Doxylamine 중독 후 합병한 비 외상성 횡문근융해증 1례 (A Case of Non-traumatic Rhabdomyolysis after Doxylamine Overdose)

  • Doxylamine is an antihistamine of the ethanolamine class. It is used primarily as a sleep-inducing agent. Clinicians should be aware of the complications in rhabdomyolysis patients who ingest doxylamine succinate and over-the-counter antihistamines. The easy availability of these substances increases the potential not only of intentional overdose by adults but also of inadvertent ingestion by children. Prompt intervention and careful assessment of renal function, urinary output, and serum creatine kinase levels may represent the difference between an uncomplicated and acute renal failure. Recognition of the potential for rhabdomyolysis and institution of vigorous treatment may prevent acute renal failure in patients who have taken an overdose of the drug. A 14-year-old male was found to have hematuria and oliguria. Evaluation of the patient revealed myoglobinuria, and a creatine kinase(CK) level of 117,563 IU/L. He was recovered by massive fluid administration, urine alkalization and mannitol infusion. We report a case of a suicide attempt in a child where ingestion of the doxylamine complicated by non-traumatic rhabdomyolysis with brief review related literatures.

약인성 급성 신부전 환자의 한약치료에 대한 증례 보고 (A Case Report of Drug-Induced Acute Renal Failure Treated with Herbal Medicine)

  • Objective: To report the effects of Siryeng-tang (柴苓湯) and Chijasi-tang (梔子豉湯) on a patient with drug-induced acute renal failure. Methods: A female patient was diagnosed with acute renal failure. The symptoms decreased urine output, tachycardia, dyspnea, and chest discomfort. The patient was treated with Siryeng-tang for one day and Chijasi-tang for about 5 days. Results: After taking Siryeng-tang, urine output increased and the values for blood urea nitrogen and creatinine were decreased to normal. Taking Chijasi-tang improved other related symptoms. Conclusions: Siryeng-tang and Chijasi-tang can improve the symptoms of acute renal failure.

  • A 7-year-old, female Shih-tzu dog with clinical signs of hematuria, oliguria, stranguria was brought to the Veterinary Teaching Hospital, Chonnam National University. Ultrasonography revealed the left hydroureter, hydronephrosis and urinary bladder mass ($12.1\;{\times}\;22.0\;mm$). The dog was finally diagnosed as the secondary unilateral obstructive uropathy by urinary bladder mass. The treatment such as unilateral nephroureterectomy and chemotherapy using piroxicam were performed to the dog. After 8 months of treatment, the polyp was not detected by ultrasonography. We concluded that urinary bladder mass was cured completely.

골주사를 이용한 사교상(Snake bite) 환자의 경과 관찰 2예 보고 (Two Cases of Bone Scan in Snake Bite)

  • It is very difficult to check the severity and clinical course of the toxicity in snake bite patients by virtes of clinical manifestation and laboratory tests. And we observed the. findings of bone scan with 99mTc-MDP in two snake bite patients. First patient was bitten in the right ankle with local pain and swelling. The finding of bone scan of him was increased uptake of radionuclide in the soft tissue of right leg and thigh. Others were normal findings. Second patient was bitten in the right hand. But his symptom was severe and he complained local pain and swelling, nausea, blurred vision, and oliguria. The bone scan findings of second patient was; Increased uptake of radionuclide in the soft tissue of whole body. Decreased uptake in the bone tissue. Renal outline was not delineated. Follow up study 10 days after, revealed more improved findings in the scan.

Bridge to Transplantation with a Left Ventricular Assist Device

  • A 61-year-old female patient was diagnosed with dilated cardiomyopathy with severe left ventricle dysfunction. Two days after admission, continuous renal replacement therapy was performed due to oliguria and lactic acidosis. On the fifth day, an intra-aortic balloon pump was inserted due to low cardiac output syndrome. Beginning 4 days after admission, she was supported for 15 days thereafter with an extracorporeal left ventricular assist device (LVAD) because of heart failure with multi-organ failure. A heart transplant was performed while the patient was stabilized with the LVAD. She developed several complications after the surgery, such as cytomegalovirus pneumonia, pulmonary tuberculosis, wound dehiscence, and H1N1 infection. On postoperative day 19, she was discharged from the hospital with close follow-up and treatment for infection. She received follow-up care for 10 months without any immune rejection reaction.

대동맥중격결손증[수술치험 1예] (Aorticopulmonary Window: one case report)

  • Aorticopulmonary window is a rare anomaly among congenital heart disease. Various terms have been suggested including A-P window, A-P fenestration, fistula, aorticseptal defect etc. The defect lies usually between the left side of the ascending aorta and right wall of the pulmonary artery just anterior to the origin of the right main pulmonary artery. We have experienced one case of aorticopulmonary septal defect which was diagnosed as V5D with pulmonary hypertension in 1 4/12 year old, 7.2 Kg, male patient. Operation was done under the hypothermic cardiopulmonary bypass using 5t. Thomas cardioplegic solution. Vertical right ventriculotomy over the anterior wall of RVOT revealed no defect in the ventricular septum, and incision was extended up to the main pulmonary artery to find the source of massive regurgitation of blood through MPA. Finger tip compression of the aorticopulmanary window was replaced with Foley bag catheter balloon, and the $7{\times}10$ mm aorticoseptal defect located 15mm above the pulmonic valve was sutured continuously wih 3-0 nylon suture during azygos flow of cardiopulmonary cannula which was located distal to the window resulted massive air pumping systemically, and temporary reversal of pumping was tried to minimize cerebral air embolism. Remained procedure was done as usual, and pump off was smooth and uneventful. Postoperatively, patient was attacked frequent opistotonic seizure with no recovery sign mentally and p.hysically. Vital signs were gradually worsen with peripheral cyanosis and oliguria, and cardiac activity was arrested 1485 minutes after operation. Autopsy was performed to find the sutured window and massive edema of the brain.

Disseminated Histiocytic Sarcoma with Ureteric Involvement in a Jindo Dog

  • We describe the case of a Jindo dog that presented with a 2-month history of an abscess-like mass on the skin of the left hip. The patient was initially diagnosed with a suspected insect bite and was treated with a topical dressing. After observing no improvements, surgical debridement was done to remove the abscess. Nine days after surgery, the patient started showing severe anorexia, vomiting, lethargy, and oliguria. Azotemia was detected, and the patient was diagnosed with acute kidney injury. Ultrasonography depicted an abdominal mass (4.6 × 7.5 cm) in the right ureter, and severe hydronephrosis in the right kidney. The patient was euthanatized. Results of the necropsy, histopathologic examination, and immunohistochemistry for tumor cells, confirmed that the patient had developed hydronephrosis and disseminated histiocytic sarcoma which had metastasized to the lungs. To the best of our knowledge, this is the first report of acute renal failure produced by ureter invasion and urinary tract obstruction resulting from the abdominal mass of disseminated histiocytic sarcoma in dogs.

Surgical Repair of Aortocaval Fistula Presenting with Cardiogenic Shock

  • Aortocaval fistula (ACF) occurs in <1% of all abdominal aortic aneurysms (AAAs), and in 3% to 7% of all ruptured AAAs. The triad of clinical findings of AAA with ACF are abdominal pain, abdominal machinery bruit, and a pulsating abdominal mass. Other findings include pelvic venous hypertension (hematuria, oliguria, scrotal edema), lower-limb edema with or without arterial insufficiency or venous thrombus, shock, congestive heart failure, and cardiac arrest. Surgery is the main treatment modality. We report successful surgical treatment in a patient with a ruptured AAA with ACF who presented with cardiogenic shock.

담도 스텐트 시술 후 장천공 증례보고 및 장천공에 대한 고찰 (A case report of gastrointestinal perforation in patient after biliary stent insertion and the overview of gastrointestinal perforation)

  • Objectives: This study aimed to emphasize the importance of accurate and timely diagnosis of acute abdominal pain with simple radiography by reporting a case of gastrointestinal perforation. Methods: We closely observed the diagnosis and progress of acute abdominal pain after biliary stent and reviewed the outline of gastrointestinal perforation. Results: Patient diagnosed with urethral cancer metastasis to lung and peritoneum was treated with complex Korean medicinal treatments to deal with anorexia, abdominal pain, jaundice and oliguria. During hospitalization, the patient's acute abdominal pain after biliary stent was diagnosed with gastrointestinal perforation by using plain chest and abdominal radiography. Conclusion: Using simple radiography to find out the emergency diseases such as perforation in acute abdominal pain is important clinically.

Angiotensin receptor blocker induced fetopathy: two case reports and literature review

  • The administration of angiotensin type 2 receptor blockers (ARBs) during pregnancy is known to cause ARB fetopathy, including renal insufficiency. We aimed to analyze the outcomes of two patients who survived ARB fetopathy and perform an accompanying literature review. Case 1 was exposed antenatally from a gestational age of 30 weeks to valsartan because of maternal pregnancy-induced hypertension. The patient presented with oliguria immediately after birth, and renal replacement therapy was administered for 24 days. Seven years after birth, renal function was indicative of stage 2 chronic kidney disease (CKD) with impaired urinary concentration. Case 2 had a maternal history of hypertension and transient ischemic attack and was treated with olmesartan until 30 weeks of pregnancy. Renal replacement therapy was performed for 4 days since birth. After 8 years, the patient is with CKD stage 2, with intact tubular function. Recent reports suggest that ARB fetopathy might manifest as renal tubular dysgenesis and nephrogenic diabetes insipidus, in contrast to mild alterations of glomerular filtration. Tubular dysfunction may induce CKD progression and growth retardation. Patients with ARB fetopathy should be monitored until adulthood. The ARB exposure period might be a critical factor in determining the severity and manifestations of fetopathy.
