'Simplon'과 분홍색의 'Spinoza'를 주두수분에 의해 교잡을 하여 획득한 교배실생 중에서 화색이 백색이고 상향개화성이며 향기가 강하고 화형이 안정된 'O-03-16' 계통을 2003년에 선발하였다. 이 계통의 특성검정은 조직배양에 의한 대량증식, 순화 및 양구를 거쳐 특정검정을 수행하였다. 2007년 '원교C1-92호'로 수원에서 3차 특성검정 및 계통 평가회의 기호성 조사를 통해 최종적으로 절화용 오리엔탈나리로 선발하여 'Pacific Wave'라 명명하였다. 'Pacific Wave'품종의 개화기는 2월부터 가온했을 때 6월 중순이었고, 꽃의 향기가 강하다. 화색은 백색에 황색(RHS, W155C + Y9A)심이 있고, 포장수송상 유리한 상향개화성이다. 식물의 초장은 115cm이고, 꽃의 크기는 20.1 cm이다. 꽃의 화피장은 12.2 cm, 화피폭은 4.2 cm이다. 엽장은 12.3 cm이고, 엽폭은 2.9cm 이다. 꽃에는 돌기반점이 있으며, 주두는 진한 회색이고 화분은 붉은 갈색이다. 구근의 크기는 19.6cm, 중량은 82.5 g이다. 구근 수확 후 -1 또는 $-2^{\circ}C$의 조건에서 동결저장을 하여 정식시기를 달리하면 주년개화가 가능하다. 엽소현상을 방지하기 위해서는 칼슘시비 및 대구의 외인편을 제거해 주는 것이 필요하고, 여름 장마시기에는 잎마름병 방제가 필요하다.

벼 내도복 뽀얀멥쌀 신품종 '설갱' (A Lodging Tolerant, Opaque Rice Cultivar 'Seolgaeng')

  • '설갱'은 다양한 전분소재를 육성할 목적으로 1991년 하계에 양질다수성 품종인 일품벼에 돌연변이 유도물질(MNU)을 처리하여 $M_1$세대 697개체를 얻었다. 1992년 하계온실에서 $M_2$세대 113개체를 선발하여 1992/'93 동계 온실에서 $M_3$집단을 육성하였다. $M_4$세대부터는 포장에서 계통선발법에 의해 주요 병충해 및 미질검정을 병행하여 다양한 전분 신소재 계통을 선발 고정시켰다. 중만생종이며 전분의 형태가 아밀로스 함량은 일반 메벼와 같고 투명도는 찰벼와 같은 뽀얀메 일품(MNU) 28-3-GH1-2-5-3 계통을 선발하여 '수원 461호'로 계통명을 부여한 후 1999년부터 2001년까지 3년간 지역적응시험 실시 결과 그 우수성이 인정되어 2001년 12월 직무육성 신품종 선정위원회에서 국가목록등재품종으로 선정됨과 동시에 '설갱' 으로 명명하였다. 그 주요 특성을 요약하면 다음과 같다. 1. '설갱'의 출수기는 보통기 보비재배에서 평균 출수기가 8월 21일로 화성벼보다 9일 늦은 중만생이며, 간장은 83 cm로 화성벼보다 6 cm 짧으며 이삭당 벼알수는 화성벼보다 23개 많으나 등숙률은 73.3%로 화성벼보다 14.3% 낮았다. 2. '설갱'은 잎도열병, 흰잎마름병, 바이러스병 및 멸구류에는 약한 반응을 보였다. 3. '설갱'은 내냉성에는 중강 정도의 특성이었으며, 벼 키와 3절 간장이 화성벼보다 짧고 도복지수가 낮아서 도복에 강하다. 4. '설갱' 쌀의 외관은 찹쌀과 같은 뽀얀특성을 지니고 있으나 아밀로스 함량은 19.3%로 일반메벼와 같은 특성을 보였으며, 식미 관능검정에서 밥맛이 화성벼보다 양호하였으나, 도정율은 화성벼보다 떨어졌다. 5. '설갱' 쌀의 발효적성 검정 결과 황국균 발효에서 일품벼보다 균사의 밀도 및 향이 우수하며 홍국균 고체 발효에서도 일품벼보다 색소 및 균사체 상태가 우수하였다. 6. '설갱'의 쌀 수량은 보통기 보비재배에서 평균 5.27 MT/ha으로 대비품종 화성벼 및 동진벼와 비슷하였고, 남부평야지 이모작 재배에서는 5.08 MT/ha로 동진벼보다 3% 감수하였으나, 만식재배에서는 5.75 MT/ha로 동진벼보다 6% 증수하였다. 7. 재배적지는 중부 및 남부 평야지이다.

개인정보보호 분야의 연구자 네트워크와 성과 평가 프레임워크: 소셜 네트워크 분석을 중심으로 (The Framework of Research Network and Performance Evaluation on Personal Information Security: Social Network Analysis Perspective)

  • 개인정보 분야에서의 다양한 정보 보안 이슈가 발생함에 따라 해당 분야의 전문가를 확인하기 위한 프레임워크는 매우 중요한 영역이 되었다. 전문가 탐색과정은 주로 연구 업적 등을 통한 주관적인 평가가 일반적이지만 보다 객관적인 방식을 통한 선정이 매우 중요하다. 소셜 네트워크 분석기법의 응용이 다양한 영역에서 활용됨에 따라 본 연구는 개인정보보호분야의 전문가를 확인하고 해당 전문가들의 연구실적을 판단하기 위한 분석 프레임워크를 제시하고자 하였다. 본 연구는 연구 목적에 따라 개인정보보호 연구영역의 연구성과 자료를 바탕으로 소셜 네트워크 분석을 실시하고 핵심연구자의 성과를 분석하였다. 수집된 데이터는 연구의 공저자, 발행기관, 소속기관 등의 네트워크 구성에 활용되어 핵심전문가 집단을 관리하기 위한 프레임워크를 제시하였다. 본 연구는 NDSL에서 최근 5년 동안 발표된 논문들을 중심으로 자료를 수집하였다. 연구자들이 학술 정보를 교환하는 정기 간행물인 학술지를 바탕으로 연구 네트워크를 형성하는 네트워크 자료를 수집함으로써 연구활동에 대한 정보를 분석할 수 있었다. 일반적으로 연구자들은 연구 결과를 논문으로 발표하고, 발표된 논문들이 다수의 관련 분야 전문가들에게 공유된다는 점에서 학술연구지는 연구자들의 지식관련 의사소통 공간이며 지식의 구조화에 핵심적인 역할을 수행한다. 그에 따라 본 연구의 연구 대상 분야로 설정한 개인정보보호 분야의 연구 구조를 이해하기 위해 국내에서 발표된 관련 분야의 논문들을 연구 대상으로 자료가 수집되었다. 특히 자료의 선별 기준은 국내 최대의 데이터베이스를 보유하고 있는 NDSL에서 개인정보보호 관련 키워드를 보유한 논문 데이터를 수집 및 정제하여 분석 자료로 사용하였다. 2005년부터 2013년까지 약 2,000개의 연구결과 중 주제 관련성, 공저자 추출 등을 수집하였다. 데이터 수집 이후 연구 분석을 위한 데이터 처리를 통하여 통해 총 784개의 논문을 선정하고 분석대상으로 확정하였다. 분석 결과, 개인정보보호 연구영역의 전문가 집단을 이용한 연구논문 성과에 대한 분석은 핵심 연구자들을 추출해내고 전문가 집단을 관리하는 데 도움을 제공할 수 있다. 특히 소속집단 및 연구논문 발행기관을 분석함으로써 개인정보보호 연구영역에서 확인되지 않았던 연구자들의 연구 논문 게재의 공저자 네트워크가 매우 밀접함을 확인할 수 있다. 또한 연구논문의 발행기관 및 소속집단의 특성을 추출함으로써 개인정보보호 영역의 전문가 평가지표로서 소셜 네트워크 지표들의 활용가능성을 확인하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. In this paper, a simple reflective infrared optic system with a 2-dimensional optic scanner is proposed for the realization of medical infrared thermography system.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction.
